1. Preparing for the Worst: Management and Predictive Factors of Open Conversion During Minimally Invasive Renal Tumor Surgery (UroCCR-135 Study)

Abstract: Background and objective: Data regarding open conversion (OC) during minimally invasive surgery (MIS) for renal tumors are reported from big databases, without precise description of the reason and management of OC. The objective of this study was to describe the rate, reasons, and perioperative outcomes of OC in a cohort of patients who underwent MIS for renal tumor initially. The secondary objective was to find the factors associated with OC. Methods: Between 2008 and 2022, of the 8566 patients included in the UroCCR project prospective database (NCT03293563), who underwent laparoscopic or robot-assisted minimally invasive partial (MIPN) or radical (MIRN) nephrectomy, 163 experienced OC. Each center was contacted to enlighten the context of OC: “emergency OC” implied an immediate life-threatening situation not reasonably manageable with MIS, otherwise “elective OC”. To evaluate the predictive factors of OC, a 2:1 paired cohort on the UroCCR database was used. Key findings and limitations: The incidence rate of OC was 1.9% for all cases of MIS, 2.9% for MIRN, and 1.4% for MIPN. OC procedures were mostly elective (82.2%). The main reason for OC was a failure to progress due to anatomical difficulties (42.9%). Five patients (3.1%) died within 90 d after surgery. Increased body mass index (BMI; odds ratio [OR]: 1.05, 95% confidence interval [CI]: 1.01–1.09, p = 0.009) and cT stage (OR: 2.22, 95% CI: 1.24–4.25, p = 0.008) were independent predictive factors of OC. Conclusions and clinical implications: In MIS for renal tumors, OC was a rare event (1.9%), caused by various situations, leading to impaired perioperative outcomes. Emergency OC occurred once every 300 procedures. Increased BMI and cT stage were independent predictive factors of OC. Patient summary: The incidence rate of open conversion (OC) in minimally invasive surgery for renal tumors is low. Only 20% of OC procedures occur in case of emergency, and others are caused by various situations. Increased body mass index and cT stage were independent predictive factors of OC.

Abstract: Background: French health authorities recommend the testing of HbA1c every 3 to 6 months in patients with diabetes. The successive containments linked to the SARSCoV-2 pandemic may have had a profound impact on the follow-up of diabetic patients in primary care. The objective of this study was to investigate the effect of containments on the volume of HbA1c assays and on HbA1c values of type 2 diabetic patients monitored in primary care in Northern France compared to usual seasonal variations. Methods: Epidemiological study of a multicentre retrospective cohort, collecting the number and the value of HbA1c assays from type 2 diabetic patients, carried out in 73 private medical analysis laboratories in Northern France region between October 1, 2018 and October 31, 2022. Results: A total of 196,744 patients and 828,037 tests were included. The first and third containments were associated with a significant decrease in the mean number of performed HbA1c tests, the decrease in the 2nd containment did not reach significance. We observed a significant decrease in HbA1c testing during the winter holiday season and the summer months. The variations in HbA1c values were not substantial compared to seasonal variations.

4. The effects of perceived therapist guidance and advice on adherence to home-based exercise programs in mothers of children with cerebral palsy in Rwanda

Abstract: Background: The alarming prevalence of non- or poor adherence to Home-based Exercise Programs (34–79.2 %, HEP) in parents of Children with Cerebral Palsy (C-CP) is a global health concern due to its detrimental effect on treatment outcomes. Objective: To examine whether Perceived Therapist Guidance and Advice (TGA) or social support moderate the effects of PTSD, depression, parenting stress, low self-efficacy, the burden of care, or the effects of perceived family stigma on adherence to HEP. Method: A stratified random sample of 301 mothers of C-CP attending the largest seven physical rehabilitation hospitals or centres in Rwanda participated in this study. Data were collected using validated measures of all investigated constructs. We conducted hierarchical multiple linear regressions, and the nature of moderation was scrutinized using the process macro 4.1 model number 1 within SPSS version 28. Results: The prevalence of adherence to HEP in the present sample was 32.9 %. Mothers' TGA and social support were positively associated with adherence to HEP; whereas mothers' stressors, including PTSD, parenting stress, depression symptoms, low self-efficacy, burden of care, and family stigma were negatively associated with adherence to HEP. In addition, TGA significantly moderated the associations of the different mothers' stressors with adherence to HEP, leading to increased HEP adherence for mothers with high stressor levels. Social support did not moderate any of these associations. Our respondents often reported poor family and friends’ social support due to family stigma-related to caring C-CP. Conclusions: Therapists play a vital role by providing education, support, and counselling to parents, emphasizing the importance of adherence, and addressing any stigma-related concerns, especially for mothers who experience high levels of stress. Therapists should be aware that mothers of C-CP frequently relied solely on their support to improve adherence to HEP.

Abstract: Summary: Background: There are limited data on the effectiveness of differentiated service delivery (DSD) for HIV care during sociopolitical turmoil. We assessed outcomes with a DSD model of care that includes patient choice between community-based antiretroviral therapy (ART) centres, home-based ART dispensing, or facility-based care at GHESKIO clinic during a period of severe civil unrest in Port-au-Prince, Haiti. Methods: This retrospective analysis included data on patients with at least one HIV visit at GHESKIO between May 1, 2019, and December 31, 2021. Multivariable logistic regression models were used to assess predictors of attending ≥1 community visit during the study period, and failure to attend timely visits. HIV-1 RNA test results were reported among patients who had been ART for ≥3 months at last visit. Findings: Of the 18,625 patients included in the analysis, 9659 (51.9%) attended at least one community visit. The proportion of community visits ranged from 0.3% (2019) to 44.1% (2021). Abstract: With more than 130 clinical trials and 8 approved gene therapy products, adeno-associated virus (AAV) stands as one of the most popular vehicles to deliver therapeutic DNA in vivo. One critical quality attribute analyzed in AAV batches is the presence of residual DNA, as it could pose genotoxic risks or induce immune responses. Surprisingly, the presence of small cell-derived RNAs, such as microRNAs (miRNAs), has not been investigated previously. In this study, we examined the presence of miRNAs in purified AAV batches produced in mammalian or in insect cells. Our findings revealed that miRNAs were present in all batches, regardless of the production cell line or capsid serotype (2 and 8). Quantitative assays indicated that miRNAs were co-purified with the recombinant AAV particles in a proportion correlated with their abundance in the production cells. The level of residual miRNAs was reduced via an immunoaffinity chromatography purification process including a tangential flow filtration step or by RNase treatment, suggesting that most miRNA contaminants are likely non-encapsidated. In summary, we demonstrate, for the first time, that miRNAs are co-purified with AAV particles. Further investigations are required to determine whether these miRNAs could interfere with the safety or efficacy of AAV-mediated gene therapy.

Abstract: Summary: Super-resolution imaging provides unprecedented visualization of sub-cellular structures, but the two main techniques used, single-molecule localization microscopy (SMLM) and stimulated emission depletion (STED), are not easily reconciled. We present a protocol to super-impose nanoscale protein distribution reconstructed with SMLM to sub-cellular morphology obtained in STED. We describe steps for tracking cells on etched coverslips and registering images from two different microscopes with 30-nm accuracy. 8. Lithium and endocrine disruption: A concern for human health?

Abstract: Lithium is a key medication for the treatment of psychiatric disorders and is also used in various industrial applications (including battery production and recycling). Here, we review published data on the endocrine-disrupting potential of lithium, with a particular focus on the thyroid hormone system. To this end, we used PubMed and Scopus databases to search for, select and review primary research addressing human and animal health endpoints during or after lithium exposure at non-teratogenic doses. Given the key role of thyroid hormones in neurodevelopmental processes, we focused at studies of the neural effects of developmental exposure to lithium in humans and animals. Our results show that lithium meets the World Health Organization’s definition of a thyroid hormone system disruptor – particularly when used at therapeutic doses. When combined with knowledge of adverse outcome pathways linking molecular initiating events targeting thyroid function and neurodevelopmental outcomes, the neurodevelopmental data reported in animal experiments prompt us to suggest that lithium influences neurodevelopment. However, we cannot rule out the involvement of additional modes of action (i.e. unrelated to the thyroid hormone system) in the described neural effects.Given the increasing use of lithium salts in new technologies, attention must be paid to this emerging pollutant – particularly with regard to its potential effects at environmental doses on the thyroid hormone system and potential consequences on the developing nervous system.

Abstract: Summary: Background: It is uncertain which biological features underpin the response of rectal cancer (RC) to radiotherapy. No biomarker is currently in clinical use to select patients for treatment modifications. Methods: We identified two cohorts of patients (total N = 249) with RC treated with neoadjuvant radiotherapy (45Gy/25) plus fluoropyrimidine. This discovery set included 57 cases with pathological complete response (pCR) to chemoradiotherapy (23%). Pre-treatment cancer biopsies were assessed using transcriptome-wide mRNA expression and targeted DNA sequencing for copy number and driver mutations. Biological candidate and machine learning (ML) approaches were used to identify predictors of pCR to radiotherapy independent of tumour stage. Findings were assessed in 107 cases from an independent validation set (GSE87211). Findings: Three gene expression sets showed significant independent associations with pCR: Fibroblast-TGFβ Response Signature (F-TBRS) with radioresistance; and cytotoxic lymphocyte (CL) expression signature and consensus molecular subtype CMS1 with radiosensitivity. These associations were replicated in the validation cohort. In parallel, a gradient boosting machine model comprising the expression of 33 genes generated in the discovery cohort showed high performance in GSE87211 with 90% sensitivity, 86% specificity. Biological and ML signatures indicated similar mechanisms underlying radiation response, and showed better AUC and p-values than published transcriptomic signatures of radiation response in RC. Interpretation: RCs responding completely to chemoradiotherapy (CRT) have biological characteristics of immune response and absence of immune inhibitory TGFβ signalling. These tumours may be identified with a potential biomarker based on a 33 gene expression signature. This could help select patients likely to respond to treatment with a primary radiotherapy approach as for anal cancer. Conversely, those with predicted radioresistance may be candidates for clinical trials evaluating addition of immune-oncology agents and stromal TGFβ signalling inhibition. Abstract: Misdiagnosis of Mycobacterium heraklionense tenosynovitis is common due to the challenging identification and perceived rarity of the disease. This can result in delayed therapy initiation and potentially irreversible consequences. In this report, we present an additional case of hand tenosynovitis, which highlights the diagnostic and management challenges of Mycobacterium heraklionense tenosynovitis and provides further evidence of its emergence as a cause of tenosynovitis. 12. Health economic impact of patients with phenylketonuria (PKU) in France – A nationwide study of health insurance claims data

Abstract: Background: Phenylketonuria (PKU) is an inherited metabolic disease. If left untreated, it can lead to severe irreversible intellectual disability and can cause seizures, behavior disturbance, and white matter disease. This study aimed at evaluating the health economic impact of patients with PKU in France. Methods: This retrospective observational study used health insurance claims data from the French SNDS (Système National des Données de Santé) database, which contains data from over 66 million French inhabitants. Patients with PKU were identified by ICD-10 diagnosis codes E70.0 (PKU) and E70.1 (Other hyperphenylalaninemia) documented as a chronic condition (affection de longue durée – ALD) or in the inpatient setting in the SNDS database between 2006 and 2018. Patients with PKU were matched to controls without PKU by age, sex, and region. Patients with early- and late-diagnosed PKU were defined as patients born after and before the implementation of nationwide newborn screening in France in 1972, respectively. Outcomes were analyzed for the year 2018. Results: Overall, 3549 patients with PKU were identified in the database on January 1st, 2018. Of those, 3158 patients versus 15,703 controls with at least one healthcare consumption in 2018 were available for outcome analyses. Abstract: Objective: This pilot study tested the feasibility of a future efficacy trial examining the effect of CONNECTDROP®, a novel Medication Event Monitoring System (MEMS) paired with a mHealth application, on medication adherence in patients with glaucoma. Design: A single-center, single-arm, prospective interventional pilot study (NCT04552964). Participants: Adults with glaucoma managed with at least a fixed combination of timolol/dorzolamide who are adherent to treatment. Methods: Participants (n = 31) were provided with the MEMS device and a smartphone with the application installed. They were required to use the MEMS with their usual timolol/dorzolamide prescription for 9 weeks. The study endpoint was at the end of week 9, when all study materials were returned, and participants completed a 17-item patient satisfaction questionnaire. Data collected continuously by each MEMS for the 9 weeks were analyzed for their suitability to quantify adherence of the individual participant and characterize adherence trends within the study cohort. Clinical data were collected at baseline, week 8, and week 9 for the safety evaluation. Main Outcome Measures: The primary outcome was global patient satisfaction after 9 weeks. Secondary outcome measures included participant feedback on handling the MEMS and its usability, along with that of the connected application. Objective data were used to determine participant medication adherence. The proportion of participants who successfully changed the MEMS to a new bottle at week 8 was reported. Results: The MEMS-connected device achieved a global satisfaction score of 74.1% from study participants after 9 weeks. Furthermore, 70.4% of participants found the MEMS easy to use. However, only 59.2% reported feedback from the mHealth application useful in reminding them to take their treatment. MEMS-derived data showed that 70.4% of participants achieved an ''adherence score'' of 80% or above after 8 weeks and that 40.7% who completed the study had not changed the bottle correctly. No adverse events (AEs) were reported. Conclusion: In this pilot study, the CONNECTDROP device was able to monitor daily intake of anti-glaucomatous medication over 2 months and had high satisfaction amongst this cohort of patients and was easy to use. The objective adherence data obtained appears reliable but must be validated for use in an efficacy trial. Abstract: Summary: Background: Research in healthy young adults shows that characteristic patterns of brain activity define individual “brain-fingerprints” that are unique to each person. However, variability in these brain-fingerprints increases in individuals with neurological conditions, challenging the clinical relevance and potential impact of the approach. Our study shows that brain-fingerprints derived from neurophysiological brain activity are associated with pathophysiological and clinical traits of individual patients with Parkinson’s disease (PD). Methods: We created brain-fingerprints from task-free brain activity recorded through magnetoencephalography in 79 PD patients and compared them with those from two independent samples of age-matched healthy controls (N = 424 total). We decomposed brain activity into arrhythmic and rhythmic components, defining distinct brain-fingerprints for each type from recording durations of up to 4 min and as short as 30 s. Findings: The arrhythmic spectral components of cortical activity in patients with Parkinson’s disease are more variable over short periods, challenging the definition of a reliable brain-fingerprint. However, by isolating the rhythmic components of cortical activity, we derived brain-fingerprints that distinguished between patients and healthy controls with about 90% accuracy. The most prominent cortical features of the resulting Parkinson’s brain-fingerprint are mapped to polyrhythmic activity in unimodal sensorimotor regions. Leveraging these features, we also demonstrate that Parkinson’s symptom laterality can be decoded directly from cortical neurophysiological activity. Furthermore, our study reveals that the cortical topography of the Parkinson’s brain-fingerprint aligns with that of neurotransmitter systems affected by the disease’s pathophysiology. Interpretation: The increased moment-to-moment variability of arrhythmic brain-fingerprints challenges patient differentiation and explains previously published results. We outline patient-specific rhythmic brain signaling features that provide insights into both the neurophysiological signature and symptom laterality of Parkinson’s disease. Thus, the proposed definition of a rhythmic brain-fingerprint of Parkinson’s disease may contribute to novel, refined approaches to patient stratification. Symmetrically, we discuss how rhythmic brain-fingerprints may contribute to the improved identification and testing of therapeutic neurostimulation targets. Abstract: Light plays a crucial role in the growth, development, and survival of aquatic organisms. To evaluate the survival of European spiny lobster (Palinurus elephas) larvae in relation to different light intensities (26, 359, and 910 lux, equivalent to 0.35, 4.74, and 12.29 μmol·m−2·s−1, respectively), this study monitored the survival rates of reared spiny lobster from the early instar phyllosoma stages to their metamorphosis into the puerulus stage (111 days after hatching) at the three light intensities. The newly hatched larvae were cultured in 65-L Kreisel tanks with 200 individuals each (three replicates per treatment, i.e., light intensity). Phyllosoma stages I to III larvae showed no significant differences in survival rates under different light intensities. However, light intensity did have a significant impact on the survival rates of phyllosoma stages IV and V as well as the metamorphosis of phyllosoma VI into puerulus. The highest survival rates were observed under high and medium light intensity conditions. At the end of the experiment, the highest mean survival rate for stage VI, the metamorphosis into the puerulus stage, was 42.0 ± 2.52 % under high light intensity. These survival rates for reared pueruli were 84 times higher than those achieved in previous studies. The present study results show promise for the successful implementation of commercial scale rearing protocols for European spiny lobster and have implications for the design of crustacean larvae culture systems.

Abstract: Heart failure (HF) increases the risk of developing atrial fibrillation (AF), leading to increased morbidity and mortality. Therefore, better prediction of this risk may improve treatment strategies. Although several predictors based on clinical data have been developed, the establishment of a transcriptome-based predictor of AF incidence in HF has proven to be more problematic. We hypothesized that the transcriptome profile of coronary sinus blood samples of HF patients is associated with AF incidence. We therefore enrolled 192 HF patients who were selected for biventricular cardioverter defibrillator implantation. Both coronary sinus and peripheral blood samples were obtained during the procedure. Patients were followed-up during two years and AF occurrence was based on interrogation of the defibrillator. A total of 96 patients stayed in sinus rhythm (SR) during follow-up, 13 patients developed AF within 1 year and 10 patients developed AF during the second year of follow up. Gene expression profiling of coronary sinus samples led to the identification of 321 AF predictor genes based on their differential expression between patients developing AF within 1 year of blood sampling and patients remaining in SR. The expression levels of these genes were combined to obtain a molecular atrial fibrillation prediction score for each patient which was significantly different between both patient groups (Mann-Whitney, p = 0.00018). We conclude that the cardiac blood transcriptome of HF patients should be further investigated as a potential AF risk prediction tool.

Abstract: In today's fiercely competitive and rapidly evolving telecommunications sector, technological change is a critical driver of organizational survival and success. This research delves into how organizations can harness technological advancements to enhance human resource management and optimize service delivery, ultimately bolstering productivity and performance. Drawing on theoretical frameworks including the Resource-Based View (RBV) theory, Human Capital Theory, and the Job Characteristics Model, this study examines the impact of technological change on digital HR practices. Data were collected from 110 employees at Congo Telecom in Brazzaville, Republic of Congo, using stratified random sampling. Employing the PLS-SEM methodology through SmartPLS, the analysis reveals that technological change exerts a direct and positive influence on digital HR practices and employee attitudes. Additionally, it uncovers the positive effects of employee competency, employee attitudes, and law compliance on digital HR practices. Notably, employee attitudes are found to partially mediate the relationship between technological change and digital HR practices. This study thus contributes theoretically by integrating various concepts from different disciplines such as marketing, innovation, human resources, and telecommunication to investigate the impact of technological change on digital HR practices.

Abstract: Introduction: Efforts to reduce the global burden of communicable and non-communicable diseases can significantly benefit from comprehensive immunisation programmes. Organisations such as the World Health Organization (WHO), regional partners, and national governments are deeply committed to this cause. Given that Africa has the world's second-largest population, approximately 1.3 billion people, the continent's health challenges underscore the critical need for such immunisation strategies. However, the continent recorded the lowest vaccination rate, less than 30 %, during the recent coronavirus disease 2019 pandemic, whereas global coverage was close to 70 %. The most commonly encountered vaccine-preventable diseases in Africa are chickenpox, diphtheria, hepatitis A, hepatitis B, human papillomavirus, measles, and mumps. Others include polio, pneumococcal diseases, tetanus, rubella, and Haemophilus influenzae type B. Objectives: To understand the general vaccination portfolio of the African continent, discuss awareness and concerns about the acceptability of vaccines in Africa, and analyse the effects of common vaccine-preventable diseases and vaccine communication. The overall objective was to evaluate vaccination and the status of vaccine-preventable diseases in Africa. Methodology: The authors relied on information from published academic resources and articles in PubMed, the National Center for Biotechnological Information, Google Scholar, the WHO, and the United States Centers for Disease Control databases, among others. Discussion: This review summarises the awareness and acceptability of vaccines in Africa, the level of understanding of vaccines, and offers solutions for how vaccines can reach every member of the African population. Conclusion: The most recent pandemic identified the need for national governments to increase investment in vaccine manufacturing. Hence, the new commitments of African leaders and relevant stakeholders to raise awareness of vaccine use, invest in vaccine manufacturing, training, and other strategies aimed at reducing vaccine-preventable diseases will make the African population healthier and wealthier.

Abstract: Mutations in the DES gene, which encodes the intermediate filament desmin, lead to desminopathy, a rare disease characterized by skeletal muscle weakness and different forms of cardiomyopathies associated with cardiac conduction defects and arrhythmias. We generated human induced pluripotent stem cells (hiPSC) from a patient carrying the DES p.R406W mutation, and employed CRISPR/Cas9 to rectify the mutation in the patient's hiPSC line and introduced the mutation in an hiPSC line from a control individual unrelated to the patient. These hiPSC lines represent useful models for delving into the mechanisms of desminopathy and developing new therapeutic approaches.

Abstract: Phosphorites from the Kodjari deposits of Burkina Faso are composite rocks with a fine-grained matrix phase and dispersed larger grains. Chemical analysis by XRF revealed a CaO content of 35.98–41.33 wt% and a P2O5 content of 26.85–30.75 wt%. X-ray diffraction patterns show the abundant presence of carbonate-fluorapatite (Francolite), Hydroxyapatite and Quartz, with the minor phases Wavellite, Maricite and phyllosilicates. The mineralogical compositions by Quantitative Rietveld Analysis shows the presence of 75.41–83.85 wt% of phosphate phases, with carbonate structural groups in Francolite and Hydroxyapatite, while F ions are in the Francolite structure. FTIR analyses also validates the identification of mineral phases with the observation of bands from structural phosphate groups, from carbonate groups and from structural F. This study highlights the effectiveness of Quantitative Rietveld Analysis in quantifying the mineralogical composition of phosphorites. The method is suitable for monitoring the exploitation of mineral deposits.

Abstract: Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

22. A re-conceptualisation of the Batwa's right to recognition as a minority and indigenous people in Rwanda: A human rights-based approach

Abstract: There have been divergent views by various human rights scholars and actors on how to address the question of vulnerability among indigenous and minority groups in Rwanda. Even though factors influencing vulnerability among these groups have received increasing scholarly attention, findings from these studies fall short of reflecting on the role of identity and discrimination. Whereas the government in Rwanda after the 1994 genocide sought to enhance unity and reconciliation by outlawing all ethnic identities, and instead conflating all vulnerable groups into a nomenclature namely, a Historically Marginalised People (HMP), a divergent group of scholars, human rights activists, the United Nations and African Union's instruments of human rights describe this approach as antithetical to the promotion and protection of the rights of indigenous and minority groups. Using the human rights-based approach, this paper critically interrogates the issue of identity and how it has affected the enjoyment of the rights among the minority and indigenous Batwa people in Rwanda.The study employed a qualitative research design with data collected through Focus Group Discussions (FGDs) as well as in-depth interviews (IDIs). Respondents included members of the Batwa community, officials of selected public institutions, leaders in Non-Governmental Organisations (NGOs), and cooperative societies working towards the promotion of rights of the HMP. Data analysis involved the process of transcription and coding of data and the eventual reflexive thematic analysis.The findings show that the rights of the Batwa community to recognition and self-identity have varied meanings and implications to different members of the community as well as officials from public institutions and other relevant actors. To the existing research, the study affirms that the inconsistencies surrounding the Batwa's identity have accentuated their vulnerability.In terms of policy, the study suggests that in the short run, the Batwa community may require enhanced special temporary social protection measures in order to ameliorate their conundrum arising from identity ambiguity without compromising Rwanda's national unity. Further studies, should consider the role of land, forest and culture in further marginalising the Batwa, as well as quantitative methodology for inferential analysis.

23. Estetrol/drospirenone versus 17α-ethinylestradiol/drospirenone: An extended one generation test to evaluate the endocrine disruption potential on zebrafish (Danio rerio)

Abstract: Combined oral contraceptives, comprising of both an oestrogen and a progestin component, are released in aquatic environments and potentially pose a risk to aquatic wildlife by their capacity to disrupt physiological mechanisms.In this study, the endocrine disruptive potential of two mixtures, 17α-ethinylestradiol (EE2), a synthetic oestrogen, or estetrol (E4), a natural oestrogen, with the progestin drospirenone (DRSP) have been characterised in three generations of zebrafish, according to an adapted Medaka Extended One Generation Reproduction Test. Zebrafish (Danio rerio) were exposed to a range of concentrations of EE2/DRSP and E4/DRSP (∼1×, ∼3×, ∼10× and ∼30× predicted environmental concentration, PEC). Survival, growth, hatching success, fecundity, fertilisation success, vitellogenin (VTG), gonad histopathology, sex differentiation, and transcriptional analysis of genes related to gonadal sex steroid hormones synthesis were assessed.In the F0 generation, exposure to EE2/DRSP at ∼10 and ∼30× PEC decreased fecundity and increased male VTG concentrations. The highest concentration of EE2/DRSP also affected VTG concentrations in female zebrafish and the expression of genes implicated in steroid hormones synthesis. In the F1 generation, sex determination was impaired in fish exposed to EE2/DRSP at concentrations as low as ∼3× PEC. Decreased fecundity and fertility, and abnormal gonadal histopathology were also observed. No effects were observed in the F2 generation. In contrast, E4/DRSP induced only minor histopathological changes and an increase in the proportion of males, at the highest concentration tested (∼30× PEC) in the F1 generation and had no effect on hatching success of F2 generation.Overall, this study suggests that the combination E4/DRSP has a more favourable environmental profile than EE2/DRSP.

24. Corneal Infantile Myofibromatosis Caused by Novel Activating Imatinib-Responsive Variants in PDGFRB

Abstract: Purpose: To investigate the genetic cause, clinical characteristics, and potential therapeutic targets of infantile corneal myofibromatosis. Design: Case series with genetic and functional in vitro analyses. Participants: Four individuals from 2 unrelated families with clinical signs of corneal myofibromatosis were investigated. Methods: Exome-based panel sequencing for platelet-derived growth factor receptor beta gene (PDGFRB) and notch homolog protein 3 gene (NOTCH3) was performed in the respective index patients. One clinically affected member of each family was tested for the pathogenic variant detected in the respective index by Sanger sequencing. Immunohistochemical staining on excised corneal tissue was conducted. Functional analysis of the individual PDGFRB variants was performed in vitro by luciferase reporter assays on transfected porcine aortic endothelial cells using tyrosine kinase inhibitors. Protein expression analysis of mutated PDGFRB was analyzed by Western blot. Main Outcome Measures: Sequencing data, immunohistochemical stainings, functional analysis of PDGFRB variants, and protein expression analysis. Results: We identified 2 novel, heterozygous gain-of-function variants in PDGFRB in 4 individuals from 2 unrelated families with corneal myofibromatosis. Immunohistochemistry demonstrated positivity for alpha-smooth muscle actin and β-catenin, a low proliferation rate in Ki-67 (< 5%), marginal positivity for Desmin, and negative staining for Caldesmon and CD34. In all patients, recurrence of disease occurred after corneal surgery. When transfected in cultured cells, the PDGFRB variants conferred a constitutive activity to the receptor in the absence of its ligand and were sensitive to the tyrosine kinase inhibitor imatinib. The variants can both be classified as likely pathogenic regarding the American College of Medical Genetics and Genomics classification criteria. Conclusions: We describe 4 cases of corneal myofibromatosis caused by novel PDGFRB variants with autosomal dominant transmission. Imatinib sensitivity in vitro suggests perspectives for targeted therapy preventing recurrences in the future. Abstract: Summary: In the ciliate Paramecium, precise excision of numerous internal eliminated sequences (IESs) from the somatic genome is essential at each sexual cycle. DNA double-strands breaks (DSBs) introduced by the PiggyMac endonuclease are repaired in a highly concerted manner by the non-homologous end joining (NHEJ) pathway, illustrated by complete inhibition of DNA cleavage when Ku70/80 proteins are missing. We show that expression of a DNA-binding-deficient Ku70 mutant (Ku70-6E) permits DNA cleavage but leads to the accumulation of unrepaired DSBs. We uncoupled DNA cleavage and repair by co-expressing wild-type and mutant Ku70. High-throughput sequencing of the developing macronucleus genome in these conditions identifies the presence of extremities healed by de novo telomere addition and numerous translocations between IES-flanking sequences. Coupling the two steps of IES excision ensures that both extremities are held together throughout the process, suggesting that DSB repair proteins are essential for assembly of a synaptic precleavage complex.

26. Assessing the vulnerability of urban drinking water intakes to water scarcity under global change: A bottom-up approach

Abstract: Drinking water intakes (DWIs) face significant pressure due to global changes, including urbanization and climate change. The common approach relies mainly on climate projections generated by global climate models to simulate large scale hydroclimatic conditions. However, it is crucial to discern the impact of global changes on water scarcity at the local level, including in regions where available data are limited. This paper proposes an approach that focuses on studying the vulnerability of surface DWIs to low water levels and water demand in current and future climates within a cold-climate region.Low flows at DWIs were simulated using historical water level data obtained from hydrometric stations situated along the studied river. After defining four scenarios for climate change and anthropogenic activities affecting raw water withdrawals at DWIs, the full potential range of level variations was simulated. This study employed a combined water scarcity index derived from two sub-indices based on water level and water demand. The resulting index ranges from 0 to 1, where a higher value indicates a greater vulnerability to water scarcity. The simulation results demonstrate the vulnerability of water scarcity in both current and future climates.The calculated index, selecting the current vulnerability to water scarcity for the five studied DWIs, ranged from 0.61 to 0.76. The results for the vulnerability of these DWIs under future climate conditions exhibited significant variability across the different scenarios representing possible maximum daily withdrawal. These scenarios were defined to encompass a spectrum of options related to the government's policy for drinking water conservation strategy implementation. While exploring the full range of potential risks, the study's results demonstrated that the DWIs were especially vulnerable to anthropogenic changes affecting water demand.The framework developed in this study can provide a decision-support basis for municipalities and water managers to adapt to global change and achieve greater water supply resilience.

Abstract: This research suggests a study model of determinants of investment decisions in the context of financial innovation. The paper considers the technology innovation dimension, which influences stakeholders' (customer and employee) satisfaction, bank performance, and investment decisions. This research aims to bridge the gap by analyzing the effect of the innovation component, financial innovation on investment decisions and considering the mediated link via stakeholders' satisfaction and the bank's performance as well as the moderating role of Internet security (IS) and utilizing data gathered from 575 banks' employees and customers in Congo. Employing Structural Equation Modelling to evaluate the hypotheses, the research obtains that: (1) financial innovation (FI) is negatively associated with investment decisions (ID), and positively related to employee satisfaction (ES), customer satisfaction (CS), and bank performance (BP). (2) BP, CS, ES, and IS are related significantly and positively to ID. (3) BP, CS, and employee satisfaction do mediate the relationship between FI and ID. (4) Internet security does not moderate the effect of FI on ID. The investigation emphasizes the role of financial innovation and Internet security in investment decision-making for the providers of financial innovation services. Certainly, it recommends that financial institutions should consider this as well as internet security in their investment decisions. This article contributes to financial literature and bank management by offering supporting theories and a framework that facilitates the identification of pertinent strategic resources and constructs.

28. Can natural history collection specimens be used as aquatic microplastic pollution bioindicators?

Abstract: Microplastic pollution has risen to such a level that concerns are being raised regarding its consequences on the environment, especially the marine environment. Understanding microplastic pollution temporal dynamics is critical but requires time-series. However, concerns about microplastic pollution being recent, long term monitoring programs have only started very recently and remain scarce. Natural History Collections that represent archives from the past can constitute time-series. Although underused, they have evinced their success to study various stressors including pollutants, in particular when using bioindicators. Bioindicator species should be defined with regards to the studied environmental disturbance according to established criteria. Those criteria include occurring frequently, being sensitive to the pollutant, and allowing, via their monitoring, a summary of the pollutant’s impacts at molecular, organismal or population levels. However, to analyse bioindicator species in Natural History Collection time-series, several specificities need to be considered. Starting from a review of articles that utilised such collections to study microplastic evolution in a given ecosystem, and focusing on their methodologies, we emphasise Natural History Collection features that need to be taken into account when choosing the most adequate taxon and extraction techniques. In particular we discuss four collection features: sampling heterogeneity, taxonomic misidentification, past environmental contamination and specimen destruction and provide leads to address these issues. We believe that combining the concept of bioindicator with valuable samples from Natural History Collections is of particular interest to monitor past microplastic pollution and better predict future trends. This constitutes a necessary step in assessing the basal level and the continuing evolution of this ever-increasing pollution.

Abstract: Inherited retinal diseases are a leading and untreatable cause of blindness and are therefore candidate diseases for gene therapy. Recombinant vectors derived from adeno-associated virus (rAAV) are currently the most promising vehicles for in vivo therapeutic gene delivery to the retina. However, there is a need for novel AAV-based vectors with greater efficacy for ophthalmic applications, as underscored by recent reports of dose-related inflammatory responses in clinical trials of rAAV-based ocular gene therapies. Improved therapeutic efficacy of vectors would allow for decreases in the dose delivered, with consequent reductions in inflammatory reactions. Here, we describe the development of new rAAV vectors using bioconjugation chemistry to modify the rAAV capsid, thereby improving the therapeutic index. Covalent coupling of a mannose ligand, via the formation of a thiourea bond, to the amino groups of the rAAV capsid significantly increases vector transduction efficiency of both rat and nonhuman primate retinas. These optimized rAAV vectors have important implications for the treatment of a wide range of retinal diseases.

Abstract: Summary: The ribosome-tethered N-terminal acetyltransferase A (NatA) acetylates 52% of soluble proteins in Arabidopsis thaliana. This co-translational modification of the N terminus stabilizes diverse cytosolic plant proteins. The evolutionary conserved Huntingtin yeast partner K (HYPK) facilitates NatA activity in planta, but in vitro, its N-terminal helix α1 inhibits human NatA activity. To dissect the regulatory function of HYPK protein domains in vivo, we genetically engineer CRISPR-Cas9 mutants expressing a HYPK fragment lacking all functional domains (hypk-cr1) or an internally deleted HYPK variant truncating helix α1 but retaining the C-terminal ubiquitin-associated (UBA) domain (hypk-cr2). We find that the UBA domain of HYPK is vital for stabilizing the NatA complex in an organ-specific manner. The N terminus of HYPK, including helix α1, is critical for promoting NatA activity on substrates starting with various amino acids. Consequently, deleting only 42 amino acids inside the HYPK N terminus causes substantial destabilization of the plant proteome and higher tolerance toward drought stress.

Abstract: Most chemical elements are crucial for life maintenance, but the intake of non-essential elements or inadequate concentrations of essential ones can have major consequences on wildlife health. However, concentrations of minor and trace elements remain largely unknown in free-ranging animals. This study aimed to establish the first reference values for 22 essential (Ca, Co, Cr, Cu, Fe, K, Mg, Mn, Mo, Na, Ni, Se, Zn) and non-essential (Al, As, Cd, Hg, Pb, Sb, Sr, Ti, Tl) elements in roe deer hair (Capreolus capreolus), to assess the degree of covariation between elements, and to investigate how individual characteristics and environmental context influence minor and trace element profiles. To do so, 758 hair samples from 542 individuals were collected between 2016 and 2019 in five roe deer populations (i.e. Ain, Chizé, Châteauvillain-Arc-en-Barrois, Jura and Trois-Fontaines) inhabiting contrasted habitats in France. Compared to the scarce literature on minor and trace elements in roe deer hair, concentrations were generally lower, and correlations between elements were mostly positive. Element concentrations were also heterogeneously distributed among populations, with overall greater heterogeneity in two (i.e. Jura and Châteauvillain-Arc-en-Barrois) of the five populations, which could be explained by their specific environmental characteristics (e.g. vegetation, climate, altitude). Overall, minor and trace element concentrations did not differ according to age and sex. Further studies on element bioavailability are needed to clarify whether the observed heterogeneity of element concentrations in roe deer hair is primarily driven by individual or by geographical characteristics.

Abstract: Summary: We investigate the bacterial and fungal composition and functionality of the Ju|’hoansi intestinal microbiome (IM). The Juǀʼhoansi are a hunter-gatherer community residing in northeastern Namibia. They formerly subsisted by hunting and gathering but have been increasingly exposed to industrial dietary sources, medicines, and lifestyle features. They present an opportunity to study the evolution of the human IM in situ, from a predominantly hunter-gatherer to an increasingly Western urban-forager-farmer lifestyle. Their bacterial IM resembles that of typical hunter-gatherers, being enriched for genera such as Prevotella, Blautia, Faecalibacterium, Succinivibrio, and Treponema. Fungal IM inhabitants include animal pathogens and plant saprotrophs such as Fusarium, Issatchenkia, and Panellus. Our results suggest that diet and culture exert a greater influence on Ju|’hoansi IM composition than age, self-identified biological sex, and medical history. The Ju|’hoansi exhibit a unique core IM composition that diverges from the core IMs of other populations.

Abstract: Decades of biological and clinical research have led to important advances in recombinant adeno-associated viruses rAAV-based gene therapy gene therapy. However, several challenges must be overcome to fully exploit the potential of rAAV vectors. Innovative approaches to modify viral genome and capsid elements have been used to overcome issues such as unwanted immune responses and off-targeting. While often successful, genetic modification of capsids can drastically reduce vector yield and often fails to produce vectors with properties that translate across different animal species, such as rodents, non-human primates, and humans. Here, we describe a chemical bioconjugation strategy to modify tyrosine residues on AAV capsids using specific ligands, thereby circumventing the need to genetically engineer the capsid sequence. Aromatic electrophilic substitution of the phenol ring of tyrosine residues on AAV capsids improved the in vivo transduction efficiency of rAAV2 vectors in both liver and retinal targets. This tyrosine bioconjugation strategy represents an innovative technology for the engineering of rAAV vectors for human gene therapy.

Abstract: Patients with new-onset left bundle branch block (LBBB) after transcatheter aortic valve implantation (TAVI) are at risk of developing delayed high-degree atrioventricular block. Management of new-onset LBBB post-TAVI remains controversial. In the Comparison of a Clinical Monitoring Strategy Versus Electrophysiology-Guided Algorithmic Approach in Patients With a New LBBB After TAVI (COME-TAVI) trial, consenting patients with new-onset LBBB that persists on day 2 after TAVI, meeting exclusion/inclusion criteria, are randomized to an electrophysiological study (EPS)-guided approach or 30-day electrocardiographic monitoring. In the EPS-guided approach, patients with a His to ventricle (HV) interval ≥ 65 ms undergo permanent pacemaker implantation. Patients randomized to noninvasive monitoring receive a wearable continuous electrocardiographic recording and transmitting device for 30 days. Follow-up will be performed at 3, 6, and 12 months. The primary endpoint is a composite outcome designed to capture net clinical benefit. The endpoint incorporates major consequences of both strategies in patients with new-onset LBBB after TAVI, as follows: (i) sudden cardiac death; (ii) syncope; (iii) atrioventricular conduction disorder requiring a pacemaker (for a class I or IIa indication); and (iv) complications related to the pacemaker or EPS. The trial incorporates a Bayesian design with a noninformative prior, outcome-adaptive randomization (initially 1:1), and 2 prespecified interim analyses once 25% and 50% of the anticipated number of primary endpoints are reached. The trial is event-driven, with an anticipated upper limit of 452 patients required to reach 77 primary outcome events over 12 months of follow-up. In summary, the aim of this Bayesian multicentre randomized trial is to compare 2 management strategies in patients with new-onset LBBB post-TAVI—an EPS-guided approach vs noninvasive 30-day monitoring. Abstract: Succinic semialdehyde dehydrogenase deficiency (SSADHD) is a neurometabolic disorder caused by ALDH5A1 mutations presenting with autism and epilepsy. SSADHD leads to impaired GABA metabolism and results in accumulation of GABA and γ-hydroxybutyrate (GHB), which alter neurotransmission and are thought to lead to neurobehavioral symptoms. However, why increased inhibitory neurotransmitters lead to seizures remains unclear. We used induced pluripotent stem cells from SSADHD patients (one female and two male) and differentiated them into GABAergic and glutamatergic neurons. SSADHD iGABA neurons show altered GABA metabolism and concomitant changes in expression of genes associated with inhibitory neurotransmission. In contrast, glutamatergic neurons display increased spontaneous activity and upregulation of mitochondrial genes. CRISPR correction of the pathogenic variants or SSADHD mRNA expression rescue various metabolic and functional abnormalities in human neurons. Our findings uncover a previously unknown role for SSADHD in excitatory human neurons and provide unique insights into the cellular and molecular basis of SSADHD and potential therapeutic interventions.

Although many longitudinal studies after NAIS have shown that predicting the occurrence of gross motor difficulties is easier, accurately predicting hand motor function (for both hands) from morphometric MRI remains complicated. The hypothesis of an association between the structural organization of the basal ganglia (BG) and thalamus with hand motor function seems intuitive given their key role in sensorimotor function. Neuroimaging studies have frequently investigated these structures to evaluate the correlation between their volumes and motor function following early brain injury. However, the results have been controversial. We hypothesize the involvement of other structural parameters. Method: The study involves 35 children (mean age 7.3 years, SD 0.4) with middle cerebral artery NAIS who underwent a structural T1-weighted 3D MRI and clinical examination to assess manual dexterity using the Box and Blocks Test (BBT). Graphs are used to represent high-level structural information of the BG and thalami (volumes, elongations, distances) measured from the MRI. A graph neural network (GNN) is proposed to predict children’s hand motor function through a graph regression. Abstract: Summary: Background: Smoking cigarettes produces carbon monoxide (CO), which can reduce the oxygen-carrying capacity of the blood. We aimed to determine whether elevated expiratory CO levels would be associated with a worse prognosis in smokers presenting with acute cardiac events. Methods: From 7 to 22 April 2021, expiratory CO levels were measured in a prospective registry including all consecutive patients admitted for acute cardiac event in 39 centres throughout France. The primary outcome was 1-year all-cause death. Initial in-hospital major adverse cardiac events (MAE; death, resuscitated cardiac arrest and cardiogenic shock) were also analysed. The study was registered at ClinicalTrials.gov (NCT05063097). Findings: Among 1379 patients (63 ± 15 years, 70% men), 368 (27%) were active smokers. Expiratory CO levels were significantly raised in active smokers compared to non-smokers. A CO level >11 parts per million (ppm) found in 94 (25.5%) smokers was associated with a significant increase in death (14.9% for CO > 11 ppm vs. 2.9% for CO ≤ 11 ppm; p 11 ppm was associated with a significant increase in MAE in smokers during initial hospitalisation after adjustment for comorbidities (odds ratio [OR] 15.75, 95% CI [5.56–44.60]) or parameters of in-hospital severity (OR 10.67, 95% CI [4.06–28.04]). In the overall population, CO > 11 ppm but not smoking was associated with an increased rate of all-cause death (HR 4.03, 95% CI [2.33–6.98] and 1.66 [0.96–2.85] respectively). Interpretation: Elevated CO level is independently associated with a 6-fold increase in 1-year death and 10-fold in-hospital MAE in smokers hospitalized for acute cardiac events. Abstract: In Huntington disease, cellular toxicity is particularly caused by toxic protein fragments generated from the mutant huntingtin (HTT) protein. By modifying the HTT protein, we aim to reduce proteolytic cleavage and ameliorate the consequences of mutant HTT without lowering total HTT levels. To that end, we use an antisense oligonucleotide (AON) that targets HTT pre-mRNA and induces partial skipping of exon 12, which contains the critical caspase-6 cleavage site. Here, we show that AON-treatment can partially restore the phenotype of YAC128 mice, a mouse model expressing the full-length human HTT gene including 128 CAG-repeats. Wild-type and YAC128 mice were treated intracerebroventricularly with AON12.1, scrambled AON or vehicle starting at 6 months of age and followed up to 12 months of age, when MRI was performed and mice were sacrificed. AON12.1 treatment induced around 40% exon skip and protein modification. The phenotype on body weight and activity, but not rotarod, was restored by AON treatment. Genes differentially expressed in YAC128 striatum changed toward wild-type levels and striatal volume was preserved upon AON12.1 treatment. However, scrambled AON also showed a restorative effect on gene expression and appeared to generally increase brain volume.

Abstract: - Predator teeth, especially canines, leave circular marks as pits and punctures on the bone surface of prey. The morphology and dimensions of puncture marks are related to different factors, both in terms of the predator itself (age, sex) and the nature of the impacted bone. The aim of this study is to examine the variability of puncture marks made by various predator species through the biometrical analysis of canines. Such an approach highlights the wide dimensional overlap and helps define the range of potential puncture production from smaller to larger carnivore species. The results draw attention to the use of simple measurements in the analysis of tooth marks on bone. Notwithstanding, more sophisticated multifactorial integrative approaches (predator and prey) are still needed.

Abstract: The relationship between student ICT use and learning outcomes is being debated. Few studies on the topic examined different forms of ICT use and heterogenous relationships with learning outcomes. This study addresses this gap by examining the association between different forms of ICT use and learning outcomes and by exploring variations in associations based on student-perceived ICT competence and across the learning distribution. The Japanese sample of PISA 2018 data was analyzed employing a regression approach accounting for the complexity of the PISA design and unconditional quantile regression. Results suggest that the variables student ICT use outside school for schoolwork activities and at school in general are not statistically related to reading and science achievements, but the former is weakly related to mathematics. The two forms of ICT use improve learning among students with lower ICT competence and also show statistical significance for low- and high-achieving students. Moreover, subject-related uses of ICT during lessons and outside lessons are positively associated with the three measures of learning achievements investigated. The association remains consistent by student ICT competence and across the learning distribution. In contrast, ICT use outside of school for leisure is negatively associated with all three measures of learning achievements. The association remains negative by student ICT competence and across the learning distribution. These findings suggest that education stakeholders promoting student ICT use should be cautious by considering forms of ICT use, student ICT competence, and heterogenous abilities.

Abstract: The subretinal injection protocol for the only approved retinal gene therapy (voretigene neparvovec-rzyl) includes air tamponade at the end of the procedure, but its effects on the subretinal bleb have not been described. In the present study, we evaluated the distribution of enhanced green fluorescent protein (EGFP) after subretinal injection of AAV2 in non-human primates (NHP) without (group A = 3 eyes) or with (group B = 3 eyes) air tamponade. The retinal expression of EGFP was assessed 1 month after subretinal injection with in vivo fundus photographs and fundus autofluorescence. In group A (without air), EGFP expression was limited to the area of the initial subretinal bleb. In group B (with air), EGFP was expressed in a much wider area. These data show that the buoyant force of air on the retina causes a wide subretinal diffusion of vector, away from the injection site. In the present paper, we discuss the beneficial and deleterious clinical effects of this finding. Whereas subretinal injection is likely to become more common with the coming of new gene therapies, the effects of air tamponade should be explored further to improve efficacy, reproducibility, and safety of the protocol.

Abstract: In advanced scapholunate instability, a scapholunate repair by open or arthroscopic ligamentoplasty is indicated. Although the radiographic results and functional scores are more or less satisfactory for open ligamentoplasty, it is often responsible for a decrease in joint amplitude postoperatively. Arthroscopic techniques are therefore of great interest, since they respect the joint capsule, but they remain technically difficult and demanding surgeries, requiring a good deal of experience in arthroscopy and using bone tunnels that are potentially a source of complications, as well as pinning to the palmar side of the wrist, which is potentially dangerous for the palmar structures of the wrist. We present a surgical technique of scapholunate ligamentoplasty under arthroscopy, focusing only on the dorsal scapholunate complex: all-dorsal arthroscopic ligamentoplasty. The advantages of all-dorsal arthroscopic ligamentoplasty are that it requires little material, does away with palmar approaches, and focuses only on the dorsal side, simplifying surgery and avoiding the creation of bone tunnels potentially at risk of fracture or avascular necrosis.

Abstract: Introduction: Fadogiella stigmatoloba, Hygrophylla auriculata, Hylodesmum repandum and Porphyrostemma chevalieri are used against malaria in traditional medicine in the Democratic Republic of the Congo (DRC). To evaluate their potential in the treatment of this disease, the in vitro antiplasmodial property of these four plants was evaluated. All experiments were conducted on methanolic extracts performed on selected organ parts of these plants. Methods: The methanolic extracts, obtained by maceration, were firstly screened in vitro against the chloroquine sensitive (3D7) and resistant (W2) Plasmodium falciparum strains by the measurement of lactate dehydrogenase activity, and on human keratinocytes (HaCat) cells by the MTT assay to determine their selectivity indices (SI). Secondly, the antioxidant activity of the same extracts was evaluated using DPPH and FRAP assays. Finally, the presence of specific phytochemical constituents was evaluated using standard methods and tentatively identified by GC-MS. Results: An optimum antiplasmodial activity (IC50 = 3.4 ± 0.7 μg/mL, for 3D7, SI = 58.2; IC50 = 7 ± 1.0 μg/mL, for W2, SI = 28.3) was obtained with the leave extract of P. chevalieri. The leaves (for F. stigmatoloba and H. repandum), and the aerial part (for H. repandum) extracts showed promising and moderate antiplasmodial activities against respectively the 3D7 strain (IC50: 100 μg/mL) on HaCat cells. For the antioxidant test, the most interesting activity was obtained with the leaf extract of P. chevalieri. The GC–MS analysis of these four plants species extracts revealed the presence of various compounds, such as Ethyl 2-nonenoate, 2-(2-Hydroxy-2-phenylethyl)-3,5,6-trimethyl pyrazine, Palmitic Acid, Ethyl palmitate, Ethyl linolenate, and N-Acetyltyramine. Conclusion: Based on the obtained results, P. chevalieri could be selected for further investigations or ⁄and for the management of malaria after standardization.

Abstract: Operational systems providing irrigation advisories to agricultural extension workers are paramount, particularly in West Africa where the yield gap represents the greatest agriculture growth-led opportunity. The proposed framework for Burkina Faso, an irrigation decision support system (DSS), is based on in-situ weather and field observations necessary for feeding the atmosphere, soil, and crop modules of crop-water productivity models (e.g., AquaCrop). To optimize water resources, incoming irrigation and precipitation, and outgoing evapotranspiration are constantly monitored and adjusted. The findings of the proposed semi-automatic irrigation DSS indicate that water stresses affecting the canopy cover and stomatal closure are minimized if the proposed irrigation schemes are generated and improved with five-day weather observations. The source of uncertainty in crop models’ evapotranspiration estimations is reduced by systematically comparing the observed crop evapotranspiration (ETc) with historical ETc records. An increase in yields is observed in all studied crops, from 1960 to 2018 kg/ha (tomato dry yields), from 2571 to 2799 kg/ha (maize), and from 1279 to 1385 kg/ha (quinoa) when comparing the 2020–21 and 2021–22 experiments. Results show an optimization of water resources, with a higher evapotranspired water productivity (WPET, expressed as dry weight) when comparing the two experiments, from 0.86 to 0.97 kg/m3 for tomato, from 0.85 to 0.86 kg/m3 for maize, and from 0.67 to 0.73 kg/m3 for quinoa, respectively in 2020–21 and 2021–22. The proposed irrigation DSS can be used to inform extension workers and technical agronomic experts about real-time crop water requirements and, thus, assist the Climate Risk and Early Warning Systems (CREWS) initiative that aims to improve access to weather information for decision-support in agriculture. Afterwards, extension agents can catalyze irrigation advisories and support farmers improve irrigation management at the field level to, ultimately, obtain higher yields.

Abstract: Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.
