Objectives: To assess the internal and external validity of a cluster randomized controlled trial (cRCT) evaluating a decision tool with supportive interventions for the empirical treatment of urinary tract infections (UTIs) in nursing homes (NHs), and to identify facilitators and barriers in implementing this antibiotic stewardship intervention., Design: Mixed-methods process evaluation study., Setting and Participants: Physicians, nursing staff, client council members, and residents of Dutch NHs., Methods: We used cRCT data of the ANNA study (Antibiotic Prescribing and Non-prescribing in Nursing Home Residents With Signs and Symptoms Ascribed to Urinary Tract Infection). In addition, we sent out an online evaluation questionnaire, conducted semistructured interviews with physicians and nursing staff, and consulted client council members., Results: Internal validity was lowered: control group physicians participated in several non-study-related activities regarding UTI. External validity was good: almost all intervention components had a high fidelity (52%-74%) and were perceived as relevant (physicians: 7.2-8.6 of 10, nursing staff: 6.5-8.5 of 10) and feasible (physicians: 7.5 of 10, nursing staff 6.4 of 10), with feasibility for residents with dementia and urine incontinence needing attention. The most common reason for deviating from the advice generated by the decision tool was an unclear illness presentation. Identified facilitators to implementation were confidence in the intervention, repeated intervention encounter, and having "champions" in the NH. Barriers were limited involvement of nursing staff, unstable nursing teams, residents' and representatives' belief that antibiotics should be prescribed, and a low antibiotic prescribing threshold within the NH culture., Conclusions and Implications: Lowered internal validity may have reduced the study effect. Attention should be paid to the feasibility of the intervention in residents with dementia and urinary incontinence. Objective: This article describes the process evaluation of the Depression in Elderly with Long-Term Afflictions (DELTA) intervention, a nurse-led minimal psychological intervention to reduce depression in chronically ill elderly persons. The aim was to assess whether the DELTA intervention was implemented and received as planned, which barriers were encountered and whether nurses and patients were satisfied with the intervention., Methods: Quantitative and qualitative data were gathered from all 183 patients who participated in the intervention and from four nurses who administered the intervention. Data were assessed by means of self-administered questionnaires, checklists, and a group interview with the nurses., Results: Although several barriers were encountered, the intervention was well performed by nurses and well received by patients. However, a small proportion of the patients found the intervention not useful and thought they would not benefit., Conclusion: The implementation of the minimal psychological intervention aimed at reducing depression in chronically ill elderly persons was successful and therefore feasible., Practice Implications: By making the intervention part of regular care for patients with a chronic condition, patients can be monitored and, if necessary, support can be provided to implement the intervention in their daily life.

Objective: To obtain insight into the involvement of a patient's social network in the process leading to the administration of euthanasia and assisted suicide (EAS)., Design: Descriptive, retrospective., Method: Data were collected by means of anonymous questionnaires sent to a random sample of 521 general practitioners (GPs) in the Province of North Holland and 521 GPs from the rest of the Netherlands and all 713 Dutch nursing home physicians (NHPs). The data were collected over the period 1986 to June 1990 inclusive., Results: GPs and NHPs had discussions most often with the partner of the patient about the patient's request for EAS, the doctor's intention to administer EAS and the actual administering of EAS. According to both the GPs and the NHPs the social network practically always agreed with the doctor's decision to administer EAS. The persons who were most often present at the patient's bedside when EAS was administered were the patient's partner and children (29%). Often, especially in the cases of GPs, the decision not to report EAS was determined by the desire of relatives and/or the physician not to involve the relatives in a judicial inquiry., Conclusion: To a large extent the social network was involved in the patient's request for EAS, the doctor's intention to administer EAS and the actual administering of EAS. This seems to be important both for good decision-making with regard to EAS and for helping relatives to come to terms with the loss of a dear one.

Bronchopulmonary carcinoid tumors are tumors with a low malignant potential. They metastasize in 5-15% of cases. Accurate histologic preoperative prediction of the presence of regional lymph node metastases is not possible at this time. A retrospective quantitative pathologic analysis was performed to investigate the possibility of predicting the presence or absence of regional lymph node metastases in 24 patients with bronchopulmonary carcinoid tumors. The results of univariate analysis showed that large tumor size was associated significantly more frequently with regional lymph node metastases than small tumor size (P less than 0.01). The other quantitative features, ie, a larger mean nuclear area, higher standard deviation of the nuclear area and the presence of an aneuploid DNA index, were frequently associated with regional lymph node metastases, but this tendency was not significant. In multivariate analysis the combination of tumor size and mean nuclear area predicted the presence or absence of regional lymph node metastases correctly in 80 and 94% of the cases, respectively. These results indicate that the combination of tumor size and mean nuclear area may serve as a guideline to predict the presence of regional lymph node metastases.
