Background: There is a lack of evidence-based guidelines for the administration methods of ceftriaxone in emergency departments (EDs), resulting in the reliance on individual institutional protocols for decision-making., Objective: This study was performed to compare the effects of administering ceftriaxone via intravenous push (IVP) and intravenous piggyback (IVPB) on 28-day mortality in patients with sepsis., Methods: This was a retrospective study of patients aged 18 years or older with sepsis or septic shock who visited an ED and were treated with ceftriaxone as an initial antibiotic between March 2010 and February 2019. Patients were divided into the IVP group and the IVPB group based on the administration method. The primary outcome was 28-day mortality, and multivariable Cox proportional hazards regression analysis was performed to evaluate the relationship between antibiotic administration methods and 28-day mortality., Results: During the study period, a total of 939 patients were included in the final analysis, and the overall mortality rate was 12.2%. The antibiotic administration time was significantly lower in the IVP group than in the IVPB group, and the rates of antibiotic administration within 1 h and within 3 h were higher in the IVP group than in the IVPB group (p < 0.05). Introduction: This research aimed to compare the estimation error of the root axis using 3-dimensional (3D) tooth models at the midtreatment stage between the whole-surface scan (WSS) and lingual-surface scan (LSS) methods., Methods: The sample consisted of 208 teeth (26 each of central incisors, canines, second premolars, and first molars in the maxillary and mandibular dentition) from 13 patients whose pre- and midtreatment intraoral scan and cone-beam computed tomography (CBCT) were available. The 3D tooth models were constructed by merging the intraoral-scan crowns and the CBCT-scan roots obtained at the pretreatment stage. To estimate the root axis at the midtreatment stage, we superimposed the individual 3D tooth models onto the midtreatment intraoral scan obtained by the WSS and LSS methods. The midtreatment CBCT scan was used as the gold standard to determine the real root axis. The estimated root axis in terms of mesiodistal angulation and buccolingual inclination was measured in the WSS and LSS methods, and statistical analysis was performed., Results: The estimation errors of the mesiodistal angulation and buccolingual inclination were <2.0° in both methods. The LSS method demonstrated a statistically larger but clinically insignificant estimation error than the WSS method in the mandibular canine (mesiodistal angulation, 1.95° vs 1.62°) and the total tested teeth (mesiodistal angulation, 1.40° vs 1.29°; buccolingual inclination, 1.51° vs 1.41°)., Conclusions: Because the estimation errors of the root axis angle using the 3D tooth model by the WSS and LSS methods were within the clinically acceptable range, the root axis can be estimated by both methods., (Copyright © 2020 American Association of Orthodontists. Given the increased interest in public hygiene due to outbreaks of food poisoning, increased emphasis has been placed on developing novel monitoring systems for point-of-care testing (POCT) to evaluate pathogens causing foodborne illnesses. Here, we demonstrate a pathogen evaluation system utilizing simple film-based microfluidics, featuring simultaneous gene amplification, solution mixing, and electrochemical detection. To minimize and integrate the various functionalities into a single chip, patterned polyimide and polyester films were mainly used on a polycarbonate housing chip, allowing simple fabrication and alignment, in contrast to conventional polymerase chain reaction, which requires a complex biosensing system at a bench-top scale. The individual integrated sensing chip could be manually fabricated in 10 min. Using the developed film-based integrated biosensing chip, the genes from the pathogens causing foodborne illnesses were simultaneously amplified based on multiple designed microfluidic chambers and Hoechst 33258, which intercalates into double-stranded DNA, to generate the electrochemical signal. The target pathogen gene was accurately analyzed by square wave voltammetry (SWV) within the 25 s, while the gel electrophoresis required about 30 min. Based on the developed integrated biosensing chip, the 1.0 × 10 1 and 1.0 × 10 2  colony-forming unit (CFU) of Staphylococcus aureus and Escherichia coli were sensitively detected with high reproducibility in the 25 s. On the basis of the significant features of the film-based molecular analysis platform, we expect that the developed sensor could be applied to the screening of various pathogens as a POCT device., (Copyright © 2018 Elsevier B.V. Background: Dietary factors contribute to the risk of developing metabolic syndrome, a disorder associated with an increased risk of developing cardiovascular disease, diabetes mellitus, and some cancers., Objective: The objective of this study was to evaluate the association between the intake frequencies of certain food groups, eating habits, and the risk of metabolic syndrome in a cross-sectional study of Korean men., Methods: Study participants were recruited from the National Cancer Center in South Korea. A total of 7,081 men aged 30 years and older were recruited between August 2002 and May 2007. Metabolic syndrome was defined as having three or more of the following conditions: obesity, high blood pressure, low high-density lipoprotein cholesterol level, high triglyceride level, and high fasting blood glucose level. The association of metabolic syndrome and sociodemographic characteristics, food intake frequencies, and eating habits assessed by a food frequency questionnaire, was examined., Results: The prevalence rate of metabolic syndrome for men aged 30 to 39, 40 to 49, 50 to 59, and 60+ years was 18.2%, 19.8%, 21.9%, and 20.5%, respectively. The study participants with metabolic syndrome had significantly higher family history of type 2 diabetes mellitus (27.6% vs 21.6%, P<0.001), and were more likely to be current smokers (50.1% vs 45.3%, P=0.005) than their counterparts. Among food group items, participants with metabolic syndrome showed significantly higher intake of seaweed (odds ratio [OR] 1.25, 95% confidence interval [CI] 1.05 to 1.50), and oily foods (OR 1.28, 95% CI 1.04 to 1.57) than participants without metabolic syndrome. In addition, the group with metabolic syndrome was more likely to eat quickly (OR 2.23, 95% CI 1.60 to 3.12 for fast vs slow) and to overeat frequently (OR 2.37, 95% CI 1.85 to 3.05 for more than 4 times a week vs less than once a week)., Conclusions: The results suggest that high intake of seaweed and oily foods as well as eating habits such as eating faster and frequent overeating, are associated with the risk of metabolic syndrome. In contrast, high fruit intake may be associated with a lower risk of metabolic syndrome. The importance of dietary habits in metabolic syndrome development needs to be pursued in further studies.

Docosahexaenoic acid (DHA; 22:6n-3) and n-6 docosapentaenoic acid (DPAn-6; 22:5n-6) are components of enriched animal feed and oil derived from Schizochytrium species microalgae. A one generation, artificial rearing model from day 2 after birth onward (AR) and a dam-reared control group (DAM) were used to examine DPAn-6 feeding on the fatty acid composition of various rat tissues at 15 weeks of age. Four AR diets were based on an n-3 fatty acid-deficient, 18:2n-6-based artificial milk with 22:6n-3 and/or 22:5n-6 added: AR-LA, AR-DHA, AR-DPAn-6, and AR-DHA+DPAn-6. The 22:6n-3 levels for the DAM, AR-DHA, and AR-DHA+DPAn-6 groups tended to be similar and higher than in the AR-LA and AR-DPAn-6 groups. The levels of 22:5n-6 tended to be higher only in the absence of dietary 22:6n-3. Adipose levels of 22:5n-6 was the only exception, as 22:5n-6 was significantly higher in AR-DHA+DPAn-6 than was observed in either the DAM or the AR-DHA group. There were no differences in 20:4n-6 levels within the tissues examined. In conclusion, 22:5n-6 replaces 22:6n-3 in the absence of 22:6n-3 only and does not appear to compete with 22:6n-3 in the presence of dietary 22:6n-3, suggesting that oils containing 22:5n-6 and 22:6n-3 may be a good dietary source of 22:6n-3.

Lead (Pb) exposure has been reported to increase arachidonic (AA) and docosahexaenoic (DHA) acids. To determine whether Pb effects on fatty acid composition are influenced by dietary (n-3) fatty acid restriction, weanling female rats were fed either an (n-3)-adequate or -deficient diet to maturity and mated. At parturition, dams in each group were subdivided to receive either 0.2% Pb or Na-acetate in their drinking water during lactation only. Pups were analyzed for fatty acid content in liver, plasma, and brain at either 3 or 11 wk. The (n-3)-deficient diets markedly decreased total (n-3) fatty acids, and increased total (n-6) fatty acids including both AA and docosapentaenoic (n-6) in each compartment (P < 0.05). The main effects of Pb were in the livers of weanling rats where there was a 56% loss in total fatty acid concentration concurrent with increased relative percentages of AA and DHA. Thus, because there was a greater percentage of liver nonessential fatty acid lost relative to the essential fatty acids (EFA), there was no net change in AA concentration. There was a diet x Pb interaction for a decrease in liver DHA concentration evident only in the (n-3)-adequate group. There were also diet x Pb interactions in plasma at 11 wk and in brain at 3 wk. These data are consistent with the hypothesis of a Pb-induced increase in fatty acid catabolism, perhaps as a source of energy.

Docosahexaenoic acid [22:6(n-3), DHA] is important for optimal infant central nervous system development, and lead (Pb) exposure during development can produce neurological deficits. Long-Evans strain rats were fed either an (n-3) deficient [(n-3) Def] diet to produce brain DHA deficiency, or an adequate [(n-3) Adq] diet through 2 generations. At the birth of the 2nd generation, the dams were subdivided into 4 groups and supplied drinking water containing either 5.27 mmol/L (Pb) or sodium (Na) acetate until weaning. Rats were killed at 3 wk (weaning) and 11 wk (maturity) for brain Pb and fatty acid analysis. Spatial task and olfactory-cued behavioral assessments were initiated at 9 wk. Rats in the (n-3) Def group had a 79% lower concentration of brain DHA compared with the (n-3) Adq group with no effect of Pb exposure. At weaning, Pb concentrations were 7.17 +/- 0.47 nmol Pb/g of brain (wet weight) in the (n-3) Adq-Pb group and 6.49 +/- 0.63 nmol Pb/g of brain (wet weight) in the (n-3) Def-Pb group. At maturity, the brains contained 1.30 +/- 0.22 and 1.07 +/- 0.12 nmol Pb/g (wet weight), respectively. In behavioral testing, significant effects of both Pb and DHA deficiency were observed in the Morris water maze probe trial and in 2-odor olfactory discrimination acquisition and olfactory-based reversal learning tasks. Both lactational Pb exposure and (n-3) fatty acid deficiency led to behavioral deficits with additive effects observed only in the acquisition of 2-odor discriminations.

Rat pups born to dams fed a diet with 3.1% of total fatty acids as alpha-linolenic acid (LNA) were fed, using an artificial rearing system, either an n-3-deficient (n-3-Def) or an n-3-adequate (n-3-Adq) diet. Both diets contained 17.1% linoleic acid, but the n-3-Adq diet also contained 3.1% LNA. The percentage of brain docosahexaenoic acid (DHA) continuously decreased (71%) with time over the 29 days of the experiment, with concomitant increases in docosapentaenoic acid (DPAn-6). In the retina, the percentage of DHA rose in the n-3-Adq group, with an apparent increased rate around the time of eye opening. However, there was a flat curve for the percentage of DHA in the n-3-Def group and a rising DPAn-6 with time. Liver DHA was highest at the time of birth in the n-3-Adq group but fell off somewhat over the course of 29 days. This decrease was more pronounced in the n-3-Def group, and the DPAn-6 rose considerably during the second half of the experiment.
