Abstract: Objectives: The challenges in diagnosing keratoconus (KC) have led researchers to explore the use of artificial intelligence (AI) as a diagnostic tool. AI has emerged as a new way to improve the efficiency of KC diagnosis. This study analyzed the use of AI as a diagnostic modality for KC. Methods: This study used a systematic review and meta-analysis following the 2020 Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. We searched selected databases using a combination of search terms: “((Artificial Intelligence) OR (Diagnostic Modality)) AND (Keratoconus)” from PubMed, Medline, and ScienceDirect within the last 5 years (2018–2023). Following a systematic review protocol, we selected 11 articles and 6 articles were eligible for final analysis. The relevant data were analyzed with Review Manager 5.4 software and the final output was presented in a forest plot. Results: This research found neural networks as the most used AI model in diagnosing KC. Neural networks and naïve bayes showed the highest accuracy of AI in diagnosing KC with a sensitivity of 1.00, while random forests were >0.90. All studies in each group have proven high sensitivity and specificity over 0.90. Conclusions: AI potentially makes a better diagnosis of the KC with its high performance, particularly on sensitivity and specificity, which can help clinicians make medical decisions about an individual patient.

Cold Recycled Mixtures (CRM) offer many benefits, including cost-effectiveness and sustainability. Large existing Reclaimed Asphalt Pavement (RAP) stockpiles that represent an environmental and storage challenge can be used for bitumen emulsion Cold Central Plant Recycling (CCPR). A pavement rehabilitation demonstration project using Cold Recycled Mixture with Bitumen Emulsion (CRM-E) as binder course with 100% RAP was carried out on August 25, 2020. The project was divided into three sections to evaluate the mill and pave strategy using CRM-E as an alternative to traditional Hot Mixture Asphalt (HMA). The wearing course was the same asphalt concrete for the three sections. The difference between Section 1 and 2 that used CRM-E was to either pave the asphalt concrete overlay on the same day or to delay its application for 3 days to give additional time for the CRM-E to cure. Section 3 had no CCPR and consisted of two HMA layers. The field performance of the rehabilitated sections was monitored in 4 phases after 14 days, 3 months, 6 months and 12 months. The performance of the sections was assessed based on Falling Weight Deflectometer (FWD), field rutting measurements and cracking distress surveys. Cores were also taken over time to compare the CRM-E and HMA binder course properties in terms of resilient modulus, dynamic creep, indirect tensile strength and tensile strength ratio. The research showed that CRM-E and HMA performance was similar in terms of FWD deflections, field rutting, in situ cracking, and moisture resistance of the field cores. CRM-E showed superior performance in terms of resilient modulus and dynamic creep compared with asphalt concrete binder course based on testing of field cores taken after 6 and 12 months. The research showed that same day CRM-E HMA overlay is a viable construction option.

Interstitial fluid (ISF) flow is identified as an essential physiological process that plays an important role in the development and progression of brain tumours. However, the relationship between the permeability of the tumour tissue, a complex porous medium, and the interstitial fluid flow around the tumour cells at the microscale is not well understood. To shed light on this issue, and in the absence of experimental techniques that can provide direct measurements, we develop a computational model to predict the tissue permeability of brain tumours in different grades by analysing the ISF flow at the pore scale. The 3-D geometrical models of tissue extracellular spaces are digitally reconstructed for each grade tumour based on their morphological properties measured from microscopic images. The predictive accuracy of the framework is validated by experimental results reported in the literature. Our results indicate that high-grade brain tumours are less permeable despite their higher porosity, whereas necrotic areas of glioblastoma are more permeable than the viable tumour areas. This implies that tissue permeability is primarily governed by both tissue porosity and the deposition of hyaluronic acid (HA), a key component of the extracellular matrix, while the HA deposition can have a greater effect than macro-level porosity. Parametric studies show that tissue permeability falls exponentially with increasing HA concentration in all grades of brain tumours, and this can be captured using an empirically derived relationship in a quantitative manner. These findings provide an improved understanding of the hydraulic properties of brain tumours and their intrinsic links to tumour microstructure. This work can be used to reveal the intratumoural physiochemical processes that rely on fluid flow and offer a powerful tool to tune textured and porous biomaterials for desired transport properties. STATEMENT OF SIGNIFICANCE: Interstitial fluid flow in the extracellular space of brain tumours plays a crucial role in their progression, development, and response to drug treatments. However, the mechanisms of interstitial fluid transport around tumour cells and the characterization of these microscale transports at the tissue scale to meet clinical requirements are largely unknown. In the absence of advanced experimental techniques to capture these pore-scale transport phenomena, we have developed and validated a computational framework to successfully reveal these phenomena across all grades of brain tumours. Background: Cerebral Palsy (CP) is a major cause of motor and cognitive disability in children due to injury to the developing brain. Early intensive sensorimotor rehabilitation has been shown to change brain structure and reduce CP symptoms severity. We combined environmental enrichment (EE) and treadmill training (TT) to observe the effects of a one-week program of sensorimotor stimulation (EETT) in animals exposed to a CP model and explored possible mechanisms involved in the functional recovery., Methods: Pregnant Wistar rats were injected with Lipopolysaccharide (LPS - 200 µg/kg) intraperitoneally at embryonic days 18 and 19. At P0, pups of both sexes were exposed to 20' anoxia at 37 °C. From P2 to P21, hindlimbs were restricted for 16 h/day during the dark cycle. EETT lasted from P21 to P27. TT - 15 min/day at 7 cm/s. EE - 7 days in enriched cages with sensorimotor stimulus. Functional 3D kinematic gait analysis and locomotion were analyzed. At P28, brains were collected for ex-vivo MRI and histological assessment. Neurotrophins and key proteins involved in CNS function were assessed by western blotting., Results: CP model caused gross and skilled locomotor disruption and altered CNS neurochemistry. EETT reversed locomotor dysfunction with minor effects over gait kinematics. Brain disorders represent an ever-increasing health challenge worldwide. While conventional drug therapies are less effective due to the presence of the blood-brain barrier, infusion-based methods of drug delivery to the brain represent a promising option. Since these methods are mechanically controlled and involve multiple physical phases ranging from the neural and molecular scales to the brain scale, highly efficient and precise delivery procedures can significantly benefit from a comprehensive understanding of drug-brain and device-brain interactions. Behind these interactions are principles of biophysics and biomechanics that can be described and captured using mathematical models. Although biomechanics and biophysics have received considerable attention, a comprehensive mechanistic model for modeling infusion-based drug delivery in the brain has yet to be developed. Therefore, this article reviews the state-of-the-art mechanistic studies that can support the development of next-generation models for infusion-based brain drug delivery from the perspective of fluid mechanics, solid mechanics, and mathematical modeling. The supporting techniques and database are also summarized to provide further insights. Finally, the challenges are highlighted and perspectives on future research directions are provided. STATEMENT OF SIGNIFICANCE: Despite the immense potential of infusion-based drug delivery methods for bypassing the blood-brain barrier and efficiently delivering drugs to the brain, achieving optimal drug distribution remains a significant challenge. Background: Hospitalized neonates are vulnerable to infection and have high rates of antibiotic utilization., Methods: Fourteen South African neonatal units (seven public, seven private sector) assembled multidisciplinary teams involving neonatologists, microbiologists, pharmacists, and nurses to implement prospective audit and feedback neonatal antimicrobial stewardship (NeoAMS) interventions. The teams attended seven online training sessions. Pharmacists conducted weekday antibiotic prescription reviews in the neonatal intensive care unit and/or neonatal wards providing feedback to the clinical teams. Anonymized demographic and NeoAMS interventions data were aggregated for descriptive purposes and statistical analysis., Findings: During the 20-week NeoAMS intervention in 2022, 565 neonates were enrolled. Pharmacists evaluated seven hundred antibiotic prescription episodes; rule-out sepsis (180; 26%) and culture-negative sepsis (138; 20%) were the most frequent indications for antibiotic prescription. For infection episodes with an identified pathogen, only 51% (116/229) of empiric treatments provided adequate antimicrobial coverage. Pharmacists recommended 437 NeoAMS interventions (0·6 per antibiotic prescription episode), with antibiotic discontinuation (42%), therapeutic drug monitoring (17%), and dosing (15%) recommendations most frequent. Neonatal clinicians' acceptance rates for AMS recommendations were high (338; 77%). Mean antibiotic length of therapy decreased by 24% from 9·1 to 6·9 days (0·1 day decrease per intervention week; P = 0·001), with the greatest decline in length of therapy for culture-negative sepsis (8·2 days (95% CI 5·7-11·7) to 5·9 days (95% CI 4·6-7·5); P = 0·032)., Interpretation: This neonatal AMS programme was successfully implemented in heterogenous and resource-limited settings. Pharmacist-recommended AMS interventions had high rates of clinician acceptance. Background: Variants in Filamin-C (FLNC) have been associated with various hereditary cardiomyopathies. Recent literature reports a prevalence of sudden cardiac death (SCD) of 13-25% among carriers of truncating-variants, with mean age of 42±15 years for first SCD event. This study reports two familial cases of SCD and the results of cascade screening of their large family., Methods: Molecular-autopsy of the SCD victims revealed a novel truncating-variant in the FLNC gene (chr 7:128496880 [hg19]; NM_001458.5; c.7467_7474del; p.(Ser2490fs)). We screened thirty-two family members following genetic counseling, and variant carriers underwent a comprehensive workup followed by consultation with a cardiologist with expertise in the genetics of cardiac diseases., Results: Seventeen variant carriers were identified: ages between 9 and 85 (mean 47±26). Fifteen underwent clinical evaluation. To date, none of the identified carriers has had major adverse events. In evaluated patients, ECG showed right-axis deviation in 60% (n = 9). Holter recorded frequent premature ventricular contractions (PVCs) (991±2030 per 24 h) in 33% (n = 5) with 4 patients having polymorphic PVC morphology. Three carriers had echocardiographic evidence of mild left-ventricular (LV) systolic dysfunction and another with mild LV dilatation. Cardiac magnetic-resonance (CMR) exhibited late‑gadolinium-enhancement in 10 out of 11 exams, mainly in the mid-myocardium and sub-epicardium, frequently involving the septum and the inferior-lateral wall., Conclusion: This large FLNC truncating variant carrier family exhibits high cardiomyopathy penetrance, best diagnosed by CMR, with variable clinical expressions. The constitutive model for the porosity-permeability relationship is a powerful tool to estimate and design the transport properties of porous materials, which has attracted significant attention for the advancement of novel materials. However, in comparison with other materials, biomaterials, especially natural and artificial tissues, have more complex microstructures e.g. high anisotropy, high randomness of cell/fibre dimensions/position and very low porosity. Consequently, a reliable microstructure-permeability relationship of fibrous biomaterials has proven elusive. To fill this gap, we start a mathematical derivation from the fundamental brain white matter (WM) formed by nerve fibres. This is augmented by a numerical characterisation and experimental validations to obtain an anisotropic permeability tensor of the brain WM as a function of the tissue porosity. A versatile microstructure generation software (MicroFiM) for fibrous biomaterial with complex microstructure and low porosity was built accordingly and made freely accessible here. Moreover, we propose an anisotropic poro-hyperelastic model enhanced by the newly defined porosity-permeability tensor relationship which precisely captures the tissues macro-scale permeability changes due to the microstructural deformation in an infusion scenario. The constitutive model, theories and protocols established in this study will both provide improved design strategies to tailor the transport properties of fibrous biomaterials and enable the non-invasive characterisation of the transport properties of biological tissues. This will lead to the provision of better patient-specific medical treatments, such as drug delivery. STATEMENT OF SIGNIFICANCE: Due to the microstructural complexity, a reliable microstructure-permeability relationship of fibrous biomaterials has proven elusive, which hinders our way of tuning the fluid transport property of the biomaterials by directly programming their microstructure. The same problem hinders non-invasive characterisations of fluid transport properties in biological tissues, which can significantly improve the efficiency of treatments e.g. drug delivery, directly from the tissues accessible microstructural information, e.g. porosity. Here, we developed a validated mathematical formulation to link the random microstructure to a fibrous material's macroscale permeability tensor. This will advance our capability to design complex biomaterials and make it possible to non-invasively characterise the permeability of living tissues for precise treatment planning. Many brain disorders, including Alzheimer's Disease and Parkinson's Disease, and drug delivery procedures are linked to fluid transport in the brain; yet, while neurons are extremely soft and can be easily deformed, how the microscale channel flow interacts with the neuronal structures (especially axons) deformation and how these interactions affect the macroscale tissue function and transport properties is poorly understood. Misrepresenting these relationships may lead to the erroneous prediction of e.g. disease spread, drug delivery, and nerve injury in the brain. However, understanding fluid-neuron interactions is an outstanding challenge because the behaviours of both phases are not only dynamic but also occur at an extremely small length scale (the width of the flow channel is ∼100 nm), which cannot be captured by state-of-the-art experimental techniques. Here, by explicitly simulating the dynamics of the flow and axons at the microstructural level, we, for the first time, establish the link between micromechanical tissue response to the physical laws governing the macroscopic transport property of the brain white matter. We found that interactions between axons and the interstitial flow are very strong, thus playing an essential role in the brain fluid/mass transport. Furthermore, we proposed the first anisotropic pressure-dependent permeability tensor informed by microstructural dynamics for more accurate brain modelling at the macroscale, and analysed the effect of the variation of the microstructural parameters that influence such tensor. These findings will shed light on some unsolved issues linked to brain functions and medical treatments relying on intracerebral transport, and the mathematical model provides a framework to more realistically model the brain and design brain-tissue-like biomaterials. STATEMENT OF SIGNIFICANCE: This study reveals how neurons interact with the fluid flowing around them and how these microscale interactions affect macroscale transport behaviour of the brain tissue. The findings provide unprecedented insights into some unsolved issues linked to brain functions and medical treatments relying on intracerebral fluid transport. Furthermore, we, for the first time, established a microstructure-informed permeability tensor as a function of local hydraulic pressure and pressure gradient for the brain tissue, which inherently captures the dynamic transport property of the brain. The genus Gasterophilus (Diptera, Gastrophilidae) is an obligate parasite of the equine family that causes widespread myiasis in desert steppe. Based on four common naturally excreted Gasterophilus larvae collected systematically in the Karamaili Ungulate Nature Reserve from March to September 2021, this paper studies the population dynamics and ontogenetic laws of horse flies, and discuss the coexistence pattern and population dynamics prediction of horse flies. The results showed that the Gasterophilus larvae had obvious concentrated development period, and the time of population peaks was different, the earliest was G. nigricornis (late March), followed by G. pecorum-Ⅰ (mid-April), G. nasalis (late April), G. intestinalis (early May), G. pecorum-Ⅱ (mid-August). The order of development threshold temperature "C nigricornis < C pecorum-Ⅰ ≤ C pecorum -Ⅱ < C nasalis < C intestinalis " is consistent with the peak order of different larval populations. The life history survival rate (L) was as follows: L nigricornis (83.97%) ≥ L intestinalis (81.25%) > L nasalis (72.42%) ≥ L pecorum-Ⅱ (71.65%) > L pecorum-Ⅰ (39.23%). This study combined indoor experiments and field surveys revealed the development of horse fly populations with different life strategies in desert grasslands. Based on the different development threshold temperatures of several horse flies, the staggered population dynamics of Gasterophilus form continuous infection stress on the host. Brain mechanics is a topic of deep interest because of the significant role of mechanical cues in both brain function and form. Specifically, capturing the heterogeneous and anisotropic behaviour of cerebral white matter (WM) is extremely challenging and yet the data on WM at a spatial resolution relevant to tissue components are sparse. To investigate the time-dependent mechanical behaviour of WM, and its dependence on local microstructural features when subjected to small deformations, we conducted atomic force microscopy (AFM) stress relaxation experiments on corpus callosum (CC), corona radiata (CR) and fornix (FO) of fresh ovine brain. Our experimental results show a dependency of the tissue mechanical response on axons orientation, with e.g. the stiffness of perpendicular and parallel samples is different in all three regions of WM whereas the relaxation behaviour is different for the CC and FO regions. An inverse modelling approach was adopted to extract Prony series parameters of the tissue components, i.e. axons and extra cellular matrix with its accessory cells, from experimental data. Using a bottom-up approach, we developed analytical and FEA estimates that are in good agreement with our experimental results. Our systematic characterisation of sheep brain WM using a combination of AFM experiments and micromechanical models provide a significant contribution for predicting localised time-dependent mechanics of brain tissue. This information can lead to more accurate computational simulations, therefore aiding the development of surgical robotic solutions for drug delivery and accurate tissue mimics, as well as the determination of criteria for tissue injury and predict brain development and disease progression., (Copyright © 2021 Elsevier Ltd. Indonesia, the world's largest archipelagic country and the fourth most populated nation, has struggled with coastal water pollution in the last decades. With the increasing population in coastal urban cities, more land-based pollutants are transported to the coastal water and adversely affected the tropical ecosystems. This paper provides an overview of anthropogenic pollutant studies in Indonesian coastal water and ecosystems from 1986 to 2021. Nutrients, heavy metals, organic pollutants, and plastic debris are the most-studied contaminants. We found that 82%, 54% and 50% of the studies exceeding nutrients, heavy metals, and organic pollutants standard limit, respectively; thus, indicating poor water quality status in part of Indonesian coastal water. The coral reef ecosystems is found to be the most sensitive to anthropogenic disturbance. The potential effect of climate change, new coastal pollution hotspots in eastern Indonesia, marine anthropogenic sources, legacy/emerging pollutants, and the need for research related to the biological contamination, are discussed for future opportunities., (Copyright © 2021 Elsevier Ltd. Brain tissue is a heterogeneous material, constituted by a soft matrix filled with cerebrospinal fluid. The interactions between, and the complexity of each of these components are responsible for the non-linear rate-dependent behaviour that characterises what is one of the most complex tissue in nature. Here, we investigate the influence of the cutting rate on the fracture properties of brain, through wire cutting experiments. We also present a computational model for the rate-dependent behaviour of fracture propagation in soft materials, which comprises the effects of fluid interaction through a poro-hyperelastic formulation. The method is developed in the framework of finite strain continuum mechanics, implemented in a commercial finite element code, and applied to the case of an edge-crack remotely loaded by a controlled displacement. Loading-induced cartilage exudation causes loss of fluid from the tissue, joint space thinning and, in a long term prospective, the insurgence of osteoarthritis. Fortunately, experiments show that joints recover interstitial fluid and thicken during articulation after static loading, thus reversing the exudation process. Here, we provide the first original theoretical explanation to this crucial phenomenon, by implementing a numerical model capable of accounting for the multiscale porous lubrication occurring in joints. We prove that sliding-induced rehydration occurs because of hydrodynamic reasons and is specifically related to a wedge effect at the contact inlet. Furthermore, numerically predicted rehydration rates are consistent with experimentally measured rates and corroborate the robustness of the model here proposed. The paper provides key information, in terms of fundamental lubrication multiscale mechanisms, to understand the rehydration of cartilage and, more generally, of any biological tissue exhibiting a significant porosity: such a theoretical framework is, thus, crucial to inform the design of new effective cartilage-mimicking biomaterials. STATEMENT OF SIGNIFICANCE: Motion and, precisely, joints articulation ensures that cartilage tissues preserve adequate level of hydration and, thus, maintain excellent mechanical properties in terms of high resilience, considerable load-carrying capacity and remarkably low friction. Conversely, when statically loaded, cartilage starts to exudate, causing joint space thinning and, in the long term, possible osteoarthritis; joints motion is, thus, the key to prevent the degradation of the tissues. By developing a numerical multiscale lubrication theory, and by corroborating this approach with experiments, we provide the first original theoretical explanation to this motion-induced cartilage rehydration mechanism. Many studies on human intestinal microbiota indicate that gender difference is one of the key factors influencing microbial community composition. To date, the degree of influence that gender has on equid intestinal microbiota has not been reported. Thus, microbiota was analyzed in feces of seven female Przewalski's horses (FRPHs) and seven male Przewalski's horses (MRPHs) in this study, determining which microbiota characteristics respond to gender biases. The microbial community composition and structure were explored by 16S rRNA sequencing, followed by diversity analysis and difference analysis. Female Przewalski's horses showed higher Shannon diversity than MRPHs, no difference in Simpson diversity, and displayed difference in beta diversity. Although gender had little effect on the overall microbiota, it significantly changed the dominant microbial community in each classification level. Male Przewalski's horses contained significantly higher amounts of microorganisms related with diseases, including spirochetes (phylum), deltaproteobacteria (class), fibrobacteria (class), spirochaetia (class), desulfovibrionales (order), fibrobacterales (order), spirochaetales (order), and spirochaetaceae (family). Female Przewalski's horses showed less than MRPHs in the top 10 genera. To our knowledge, this study is the first to document the gender-related intestinal microbiota profile in equines and discovered notable differences between the gender, which suggests that gender should be considered as a biological variable in future microbiota studies., (Copyright © 2020 Elsevier Inc. Our sense of fine touch deteriorates as we age, a phenomenon typically associated with neurological changes to the skin. However, geometric and material changes to the skin may also play an important role on tactile perception and have not been studied in detail. Here, a finite element model is utilised to assess the extent to which age-related structural changes to the skin influence the tactile stimuli experienced by the mechanoreceptors. A numerical, hyperelastic, four-layered skin model was developed to simulate sliding of the finger against a rigid surface. The strain, deviatoric stress and strain energy density were recorded at the sites of the Merkel and Meissner receptors, whilst parameters of the model were systematically varied to simulate age-related geometric and material skin changes. The simulations comprise changes in skin layer stiffness, flattening of the dermal-epidermal junction and thinning of the dermis. It was found that the stiffness of the skin layers has a substantial effect on the stimulus magnitudes recorded at mechanoreceptors. Additionally, reducing the thickness of the dermis has a substantial effect on the Merkel disc whilst the Meissner corpuscle is particularly affected by flattening of the dermal epidermal junction. In order to represent aged skin, a model comprising a combination of ageing manifestations revealed a decrease in stimulus magnitudes at both mechanoreceptor sites. The result from the combined model differed from the sum of effects of the individually tested ageing manifestations, indicating that the individual effects of ageing cannot be linearly superimposed. Each manifestation of ageing results in a decreased stimulation intensity at the Meissner Corpuscle site, suggesting that ageing reduces the proportion of stimuli meeting the receptor amplitude detection threshold. This model therefore offers an additional biomechanical explanation for tactile perceptive degradation amongst the elderly. Applications of the developed model are in the evaluation of cosmetics products aimed at mitigating the effects of ageing, e.g. through skin hydration and administration of antioxidants, as well as in the design of products with improved tactile sensation, e.g. through the optimisation of materials and surface textures., (Copyright © 2020 Elsevier Ltd. The COVID-19 outbreak in China led to an extraordinary threat to public health and wellbeing. This study examined the psychological impact of media use among people indirectly exposed to the disease during the initial phase of the outbreak. We conducted an internet-based survey on January 28, 2020 (one week after the official declaration of person-to-person transmission of the coronavirus). Media use (media forms, content of media exposure, and media engagement) related to the outbreak and psychological outcomes (positive and negative affect, anxiety, depression, and stress) of 917 Chinese adults was assessed. A series of multivariable regressions were conducted. The results showed that use of new media, rather than traditional media, was significantly associated with more negative affect, depression, anxiety, and stress. Viewing stressful content (i.e., severity of the outbreak, reports from hospital) was associated with more negative affect and depression. Media engagement was also associated with more negative affect, anxiety, and stress. However, viewing heroic acts, speeches from experts, and knowledge of the disease and prevention were associated with more positive affect and less depression. The study suggested new media use and more media engagement was associated with negative psychological outcomes, while certain media content was associated with positive psychological impact. A new electroanalytical method has been developed for the determination of polar antioxidant compounds in extra virgin olive oils. This method is based on the extraction of polar antioxidant compounds from extra-virgin olive oils by means of a deep eutectic solvent and their determination by a modified screen-printed electrode platform. The platform sensitivity was increased by modifying the working electrode with MWCNT and TiO 2 nanoparticles as modifiers and Nafion as a binder. The platform showed very good sensitivity in detecting polar antioxidant compounds in extra-virgin olive oils in a fairly wide range of concentrations. The measurements were performed by using square wave voltammetry. The extraction was performed without using organic solvents, making the method environmentally friendly. The proposed method has been compared with a common spectrophotometric one, the results appeared in good agreement. Objective: To determine the rate of sonographic placental markers and their predictive value for preeclampsia and fetal growth restriction in women with chronic kidney disease (CKD)., Study Design: A retrospective cohort study of women with CKD followed at a tertiary referral center between 2016 and 2019 (n = 86). All women underwent 2nd trimester sonographic placental examinations that included assessment of placental morphology, umbilical cord, and uterine artery Doppler. Continuous placental markers were converted to multiples on medians (MoM)., Main Outcome Measures: Predictive value of sonographic markers for preeclampsia and birthweight < 10th percentile., Results: Women in the cohort had a high rate of preeclampsia (24.4%), birthweight < 10th% (26.7%), and preterm birth (30.2%). The most important markers were placental volume and uterine artery Doppler: the risk of preeclampsia was elevated in women with low placental volume (51.7% vs. 10.9%; OR = 8.79 [2.70-28.59] for preeclampsia; and 40.0% vs. 9.1%; OR = 6.67 [1.85-24.04] for preterm preeclampsia), and in women with bilateral uterine artery notching (62.5% vs. 20.8%; OR = 6.35 [1.37-29.45] for preeclampsia; and 62.5% vs. 10.4%; OR = 14.38 [1.29-71.75] for preterm preeclampsia). The combination of both markers had the strongest predictive value for preeclampsia (positive likelihood ratio = 8.25 [6.84-9.95]). Low placental volume and bilateral uterine notching were also associated with birthweight < 10th percentile., Conclusion: A 2nd-trimester sonographic placental study can identify a subgroup of women with CKD who are at most risk of preeclampsia and fetal growth restriction. Submarine groundwater discharge (SGD) can be an important pathway for chemical or biological pollutants from land to the ocean around the world. However, studies on the microbial communities associated with SGD in Southeast Asia, which has been hypothesized as SGD hotspot, remain scarce. In this study, we examined the microbial community composition with 16S rRNA gene sequencing along the hydrological continuum of an SGD site in a tropical urban area of Indonesia. Of the observed parameters in this study, salinity and temperature were the most determinant variables explaining patterns in microbial community composition. The bacterial taxon Burkholderiaceae was predominantly found in low salinity samples, including those from terrestrial groundwater and brackish pore water, while cyanobacteria of the genus Synechococcus sp. CC9902 were indicative of saline SGD and seawater samples. The composition of microbial taxa in each sample pointed to the influence of shallow terrestrial groundwater in the beach pore water, while seawater recirculation dominated the SGD sampling points situated further offshore. We identified taxa containing fecal indicators and potential pathogens at the SGD compartments; however, while a likely explanation, we could not conclude with certainty that SGD was a conduit for these bacteria. Overall, the results from this study show that microbial community analysis can highlight hydrological processes and water quality at the SGD site; thus, they could be useful for environmental policymakers to formulate water management strategies in coastal areas., (Copyright © 2019 Elsevier B.V. A newly modified electrode based on glassy carbon (GC) has been prepared and characterized electrochemically for application in electroanalytical chemistry. In particular, a GC screen-printed electrode (SPE) has been modified with nanostructures, namely multi-walled carbon nanotubes (MWCNTs), and TiO2 nanoparticles, and combined with a new generation of eco-friendly room-temperature ionic liquids (RTILs). The green RTILs here used are suitable for the immobilization of enzymes on the electrode surface and, additionally, facilitate the kinetics of electron transfer due to their intrinsic electrical conductivity. Upon evaluation of these newly modified electrodes we found an improvement in terms of electrochemically active area (Aea) with respect to the electrodes we previously reported. The modified SPEs were then used as substrates for the construction of two enzymatic biosensors for analytical applications: the first is an enzymatic biosensor based on alcohol dehydrogenase (ADH) for the analysis of ethyl alcohol; the second biosensor is based on lipase enzyme and has been tested for the analysis and the classification of Extra Virgin Olive Oil (EVOO). The performances of the here projected sensors appear comparable with biosensors having similar finalities. Objectives: Previous meta-analyses of root caries incidence and increment studies reported different estimates due to the limited number of studies, heterogeneity and variations in studies included. Currently, new publications and approaches to handle heterogeneity are available. This research aims to systematically review and meta-analyse root caries incidence and increment, and use meta-regression to analyse heterogeneity., Sources: PUBMED and EMBASE databases were searched systematically., Study Selection: Longitudinal studies on root caries incidence and increment, published in English language prior to 2017, were independently checked by two authors. A pooled incidence and increment of decayed/filled root surfaces (DFS) was estimated and meta-regression analysis was performed by length of follow-up (<2 years; 2years; 3-4years and ≥5years) and study type (observational population-based and clinical trial)., Data: Of 737 articles, 20 were included for meta-analysis. The annualised root caries incidence and increment were 18.25%[CI = 13.22%-23.28%] and 0.45[CI = 0.37-0.53] root DFS respectively. Length of follow-up influenced the estimates, but not the study type. The annual root DFS incidence and increment from studies <2years were 32.95%[CI = 29.13%-36.77%] and 0.64[CI = 0.38-0.89] root surfaces respectively. Studies with 5+years follow-up, the annualised root caries incidence and increment were 9.4%[CI = 3.32%-15.48%] and 0.43[CI = 0.21-0.64] root surfaces respectively., Conclusions: Length of follow-up influenced root caries estimates due to a bias towards relatively healthier older adults retained in the study. Root caries increased over time even among the healthier older adults., Clinical Significance: The increase in root caries, even among the healthier older adults, should be considered by both clinicians and healthcare planners/policy makers in their provision of services., (Copyright © 2018 Elsevier Ltd. Groundwater discharge is known to transport nutrients into estuaries at several locations around the world. However, few studies report groundwater-associated nutrient fluxes from tropical developing regions such as Southeast Asia, even though this area shows the strongest human modifications in the coastal zone worldwide. We investigated groundwater nutrient flux into two streams and estuaries (Awur and Sekumbu Bay) in the urban area of Jepara, Indonesia, and its relation with the land usage surrounding the estuaries. We found that average concentrations of NO 3 , NH 4 , and PO 4 in Jepara's aquifer reached 145μM, 68μM, and 14μM, respectively, and our results indicate that these were mainly originated from untreated sewage, agriculture, and manure input. Approximately 2200tonNyear -1 and 380tonPyear -1 were removed in the soil and aquifer before the nutrients were discharged into the river. The total groundwater discharge into the river and estuary was estimated to 461×10 3 m 3 d -1 , or up to 42% of the river discharge. Discharge of groundwater-associated NO 3 (72×10 3 mold -1 ), NH 4 (34×10 3 mold -1 ), PO 4 (5×10 3 mold -1 ), and additional surface runoff may contribute to eutrophication and a decrease of nearshore surface water quality. Nutrient concentrations in groundwater, river, and coastal seawater in the Jepara region are similar to those found in major urban areas in Southeast Asia, e.g. Manila and Bangkok, even though Jepara has smaller size and population. Thus, our results indicate that medium populated cities with highly modified regional land use can contribute a significant amount of nutrient discharge in the coastal area and should be included in global assessments of nutrient budget calculation., (Copyright © 2018. This research investigated the in-vitro wear and friction performance of polycarbonate urethane (PCU) 80A as they interact with articular cartilage, using a customised multidirectional pin-on-plate tester. Condyles were articulated against PCU 80A discs (Bionate ® I and Bionate ® II) (configuration 1) and the results arising from these tests were compared to those recorded during the sliding of PCU pins against cartilage plates (configuration 2). Configuration 1 produced steadily increasing coefficient of friction (COF) (up to 0.64 ± 0.05) and had the same trend as the cartilage-on-stainless steel articulation (positive control). When synovial fluid rather than bovine calf serum was used as lubricant, average COF significantly decreased from 0.50 ± 0.02-0.38 ± 0.06 for condyle-on-Bionate ® I (80AI) and from 0.41 ± 0.02-0.24 ± 0.04 for condyle-on-Bionate ® II (80AII) test configurations (p < 0.05). After 15 h testing, the cartilage-on-cartilage articulation (negative control) tests showed no cartilage degeneration. However, different levels of cartilage volume loss were found on the condyles from the positive control (12.5 ± 4.2 mm 3 ) and the PCUs (20.1 ± 3.6 mm 3 for 80 AI and 19.0 ± 2.3 mm 3 for 80AII) (p > 0.05). A good correlation (R 2 =0.84) was found between the levels of average COF and the volume of cartilage lost during testing; increasing wear was found at higher levels of COF. Configuration 2 showed low and constant COF values (0.04 ± 0.01), which were closer to the negative control (0.03 ± 0.01) and significantly lower than configuration 1 (p < 0.05). The investigation showed that PCU is a good candidate for use in hemiarthroplasty components, where only one of the two articulating surfaces is replaced, as long as the synthetic material is implanted in a region where migrating cartilage contact is achieved. This research investigated in-vitro tribological performance of the articulation of cartilage-on- polyvinyl alcohol (PVA) and polyvinyl pyrrolidone (PVP) blend hydrogels using a custom-designed multi-directional wear rig. The hydrogels were prepared by repeated freezing-thawing cycles at different concentrations and PVA to PVP fractions at a given concentration. PVA/PVP blend hydrogels showed low coefficient of friction (COF) values (between 0.12 ± 0.01 and 0.14 ± 0.02) which were closer to the cartilage-on-cartilage articulation (0.03 ± 0.01) compared to the cartilage-on-stainless steel articulation (0.46 ± 0.06). The COF increased with increasing hydrogel concentration (p = 0.03) and decreasing PVP content at a given concentration (p < 0.05). The cartilage-on-hydrogel tests showed only the surface layers of the cartilage being removed (average volume loss of the condyles was 12.5 ± 4.2mm 3 ). However, the hydrogels were found to be worn/deformed. The hydrogels prepared at a higher concentration showed lower apparent volume loss. A strong correlation (R 2 = 0.94) was found between the COF and compressive moduli of the hydrogel groups, resulting from decreasing contact congruency. Background: The outcomes of pregnancy and subsequent delivery of healthy neonates in women who have undergone previous near total hemispherectomy for Sturge-Weber syndrome (SWS) have rarely been reported., Case: A woman with SWS had two successful and uncomplicated pregnancies and deliveries under epidural anaesthesia after undergoing a near total hemispherectomy with subsequent unilateral cerebral palsy. Non-contrast MRI of the brain revealed post hemispherectomy changes with no residual meningeal angiomatosis., Conclusion: As more women who undergo surgical resection for underlying SWS grow into the child-bearing years, additional reports of pregnancy and delivery outcomes in this patient population can be expected and will be valuable.

Objective: Abnormal serum screening markers have been associated with adverse pregnancy outcomes. We sought to review the performance of combined abnormal first and/or second trimester maternal serum markers used in prenatal screening for aneuploidy and open neural tube defects for predicting preeclampsia (PET), small for gestational age (SGA), and stillbirth beyond 24 weeks' gestation., Data Sources and Study Selection: Medline, EMBASE, and Cochrane Library databases were searched for studies from 1970 to May 2010 that analyzed predictive abilities of combined serum markers for defined outcomes., Data Extraction and Synthesis: Data were extracted independently by two authors, and 15 studies were included. Eight studies of 115,290 pregnancies, 11 studies of 144 853 pregnancies, and seven studies of 80 274 pregnancies examined PET, SGA, and stillbirth respectively. Because of the heterogeneity of marker combinations and thresholds, outcome definitions, and analytic methods, limited meta-analysis was possible for the outcomes of PET and SGA only. Three relatively homogeneous studies on prediction of PET, and two on prediction of SGA were meta-analyzed. Several single studies demonstrated utility in combining markers to predict adverse outcome; however, this effect was not confirmed after meta-analysis. The most common combination of markers evaluated was alpha fetoprotein and human chorionic gonadotrophin for all outcomes. The highest positive likelihood ratios for predicting PET (5.68; 95% CI 0.73 to 43.97) and SGA (6.18; 95% CI 1.84 to 20.85) were seen with combined alpha fetoprotein and human chorionic gonadotrophin (> 2.5 multiples of the median)., Conclusions: Currently, no identifiable combination of serum markers performs well as a screening test for preeclampsia, small for gestational age, and stillbirth beyond 24 weeks. Large cohort studies with standardized screening test parameters and outcomes are needed.

Objectives: We wished to determine prescribing practices of obstetricians in Canada regarding tocolytics, antenatal corticosteroids, and progesterone for women at increased risk of preterm labour and birth, and to determine whether these practices changed between 1997-98 and 2004., Methods: Two cross-sectional surveys of Canadian obstetricians were conducted. The initial survey was in 1997-98 (N = 1313); the follow-up survey was in 2004 (N = 1508)., Results: Response rates were 46.4% in 1997-98 and 43.3% in 2004. Most respondents reported that they prescribed tocolytics to women with signs and symptoms of preterm labour (97.4% in 1997-98; 92.2% in 2004; P < 0.001) but use had decreased. In 1997-98, 20.0% of respondents used tocolytics for > 48 hours during one course of treatment, whereas in 2004, only 9.6% did this (P = 0.06). The tocolytic prescribed most frequently was magnesium sulphate in 1997-98 (40.6% of respondents), whereas in 2004, it was indomethacin (47.5% of respondents). The use of repeat courses of antenatal corticosteroids decreased from 72.9% in 1997-98 to 18.7% in 2004 (P < 0.001). In 2004, only a few respondents (7.0%) prescribed progesterone for women at increased risk of preterm birth., Conclusions: Most Canadian obstetricians continue to use tocolytics for women in preterm labour, although use has decreased over time, and the most frequently prescribed tocolytic has changed from magnesium sulphate to indomethacin. The use of repeat courses of antenatal corticosteroids decreased substantially during this time, and in 2004, progesterone was not in frequent use.

Background: Pneumatic compression is a frequently prescribed physical therapy for patients affected by postmastectomy lymphedema but, despite its wide use, its efficacy has not been demonstrated in phase III studies. We performed a randomized study comparing pneumatic compression versus no treatment in patients with postmastectomy lymphedema., Patients and Methods: Patients with monolateral postmastectomy lymphedema were randomized to receive two cycles of intermittent pneumatic compression (PC group), i.e., five two-hour sessions per week for two weeks, to be repeated after a five-week interval, or to no treatment (control group). The patients in both groups were instructed as to the prophylactic hygienic care of the limb. Lymphedema was assessed by the sum of differences in circumference measurements between affected and normal limbs ('delta'). Response was defined as a > or = 25% reduction in delta value., Results: Eighty patients entered the study. No statistically significant differences in response rates between the two groups were observed: 20% in the control group (95% CI: 9%-36%), 25% in the PC group (95% CI: 13%-41%, P = 0.59). The absolute mean decrease in delta value was 1.9 +/- 3.7 cm in the PC group and 0.5 +/- 3.3 cm in the control group., Conclusions: We demonstrated that intermittent pneumatic compression has a limited clinical role in the treatment of postmastectomy lymphedema. Efforts to prevent this complication should be undertaken.

A controlled, randomized trial was carried out to evaluate the effectiveness of a new conservative treatment for postmastectomy lymphedema. Seventy-four patients were randomized to wear only an elastic sleeve for six months (group A) or to receive additionally an induction treatment with electrically stimulated lymphatic drainage (group B). The evaluation of results was focused not only on the mean variations in limb measurements in the two groups, but also on the frequency of clinically significant reductions of lymphedema (i.e., at least 25% compared to the initial values). A mean reduction of limb girth of about 17% was obtained in both groups, without significant differences between the two treatments. An 'objective response', however, was not achieved by the majority of patients. On the other hand, compliance to therapy was good and a significant result could be obtained simply and economically also in the group of patients treated only with the elastic sleeve.
