AIM: User-friendly up-to-date guidance for antimicrobial therapy in resource-constrained settings is lacking. We aim to address the gap by describing the development process of the ABXGUIDE, which includes online pathogen-directed antibiotic guidelines taking into consideration low-resource settings. BACKGROUND: The threat of antimicrobial resistance (AMR) is particularly pressing in low-and-middle-income countries (LMICs). This is due to the high burden of infectious diseases, limited resources, lacking antimicrobial stewardship (AMS) infrastructure, and limited guidance intended for these conditions. A global stakeholder survey conducted in 2022 confirmed the need for contextualized guidelines on preferred and alternative therapies in resource-constrained settings. The ABXGUIDE provides pathogen-directed antibiotic guidance for multiple bacterial infections, while considering multiple restrictive factors in low-resource settings. METHODS: ABXGUIDE development started by a literature review, compiling five or more guidelines for each infection in adults. Then antibiotic choices for different pathogens were tailored to resource-constrained settings by availability, cost, susceptibility, mode of administration, side effects, and allergies. Tabulated data, which was aligned with the WHO AWaRe classification (2021), was internally reviewed by consensus of four experts, followed by a peer review by two independent experts. RESULTS: The ABXGUIDE online platform exhibits pathogen-directed guidelines on primary therapy and alternative therapies that are suitable for LMICs. Multiple infections are included: respiratory, bloodstream, neurological, urinary, abdominal, skin and cardiac infections. ABXGUIDE offers suggestions on duration of therapy, allergies and side effects, bearing in mind limitations in resource-constrained settings. CONCLUSIONS: ABXGUIDE provides easy access to high-quality pathogen-directed treatment guidelines for multiple bacterial infections optimized for resource-constrained settings.

Determination of mechanical state of thermal oxide growing during high temperature oxidation is a challenge. Fabrication of nanogauges for monitoring during high temperature oxidation was presently investigated up to 1000 °C. Several materials were tested in terms of mechanical behaviour and maximum working temperature for gauges used as markers during thermal loading under air. The experimental determination of the optimized gauges material for high temperature oxidation was validated. SiO2 offers particularly interesting features for gauges to determine the mechanical state of metal/oxide system. In this article, a special attention has also been paid to two nano-fabrication processes, as well as their limits. The standard electron beam lithography process is well suited to build gauges for oxidation applications, and can be improved by use of reactive ion etching process. The gauges can eventually reach several micrometers height such that the oxidation layer does not cover the gauges during thermal loading for relevant monitoring at short oxidation times. To illustrate, an application to a 17-4PH stainless steel oxidized at 480 °C is proposed and stress kinetics are deduced and related to an advanced thermomechano-chemical model, as well as identification of the numerical values of different thermomechano-chemical parameters associated to mechanisms of growth or relaxation.

Objectives: This study aimed to assess compliance with legal requirements, safe medication storage and staffing standards in community pharmacies in Punjab, Pakistan. Method: We conducted a three-step cross-sectional study using observations, questionnaires and face-to-face interviews in 544 systematically-selected community pharmacies. We used descriptive statistic and one-way ANOVA to assess the data. Results: Only 23 (4.2 %) pharmacies had accurate area and only 3.9 % had appropriate walls. In total, 23.3 % had glass-fronted shelves and 38.2 % had a glass door. More than half (53.8 %) had separate narcotics shelves and 43.0 % a separate shelf of expired medicines. Less than half (47.5 %) of the pharmacies were able to maintain hygiene. About 36.2 % of the pharmacies segregated different types of product. Drugs were protected from direct sunlight in most (61.3 %) pharmacies, but the refrigerator was working properly in less than half (43.4 %) and only a very small number (2.4 %) had an alternative power supply for the refrigerator. Only 37 (6.8 %) were able to maintain an appropriate room temperature. The vast majority (93.0 %) displayed a valid drug sale license, but a qualified person/pharmacist was only present in 4.8 %. The average number of employees was 4.2, and more than 71.0 % of staff had 10–12 years of formal education. Only 0.2 % of employees could explain term “PRN”, although 57.3 % explained “IV” correctly. About 22.8 % replied correctly about the room temperature but the vast majority (97.6 %) did not know about cold chain temperature. The location of the pharmacy (p-value = 0.045) affected its performance. Conclusions: Noncompliance with legal requirements, unsafe drug storage and limited human resources reflect the poor enforcement of drug laws in Pakistan. The findings suggest that there is a need to strengthen inspection and management of community pharmacies.

The utilization of solar energy is a vital strategy in the agricultural sector's efforts to address and reduce greenhouse gas emissions. This renewable resource can greatly decrease the industry's carbon footprint and play a significant role in combating climate change. In this context, this study examines an automatic solar system installed in a south-facing agricultural greenhouse and its effect on the growth of strawberry plants in winter. Heat is transferred from the environment during the day to the structure at night using this technology, which uses water flowing in a closed circuit that is put on the greenhouse roof. This system includes a battery, a photovoltaic solar panel to power some accessories, a copper coil placed within double glass on the greenhouse's roof, a water pump circulator, and storage tanks. Two greenhouses—one experimental with a solar heating system and the other reference without one—were used for the comparative experimental investigation. Both greenhouses are constructed on the terrace of Mohammed V University's Solar Energy and Environment Laboratory in Rabat, Morocco. An environmental monitoring system was built to automatically measure environmental parameters. Real-time data visualization and analysis may be done from any place via a website with the Internet of Things integrated into the system. The greenhouse's microclimate was improved by this low-cost technology, which also allowed for winter heating. Compared to the control greenhouse, this improvement allowed for a 17-day earlier harvest and a 30% reduction in irrigation water usage. The economic analysis findings show that the system is profitable for investments and energy savings.

Objective: It has been suggested that positive psychotic symptoms reflect ‘aberrant salience’. Previously we provided support for this hypothesis in first-episode schizophrenia patients, demonstrating that delusional symptoms were associated with aberrant reward processing, indexed by the Salience Attribution Test (SAT). Here we tested whether salience processing is abnormal in schizophrenia patients with long-standing treatment-refractory persistent delusions (TRS). Method: Eighteen medicated TRS patients and 31 healthy volunteers completed the SAT, on which participants made a speeded response to earn money in the presence of cues. Each cue comprised two visual dimensions, colour and form. Reinforcement probability varied over one of these dimensions (task-relevant), but not the other (task-irrelevant). Results: Participants responded significantly faster on high-probability relative to low-probability trials, representing implicit adaptive salience; this effect was intact in TRS patients. By contrast, TRS patients were impaired on the explicit adaptive salience measure, rating high-probability stimuli less likely to be associated with reward than controls. There was little evidence for elevated aberrant salience in the TRS group. Conclusion: These findings do not support the hypothesis that persistent delusions are related to aberrant motivational salience processing in TRS patients. However, they do support the view that patients with schizophrenia have impaired reward learning.

Objective: This study employs a Developmental Origins of Health and Disease (DOHaD) approach to assess the effect of vitamin D deficiency (VDD) in childhood on the risk of osteoporosis in adulthood in an archaeological sample of skeletons dating from the 18th to 19th centuries., Materials: Femora and lumbar vertebrae of 65 adults aged 18+ years (26 diagnosed with residual rickets and 39 without) from an 18th-19th century Quaker burial ground at Coach Lane, North Shields, England., Methods: Bone mineral density (BMD) was measured for the femoral neck and first four lumbar vertebrae of each individual using a dual energy X-ray absorptiometry (DXA) scanner as a proxy for assessing osteoporotic fracture risk., Results: 3-way ANOVA revealed no statistically significant differences in BMD between individuals with and without residual rickets across age and sex., Conclusions: A combination of lifestyle and environmental factors likely influenced the BMD of people buried at Coach Lane across the life course. The impact of childhood VDD on BMD later in life can be mitigated through other factors such as physical activity and diet., Significance: This is one of the first bioarchaeological studies to take a DOHaD approach to understand osteoporosis risk in 18th-19th century England. It highlights the complexity of aetiological factors for osteoporosis and that VDD in early life does not necessarily predispose a person to osteoporosis in adulthood., Limitations: BMD is not the only indicator of osteoporosis. A Near Infrared (NIR) spectroscopic method combined with multivariate calibration was developed for the determination of the amount of sucrose in date fruits growing in the Sultanate of Oman. In this study two groups of samples were used: one group of 48 sucrose standard solutions in the concentration range from 0.01% to 50% (w/v) and another group of 54 date fruit samples of 18 different varieties. The sucrose standard samples were split in two sets, i.e. one training set of 31 samples and one test set of 17 samples. All samples were measured with a NIR spectrophotometer in the wavelength range from 700 to 2500 nm. The spectra collected were preprocessed using baseline correction and Savitzky-Golay 1st derivative. Partial least-squares regression (PLSR) was used to build the regression model with the training set of 31 samples. This model was then validated by using random leave-one-out cross-validation. Later, the PLS regression model was externally validated by using the test set of 17 samples of known sucrose concentration. The root mean squared error of prediction (RMSEP) was found to be of 1.5%, which shows a good prediction ability of the model. Finally, the PLS model was applied to the spectra of 54 date fruit samples to quantify their sucrose amount. It was found that the Khalas, Barnia Nizwi, Ajwa Almadina, Maan, and Khunizi varieties contain high amounts of sucrose, i.e. ranging from 36% to 60%, while Naghal, Fardh, Nashu and Qash Tabaq varieties contain the least amount of sucrose, ranging from 3.5% to 8.1%., (Copyright © 2015 Elsevier B.V. Inadequate bone mineral accrual during growth and accelerated bone demineralisation in adulthood are recognised as additional and serious complications for patients with cystic fibrosis (CF). However, little attention has been given to preventative strategies for this population. Inadequate bone accrual during childhood and adolescence, and premature bone loss, lead to a failure to attain an optimal peak bone mass, osteoporosis and fracture in patients with CF. Pharmaceutical treatments may assist in the improvement of bone in patients with CF, but these are usually not preferable for use in children. Evidence indicates that regular, weight-bearing exercise significantly enhances bone accrual in healthy children. This paper reviews the literature concerning the potential for weight-bearing exercise to improve bone mineral accrual in children with CF. All relevant literature since 1979 was obtained and reviewed from the Medline, PubMed, Cochrane and PEDro data base. Evidence concerning the efficacy of exercise for bone health in CF is lacking. There have been no controlled trials investigating the value of weight-bearing exercise for bone accrual in children with CF. As exercise may offer an effective and enjoyable strategy to improve the bone development in children who have CF, exercise should be a high priority for randomised controlled trials in this population.
