Abstract

Pediatric oligodendroglioma is a low-grade glioma that remains relatively rare when compared to adults. Retrospective studies have clearly defined the most common presenting symptoms and tumor locations. Delays in diagnosis are likely to occur given the nature of pediatric oligodendroglioma. Prospective studies are required to better understand the “time-clock” with respect to progression of symptoms requiring further intervention.

Abstract

Choroid plexus (CP) carcinoma is a rare intraventricular brain tumor derived from the CP epithelium and occurs mainly in young children. CP carcinoma is a World Health Organization (WHO) grade III neoplasm. Despite multidisciplinary treatment approaches, including surgical resection, radiotherapy, and adjuvant chemotherapy, the outcome remains guarded and many of the survivors exhibit significant neurologic sequelae of both the tumor and its treatment. TP53 mutations are common in CP carcinomas and have been associated with poorer outcome. Recent studies have highlighted the potential of genome-wide methylation profiling, gene expression profiling, and p53 mutation status to provide additional layers of information to improve risk stratification in CP tumors. In this chapter, we discuss the current management of CP carcinoma and future therapeutic potentials in the era of new molecular discoveries in this rare tumor. Further studies are needed to standardize the treatment paradigm for this neoplasm.

Abstract

Marrow-ablative chemotherapy with autologous hematopoietic progenitor cell rescue has been evaluated in the treatment of children, adolescents, and young adults with brain tumors, in whom conventional therapy is either considered too toxic (e.g., radiotherapy in infants) or has proven ineffective (e.g., recurrent or progressive malignant tumors). The success of this approach depends on tumor type and histology, extent of disease and surgical resection, and response to prior chemotherapy. In this chapter, we review results of marrow-ablative chemotherapy with autologous hematopoietic progenitor cell rescue as well as allogeneic transplantation in brain tumors of different histologies.

Abstract

Marrow ablative chemotherapy with hematopoietic stem cell rescue (also referred to as hematopoietic stem cell transplantation) is a therapeutic strategy used to treat children and young adults with brain tumors whose conventional therapy is either too toxic (e.g., radiotherapy in infants) or ineffective (e.g., rhabdoid tumors or progressive malignant tumors). In this strategy, high-dose chemotherapy (typically including alkylating agents and/or other radiomimetic drugs) is given to patients after initial conventional therapy, including surgery and irradiation, if possible, and conventional-dose chemotherapy. The success of marrow ablative chemotherapy with hematopoietic stem cell rescue depends on several variables, including the histological type of tumor, extent of disease and of surgical resection, and response to prior chemotherapy. In this chapter, we shall review results of marrow ablative chemotherapy with hematopoietic stem cell rescue in brain tumors of differing histologies.
