Digital, Background Guidelines recommend nonstatin lipid-lowering agents in patients at very high risk for major adverse cardiovascular events (MACE) if low-density lipoprotein cholesterol (LDL-C) remains ≥70 mg/dL on maximum tolerated statin treatment. It is uncertain if this approach benefits patients with LDL-C near 70 mg/dL. Lipoprotein(a) levels may influence residual risk. Objectives In a post hoc analysis of the ODYSSEY Outcomes (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) trial, the authors evaluated the benefit of adding the proprotein subtilisin/kexin type 9 inhibitor alirocumab to optimized statin treatment in patients with LDL-C levels near 70 mg/dL. Effects were evaluated according to concurrent lipoprotein(a) levels. Methods ODYSSEY Outcomes compared alirocumab with placebo in 18,924 patients with recent acute coronary syndromes receiving optimized statin treatment. In 4,351 patients (23.0%), screening or randomization LDL-C was 13.7 mg/dL or ≤13.7 mg/dL; corresponding adjusted treatment hazard ratios were 0.82 (95% CI: 0.72-0.92) and 0.89 (95% CI: 0.75-1.06), with Pinteraction = 0.43. Conclusions In patients with recent acute coronary syndromes and LDL-C near 70 mg/dL on optimized statin therapy, proprotein subtilisin/kexin type 9 inhibition provides incremental clinical benefit only when lipoprotein(a) concentration is at least mildly elevated. - Abstract
Cerebral venous thrombosis (CVT) accounts for approximately 1% of all stroke presentations and due to variable clinical presentation, it can present a diagnostic difficulty. The purpose of this review is to re-iterate the usefulness of noncontrast CT brain (CTB) in detecting CVT. In this pictorial essay, we highlight our experience in multiple cases where unenhanced CTB demonstrated imaging features of CVT allowing early diagnosis and ensuing prompt management of CVT. This reduces reliance on additional imaging modalities which may prove beneficial in departments where modalities such as magnetic resonance venogram which is considered gold standard imaging for CVT, are not readily available. - Abstract
Background: Vancomycin is a first-line antibiotic for methicillin-resistant Staphylococcus aureus infections or other Gram-positive infections. The area under the curve (AUC) to minimum inhibitory concentration (MIC) ratio is proposed as a therapeutic drug-monitoring parameter. How well clinical efficacy is predicted by this measure has not been established., Objective: Determine the test performance characteristics (TPC) of AUC:MIC of vancomycin for prediction of positive outcome., Data Sources: PubMed and Ovid Medline (1946 to 2018) and EMBASE (1974 to 2018). Study Eligibility Criteria and Participants: Studies of patients treated with vancomycin for any type of infection in peer reviewed publications. All patient populations were included., Interventions: Vancomycin AUC:MIC or AUC was related to patient clinical outcome., Methods: Searches of medical databases using relevant terms were performed. Screening, study reviewing, data extracting and assessing data quality was performed independently by two reviewers. Studies were stratified by type of primary outcome for calculation of pooled sensitivity, specificity and construction of hierarchical summary receiver operating characteristic (HSROC) curves., Results: Nineteen studies including 1699 patients were meta-analysed. Pooled sensitivity and specificity were 0.77 (95% CI 0.67-0.84) and 0.62 (95% CI 0.53-0.71) respectively for the seven studies with primary outcome of mortality and 0.65 (95% CI 0.53-0.75), 0.58 (95% CI 0.48-0.67) for studies with composite or clinical cure outcome (n = 12). HSROC curves suggested considerable heterogeneity. An additional 11 studies were described but could not be included for meta-analysis because data were not available. The majority of these studies (9/11) failed to demonstrate a relationship between AUC:MIC and positive clinical outcome., Conclusions: Vancomycin AUC:MIC performance was modest and inconsistent. Analysis was limited by studies without sufficient data; therefore, meta-analytic results may overestimate TPC values. 7. A longitudinal analysis of cytokines in anorexia nervosa.

- Abstract
Objective: Inflammation has been proposed to have a pathophysiological role in anorexia nervosa (AN) and to contribute to the maintenance of the disorder. Longitudinal cytokine research in AN has focused on only a few pro-inflammatory cytokines. We assessed a broad range of cytokines over time in people undergoing specialised treatment for AN., Method: We measured serum concentrations of 27 cytokines in people with AN (n = 23). Body mass index (BMI), eating disorder (ED) symptoms and general psychopathology were assessed and blood samples were collected within four weeks of the commencement of specialised ED treatment (baseline) and at 12- and 24-week follow-ups., Results: Both BMI and ED symptoms improved over the assessment period. Linear mixed models showed that log IL-6 decreased between baseline and week 12 assessments. By week 12, log IL-6 values were comparable to levels in healthy individuals. Log IL-7 increased from week 12 to week 24., Discussion: Initially elevated IL-6 serum concentrations appear to 'normalize' during the first 3-months of specialised treatment for AN and this co-occurs with improvements in ED symptoms. - Abstract
Background: An ASGE-ACG task force developed quality indicators (QI) for documenting quality endoscopic procedures. Acceptable compliance rates have not been determined., Aims: To determine our degree of compliance to the intra-procedure colonoscopy QI prior to intervention, design an educational intervention to improve those with low compliance, and to compare the degree of compliance after intervention., Methods: 300 patients undergoing colonoscopy in the pre-intervention time period followed by 300 patients after the educational intervention were reviewed. Endoscopists were instructed on the required QI and provided with their individual baseline compliance results. Dictated endoscopy reports were reviewed for compliance., Results: Four QIs; documentation of bowel preparation adequacy, appendiceal orifice, photographs of cecum, and polyp shape, had low pre-intervention achievement (64%, 53%, 20%, and 15% respectively) and significant change was observed (83%, 68%, 63%, and 54% respectively, all p<0.001). Four QIs; documentation of ileocecal valve, polyp size description, polyp location description, and follow up recommendations, had high levels of achievement prior to intervention (92%, 98%, 97% and 81% respectively) and no significant change was observed (all p≥0.16)., Conclusion: This study provides benchmarks for ASGE/QIs in colonoscopy cases in a large group practice model. Objective: To establish the inter-rater reliability of decomposition-based quantitative electromyography (DQEMG) derived motor unit number estimates (MUNEs) and quantitative motor unit (MU) analysis., Methods: Using DQEMG, two examiners independently obtained a sample of needle and surface-detected motor unit potentials (MUPs) from the tibialis anterior muscle from 10 subjects. Coupled with a maximal M wave, surface-detected MUPs were used to derive a MUNE for each subject and each examiner. Additionally, size-related parameters of the individual MUs were obtained following quantitative MUP analysis., Results: Test-retest MUNE values were similar with high reliability observed between examiners (ICC=0.87). Additionally, MUNE variability from test-retest as quantified by a 95% confidence interval was relatively low (+/-28 MUs). Lastly, quantitative data pertaining to MU size, complexity and firing rate were similar between examiners., Conclusion: MUNEs and quantitative MU data can be obtained with high reliability by two independent examiners using DQEMG., Significance: Establishing the inter-rater reliability of MUNEs and quantitative MU analysis using DQEMG is central to the clinical applicability of the technique. In addition to assessing response to treatments over time, multiple clinicians may be involved in the longitudinal assessment of the MU pool of individuals with disorders of the central or peripheral nervous system.

- Abstract
The purpose of this study was to compare intracanal bacterial reduction on teeth instrumented with 0.04 tapered nickel-titanium (NiTi) rotary instrumentation to bacterial reduction when using a stainless-steel K-file step-back technique using sterile saline irrigation. Forty-eight patients with apical periodontitis were randomly assigned treatment type. The canals were sampled before, during, and after instrumentation. The samples were incubated anaerobically for 7 days at 37 degrees C, colony-forming unit numbers calculated, and a log transformation performed to normalize the counts. Teeth exhibiting apical periodontitis were uniformly infected, whereas vital control teeth were not. A similar and uniform reduction occurred with progressive filing, regardless of technique (p < 0.0001). There was no detectable difference in colony-forming unit count after NiTi rotary or stainless-steel hand instrumentation (p = 0.42). Neither technique could predictably render canals free of bacteria. The results of this study indicate NiTi rotary and stainless-steel hand K-file step-back instrumentation techniques were not significantly different in their ability to reduce intracanal bacteria.

- Abstract
The purpose of this study was to evaluate the strengthening effect of an internal resin bonding technique. One hundred central incisors were endodontically treated and divided equally into five groups. Group 1, without any cervical preparation and with access repaired with bonded resin, served as the positive control. Teeth in groups 2 to 5 were cervically prepared to simulate the thin dentinal wall of immature teeth, and obturation material was removed to 3 mm below the cementoenamel junction (CEJ). In group 2, the negative control, the access was restored with bonded composite to the level of the CEJ. In group 3, access was restored with composite resin 3 mm apical to the CEJ using a clear post system. Group 4 was treated similarly to group 3, but an opaque post replaced the clear post. Group 5 was treated like group 3, after which a metal post was cemented into the channel left after removal of the clear post. All teeth were subjected to compression testing and the force required to fracture the teeth cervically was recorded for each group and the Kruskal-Wallis analysis performed to detect intergroup differences. All of the bonded resin techniques significantly strengthened the teeth against fracture, compared with the negative control group (p < 0.05). None of the experimental groups (3-5) were significantly different from the positive control group.

- Abstract
The relationship between ultraviolet irradiation, interleukin-1 production, and inflammatory sequelae and the pharmacologic inhibition of these events was investigated in Balb/c mice exposed to ultraviolet irradiation from a bank of six Westinghouse FS40 sunlamps. The resulting edema (66% increase), inflammatory cell infiltration, and rise in the acute-phase reactant (fourfold) serum amyloid P component was preceded by the activation of the interleukin-1 beta gene and enhanced product formation. Administration of dexamethasone, which is known to inhibit interleukin-1 production, inhibited the inflammatory response to ultraviolet irradiation. Thus, production of interleukin-1 may be one of the initial events leading to the consequences of ultraviolet irradiation exposure.

- Abstract
Monoclonal antibodies were produced against fibronectin and vitronectin, using the extracellular matrix from cultured bovine corneal endothelial cells as immunogen. Some antibodies inhibited the adhesion of BHK-21 cells to these molecules, in a standard assay with fibronectin or vitronectin coated on a plastic surface. By first coating the plastic surface with the monoclonal antibodies and subsequently binding vitronectin, the inhibitory effect was markedly increased. Divalent fibronectin required the presence of antibody in the medium as well as coated on the surface for maximal inhibition of cell adhesion. The inhibitory effect of surface-coated antibodies was stable in the presence of cells up to at least 24 h in vitro. Using this antibody coating method the surface could either be made compatible for cell adhesion (using non-inhibitory antibodies) or refractory. Assays performed by this method have considerable potential for quantifying particular functions of extracellular matrix molecules.

- Abstract
Although most intraepithelial lymphocytes (IEL) in mouse small intestine bear surface markers classically associated with T lymphocytes, the T-cell nature of these cells remains controversial. In the present study IEL from normal mice, or from mice infected with the gut nematode Trichinella spiralis, were therefore tested for their ability to produce T-cell-derived lymphokines in response to in vitro stimulation with concanavalin A (Con A) or with specific worm antigens. The data show that Con A-stimulated IEL produce minimal amounts of IL-2, and intermediate levels of IFN-gamma and IL-3 in comparison to the levels produced by spleen T cells. The FDC-P2 cell line, which proliferates in response to both IL-3 and GM-CSF, was identified as the most sensitive and reproducible indicator of lymphokine activity in supernatants from mitogen-stimulated IEL from normal mice. IEL isolated from mice infected with T. spiralis also produced high levels of FDC-P2 growth factors when challenged in vitro with Trichinella-derived antigens; however, normal IEL did not respond to this stimulus. The data thus provide evidence that antigen-sensitive T cells can arise in (or migrate to) the gut epithelium during gut infection.

16. Antigenic cross-reaction between the alpha types of human and mouse interferon.

- Abstract
Cross-neutralization of the alpha and beta types of human and mouse interferons was tested using antibodies directed against the heterologous types of interferons. The alpha type of mouse interferon (MuIFN-alpha) prepared from L cells was found to be completely neutralizable by high-titered antibody against human alpha-type interferon (HuIFN-alpha), although the titers were much lower than those obtained in neutralization reactions with the homologous interferon. MuIFN-alpha from virus-induced lymphocytes, as well as non-glycosylated MuIFN-alpha from L cells, reacted similarly. The cross-reaction was also observed by the binding of anti-HuIFN-alpha antibody to a column of immobilized MuIFN-alpha. The binding experiment indicated that only a small fraction of the anti-HuIFN-alpha antibody population is heterologous reactive. Reciprocally, HuIFN-alpha from L cells, again with relatively low antibody titers, and the antibody responsible for the heterologous reaction was shown to be the anti-MuIFN-alpha and not anti-MuIFN-beta type. It is concluded that the alpha types of human and mouse interferon bear an antigenic homology. On the other hand, no significant antigenic cross-reactivity has been detected between the beta types of human and mouse interferons.

- Abstract
To determine the effect of intraoperative albumin administration on blood use, water balance, and postoperative clinical course, we studied two groups of adult cardiac surgical patients. Group I (30 patients) received 25 gm of albumin during withdrawal of 2 units of blood prior to cardiopulmonary bypass (CPB) and 50 gm of albumin in the oxygenator prime. Group II (32 patients) received no albumin prior to the end of CPB. No difference in clinical course could be identified, nor was there a significant difference in blood use. Group I patients had lower hematocrit values intraoperatively from the time of blood withdrawal until the conclusion of operation. Coronary artery bypass operations were associated with greater positive water balance than were heat valve operations. Forty-three percent of the patients having coronary artery bypass grafting had a positive water balance greater than 5 liters, whereas 50% of those undergoing valve procedures had a balance less than 3 liters. We conclude that the principal effect of withholding albumin under these circumstances is to increase net positive water balance. The greater positive water balance does not appear to be detrimental.
