Abstract

BACKGROUND: The Global COVID Vaccine Safety (GCoVS) Project, established in 2021 under the multinational Global Vaccine Data Network™ (GVDN®), facilitates comprehensive assessment of vaccine safety. This study aimed to evaluate the risk of adverse events of special interest (AESI) following COVID-19 vaccination from 10 sites across eight countries. METHODS: Using a common protocol, this observational cohort study compared observed with expected rates of 13 selected AESI across neurological, haematological, and cardiac outcomes. Expected rates were obtained by participating sites using pre-COVID-19 vaccination healthcare data stratified by age and sex. Observed rates were reported from the same healthcare datasets since COVID-19 vaccination program rollout. AESI occurring up to 42 days following vaccination with mRNA (BNT162b2 and mRNA-1273) and adenovirus-vector (ChAdOx1) vaccines were included in the primary analysis. Risks were assessed using observed versus expected (OE) ratios with 95 % confidence intervals. Prioritised potential safety signals were those with lower bound of the 95 % confidence interval (LBCI) greater than 1.5. RESULTS: Participants included 99,068,901 vaccinated individuals. In total, 183,559,462 doses of BNT162b2, 36,178,442 doses of mRNA-1273, and 23,093,399 doses of ChAdOx1 were administered across participating sites in the study period. Risk periods following homologous vaccination schedules contributed 23,168,335 person-years of follow-up. OE ratios with LBCI > 1.5 were observed for Guillain-Barré syndrome (2.49, 95 % CI: 2.15, 2.87) and cerebral venous sinus thrombosis (3.23, 95 % CI: 2.51, 4.09) following the first dose of ChAdOx1 vaccine. Acute disseminated encephalomyelitis showed an OE ratio of 3.78 (95 % CI: 1.52, 7.78) following the first dose of mRNA-1273 vaccine. Abstract

Pulmonary artery pseudoaneurysm is a rare but life threatening complication of pulmonary tuberculosis, considered as a diagnosis and therapeutic emergency. Transarterial embolization approach has become more widespread over the last few decades, and is now considered the first-line treatment over surgery. Percutaneous embolization under computed tomography (CT) or CT scan control has recently been reported by one centre as a first-line treatment for persistent peripheral Pulmonary artery pseudoaneurysm under certain conditions. We report the case of a 23-year-old female patient admitted in emergency for moderate haemoptysis, in a context of relapsing of tuberculosis. CT scan angiogram showed a peripheral pulmonary artery pseudoaneurysm of the lower left lobe, and persisted seven days later. After multidisciplinary meeting, a minimal invasive approach was decided. The patient was treated in first-line treatment by percutaneous transthoracic embolization, under CT-guidance, using N butyl-cyanoacrylate and Lipiodol mixture, without any complication. The percutaneous minimal invasive treatment seems to be a reliable approach to treat persistent peripheral pulmonary artery pseudoaneurysm.

13. Augmenting family based treatment with emotion coaching for adolescents with anorexia nervosa and atypical anorexia nervosa: Trial design and methodological report

Abstract

This article characterizes the design, recruitment, methodology, participant characteristics, and preliminary feasibility and acceptability of the Families Ending Eating Disorders (FEED) open pilot study. FEED augments family-based treatment (FBT) for adolescents with anorexia nervosa (AN) and atypical anorexia nervosa (AAN) with an emotion coaching (EC) group for parents (i.e., FBT + EC). We targeted families high in critical comments and low warmth (assessed by the Five-Minute Speech Sample), known predictors of poor response in FBT. Eligible participants included adolescents initiating outpatient FBT, diagnosed with AN/AAN, ages 12–17, with a parent high in critical comments/low in warmth. The first phase of the study was an open pilot which demonstrated feasibility and acceptability of FBT + EC. Thus, we proceeded with the small randomized controlled trial (RCT). Eligible families were randomized to either 10 weeks of FBT + EC parent group treatment or the 10- week parent support group (control condition). The primary outcomes were parent critical comments and parental warmth, while our exploratory outcome was adolescent weight restoration. Novel aspects of the trial design (e.g., specifically targeting typical treatment non-responders), as well as recruitment and retention challenges in the context of the COVID-19 pandemic are discussed.

Abstract

Percutaneous radiologic gastrostomy (PRG) is a widely used procedure with a low rate of serious complications and with comparable short-term outcomes with percutaneous endoscopy. Hemorrhagic complications are rare (1.4%), and occur usually immediately after the procedure due to direct arterial punctures. We report on the case of a 62-year-old male patient with a diagnosis of multi-systemic atrophy disease that was referred to our tertiary center for PRG. The procedure was performed without early complications. He presented a slight bleeding 3 weeks of the procedure. A CT angiogram revealed a pseudoaneurysm of the left superior epigastric artery, in contact with the gastrostomy tube. After a failed surgical treatment, the patient was successfully treated by percutaneous embolization using a mixture of Glubran 2 and Lipiodol, under ultrasound and fluoroscopic control. This case study suggested that a slight hemorrhage following PRG may suggest a pseudoaneurysm and a CT angiogram should be performed.

Abstract

Hemoptysis could be a life-threatening event, especially when the bleeding originates from the arterial pulmonary circulation. The main cause of this type of bleeding is pulmonary artery pseudoaneurysm (PAP), which can be managed by surgical, medical or minimally invasive techniques. This study reports the case of massive hemoptysis in a 75-year-old male patient, with a former history of lobectomy. The initial CT scan showed a giant PAP from a branch of the right middle lobar pulmonary artery, within the right lower lobectomy cavity. An endovascular approach was decided. Subsequently, the feeding artery of the PAP was embolized with detachable coils. The control CT scan showed a persistent opacification of the PAP. The embolization was then completed by injection of Onyx within coils packing, with a complete thrombose of the PAP on control CT scan. This report confirms the safety and efficacy profile of an endovascular approach to treat giant PAP, using a combination of coils and Onyx.

Abstract

Gastrointestinal stromal tumours (GISTs) are malignant mesenchymal tumours with a variable clinical behaviour, marked by differentiation towards the interstitial cells of Cajal.1 GISTs belong to the family of soft tissue sarcomas (STSs) but are treated separately due to their peculiar histogenesis, clinical behaviour and specific therapy. This European Society for Medical Oncology (ESMO)–European Reference Network for Rare Adult Solid Cancers (EURACAN)–European Reference Network for Genetic Tumour Risk Syndromes (GENTURIS) Clinical Practice Guideline (CPG) will cover GISTs while other STSs are covered in the ESMO–EURACAN–European Reference Network for Paediatric Oncology (ERN PaedCan)–GENTURIS STS CPG.

23. A case study application of machine-learning for the detection of greenhouse gas emission sources

Abstract

Conclusively linking local, episodic enhancements in greenhouse gas concentrations to a specific emission source can be challenging, particularly when faced with multiple proximal sources of emissions and variable meteorology, and in the absence of co-emitted tracer gases. This study demonstrates and evaluates the efficacy of using machine-learning tools to detect episodic emissions of methane (CH4) from a shale gas extraction facility in Lancashire (United Kingdom). Two machine-learning tools (rmweather and Prophet) were trained using a two-year climatological baseline dataset collected prior to gas extraction operations at the facility. The baseline dataset consisted of high-precision trace gas concentrations and meteorological data, sampled at 1 Hz continuously between 2016 and 2019. The models showed good overall predictive capacity for baseline CH4 concentrations, with R2 values of 0.85 and 0.76 under optimised training conditions for rmweather and Prophet, respectively. CH4 concentrations were then forecast for an 18-month period from the onset of operations at the shale gas facility (in 2018). Forecast values were compared with true measurements to detect anomalous deviations that may indicate the presence of new emission events associated with the operational facility. Both models successfully detected two periods in which CH4 emissions were known to have occurred (December 2018 and January 2019) via anomalous deviations between modelled and measured concentrations. This work demonstrates the application of machine-learning models for the detection of CH4 emission events from newly built industrial sources, when used in combination with real-time atmospheric monitoring and a baseline dataset collected prior to installation.

26. Beneficial effects of multi-species mixtures on N2O emissions from intensively managed grassland swards

Abstract

In a field experiment, annual nitrous oxide (N2O) emissions and grassland yield were measured across different plant communities, comprising systematically varying combinations of monocultures and mixtures of three functional groups (FG): grasses (Lolium perenne, Phleum pratense), legumes (Trifolium pratense, Trifolium repens) and herbs (Cichorium intybus, Plantago lanceolata). Plots received 150 kg ha−1 year−1 nitrogen (N) (150 N), except L. perenne monocultures which received two N levels: 150 N and 300 N. The effect of plant diversity on N2O emissions was derived from linear combinations of species performances' in monoculture (species identity) and not from strong interactions between species in mixtures. Increasing from 150 N to 300 N in L. perenne resulted in a highly significant increase in cumulative N2O emissions from 1.39 to 3.18 kg N2O-N ha−1 year−1. Higher N2O emissions were also associated with the legume FG. Emissions intensities (yield-scaled N2O emissions) from multi-species mixture communities around the equi-proportional mixture were lowered due to interactions among species. For N2O emissions scaled by nitrogen yield in forage, the 6-species mixture was significantly lower than L. perenne at both 300 N and 150 N. In comparison to 300 N L. perenne, the same N yield or DM yield could have been produced with the equi-proportional 6-species mixture (150 N) while reducing N2O losses by 63% and 58% respectively. Compared to 150 N L. perenne, the same N yield or DM yield could have been produced with the 6-species mixture while reducing N2O losses by 41% and 24% respectively. Overall, this study found that multi-species grasslands can potentially reduce both N2O emissions and emissions intensities, contributing to the sustainability of grassland production.

Abstract

[Introduction] Healthcare-associated infections (HAI) are among the most common adverse events of medical care. Surveillance of HAI is a key component of successful infection prevention programmes. Conventional surveillance – manual chart review – is resource intensive and limited by concerns regarding interrater reliability. This has led to the development and use of automated surveillance (AS). Many AS systems are the product of in-house development efforts and heterogeneous in their design and methods. With this roadmap, the PRAISE network aims to provide guidance on how to move AS from the research setting to large-scale implementation, and how to ensure the delivery of surveillance data that are uniform and useful for improvement of quality of care., [Methods] The PRAISE network brings together 30 experts from ten European countries. This roadmap is based on the outcome of two workshops, teleconference meetings and review by an independent panel of international experts., [Results] This roadmap focuses on the surveillance of HAI within networks of healthcare facilities for the purpose of comparison, prevention and quality improvement initiatives. The roadmap does the following: discusses the selection of surveillance targets, different organizational and methodologic approaches and their advantages, disadvantages and risks; defines key performance requirements of AS systems and suggestions for their design; provides guidance on successful implementation and maintenance; and discusses areas of future research and training requirements for the infection prevention and related disciplines. The roadmap is supported by accompanying documents regarding the governance and information technology aspects of implementing AS., [Conclusions] Large-scale implementation of AS requires guidance and coordination within and across surveillance networks. Transitions to large-scale AS entail redevelopment of surveillance methods and their interpretation, intensive dialogue with stakeholders and the investment of considerable resources. Abstract

This paper introduces Grid2050, a web-based decision support framework that allows users to build and assess US low-carbon electricity generation portfolios for 2050. Although base assumptions from various sources are integrated into the tool to support a default portfolio mix, the framework encourages users to alter assumptions about the techno-economic performance and socio-political acceptance of different technologies to create their portfolios. To support that process, the framework includes built-in analytics to estimate deviations from base assumptions, a capability to explore alternative assumptions about technological learning and scale-up, and the premiums users are willing to incur to avoid or deploy different technologies. This suite of features offers a way to benchmark the consistency and feasibility of users’ energy system preferences. Its use should help inform energy system planning studies and regulatory interventions to bend the global warming curve.

Abstract

Objectives To explore interactional processes in which clinical decisions are made in situ during medical consultations, particularly the ways in which patients show agency in decision-making processes by proposing and opposing actions, and which normative dimensions and role-expectations their engagement entail. Methods Narrative analysis of verbatim transcripts of 22 naturally occurring consultations, sourced from a corpus of 212 consultations between general practitioners and patients in England. After thematically coding the whole dataset, we selected 22 consultations with particularly engaged patients for in-depth analysis. Results Patients oppose further actions more often than they propose actions, and they oppose more directly than they propose. When they explain why they propose and oppose something, they reveal their values. Patients’ role-performance changes throughout the consultations. Conclusion Assertive patients claim – and probably also achieve – most influence when they oppose actions directly and elaborate why. Patients display ambiguous role-expectations. In final concluding stages of decision-making processes, patients usually defer to GPs’ authority. Practice Implications Clinicians should be attentive to the ways in which patients want to engage in decision-making throughout the whole consultation, with awareness of normative dimensions of both process and content, and the ways in which patient’s actions are constrained by their institutional position.

34. Switzerland's PM10 and PM2.5 environmental increments show the importance of non-exhaust emissions

Abstract

Atmospheric particulate matter (PM) is a priority pollutant for urban air pollution management because of its negative effects on human health and visibility. Emissions from road traffic have been a major focus of management over the past few decades, but non-exhaust emissions i.e., emissions from brake, tyre, road wear, and the resuspension of dust have emerged to become a major source of unregulated PM in many locations. Here, a filter-based sampling campaign was conducted between 2018 and 2019 where a large number of PM constituents were quantified for five sites in Switzerland for both PM10 and PM2.5. This had the objective of investigating urban and urban-traffic PM increments in Switzerland. The results show that PM concentrations increased as the sampling locations moved along a rural to urban-traffic gradient. However, source apportionment analysis showed that sulfate-rich, nitrate-rich, and biogenic sources were not enhanced in urban environments, but road traffic and mineral dust sources were. The total mass enhancement for PM10 and PM2.5 were 2.4 μg m−3 and 2.0 μg m−3 for the urban environment while the corresponding urban-traffic enhancements were 5.7 μg m−3 and 2.8 μg m−3. Emissions from road traffic were estimated to contribute more than 75% to the urban increments and non-exhaust emissions contributed 48% (PM10) and 25% (PM2.5) to the total road traffic related increment at an urban background site and 62% (PM10) and 49% (PM2.5) at an urban-traffic site. Analysis of the composition of Switzerland's PM showed that elements associated with non-exhaust emissions, specifically the brake wear tracers of antimony, barium, copper, and iron were the metals with the greatest urban and urban-traffic enhancements. Critically, the urban increment of these elements was enhanced for both PM10 and PM2.5 by about the same magnitude as the urban-traffic increment (by 2–3 times), demonstrating non-exhaust emissions are encountered across urban areas, not just the urban-traffic environment. Therefore, non-exhaust emissions were an important contributor to the urban and urban-traffic PM10 and PM2.5 increments in Switzerland's urban areas. The relative contributions of non-exhaust emissions to the urban and urban-traffic increments could be expected to increase due to the introduction of further exhaust after-treatment technologies (such as gasoline particulate filters; GPFs) and the transition to a more electrified vehicle fleet. Abstract

REIPI-SEIMC COVID-19 group and COVID@HULP group., [Background] The clinical presentation of COVID-19 in patients admitted to hospital is heterogeneous. We aimed to determine whether clinical phenotypes of patients with COVID-19 can be derived from clinical data, to assess the reproducibility of these phenotypes and correlation with prognosis, and to derive and validate a simplified probabilistic model for phenotype assignment. Phenotype identification was not primarily intended as a predictive tool for mortality., [Methods] In this study, we used data from two cohorts: the COVID-19@Spain cohort, a retrospective cohort including 4035 consecutive adult patients admitted to 127 hospitals in Spain with COVID-19 between Feb 2 and March 17, 2020, and the COVID-19@HULP cohort, including 2226 consecutive adult patients admitted to a teaching hospital in Madrid between Feb 25 and April 19, 2020. The COVID-19@Spain cohort was divided into a derivation cohort, comprising 2667 randomly selected patients, and an internal validation cohort, comprising the remaining 1368 patients. The COVID-19@HULP cohort was used as an external validation cohort. A probabilistic model for phenotype assignment was derived in the derivation cohort using multinomial logistic regression and validated in the internal validation cohort. The model was also applied to the external validation cohort. 30-day mortality and other prognostic variables were assessed in the derived phenotypes and in the phenotypes assigned by the probabilistic model., [Findings] Three distinct phenotypes were derived in the derivation cohort (n=2667)—phenotype A (516 [19%] patients), phenotype B (1955 [73%]) and phenotype C (196 [7%])—and reproduced in the internal validation cohort (n=1368)—phenotype A (233 [17%] patients), phenotype B (1019 [74%]), and phenotype C (116 [8%]). Patients with phenotype A were younger, were less frequently male, had mild viral symptoms, and had normal inflammatory parameters. Patients with phenotype B included more patients with obesity, lymphocytopenia, and moderately elevated inflammatory parameters. Patients with phenotype C included older patients with more comorbidities and even higher inflammatory parameters than phenotype B. We developed a simplified probabilistic model (validated in the internal validation cohort) for phenotype assignment, including 16 variables. In the derivation cohort, 30-day mortality rates were 2·5% (95% CI 1·4–4·3) for patients with phenotype A, 30·5% (28·5–32·6) for patients with phenotype B, and 60·7% (53·7–67·2) for patients with phenotype C (log-rank test p, [Interpretation] Patients admitted to hospital with COVID-19 can be classified into three phenotypes that correlate with mortality. We developed and validated a simplified tool for the probabilistic assignment of patients into phenotypes. [Introduction] Healthcare-associated infections (HAI) are among the most common adverse events of medical care. Surveillance of HAI is a key component of successful infection prevention programmes. Conventional surveillance – manual chart review – is resource intensive and limited by concerns regarding interrater reliability. This has led to the development and use of automated surveillance (AS). Many AS systems are the product of in-house development efforts and heterogeneous in their design and methods. With this roadmap, the PRAISE network aims to provide guidance on how to move AS from the research setting to large-scale implementation, and how to ensure the delivery of surveillance data that are uniform and useful for improvement of quality of care., [Methods] The PRAISE network brings together 30 experts from ten European countries. This roadmap is based on the outcome of two workshops, teleconference meetings and review by an independent panel of international experts., [Results] This roadmap focuses on the surveillance of HAI within networks of healthcare facilities for the purpose of comparison, prevention and quality improvement initiatives. The roadmap does the following: discusses the selection of surveillance targets, different organizational and methodologic approaches and their advantages, disadvantages and risks; defines key performance requirements of AS systems and suggestions for their design; provides guidance on successful implementation and maintenance; and discusses areas of future research and training requirements for the infection prevention and related disciplines. The roadmap is supported by accompanying documents regarding the governance and information technology aspects of implementing AS., [Conclusions] Large-scale implementation of AS requires guidance and coordination within and across surveillance networks. Transitions to large-scale AS entail redevelopment of surveillance methods and their interpretation, intensive dialogue with stakeholders and the investment of considerable resources. This roadmap can be used to guide future steps towards implementation, including designing solutions for AS and practical guidance checklists.

Abstract

This work explores the conditions for safe heat treatment of aluminum alloys containing lithium and magnesium in molten sodium nitrate (NaNO3) bath furnaces, and conditions where industrial accidents may occur. Using calorimetry coupled to classical thermodynamics, the strength of classical thermodynamics when analyzing thermal curves was demonstrated through a series of small-scale thermal analyses of various aluminum alloys in contact with sodium nitrate. This system was selected to illustrate reactions that may lead to severe and violent heat effect phenomena. Using idealized binary alloys, severe oxidation of magnesium- and lithium-rich aluminum alloy samples were shown to occur near 500 °C, a temperature range dangerously close to the operating temperature of solution heat treatment furnaces in manufacturing processes of heat treatable aluminum alloy sheets used in the aerospace industry. Commercial aluminum alloys AW257, 2198, 2024, and 1050 were also assessed with the same tools. The temperature that needed to be reached for these commercial aluminum alloys to react with molten sodium nitrate was significantly higher than the normal operating temperature of a conventional solution heat treatment furnace.
