Background: Fluoroquinolones are a popular alternative to trimethoprim-sulfamethoxazole for Stenotrophomonas maltophilia infections., Objectives: To compare the effects of fluoroquinolones and trimethoprim-sulfamethoxazole on mortality of S. maltophilia infections., Data Sources: PubMed and EMBASE., Study Eligibility Criteria: Clinical studies reporting mortality outcomes of S. maltophilia infections., Participants: Patients with clinical infections caused by S. maltophilia., Interventions: Fluoroquinolone monotherapy in comparison with trimethoprim-sulfamethoxazole monotherapy., Methods: Systematic review with meta-analysis technique., Results: Seven retrospective cohort and seven case-control studies were included. Three cohort studies were designed to compare the two drugs, whereas others had other purposes. A total of 663 patients were identified, 332 of which were treated with trimethoprim-sulfamethoxazole (50.1%) and 331 with fluoroquinolones (49.9%). Levofloxacin was most frequently used among fluoroquinolones (187/331, 56.5%), followed by ciprofloxacin (114/331, 34.4%). The overall mortality rate was 29.6%. Using pooled ORs for the mortality of each study, fluoroquinolone treatment (OR 0.62, 95% CI 0.39-0.99) was associated with survival benefit over trimethoprim-sulfamethoxazole treatment, with low heterogeneity (I 2  = 18%). Specific fluoroquinolones such as ciprofloxacin (OR 0.44, 95% CI 0.17-1.12) and levofloxacin (OR 0.78, 95% CI 0.48-1.26) did not show a significant difference in comparison with trimethoprim-sulfamethoxazole. In the sub-group analyses of adult and bacteraemic patients, significant differences in mortality were not observed between fluoroquinolones and trimethoprim-sulfamethoxazole., Conclusions: Based on a meta-analysis of non-randomized studies, fluoroquinolones demonstrated comparable effects on mortality of S. maltophilia infection to trimethoprim-sulfamethoxazole, supporting the use of fluoroquinolones in clinical S. maltophilia infections. Although the pooled analysis of overall studies favoured fluoroquinolones over trimethoprim-sulfamethoxazole, the studies included were observational, and sub-group analyses of certain fluoroquinolone agents did not show statistical differences with trimethoprim-sulfamethoxazole. This experiment was conducted to investigate the effect of feeding duration of diets containing corn distillers dried grains with solubles (DDGS) on productive performance, egg quality, and lutein and zeaxanthin concentrations of egg yolk in laying hens. A total of 300 57-week-old Hy-Line Brown laying hens were randomly assigned to one of 5 treatment groups (feeding duration) with 6 replicates consisting of 5 consecutive cages with 2 hens per cage. Diets were formulated to contain either 0% (the control diet) or 20% DDGS. Experimental diets were fed to hens for 12 wk. The feeding duration of diets containing 20% DDGS was 0, 3, 6, 9, or 12 wk before the conclusion of the experiment. Feeding the diet containing 20% DDGS for 3, 6, or 9 wk followed feeding the control diet for 9, 6, or 3 wk, respectively. The data for productive performance were summarized for 12 wk of the feeding trial. Results indicated that increasing feeding duration of diets containing 20% DDGS had no effects on productive performance of laying hens, but increased egg yolk color (linear, P < 0.01), hunter a* value (linear and quadratic, P < 0.01), and b* values (linear, P < 0.05) with a decrease in hunter L* value (linear and quadratic, P < 0.05). Lutein and zeaxanthin concentrations of egg yolks also were increased (linear, P < 0.01) by increasing the feeding duration of diets containing 20% DDGS. In conclusion, feeding diets containing 20% DDGS to laying hens has no adverse effects on productive performance. We conducted a 2-year multicentre prospective observational study to determine the epidemiology of and mortality associated with invasive fungal diseases (IFDs) among patients with haematological disorders in Asia. Eleven institutions from 8 countries/regions participated, with 412 subjects (28.2% possible, 38.3% probable and 33.5% proven IFDs) recruited. The epidemiology of IFDs in participating institutions was similar to Western centres, with Aspergillus spp. (65.9%) or Candida spp. (26.7%) causing the majority of probable and proven IFDs. The overall 30-day mortality was 22.1%. Progressive haematological disorder (odds ratio [OR] 5.192), invasive candidiasis (OR 3.679), and chronic renal disease (OR 6.677) were independently associated with mortality., (Copyright © 2015 European Society of Clinical Microbiology and Infectious Diseases. The antifungal activity and mechanism of a 23-mer peptide, PMAP-23, derived from pig myeloid was investigated. PMAP-23 displayed strong antifungal activity against yeast and mold. To investigate the antifungal mechanism of PMAP-23, fluorescence activated flow cytometry and confocal laser scanning microscopy were performed. Candida albicans treated with PMAP-23 showed higher fluorescence intensity by propidium iodide(PI) staining, which was similar to that of Melittin than untreated cells. Confocal microscopy showed that the peptide was located in the plasma membrane. The action of peptides against fungal cell membranes was examined by treating prepared protoplasts of C. albicans with the peptide and lipid vesicle titration test. The result showed that the peptide prevented the regeneration of fungal cell walls and induced release of the fluorescent dye trapped in the artificial membrane vesicles, indicating that the peptide exerts its antifungal activity by acting on the plasma lipid membrane., (Copyright 2001 Academic Press.)

CRAMP-18 (GEKLKKIGQKIKNFFQKL) is the antibacterial sequence derived from CRMAP, a member of cathelicidin-derived antimicrobial peptides. To develop the novel antibiotic peptides useful as therapeutic drugs requires strong antibiotic activity against bacterial and fungal cells without hemolytic effect. To this goal, the analogues were designed to increase only net positively charge by Lys-substitution of positions 2, 9, 13, or 16 at the hydrophilic helix face of CRAMP-18 without any change at the hydrophobic helix face. In particular, Lys-substitution (K(2)-CRAMP-18) of position 2 in CRAMP-18 induced the enhanced antibiotic activity without any increase in hemolysis. Thus, this peptide may provide a useful template for the design novel antibiotic peptides for the treatment of infectious diseases. Additional CD spectra studies suggested that the alpha-helical structure of the peptides plays an important role in killing bacterial and fungal cells, but the increase of alpha-helical content is less connected with the enhanced antibiotic activity., (Copyright 2000 Academic Press.)

The inhibitory effects of the traditional herbal medicine Jindangwon (JDW) on streptozotocin (ST)-induced diabetic mellitus were studied using the ST-treated diabetic model. Glucokinase activity of pancreatic islets was severely impaired by ST treatment. However, when ST-treated islets were treated with 1 mg/ml of JDW, the enzyme activities of glucokinase and hexokinase were protected, glucose-6-phosphatase was not. When the effects of JDW on ST-induced ATP/ADP ratio of islets were assayed, JDW was effective in restoring of ATP/ADP ratio. In addition, ST decreased the enzyme activities of PDH, while JDW had a protective effect on the enzyme. ST-induced cGMP accumulation was significantly inhibited by JDW treatment. Furthermore, ST-induced nitrite formation was significantly inhibited by JDW treatment. JDW also showed the suppressed nitrite production in ST-treated pancreatic islet cells. When the islets (200/condition) were treated with ST (5 mM for 30 min), and then JDW was added to the ST-treated cells, 1.0 mg/ml of JDW showed the activated and recovered aconitase activity in pancreatic islet cells. When the effect of ST on the gene expression of pancreatic GLUT2 and glucokinase were examined, the level of GLUT2 and glucokinase mRNA in pancreatic islets was significantly decreased. However, JDW protected and improved the expression of protein and genes, indicating that JDW is effective on ST-induced inhibition of gene expression of GLUT2, glucokinase and proinsulin in islets. These results suggested that JDW is effective in this model to treat ST-induced diabetes.

Proteins that bind and hydrolyze ATP are frequently involved in the early steps of DNA replication. Recent studies of Saccharomyces cerevisiae suggest that two members of the AAA+ ATPase family--the origin recognition complex and Cdc6p--have separable roles for ATP binding and ATP hydrolysis during eukaryotic DNA replication. Intriguingly, the proposed regulation of these eukaryotic replication proteins by ATP has functional similarities to the ATP-dependent control of the DnaA and DnaC initiation factors from Escherichia coli. Comparison of the ATP regulation of these factors suggests that ATP binding and hydrolysis acts as a molecular switch that couples key events during initiation of replication. This switch results in a significant change in protein function.

SUMMARY. Recent research has enhanced the understanding of instability of the spine. The principles of this research have been incorporated into the evaluation and treatment of the unstable thorax. Rotational instability of the midthorax is commonly seen following trauma to the chest. Specific mobility and stability tests have been developed to detect this instability. The tests are derived from a biomechanical model of evaluation. Treatment is based on sound stabilization principles and although the segment will remain unstable on passive testing, the patient can be trained to control the biomechanics of the thorax and return to a high level of function. It has been suggested that changes in bioactive luteinizing hormone in women occur toward midcycle and may result in increased bioactive/immunoreactive luteinizing hormone ratios. To determine whether dopamine or norepinephrine modulate immunoreactive and bioactive luteinizing hormone secretion, 15 ovulatory women were studied in the mid to late follicular phase. Dopamine in two doses (0.5 micrograms/kg/min and 4 micrograms/kg/min) and norepinephrine, 0.1 microgram/kg/min, were infused for 4 hours, and metoclopramide, 10 mg intravenously, was also given to determine whether dopamine receptor antagonism results in changes. Bioactive luteinizing hormone and the bioactive/immunoreactive luteinizing hormone ratio increased in women from the early to late follicular phase (p less than 0.05). Both intravenous doses of dopamine resulted in significant decrements in immunoreactive luteinizing hormone (20 +/- 2 and 20 +/- 3%, p less than 0.02) and bioactive luteinizing hormone (36.7% and 43.2%, p less than 0.05). With dopamine there was also a significant decrease in the bioactive/immunoreactive luteinizing hormone ratio (p less than 0.02). Intravenous norepinephrine, however, resulted in no changes in either immunoreactive or bioactive luteinizing hormone levels. Metoclopramide also did not change immunoreactive or bioactive luteinizing hormone levels. These data suggest that although endogenous dopaminergic blockade may not play a significant role in determining basal levels of luteinizing hormone, decreases in dopamine at the pituitary level may increase the bioactive/immunoreactive luteinizing hormone ratio. Norepinephrine does not appear to exert major changes in immunoreactive or bioactive luteinizing hormone.
