Abstract

The nuclear structure of $^{66}$Se, nucleus beyond the N=Z line on the proton-rich side of the valley of stability, was investigated by the neutron knock-out reaction $^{67}$Se($^{12}$C,X)$^{66}$Se using a $^{12}$C target. The analysis of the singles spectrum of the γ-rays emitted during the de-excitation of the populated low-lying excited states revealed two previously detected (927(4) keV, 1460(32) keV) and three new (744(6) keV, 1210(17) keV, 1661(23) keV) transitions. The 744-keV, the 1210-keV, and the 1460-keV transitions were found to be in coincidence with the one at 927 keV. The spectrum coincident with the 927-keV transition showed a further possible transition at 299(35) keV, which was obscured by significant atomic background in the singles spectrum. This transition might correspond to a peak previously reported at 273(5) keV that could not be assigned to $^{66}$Se unambiguously. Based on a comparison of the experimental data to theoretical calculations, four new excited states are proposed which suggest that $^{66}$Se exhibits shape coexistence.

Abstract

Gamma decays were observed in $^{56}$Ca and $^{58}$Ca following quasi-free one-proton knockout reactions from $^{57,59}$Sc beams at ≈200 MeV/nucleon. For $^{56}$Ca, a γ ray transition was measured to be 1456(12) keV, while for $^{58}$Ca an indication for a transition was observed at 1115(34) keV. Both transitions were tentatively assigned as the 21+→0gs+ decays, and were compared to results from ab initio and conventional shell-model approaches. A shell-model calculation in a wide model space with a marginally modified effective nucleon-nucleon interaction depicts excellent agreement with experiment for 21+ level energies, two-neutron separation energies, and reaction cross sections, corroborating the formation of a new nuclear shell above the N = 34 shell. Its constituents, the 0f5/2 and 0g9/2 orbitals, are almost degenerate. This degeneracy precludes the possibility for a doubly magic $^{60}$Ca and potentially drives the dripline of Ca isotopes to $^{70}$Ca or even beyond.

Abstract

Detailed γ-ray spectroscopy of the exotic neon isotope $^{28}$Ne has been performed for the first time using the one-neutron removal reaction from $^{29}$Ne on a liquid hydrogen target at 240 MeV/nucleon. Based on an analysis of parallel momentum distributions, a level scheme with spin-parity assignments has been constructed for $^{28}$Ne and the negative-parity states are identified for the first time. The measured partial cross sections and momentum distributions reveal a significant intruder p-wave strength providing evidence of the breakdown of the N=20 and N=28 shell gaps. Only a weak, possible f-wave strength was observed to bound final states. Large-scale shell-model calculations with different effective interactions do not reproduce the large p-wave and small f-wave strength observed experimentally, indicating an ongoing challenge for a complete theoretical description of the transition into the island of inversion along the Ne isotopic chain.

Abstract

Unbound states in C17 were investigated via one-neutron removal from a C18 beam at an energy of 245 MeV/nucleon on a carbon target. The energy spectrum of C17, above the single-neutron decay threshold, was reconstructed using invariant mass spectroscopy from the measured momenta of the C16 fragment and neutron, and was found to exhibit resonances at Er=0.52(2), 0.77(2), 1.36(1), 1.91(1), 2.22(3) and 3.20(1) MeV. The resonance at Er=0.77(2) MeV [Ex=1.51(3) MeV] was provisionally assigned as the second 5/2+ state. The two resonances at Er=1.91(1) and 3.20(1) MeV [Ex=2.65(2) and 3.94(2) MeV] were identified, through comparison of the energies, cross sections and momentum distributions with shell-model and eikonal reaction calculations, as p-shell hole states with spin-parities 1/21− and 3/21−, respectively. A detailed comparison was made with the results obtained using a range of shell-model interactions. The YSOX shell-model Hamiltonian, the cross-shell part of which is based on the monopole-based universal interaction, was found to provide a very good description of the present results and those for the neighbouring odd-A carbon isotopes – in particular for the negative parity cross-shell states.

Abstract

States in the N = 35 and 37 isotopes Ca-55,Ca-57 have been populated by direct proton-induced nucleon removal reactions from Sc-56,Sc-58 and Ca-56 beams at the RIBF. In addition, the (p, 2p) quasi-free single-proton removal reaction from Ca-56 was studied. Excited states in K-55, Ca-55, and Ca-57 were established for the first time via in-beam gamma-ray spectroscopy. Results for the proton and neutron removal reactions from Ca-56 to states in K-55 and Ca-55 for the level energies, excited state lifetimes, and exclusive cross sections agree well with state-of-the-art theoretical calculations using different approaches. The observation of a short-lived state in Ca-57 suggests a transition in the calcium isotopic chain from single-particle dominated states at N = 35 to collective excitations at N = 37. 8. Three-body breakup of He and its halo structure

Abstract

The Borromean halo nucleus $^{6}$He has been studied by a kinematically complete measurement of Coulomb and nuclear breakup into α + 2n on Pb and C targets at 70 MeV/nucleon. Fully quantum-mechanical four-body breakup calculations reproduce the energy and angular differential cross sections below Erel∼1 MeV for both targets. The model used here reproduces the $^{6}$He ground-state properties as well as α-n and n-n scattering data and predicts an average opening angle 〈θnn〉 of 68$^{∘}$ between the two halo neutrons. However, the model underestimates the breakup cross sections for higher Erel, indicating a possible contribution from the inelastic breakup. Alternatively, we examine the empirically modified calculations that reproduce the energy-differential cross sections for a wide range of scattering angles for both targets. The extracted B(E1) peaks at Erel∼1.4 MeV and amounts to 1.6(2) e$^{2}$fm$^{2}$ for Erel ≤ 20 MeV, resulting 〈θnn〉 = 56−10+9 degrees. In either interpretation, the current results show evidence of the dineutron spatial correlation in $^{6}$He.

Abstract

The nuclear structure of $^{51}$Ar, an uncharted territory so far, was studied by the (p,2p) reaction using γ-ray spectroscopy for the bound states and the invariant mass method for the unbound states. Two peaks were detected in the γ-ray spectrum and six peaks were observed in the $^{50}$Ar+n relative energy spectrum. Comparing the results to our shell-model calculations, two bound and six unbound states were established. Three of the unbound states could only be placed tentatively due to the low number of counts in the relative energy spectrum of events associated with the decay through the first excited state of $^{50}$Ar. The low cross sections populating the two bound states of $^{51}$Ar could be interpreted as a clear signature for the presence of significant subshell closures at neutron numbers 32 and 34 in argon isotopes. It was also revealed that due to the two valence holes, unbound collective states coexist with individual-particle states in $^{51}$Ar.

Abstract

We report on the first γ-ray spectroscopy of $^{51,53}$K produced via the $^{52,54}$Ca(p,2p) reactions at ∼250 MeV/nucleon. Unambiguous final-state angular-momentum assignments were achieved for beam intensities down to few particles per second by using a new technique based on reaction vertex tracking combined with a thick liquid-hydrogen target. Through γ-ray spectroscopy and exclusive parallel momentum distribution analysis, 3/2$^{+}$ ground states and 1/2$^{+}$ first excited states in $^{51,53}$K were established quantifying the natural ordering of the 1d3/2 and 2s1/2 proton-hole states that are restored at N = 32 and 34. State-of-the-art ab initio calculations and shell-model calculations with improved phenomenological effective interactions reproduce the present data and predict consistently the increase of the E(1/21+) - E(3/21+) energy differences towards N = 40.

Abstract

Excited states in the N = 40 isotone Ti-62 were populated via the V-63( p, 2p)Ti-62 reaction at similar to 200MeV/nucleonat the Radioactive Isotope Beam Factory and studied using gamma-ray spectroscopy. The energies of the 2(1)(+) -> 0(gs)(+) and 4(1)(+) -> 2(1)(+) transitions, observed here for the first time, indicate a deformed Ti-62 ground state. These energies are increased compared to the neighboring Cr-64 and Fe-66 isotones, suggesting a small decrease of quadrupole collectivity. The present measurement is well reproduced by large-scale shell-model calculations based on effective interactions, while ab initio and beyond mean-field calculations do not yet reproduce our findings. The shell-model calculations for Ti-62 show a dominant configuration with four neutrons excited across the N = 40 gap. Likewise, they indicate that the N = 40 island of inversion extends down to Z = 20, disfavoring a possible doubly magic character of the elusive Ca-60. (C) 2019 The Authors. Abstract

Abstract

We present an investigation of the structure of $^{13}$Be obtained via a kinematically complete measurement of the (p,pn) reaction in inverse kinematics at 265 MeV/nucleon. The relative energy spectrum of $^{13}$Be is compared to Transfer-to-the-Continuum calculations which use as structure inputs the overlaps of the $^{14}$Be ground-state wave function, computed in a three-body model, with the unbound states of the $^{13}$Be residual nucleus. The key role of neutron p-wave orbital in the interpretation of the low-relative-energy part of the spectrum is discussed.

Abstract

Excited states of 77Cu have been investigated by use of single proton knock-out reaction at the Radioactive Isotope Beam Factory in RIKEN in order to reveal the main components of the proton single-particle states. Three excited states were observed at 271, 902 and 2068 keV in 77Cu. The lowest-energy excited state follows the trend predicted for the crossing of the 3/21− and 5/21− states. Comparing the excitation energies of the 3/2−, 5/2− and 7/2− levels from 69Cu to 77Cu one can see that the Z=28 shell gap between the p3/2 and f7/2 states is rather stable, while the f5/2−f7/2 spin–orbit splitting decreases by ∼1.5 MeV in agreement with shell model calculations using the tensor force.

Abstract

Two-proton relative momentum (qpp) and opening angle (θpp) distributions from the three-body decay of two excited proton-rich nuclei, namely Al23→p+p+Na21 and Mg22→p+p+Ne20, have been measured with the projectile fragment separator (RIPS) at the RIKEN RI Beam Factory. An evident peak at qpp∼20 MeV/c as well as a peak in θpp around 30° are seen in the two-proton break-up channel from a highly-excited 22Mg. In contrast, such peaks are absent for the 23Al case. It is concluded that the two-proton emission mechanism of excited 22Mg is quite different from the 23Al case, with the former having a favorable diproton emission component at a highly excited state and the latter dominated by the sequential decay process.

Abstract

One-neutron knockout from 24 O leading to the first excited state in 23 O has been measured for a proton target at a beam energy of 62 MeV/nucleon. The decay energy spectrum of the neutron unbound state of 23 O was reconstructed from the measured four momenta of the 22 O fragment and emitted neutron. A sharp peak was found at Edecay=50±3 keV , corresponding to an excited state in 23 O at 2.78±0.11 MeV , as observed in previous measurements. The longitudinal momentum distribution for this state was consistent with d -wave neutron knockout, providing support for a Jπ assignment of 5/2+ . The associated spectroscopic factor was deduced to be C2S(0d5/2)=4.1±0.4 by comparing the measured cross section ( σ−1nexp=61±6 mb ) with a distorted wave impulse approximation calculation. Such a large occupancy for the neutron 0d5/2 orbital is in line with the N=16 shell closure in 24 O.

Abstract

With the common goal of more accurately and consistently quantifying ambient concentrations of free metal ions and natural organic ligands in aquatic ecosystems, researchers from 15 laboratories that routinely analyze trace metal speciation participated in an intercomparison of statistical methods used to model their most common type of experimental dataset, the complexometric titration. All were asked to apply statistical techniques that they were familiar with to model synthetic titration data that are typical of those obtained by applying state-of-the-art electrochemical methods - anodic stripping voltammetry (ASV) and competitive ligand equilibration-adsorptive cathodic stripping voltammetry (CLE-ACSV) - to the analysis of natural waters. Herein, we compare their estimates for parameters describing the natural ligands, examine the accuracy of inferred ambient free metal ion concentrations ([Mf]), and evaluate the influence of the various methods and assumptions used on these results.The ASV-type titrations were designed to test each participant's ability to correctly describe the natural ligands present in a sample when provided with data free of measurement error, i.e., random noise. For the three virtual samples containing just one natural ligand, all participants were able to correctly identify the number of ligand classes present and accurately estimate their parameters. For the four samples containing two or three ligand classes, a few participants detected too few or too many classes and consequently reported inaccurate 'measurements' of ambient [Mf]. Since the problematic results arose from human error rather than any specific method of analyzing the data, we recommend that analysts should make a practice of using one's parameter estimates to generate simulated (back-calculated) titration curves for comparison to the original data. Abstract

The neutron-rich carbon isotopes 19,17C have been investigated via proton inelastic scattering on a liquid hydrogen target at 70 MeV/nucleon. The invariant mass method in inverse kinematics was employed to reconstruct the energy spectrum, in which fast neutrons and charged fragments were detected in coincidence using a neutron hodoscope and a dipole magnet system. A peak has been observed with an excitation energy of 1.46(10) MeV in 19C, while three peaks with energies of 2.20(3), 3.05(3), and 6.13(9) MeV have been observed in 17C. Deduced cross sections are compared with microscopic DWBA calculations based on p-sd shell model wave functions and modern nucleon–nucleus optical potentials. Jπ assignments are made for the four observed states as well as the ground states of both nuclei.

Abstract

Background: Tissue plasminogen activator (tPA) is an effective treatment for acute ischemic stroke. Although initial improvement is observed when administered for branch atheromatous disease (BAD), some cases subsequently worsen. Clinical data on the characteristics of these patients is lacking, and the benefits of tPA are unclear., Objective: To analyze rebound cases and elucidate the clinical characteristics and outcomes associated with tPA administration in BAD., Methods: This multicenter retrospective study was conducted in Japan. Worsening after initial improvement of a condition is termed as rebound, and such cases were compared with other types of ischemic stroke in patients with and without rebound. The characteristics of patients with BAD who rebounded were examined., Results: The study included 93 patients. Among the patients who were administered tPA, the NIHSS scores at 24 h and 7 days post-tPA were significantly higher in patients with BAD than in patients with other types of infarcts. The group with BAD exhibited a significantly higher rate of rebound than other groups (37.5 % vs. 0 %, P < 0.001). However, no differences were observed in outcomes between patients who experienced rebound after tPA administration and those who did not., Conclusions: Reevaluation and changing the strategy of tPA use in patients with BAD may be necessary. Abstract

Breast cancer (BC) is classified into four major histological subtypes, namely luminal A, luminal B, HER2, and basal-like, and its treatment is based on these subtypes. The use of immune checkpoint inhibitors against BC depends on the expression of PD-1/PD-L1. Another tumor immune system-the cGAS-STING pathway-is a potential target for cancer immunotherapy. However, the status of the cGAS-STING pathway in BC has not been fully established. Therefore, we investigated the expression status of the cGAS-STING pathway and immune-related proteins in BC. We classified 111 BCs into six groups-29 hormone receptor-positive carcinomas, 12 HER2+ carcinomas (HER2), 8 luminal-HER2 carcinomas, 26 triple-negative breast carcinomas (TNBCs), 21 lobular carcinomas (LC), and 15 carcinomas with apocrine differentiation (CAD)-and investigated the relationship between BC and tumor immunity via the cGAS-STING pathway using histopathological and immunohistochemical methods. Expression of cGAS was high in CADs (100%) and low in TNBCs (35%); STING-positive lymphocytes were high in TNBC (85%, P = 0.0054). Expression of pSTAT3 was significantly high in patients with TNBC (≥10%, 88%). The proportion of PD-L1-positive tumor cells was higher in TNBCs (54%) than in other BCs (30%). SRGN expression was significantly higher in the TNBC group than in the other BC groups (58%). Tumor immune responses may differ among tumor subtypes. The cGAS-STING pathway may be functional in TNBC and CAD but not in LC. 35. HTRA1-related cerebral small-vessel disease causes cerebral microbleeds on the brainstem surface.

Abstract

Background and Objectives: Cerebral autosomal recessive arteriopathy with subcortical infarcts and leukoencephalopathy (CARASIL) has recently been known as HTRA1-related cerebral small-vessel disease (CSVD), it is caused by variants in HTRA1. Recently, it has been reported to develop in heterozygotes with some variants of the gene. Multiple prospective studies have reported that the frequency of heterozygous HTRA1 variants developing CSVD is 2 - 6.5 % in CARASIL. Heterozygous variant cases lack unique clinical features, have an older age of onset, and are difficult to detect. Characteristic findings are required to identify such cases., Method: Magnetic resonance imaging (MRI) images of cases that experienced cerebral infarction and carried heterozygous variants in HTRA1 were reviewed., Results: Four cases of heterozygous HTRA1-related CSVD in two families (Family 1: c.754G > A, p.A252T; three males. Family 2: c.497G > T, p.R166L, one female). In all cases, white matter lesions with lacunar infarcts were observed in the periventricular and basal ganglia, external capsule, and brainstem. Moreover, T2 star weighted image (T2*WI) low presented dot-like lesions were present along the surface of the brainstem, which have only been reported in one homozygous case. Susceptibility-weighted imaging (SWI) was performed in two cases, and the dot-like lesions on T2*WI resembled a pearly tiara along the surface of the brainstem., Conclusion: Brainstem surface on T2*WI low showed dot-like lesions, which are not generally observed in patients with stroke and can be characteristic of HTRA1-CSVD associated with heterozygous variant. Abstract

Background and Aims: In advanced atherosclerotic lesions, macrophage deaths result in necrotic core formation and plaque vulnerability. Cyclophilin D (CypD) is a mitochondria-specific cyclophilin involved in the process of cell death after organ ischemia-reperfusion. However, the role of CypD in atherosclerosis, especially in necrotic core formation, is unknown. Therefore, this experiment aims to clarify the role of CypD in necrotic core formation., Methods: To clarify the specific role of CypD, encoded by Ppif in mice, apolipoprotein-E/CypD-double knockout (Apoe -/- Ppif -/- ) mice were generated. These mice were fed a high-fat diet containing 0.15 % cholesterol for 24 weeks to accelerate atherosclerotic lesion development., Results: Deletion of CypD decreased the necrotic core size, accompanied by a reduction of macrophage apoptosis compared to control Apoe -/- mice. In RAW264.7 cells, siRNA-mediated knockdown of CypD attenuated the release of cytochrome c from the mitochondria to the cytosol induced by endoplasmic reticulum stress inducer thapsigargin. In addition, necroptosis, induced by TNF-α and caspase inhibitor, was attenuated by knockdown of CypD. Ly-6C high inflammatory monocytes in peripheral blood leukocytes and mRNA expression of Il1b in the aorta were decreased by deletion of CypD. In contrast, siRNA-mediated knockdown of CypD did not significantly decrease Il1b nor Ccl2 mRNA expression in RAW264.7 cells treated with LPS and IFN-γ, suggesting that inhibition of inflammation in vivo is likely due to decreased cell death in the atherosclerotic lesions rather than a direct action of CypD deletion on the macrophage., Conclusions: These results indicate that CypD induces macrophage death and mediates necrotic core formation in advanced atherosclerotic lesions. Abstract

Background: Little is known regarding which patients with ischemic cardiomyopathy (ICM) should be considered for prophylactic therapies, such as an implantable cardioverter-defibrillator (ICD), in the primary percutaneous intervention era. The aim of this study was to investigate the influence of non-sustained ventricular tachycardia (NSVT) on major adverse cardiac events (MACE) in heart failure with reduced ejection fraction (HFrEF) patients., Methods: We retrospectively analyzed patients of ICM and non-ICM who underwent ICD implantation at our institute from October 2006 to August 2020. MACE were defined as composite outcome of cardiovascular death, heart failure hospitalization, and appropriate ICD therapies., Results: A total of 167 patients were enrolled [male, 138 (83 %); age, 62.1 ± 11.7 years; left ventricular ejection fraction, 23.5 ± 6.1 %; left ventricular diastolic diameter, 67.4 ± 9.0 mm; atrial fibrillation, 47 (28 %); NSVT, 124 (74 %); use of class III antiarrhythmic drugs, 55 (33 %); ischemic cardiomyopathy, 56 (34 %); cardiac resynchronization therapy, 73 (44 %)]. The median follow-up duration was 61 months. MACE occurred with 71 patients (43 %). When comparing baseline characteristics of the patients, left ventricular ejection fraction (p = 0.02) and atrial fibrillation (p = 0.04) were significantly associated with MACE. The multivariable Cox analysis for the target variable MACE identified atrial fibrillation (hazard ratio 2.00; 95 % confidence index 1.18-3.37; p = 0.01) as an independent predictor for MACE., Conclusions: Prior NSVT before ICD implantation was not an independent predictor of future MACE in patients with HFrEF with primary prophylactic ICD. In contrast, atrial fibrillation was associated with worse prognosis. Abstract

Introduction: Patients with combined burns and trauma are often seen in the United States. The combination of trauma with burns increases mortality. In contrast, the characteristics and outcomes of these cases remain unknown in Japan. This study investigated the characteristics and outcomes of trauma associated with burns in Japan., Methods: This multicenter retrospective cohort study was conducted by utilizing data from the Japan Trauma Data Bank for the period between 2004 and 2017. We evaluated the characteristics of burn patients (n = 5783) divided into two groups: burns only (n = 5537) and combined burns and trauma (n = 246). Clinical characteristics, including patient background, severity of trauma, injury mechanism, total body surface area affected, injury location, treatments, and clinical outcomes, were examined., Results: Most patients in both the groups were injured by flames. The number proportion of patients with 40-89% of the total body surface area affected was 1069/5537 (19.3%) in the burn-only group and 23/246 (9.3%) in the combined burn and trauma group. The in-hospital mortality was 1006/5537 (18.2%) in the burn-only group and 17/246 (6.9%) in the combined burn and trauma group., Conclusions: We demonstrated the characteristics of Japanese patients with burns only compared with those with combined burns and trauma. Abstract

Background: Clot waveform analysis (CWA) reportedly enhances the interpretation of clotting time measurement. This study aimed to compare CWA between prothrombin time (PT) and activated partial thromboplastin time (APTT) assays for better understanding how to apply CWA for assessing effects of direct oral anticoagulants (DOACs)., Methods: Samples were prepared by spiking plasma with rivaroxaban, apixaban, edoxaban, or dabigatran. To compensate the influence of fibrinogen, CWA parameters were adjusted by unifying maximum changes in transmittance in clotting reaction curves detected by the optical system., Results: Non-adjusted PT-CWA parameters unexpectedly rose at low drug concentrations but declined at high drug concentrations while adjusted PT-CWA parameters exhibited dose-dependent decrease. Both non-adjusted and adjusted APTT-CWA parameters showed dose-dependent decrease. Adjusted CWA parameters were applicable to Hill plot analysis. All DOACs exhibited Hill coefficients indicating positively cooperative effects regarding most adjusted PT-CWA parameters. Regarding adjusted APTT-CWA parameters, rivaroxaban, apixaban, and edoxaban exhibited Hill coefficients indicating no or negatively cooperative effects. The observed differences between PT-CWA and APTT-CWA suggested the implication of thrombin positive feedback in DOAC effects., Conclusion: The results revealed distinct features of DOAC effects in extrinsic and intrinsic pathways. Abstract

Niemann-Pick disease type C (NPC) is a lysosomal lipid storage disorder characterized by progressive neurodegeneration and hepatic dysfunction. A cyclic heptasaccharide, 2-hydroxypropyl-β-cyclodextrin (HP-β-CD), is currently under clinical investigation for NPC, but its adverse events remain problematic. We previously identified that a cyclic octasaccharide, 2-hydroxypropyl-γ-cyclodextrin (HP-γ-CD), also ameliorated NPC manifestations with higher biocompatibility than HP-β-CD. However, preclinical studies describing the associations between the biodistribution and pharmacodynamics of these compounds, which are essential for clinical application, are still lacking. Here, we investigated these properties of HP-γ-CD by measuring its organ biodistribution and therapeutic effect after systemic and central administration. The effect of HP-γ-CD on disturbed cholesterol homeostasis appeared within several hours after exposure and persisted for several days in NPC model cells and mice. Tissue distribution indicated that only a small fraction of subcutaneously administered HP-γ-CD rapidly distributed to peripheral organs and contributed to disease amelioration. We found that a subcutaneous dose of HP-γ-CD negligibly ameliorated neurological characteristics because it has limited penetration of the blood-brain barrier; however, an intracerebroventricular microdose unexpectedly attenuated hepatic dysfunction without the detection of HP-γ-CD in the liver. Abstract

Introduction: This study aimed to evaluate the cost-effectiveness of nirmatrelvir/ritonavir (Nir/Rit) for adult outpatients with COVID-19 from the perspective of a Japanese public healthcare payer., Methods: A cost-effectiveness simulation was conducted comparing Nir/Rit for the outpatient treatment of high-risk COVID-19 patients to best supportive care (BSC) without antiviral or antibody drugs. The analytical model was divided into two phases: the treatment phase, lasting 35 days from the start of COVID-19 treatment, and the post-treatment phase. Patients who survived the treatment phase were assumed to follow a general population survival curve. Expected costs and expected quality-adjusted life years (QALYs) for both BSC and Nir/Rit were calculated for ages 40 to 80 to obtain the incremental cost-effectiveness ratio (ICER). The robustness of the results was evaluated through deterministic and probabilistic sensitivity analysis (PSA)., Results: The ICERs for patients aged 40, 50, 60, 70, and 80 were 18,854,276 Japanese Yen (JPY)/QALY, 8,482,034 JPY/QALY, 4,976,612 JPY/QALY, 2,636,096 JPY/QALY, and 1,597,783 JPY/QALY, respectively. In the deterministic sensitivity analysis, both the mortality risk during the treatment phase and the relative mortality risk with Nir/Rit had a high impact on ICER across all ages. Abstract

Aim and Objective: Our recent report showed that soluble T-cadherin promotes pancreatic beta-cell proliferation. However, how and where the secretion of soluble T-cadherin is regulated remain unclear., Methods and Results: Soluble T-cadherin levels significantly increased in leptin receptor-deficient db/db mice with hypoinsulinaemia or in wild-type mice treated with insulin receptor blockade by S961. Similar results were observed in human subjects; Diabetic ketoacidosis patients at the time of hospitalization had increased plasma soluble T-cadherin levels, which decreased after insulin infusion therapy. Patients with recurrent ovarian cancer who were administered a phosphatidylinositol-3 kinase (PI3K)-alpha inhibitor (a new anticancer drug) had increased plasma soluble T-cadherin and plasma C-peptide levels. Endothelial cell-specific T-cadherin knockout mice, but not skeletal muscle- or cardiac muscle-specific T-cadherin knockout mice, showed a 26 % reduction in plasma soluble T-cadherin levels and a significant increase in blood glucose levels in streptozocin-induced diabetes. Abstract

The extensive emissions of black carbon (BC) from the Indo-Gangetic Plain (IGP) region of India have been well recognized. Particularly, biomass emissions from month-specific crop-residue burning (April, May, October, November) and heating activities (December-February) are considered substantial contributors to BC emissions in the IGP. However, their precise contribution to ambient BC aerosol has not been quantified yet and remains an issue of debate. Therefore, this study aims to fill this gap by quantifying the contribution of these month-specific biomass emissions to ambient BC at an urban site in IGP. This study presents the analysis of BC mass concentrations (M BC ) measured for 3 years (2020-2022) in Delhi using an optical photometer i.e., continuous soot monitoring system (COSMOS). A statistical analysis of monthly mean M BC and factors affecting the M BC (ventilation coefficients, air mass back trajectories, fire counts) is performed to derive month-wise contribution due to background concentration, conventional emission, regional transport, crop-residue burning, and heating activities. The yearly mean M BC (5.3 ± 4.7, 5.6 ± 5.0, and 5.3 ± 3.5 μg m -3 during 2020, 2021, and 2022, respectively) remained relatively consistent with repetitive monthly patterns in each year. The peak concentrations were observed from November to January and low concentrations from June to September. Anthropogenic activities contributed significantly to M BC over Delhi with background concentration contributing only 30 % of observed M BC. The percentage contribution of emissions from crop-residue burning varied from 15 % (May) to 37 % (November), while the contribution from heating activities ranged from 25 % (December) to 39 % (January). Abstract

Background: Adult-onset leukoencephalopathy with axonal spheroids and pigmented glia (ALSP) is a disease responsible for cognitive impairment in adult humans. It is caused by mutations in the colony stimulating factor 1 receptor gene (CSF1R) or alanyl-transfer (t) RNA synthetase 2 (AARS2) gene and affects brain white matter. Settlement of stages of the pathological brain lesions (Oyanagi et al. 2017) from the findings of brain imaging will be inevitably essential for prognostication., Methods: MRI images of eight patients with ALSP were analyzed semiquantitatively. White matter degeneration was assessed on a scale of 0 to 4 (none, patchy, large patchy, confluent, and diffuse) at six anatomical points, and brain atrophy on a scale 0 to 4 (none, slight, mild, moderate, and severe) in four anatomical areas. The scores of the two assessments were then summed to give total MRI scores of 0-40 points. Based on the scores, the MRI features were classified as Grades (0-4). Regression analysis was applied to mutual association between mRS, white matter degeneration score, brain atrophy score, the total MRI score and disease duration., Results: White matter degeneration score, brain atrophy score, and the total MRI score were significantly correlated with the disease duration. 46. Mechanical insights into jawbone characteristics under chronic kidney disease: A comprehensive nanoindentation approach.

Abstract

The mechanical properties of the jawbone play a critical role in determining the successful integration of dental prostheses. Chronic kidney disease (CKD) has been identified to abnormally accelerate bone turnover rates. However, the impact of CKD on the mechanical characteristics of the jawbone has not been extensively studied. This study sought to evaluate the time-dependent viscoelastic behaviors of rat jawbones, particularly in the scenarios both with and without CKD. We hypothesized that CKD might compromise the bone's innate toughening mechanisms, potentially owing to the time-dependent viscoelasticity of the bone matrix proteins. The maxillary and mandibular bones of Wistar rats were subjected to nanoindentation and Raman micro-spectroscopy. Load-hold-displacement curves from the cortical regions were obtained via nanoindentation and were mathematically characterized using a suitable viscoelastic constitutive model. Raman micro-spectroscopy was employed to identify nuanced vibrational changes in local molecular structures induced by CKD. The time course of indenter penetration onto cortical bones during the holding stage (creep behavior) can be mathematically represented by a series arrangement of the Kelvin-Voigt bodies. This configuration dictates the overall viscoelastic response observed during nanoindentation tests. The CKD model exhibited a reduced extent of viscoelastic contributions, especially during the initial ramp loading phase in both the maxillary and mandibular cortical bones. The generalized Kelvin-Voigt model comprises 2 K-Voigt elements that signify an immediate short retardation time (τ 1 ) and a subsequent prolonged retardation time (τ 2 ), respectively. Notably, the mandibular CKD model led to an increase in the delayed τ 2 alongside an increase in non-enzymatic collagen cross-linking. Background: Percutaneous left atrial appendage closure (LAAC) has increased for those who need alternative to long-term anticoagulation with non-valvular atrial fibrillation (NVAF)., Methods and Results: From September 2019, after initiating WATCHMAN (Boston Scientific, Maple Grove, MN, USA) device implantation, we established Transcatheter Modification of Left Atrial Appendage by Obliteration with Device in Patients from the NVAF (TERMINATOR) registry. Utilizing 729 patients' data until January 2022, we analyzed percutaneous LAAC data regarding this real-world multicenter prospective registry. A total of 729 patients were enrolled. Average age was 74.9 years and 28.5 % were female. Paroxysmal AF was 37.9 % with average CHADS 2 3.2, CHA 2 DS 2 -VASc 4.7, and HAS-BLED score of 3.4. WATCHMAN implantation was successful in 99.0 %. All-cause deaths were 3.2 %, and 1.2 % cardiovascular or unexplained deaths occurred during follow-up [median 222, interquartile range (IQR: 93-464) days]. Stroke occurred in 2.2 %, and the composite endpoint which included cardiovascular or unexplained death, stroke, and systemic embolism were counted as 3.4 % [median 221, (IQR: 93-464) days]. Major bleeding defined as BARC type 3 or 5 was seen in 3.7 %, and there was 8.6 % of all bleeding events in total [median 219, (IQR: 93-464) days]., Conclusions: These preliminary data demonstrated percutaneous LAAC with WATCHMAN device might have a potential to reduce stroke and bleeding events for patients with NVAF. Abstract

CD146/MCAM has garnered significant attention for its potential contribution to cardiovascular disease; however, the transcriptional regulation and functions remain unclear. To explore these processes regarding cardiomyopathy, we employed doxorubicin, a widely used stressor for cardiomyocytes. Our in vitro study on H9c2 cardiomyoblasts highlights that, besides impairing the fatty acid uptake in the cells, doxorubicin suppressed the expression of fatty acid binding protein 4 (Fabp4) along with the histone deacetylase 9 (Hdac9), bromodomain and extra-terminal domain proteins (BETs: Brd2 and Brd4), while augmented the production of CD146/MCAM. Silencing and chemical inhibition of Hdac9 further augmented CD146/MCAM and deteriorated fatty acid uptake. In contrast, chemical inhibition of BETs as well as silencing of MCAM/CD146 ameliorated fatty acid uptake. Moreover, protein kinase C (PKC) inhibition abrogated CD146/MCAM, particularly in the nucleus. Taken together, our results suggest that epigenetic dysregulation of Hdac9, Brd2, and Brd4 alters CD146/MCAM expression, deteriorating fatty acid uptake by downregulating Fabp4. This process depends on the PKC-mediated nuclear translocation of CD146. Abstract

Acetaminophen (APAP) overdoses can cause severe liver injury. In this study, the protective effect of fasudil against APAP-induced liver injury was investigated. APAP (400 mg/kg) was administered to male C57BL/6J mice to induce liver injury, and fasudil (20 or 40 mg/kg) was injected 30 min before APAP administration. Fasudil markedly suppressed APAP-induced elevation in serum transaminase activity and hepatic necrosis and significantly reduced an increase in nitrotyrosine and DNA fragmentation. However, fasudil did not affect cytochrome P450 2E1 expression, N-acetyl-p-benzoquinone imine production or c-jun N-terminal kinase activation. In contrast, fasudil significantly inhibited an APAP-induced increase in expression of the transcription factor C/EBP homologous protein (CHOP) in the liver, accompanied by transcriptional suppression of ER stress-related molecules such as Ero1α, Atf4 and Grp78. These findings indicate that suppression of CHOP expression by fasudil exhibits a remarkable protective effect against APAP liver injury by regulating ER stress. 50. Is central repair with coronary artery bypass grafting enough for multi-organ malperfusion in acute type A aortic dissection with coronary malperfusion?

Abstract

Objectives: Acute type A aortic dissection with coronary malperfusion syndrome is rare but associated with high mortality. Multi-organ malperfusion is an independent predictor of acute type A aortic dissection. Coronary malperfusion requires treatment, but it is not feasible to treat all malperfusions. The adequacy of "central repair and coronary artery bypass grafting" for patients with coronary and other organ malperfusion is unknown., Methods: Of the 299 patients who underwent surgery between 2008 and 2018, 21 patients with coronary malperfusion, who received cental repair with coronary artery graft bypass, were analyzed retrospectively. They were divided; into Group M (n = 13, coronary and other organ malperfusion) and Group O (n = 8, coronary malperfusion only). The patient background, surgical content, details of malperfusion, surgical mortality and morbidity, and long-term outcome were compared., Results: There was no difference in operation time (205 ± 30 vs. 266 ± 88, p = 0.49), but the time from arrival to circulatory arrest tended to be shorter in Group M (81 vs. 134, p = 0.05). Among Group M, cerebral malperfusion was the most common at 92%. Two of the three cases with mesenteric malperfusion died. The mortality of Group M and Group O was 13% and 15% (P = 0.85), respectively.
