Objective: The aim of this research was to evaluate the surgical complications and neurosensory deficits after coronectomy and the complete removal of mandibular third molars., Methods: The study sample included patients requiring surgical removal of mandibular third molars. A coronectomy was conducted on 220 teeth showing signs of close proximity to the inferior alveolar canal. A complete extraction was performed on 218 teeth with no risk signs. The patients were evaluated at 1 week and 1, 3, 6, 12, and 24 months after surgery for pain, swelling, neurologic deficit, dry socket, postoperative bleeding, infection, root migration, and eruption., Results: No significant difference was noted in pain and swelling; however, bleeding and dry socket were significantly higher in the odontectomy group (P = .017). The inferior alveolar nerve deficit was higher in the odontectomy group (3.7%) than the coronectomy group (0.5%) (P = .017). The percentage and distance of root migration of coronectomised teeth at 3, 6, and 12 months were 60% (2.37 ± 0.96 mm), 66% (3.35 ± 0.86 mm), and 74% (3.85 ± 0.93 mm), respectively., Conclusions: Coronectomy is a safe procedure and should be performed when the roots are closely associated with the mandibular canal. Objective: There are no studies that have evaluated the effect of Alveogyl with and without adjunct photobiomodulation therapy (PBMT) in reducing self-rated post-operative pain (SPP) in patients with alveolar osteitis (AO; dry socket). The aim of the present randomised controlled trial was to assess the effectiveness of Alveogyl with and without PBMT for the management of SPP in patients with AO., Methods: Adult nonsmokers with diagnosed AO were included. Patients were randomly divided into 4 groups. In Group 1, patients underwent mechanical curettage (MC) with copious normal saline irrigation. In Group 2, patients underwent MC + Alveogyl dressings in extraction sites which were changed every 48 hours until cessation of pain. In Group 3, patients underwent MC + Alveogyl followed by PBMT using a 660-nm indium-gallium-aluminum-phosphide diode laser. In Group 4, patients were treated solely with PBMT. The visual analogue scale was used up to 3 postoperative days to assess SPP up to 3 days at 6- (T0) and 12-hour (T1) intervals. Statistical analyses were performed using the analysis of variance and Bonferroni post hoc adjustment tests. Correlation between SPP scores and age, sex, and eruption status of teeth was assessed using logistic regression models. P values <.01 were nominated as being statistically significant., Results: In all, 14, 13, 14, and 14 individuals with AO were included in groups 1, 2, 3 and 4, respectively. All patients had undergone extraction of mandibular third molars. At baseline and on day 1, there was no difference in SPP in all groups. On days 2 and 3, mean visual analogue scale (VAS) scores at T1 (P < .01) and T2 (P < .01) intervals were significantly high in Group 2 compared with Group 3. On days 2 and 3, mean VAS scores at T1 (P < .01) and T2 (P < .01) intervals were significantly high in Group 4 compared with Group 3. There was no difference in SPP in groups 3 and 4 on day 3 at T0 and T1 intervals., Conclusions: PBMT following MC and Alveogyl dressing is more efficient in reducing SPP compared with MC with or without Alveogyl dressing in patients with AO., Competing Interests: Conflict of interest None disclosed., (Copyright © 2022 The Authors. This systematic review and meta-analysis aimed to assess the effectiveness of a warm saline mouth bath (WSMB) in preventing dry socket after tooth extractions. A systematic search for randomized controlled trials published until August 30, 2020, in seven databases was conducted: Cochrane, PubMed, Ovid Medline, Google Scholar, and OpenGrey databases, ClinicalTrials.gov, and the World Health Organization International Clinical Trials Registry. The inclusion criteria were studies investigating the use of a warm saline mouth bath postoperatively in a population of participants who had a tooth extraction, compared to no mouth rinse at all/any other mouth rinse. The primary outcome assessed in the studies was the incidence of alveolar osteitis. Only eight randomized studies met all inclusion criteria and were selected for qualitative analysis. Six of the studies compared WSMB with antimicrobial rinses, and two studies compared WSMB with no-rinse. This review found no significant difference (P > 0.05) in the incidence of alveolar osteitis between WSMB and other antimicrobial rinses. Based on the results of this review, WSMB has potential in reducing post-operative complications such as alveolar osteitis following a routine or surgical extraction of teeth. Objective: The aim of this study was to estimate the influence of flap design on alveolar osteitis (AO) and postoperative side effects following third molar surgery., Study Design: This study was designed as a randomized single-blind clinical trial. The predictor variable was flap type. Envelope flap and modified triangular flap techniques were used. The primary outcome variable was AO. The secondary outcome variables were pain, swelling, and trismus. Descriptive and bivariate statistics were computed. Statistical significance was set at P ≤ .05., Results: Eighty patients with impacted mandibular third molars participated in the study. The envelope flap design was associated with a higher incidence of AO that was not statistically significant. On the second day, postoperative pain and swelling was observed as significantly different with the envelope flap technique., Conclusions: The modified triangular flap had the advantage of less postoperative pain and swelling but had the disadvantage of AO., (Copyright © 2013 Elsevier Inc. Background: Damage to the inferior alveolar nerve (IAN) during the extraction of impacted mandibular third molars in close proximity to the mandibular canal is a postoperative complication that most frequently occurs in patients 40 years and older., Methods: The authors evaluated the postoperative complications of 43 coronectomies of impacted mandibular third molars in 37 patients (17 men and 20 women, mean age [standard deviation] 31 [2] years). The authors used cone-beam computed tomographic images to determine that all of the teeth that underwent a coronectomy were in close proximity to the IAN., Results: The authors did not observe neurological injuries to the IAN or to the lingual nerve. One patient complained of intermittent pain and, 10 months after undergoing the coronectomy, underwent a second operation for extraction of the root fragments. The root extraction did not result in any neurological deficits, because the root fragments had migrated from the mandibular canal. One case of alveolitis was reported that manifested 15 days after the coronectomy., Conclusions: Coronectomies are safer to perform than complete extractions in situations in which the third molar is in close proximity to the mandibular canal. Root migration generally is asymptomatic, but in a case in which the patient underwent a second operation, the risk of the patient's experiencing neurological injuries was reduced., Clinical Implications: Coronectomy appears to be a valid surgical alternative in patients 40 years and older who are at a higher risk of experiencing neurological deficits than are younger patients.

Objective: To investigate whether smoking has adverse effects in simple exodontia., Methods: A single-centre, prospective study of postoperative inflammatory complications in simple exodontia was performed. All procedures were conducted under similar and sterile conditions. Postoperative complications (PCs) in exodontia were classified as alveolar osteitis (AO) or alveolar infection (AI) and their incidences then added., Results: A logistic regression model for PCs revealed tooth sectioning [odds ratio (OR) = 4.3, 95% confidence interval (CI) 1.0-18.8; P = 0.050], smoking (OR = 4.5, 95% CI 1.0-18.9; P = 0.03) and amount of smoking (> 20 cigarettes/day: OR = 12.3, 95% CI 1.0-149.8; P = 0.04) to be associated with the occurrence of PCs., Conclusions: Tooth sectioning, smoking and degree of smoking are all associated with the development of PCs such as AO and AI after simple exodontia. Objectives: Obesity is a disease that affects millions of Americans. Sixty-nine million Americans are considered to be overweight including at least 1 in 5 children. Overweight and obesity as risk factors for complications associated with dentoalveolar surgery have not been extensively studied. The purpose of this study was to prospectively investigate the frequency and the nature of postoperative complications in obese patients receiving ambulatory dentoalveolar surgery., Study Design: Patients undergoing dentoalveolar outpatient ambulatory surgery at the University of Cincinnati Oral and Maxillofacial Surgery Clinic from January 10, 2005 to December 7, 2005 were enrolled in a prospective study. At the preoperative visit, height, weight, and age were recorded. The body mass index (BMI) was calculated and patients were divided into weight categories based on CDC and WHO classifications. Unfavorable outcomes studied included number of postoperative visits, infections, alveolar osteitis, soft and hard tissue problems, and postoperative bleeding. Complication categories were created for all complications identified during postoperative exams and t tests were performed to see if increased BMI correlated to higher postoperative complication rates, if multiple complications correlated to increased BMI, and to see if individual postoperative complication groups and BMI correlated. Because it was thought that complications would increase the number of postoperative exams for a particular patient, simple linear regression was used to see if BMI correlated to increased number of postoperative visits., Results: No significant differences were noted in number of postoperative complications when comparing patients' BMIs (t = -0.62, P = .5350). Simple linear regression analysis showed that increased BMI did not predict increased number of postoperative visits. Increased age was the only predictor of increased postoperative complication risk (t = -3.17, P = .0016) and of increased number of postoperative visits (t = -16.35, P < .0001, and P < .0001 by nonparametric Wilcoxon rank sum test)., Conclusions: The BMI is not statistically correlated to higher postoperative complication rates or increased number of postoperative visits. Obesity remains an important risk factor for dentoalveolar ambulatory outpatient anesthesia; however, it is not associated with poorer postoperative complication rates.

Objectives: This paper provides a comprehensive review of the etiology, pathophysiology and current treatment of dry socket., Study Design: The Medline database (Ovid version) from 1966 to 2007 was searched for the term "dry socket" published in the English language, and 317 results were obtained. The articles were screened by abstract for relevance to etiology, pathophysiology, or treatment of dry socket. Treatment papers were ranked on the quality of evidence presented as assessed using the evidence-based systematic review worksheet of the University of Alberta. A total of 62 publications were included in the final review., Results: Prevention methods remain the key to avoiding this complication. Prophylactic placement of topical antibiotics can be considered, whereas systemic antibiotics should be reserved for patients who are immunocompromised., Conclusion: This paper provided a comprehensive review of the etiology, pathophysiology, and current treatment of dry socket in dental practice.

Objectives: The purpose of this study is to assess the contribution of smoking to postoperative complications, including alveolar osteitis (dry socket), after dental extractions. In addition, it attempts to determine the effect of the ban imposed on tobacco use in the prison on postoperative complications., Study Design: All inmates having dental extractions at the Federal Correctional Institution in Danbury, CT, during the period January 2004 to April 2005, were included in this study (N = 219; mean age = 37.7 years). Data on postextraction complications were analyzed for association with smoking by using the chi-square test. Significance was set at P < .05., Results: The incidences of overall complications and alveolar osteitis were 19.6% and 5.0%, respectively. It was found that (1) there was a significant difference in overall complications between smokers and nonsmokers (P = .02), (2) there was a significant difference in the incidence of alveolar osteitis between mandibular third molar and other extractions, regardless of smoking status (P = .02), (3) surgical trauma contributed significantly to both an increase in total complications (P = .05) and alveolar osteitis (P = .01), and (4) smoking appeared to be a contributing factor to increased complications among multiple extractions (P = .03)., Conclusion: In this study, smoking, mandibular third molars, and surgical trauma were significantly associated with the increased incidence of overall complications including alveolar osteitis.

Objectives: The aim of this study was to identify risk indicators for extended operation time and postoperative complications after removal of mandibular third molars., Study Design: There were 388 molars included in the study. The teeth were removed using the buccal approach under local anesthesia. Four hours postoperatively the patient recorded his or her pain perception on a visual analogue scale (VAS). After surgery a surgeon recorded parameters regarding the tooth and if the mandibular nerve had been visible during the operation. One week postoperatively the postoperative pain and complications were recorded. Logistic regression models were made to identify risk indicators for extended operation time, postoperative pain, and complications., Results: Females were at higher risk for postoperative pain and dry socket than males. Older patients were at higher risk for extended operation time than younger patients. Radiographically fully impacted molars increased the risk of postoperative general infection. If the nerve was visible during surgery there was a higher risk of a high VAS score, postoperative pain, and general infection than if the nerve had not been visible., Conclusion: Several indicators were found to increase the risk of postoperative complications, but a visible alveolar inferior nerve during the operation was repeatedly found to be the highest single risk indicator.

Objective: The purpose of this study was to evaluate the use of a 0.2% chlorhexidine gluconate and amoxicillin plus clavulanic acid combination as a prophylactic therapy for the prevention of alveolar osteitis after mandibular third molar extractions and to investigate adverse reactions to chlorhexidine., Study Design: This randomized, placebo-controlled, parallel group study was conducted in a group of 177 subjects, from which 3 groups were formed. The first group (n = 62) received 0.2% chlorhexidine gluconate, the second group (n = 56) received a 0.2% chlorhexidine gluconate and amoxicillin plus clavulanic acid combination, and the third group (n = 59) received 0.09% sterile saline solution. All patients were recalled for the diagnosis of alveolar osteitis on the third and seventh postoperative days., Results: When patients in the antibiotic group were compared with those in the other 2 groups, a significant reduction in alveolar osteitis was noted (P <.05). An alteration in taste, the bad taste of the solution, and staining of dentures and oral tissues were the major complaints about chlorhexidine., Conclusion: It would be more beneficial to use chlorhexidine solution with a beta-lactamase inhibitor-containing antibiotic to enhance its effectiveness for the prevention of alveolar osteitis.

Objective: The purpose of this study was to evaluate whether immediate placement of medicated dry socket packing would decrease the incidence of alveolar osteitis (dry socket) with lower third molar extractions., Study Design: In 100 patients, 200 lower third molars were extracted. One half of the sockets were packed to the crest of the alveolar ridge with a one-quarter-inch radiograph-detectable filament gauze that contained 9% eugenol, 36% balsam of Peru, and 55% petroleum jelly. The medicated packing was removed 1 week after surgery. None of the patients were taking antibiotics. Patients were instructed to increase their oral hygiene before and after surgery and were to use 0.12% chlorhexidine gluconate 2 days before and 3 days after surgery. RESULTS; Two hundred bilateral lower third molars of varying difficulty were extracted. The overall alveolar osteitis rate was 34 (17%). The immediately packed lower third molar sites had an alveolar osteitis rate of 8 (8%). The sockets that were not packed with medicated packing the day of surgery had an alveolar osteitis rate of 26 (26%). The difference was statistically significant (P =.001)., Conclusion: The results of this study suggest that placement of medicated dry socket packing immediately after lower third molar extraction decreases the alveolar osteitis rate.

Tooth extraction is the dental treatment most commonly carried out among HIV positive patients. In this paper we propose a simple protocol for dental extractions in patients of this type, based on a review of the literature and on our own experience in this field. The factors to be considered when planning a dental extraction for HIV positive patients are: diagnosis of the lesion justifying the need for tooth extraction and medical assessment of the patient including the infection pathway, the stage of the disease, laboratory tests and drug therapy. The most frequent post extraction complications are a delay in the wound healing process, alveolitis and surgical wound infection. Fortunately, these complications tend to be rather uncommon and not too severe; they can be managed on an out-patient basis and their prevalence will probably tend to decrease with the introduction of the new anti-retroviral drugs for HIV-1.

Through the years, dentists who perform dentoalveolar surgery have perpetuated many myths and other unproven beliefs from one generation to another. Sometimes, these beliefs originated in older textbooks, while others were given birth by mentors sharing anecdotal experiences with their students. Even today, many of these scientifically unsupported statements are perpetuated in surgical textbooks and in continuing education forums and are passed on to students in dental schools. In today's evolving environment of evidence-based medicine and dentistry, these anecdotal observations do not withstand scrutiny. The purpose of this article is to review the more common surgical myths and to test their validity against scientific evidence.

Objective: The purpose of this study was to identify factors associated with an increased risk for post-tooth-extraction complications in a sample of HIV-positive patients., Study Design: A cohort of HIV-positive patients who required the extraction of one or more teeth was enrolled. The predictor variables were grouped into the following sets: demographics; medical and social history; preoperative clinical findings; preoperative laboratory measures (hematologic, immunologic, and nutritional); and treatment. The outcome variable was defined as the presence or absence of a complication following tooth extraction. Logistic regression techniques were used to identify variables associated with an increased risk for complications following tooth extraction., Results: During the enrollment period, 76 HIV-positive patients were enrolled into the study cohort. Seventeen patients (22.4%) had postoperative complications. Based on the bivariate statistical analyses, variables associated with the presence of postoperative complications were red blood cell count, CD8 count, total number of positive sites tested using cell-mediated immunity skin tests, and extraction technique (p < or = 0.05). Using a stepwise logistic regression technique, the variable identified as being predictive of postoperative complications was the CD8 count (p = 0.02). The post-tooth-extraction complication rate of the HIV-positive patients in this study sample was greater than the rate reported in most other studies (22% vs. 3%-5%). The complications, however, were minor and easily treated. The variable consistently identified with an increased risk for complications was the CD8 count: the lower the CD8 count, the higher the risk for complications. The CD8 count, however, had poor predictive value., Conclusion: In acute clinical situations--for example, in cases of patients with significant dental pain--the results suggest that delaying treatment to obtain laboratory studies may be of little clinical value. It may be appropriate to proceed with suitable, definitive procedure(s) to alleviate symptoms.

Objective: To evaluate the effect of polylactic acid granules placed in third molar sockets on the incidence of alveolar osteitis., Study Design: A prospective randomized study with 161 patients (322 bilaterally impacted mandibular third molars)., Results: The sockets in which the polylactic acid granules were placed had a significantly higher rate of alveolar osteitis (23.6%) than did the control sockets (13.58%). In all subgroups analyzed, the incidence of alveolar osteitis was higher in the experimental (polylactic acid) group., Conclusion: On the basis of the findings of this study, polylactic acid granules do not reduce the incidence of alveolar osteitis and may actually contribute to a higher incidence of alveolar osteitis.

Numerous literature references have suggested increased risk for localized alveolar osteitis associated with female gender, use of oral contraceptives, and point in menstrual/contraceptive cycle. However, the available information has not been systematically considered with the intent to accurately estimate the magnitude of these effects. The present review suggests that under certain conditions, some of which may be avoidable, females may have at least a two to threefold increase in osteitis risk compared with males. It appears that this greater risk may be reduced by considering hormonal cycles when scheduling elective exodontia.

This nonrandomized prospective study was conducted in a private practice setting to determine the effect of a 0.12% chlorhexidine gluconate rinse (Peridex, Proctor and Gamble, Cincinnati, Ohio) on the incidence of dry socket after removal of impacted mandibular third molars. Over a 3-year period, 371 patients (total of 654 impacted mandibular third molars) received either no treatment (group 1), 2 weeks of twice daily Peridex rinse postsurgery (group 2), or one rinse presurgery (group 3). The group that used Peridex twice daily for 2 weeks after surgery (group 2) showed a significant reduction (56%) in the incidence of dry socket when compared with either the group that did not rinse (group 1) or the group that rinsed only once just before surgery (group 3). The incidence of dry socket was higher in smokers and in females who used oral contraceptives. Twice daily use of Peridex for 2 weeks also yielded significant reductions in dry socket in smokers, nonsmokers, and in females who did not use oral contraceptives. Results from this study demonstrate that the use of Peridex for 2 weeks after surgery is effective in prevention of dry socket after surgical extraction of impacted molars.

Post-procedural complications were assessed for 331 patients with AIDS after a wide range of outpatient dental procedures. Only patients with a CD4+ cell count < or = 200 cells/mm3 were included. Patients' charts were reviewed retrospectively by the treating dentist. The overall complication rate was 0.9 percent.

The aetiology of dry socket continues to be the subject of intense research. Local disturbances of fibrinolytic activity appear to be the most satisfactory explanation for the pathogenesis of this disorder. The factors that are responsible for local changes in fibrinolytic activity are reviewed and evaluated. In the management of this disorder attention should be directed towards the prevention of local changes in fibrinolytic activity.

Current literature has noted the various side effects of oral contraceptives. Oral surgeons have recently encountered an increased incidence of postoperative localized osteitis occurring with removal of manidibular third molars. This article documents the occurrence of this phenomenon among 105 women participating in a recent study at the Clinical Center of the National Institutes of Health.

Use of oral contraceptives was associated with a significant increase in the frequency of dry socket after extraction of mandibular third molars. The probability of dry socket increases with the estrogen dose in the oral contraceptive. The risk of dry socket associated with oral contraceptives can be minimized by performing extractions during days 23 through 28 of the tablet cycle.
