Quantification of the unbound portion of platinum (Pt) in human plasma is important for assessing the pharmacokinetics of the chemotherapeutic drug cisplatin. In this study, we sought to compare the recovery of unbound Pt using Nanosep® filters to 1) traditional filters (Centrifree®, Centrisart®, Amicon®) or trichloroacetic acid (TCA) protein precipitation, and 2) unbound, bound, and total Pt concentrations in clinical specimens. For the tested filters, the impact of 1) molecular weight cut-offs, 2) centrifugation force, and 3) total Pt concentration on Pt binding in human plasma was evaluated. Pt was quantified using inductively coupled-plasma mass spectrometry. In human plasma spiked with 0.9 μg/mL Pt, the percent of unbound Pt increased at higher centrifugation speeds. By comparison, the percent of unbound Pt was highest (42.1%) following TCA protein precipitation. When total Pt was ≤0.9 μg/mL, unbound Pt (∼20-30%) was consistent across filters. Conversely, when plasma was spiked with Pt exceeding 0.9 μg/mL, the percent of unbound Pt increased from 36.5 to 48% using ultrafiltration, compared to 63.4% to 79% with TCA precipitation. In patients receiving cisplatin-containing chemotherapy, the fraction of unbound Pt at concentrations exceeding 0.9 μg/mL ranged between 35 and 90%. Moreover, the unbound fraction of Pt in plasma correlated with the concentration of unbound (R 2  = 0.738) and total Pt (R 2  = 0.335). In summary, this study demonstrates that 1) the percent of unbound Pt is influenced by total and unbound Pt levels in vitro and in clinical specimens, and 2) ultrafiltration with Nanosep® filters is a feasible method for quantifying unbound Pt concentrations in human plasma., Competing Interests: Declaration of competing interest The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper. Metal-based drugs remain a tiny minority of all drugs that are on the market. The success story of the quintessential metal-based drug cisplatin (CP), which is intravenously administered to 70% of all cancer patients, however, demonstrates the inherent potential of metal-based drugs. A distinct disadvantage of CP is the dose-limiting severe toxic-side effects that it exerts in patients. To better understand the biomolecular basis for its toxicity, we employed a metallomics method to observe all platinum metabolites that are formed in blood plasma. These investigations revealed that a highly toxic CP-derived hydrolysis product - the highly toxic monoaqua hydrolysis complex (MHC) - is formed within 5min. More importantly, the application of this research tool has unraveled the mechanisms by which the chemoprotective agents sodium thiosulfate, d-methionine, N-acetyl-cysteine and l-glutathione modulate the metabolism of CP in plasma, namely by rapidly reacting with the MHC to form platinum‑sulfur complexes. Since CP remained in plasma for a considerable time, the possibility of 'tuning' its metabolism with chemoprotective agents in a desirable way has emerged. These observations are highly relevant because these chemoprotective agents were previously shown to significantly reduce the toxicity of CP in animal models, often without appreciably affecting its anticancer efficiency. Collectively, these results suggest that the toxicity of other metal-based drugs may be overcome if their metabolism in the bloodstream is adequately tuned with a suitable chemoprotective agent. Cisplatin is a first-line chemotherapeutic for the treatment of a wide variety of cancers since its discovery in the 1960s. Although various techniques have been reported for the measurement of total platinum in biological matrices, such as inductively coupled plasma-mass spectrometry and derivatization procedures, a specific, sensitive and robust assay for the quantification of intact cisplatin is still lacking. Therefore, we present a rapid, selective, sensitive, and reliable UHPLC-MS/MS based method for the determination of intact cisplatin in human plasma in support of a Phase II clinical trial. The optimal chromatographic behavior of cisplatin was achieved on a Syncronis HILIC column (50 × 2.1mm, 1.7µm, zwitterionic stationary phase). The retention behavior of cisplatin on this zwitterion-based stationary phase was well described by an adsorptive interaction model. A simple sample preparation based on protein precipitation combined with the removal of phospholipids by HybridSPE-precipitation was developed. The method was proven to be free of a relative matrix effect. The assay was validated within a range of 20 - 10,000ng/mL using 100μL of plasma sample. The intra and inter-day precisions were all less than 7.6%, and none of the bias was greater than 13.1%, thus corroborating that the developed method is precise and accurate. Till date, no analytical method published to detect Cisplatin has been validated according to the U.S. Food and Drug Administration (FDA) guidance using liquid chromatography mass spectrometry (LC-MS/MS). We report, a validated LC-MS/MS method for quantitative determination of cisplatin in rat plasma and urine according to FDA guidlines. Cisplatin is a platinum containing compound used for the treatment of different types of cancers. Quantitative determination of cisplatin has been carried out using atomic absorption spectroscopy, high pressure liquid chromatography with phosphorescence, ultra-violet detection, or with inductively coupled plasma mass spectrometry. Few LC-MS/MS methods have been reported for the analysis of cisplatin either for direct quantification or indirect by derivatizing with organic compounds but none of the reported methods have validated the method. The developed and validated assay presented here is a highly sensitive LC-MS/MS method developed and validated for the quantitative determination of cisplatin following derivatization with diethyldithiocarbamate (DDTC) in order to detect platinum (Pt) of cisplatin, suitable for pharmacokinetic studies in rats and to further use it to study human toxicology. Chromatographic separation was achieved using a Poroshell 120 EC-C 18 column (3×50mm, 2.7μm) with a binary gradient mobile phase. Quantification was performed on a triple quadruple with electrospray ionization and detection was performed using multiple reaction monitoring. The method has a limit of detection of 1ng/mL, and the quantifiable range was 3-3000ng/mL in rat plasma and urine. The method was accurate and precise with an accuracy and precision for intra-day and inter-day of ±20% for lower limit of quantitation and of ±15% for low, mid and high quality control samples. This method was successfully applied to study the pharmacokinetic profile of cisplatin in rat plasma and urine given a range of doses from 0.5 to 3.5mg/kg., (Published by Elsevier B.V.)

Objective: To evaluate the associations between long-term serum levels of platinum (se-Pt) and neurotoxicity and ototoxicity (NTX), endocrine gonadal function (endocrine-GF), and cardiovascular disease (CVD) in testicular cancer survivors., Material and Methods: A total of 292 cisplatin-treated testicular cancer survivors (1980-1994) participated in a national follow-up study (2007-2008). Se-Pt was quantified by inductively coupled plasma mass spectrometry, and categorized in quartiles. Symptoms of NTX were assessed with scale for chemotherapy-induced neurotoxicity (SCIN), with each symptom in 4 categories and total SCIN score categorized in quartiles. Endocrine-GF was categorized according to cutoff values for the 25, 50, and 75 percentiles of luteinizing hormone (LH) and testosterone within each decadal age group established from a control group. CVD was defined as ischemic heart disease, stroke, or artery occlusion. Associations between se-Pt levels and NTX, endocrine-GF, or risk for CVD, were analyzed with ordinal logistic regression and Cox regression, respectively., Results: Median follow-up was 19 years (range: 13-28). In ordinal regression analyses, increasing quartiles of se-Pt were significantly associated with increasing quartiles of SCIN (P for trend = 0.05), increased tinnitus (P<0.001), and increased hearing impairment (P = 0.04). The association remained significant for tinnitus when adjusting for cisplatin dose. Increasing LH quartiles was associated with increasing se-Pt quartiles (P = 0.04). No association between se-Pt in quartiles and CVD was established., Conclusion: Median 19 years after treatment, increasing quartiles of se-Pt are associated with increasing SCIN score, tinnitus, hearing impairment, and increasing LH levels. Conjoint liquid chromatography (CLC) on monolithic convective interaction media (CIM) disks coupled on-line to UV and inductively coupled plasma mass spectrometry (ICP-MS) detectors was used for the first time in speciation analysis of Pt in human serum spiked with Pt-based chemotherapeutics. CIM Protein G and CIM DEAE disks were assembled together in a single housing forming a CLC monolithic column. Such a set-up allows rapid two-dimensional separation by affinity and ion-exchange (IE) modes to be carried out in a single chromatographic run. By applying isocratic elution with Tris-HCl-NaHCO3 buffer (pH 7.4) in the first minute, followed by gradient elution with 1 mol L(-1) NH4Cl (pH 7.4) in the next 9 min, immunoglobulins (IgG) were retained by the Protein G disk enabling subsequent separation of unbound Pt from Pt bound to transferrin (Tf) and albumin (HSA) on the CIM DEAE disk. Further elution with acetic acid (AcOH) in the next 3 min allowed separation of Pt associated with IgG. Separated Pt species were quantified by post-column isotope dilution-ICP-MS. Pt recovery on the CLC column was close to 100%. In comparison to commonly applied procedures that involve separation of protein peaks by size-exclusion chromatography (SEC) followed by IE separation of metal-based chemotherapeutic fractions bound to serum proteins, the CLC method developed is much faster and simpler. Its sensitivity (LOQs adequate for quantification of all separated Pt species, lower than 2.4 ng Pt mL(-1)), good selectivity and method repeatability (RSD±3%) enabled investigation of the kinetics of interaction of Pt-based chemotherapeutics with serum proteins and the distribution of Pt species in spiked human serum. Pt species present in spiked serum were bound preferentially to HSA. The proportion of Pt associated with IgG and Tf was lower than 13%. Cisplatin and especially oxaliplatin react rapidly with serum proteins, while carboplatin much less. Background: Self-expandable metallic stents (SEMSs) are effective in the palliation of malignant airway obstruction. Tumor ingrowth, however, frequently occurs because of a shortage of effective local therapy. Additionally, SEMSs are frequently associated with problems of fracture, migration, and difficult removals. Our goal was to develop a novel bioabsorbable stent with cisplatin elution to circumvent such problems., Methods: Biodegradable stents made of polycaprolactone were fabricated by a laboratory-made, microinjection molding machine. In vitro mechanical strength of the stents was compared with the strength of Ultraflex SEMSs. Polylactide-polyglycolide copolymer and cisplatin were coated onto the surfaces of the stents. Elution method and high-performance liquid chromatography (HPLC) analysis were used to examine the in vitro cisplatin release characteristics. In vivo, the stents were surgically implanted into the cervical trachea of 15 New Zealand white rabbits. Bronchoscopic examination was performed weekly (1 to approximately 5 weeks) before killing. Cisplatin concentrations in trachea, lung, and blood were analyzed by HPLC. Histologic examination was also performed., Results: The biodegradable stent exhibited mechanical strength comparable to the strength of Ultraflex SEMSs and provided a steady release of cisplatin for >4 weeks in vitro. The in vivo study showed sustained cisplatin levels in rabbit trachea for >5 weeks with a minimum drug level in blood. Histologic examination showed an intact ciliated epithelium and marked leukocyte infiltration in the submucosa of the stented area., Conclusions: Our study demonstrated that the biodegradable stents provided physical properties comparable to the properties of SEMSs and a sustained release of cisplatin for >5 weeks, which showed great potential in the treatment of malignant airway obstruction.

A new microflow injection chemiluminescence (μFI-CL) system was described for the determination of cisplatin in human serum. By using the microchip with double spiral channel configuration, the sensitivity was greatly enhanced due to more efficient mixing of the analyte and reagent solutions. Experimental results revealed that common ions in human serum, such as Mn(2+), Co(2+), Fe(3+), Cu(2+), Zn(2+), Ni(2+), Na(+), K(+), Ca(2+), Cl(-), NO(3)(-), Ac(-), CO(3)(2-), PO(4)(3-), SO(4)(2-) did not cause interference with the detection of Pt(II) by using 1,10-phenanthroline as the masking agent. Under the optimized conditions, a linear calibration curve (R(2)=0.998) over the range 2.0 × 10(-8) to 2.0 × 10(-6) mol L(-1) was obtained with the detection limit of 1.24 × 10(-9) mol L(-1). The relative standard deviation was found to be 3.46% (n=12) for 2.0 × 10(-7) mol L(-1). The sample consumption was only 2 μL with the sample throughput of 72 h(-1). It had been used for trace platinum determination in cisplatin injection and human serum samples after the dosage of cisplatin. The recovery varied from 97.6 to 103.9%. We have developed a specific assay for cisplatin in human plasma ultrafiltrate (PUF) and cell culture medium ultrafiltrate (MUF) using HPLC on-line with inductively coupled plasma mass spectrometry (ICP-MS). Separation of cisplatin (6 min) and monohydrated cisplatin (12 min) was achieved using a muBondapak C(18) column (Waters) and a mobile phase (0.075 mM sodium dodecyl sulfate and 3% methanol, adjusted to pH 2.5 with triflic acid) pumped at a flow rate of 0.5 mL/min. The analytes were detected with little background interference by ICP-MS monitoring of platinum masses (m/z 194/195). Calibration curves were linear over three orders of magnitude (0.05-8 microM) and the limit of quantitation was 0.1 microM. Intra- and inter-assay accuracy (range 91.6-113%) and precision (range 1.00-12.3%) were acceptable for PUF and MUF. The method was applied to determining cisplatin during ex vivo incubation of the drug in whole human blood at 37 degrees C. In conclusion, a specific, sensitive and reliable HPLC-ICP-MS assay has been established for determining intact cisplatin in PUF and MUF.

Cisplatin is for a long time in clinical use as efficient antitumor drug. The success in cisplatin-based chemotherapy, however, strongly depends on how careful the drug's dosages are monitored in order to reduce severe side-effects and overcome cellular resistance. The use of micellar electrokinetic chromatography with direct UV detection is described for the determination of intact cisplatin in human serum. The main product of drug's hydrolytic metabolism, cis-diammineaquachloroplatinum(II), was quantified using capillary zone electrophoresis in combination with indirect UV detection and on-line transient isotachophoresis preconcentration. The detection limits of platinum species studied were about 2-3 micromol l(-1) that allows the proposed methods to be applied for quantification of the administered levels of cisplatin as well as drug's active metabolite. Changes in the speciation of cisplatin occurring after intravenous administration can also be monitored using these simple and convenient CE techniques.

A method for determination of cis-diaminedichloroplatinum (II) (cisplatin) in ultrafiltered plasma, cell, and tumour samples is described. Cisplatin separation was carried out on a reversed-phase column using methanol-acetonitrile-water as the mobile phase. The flow rate was maintained constant at 1.6 mL/min and analysis was performed at 23 degrees C. Detection was carried out by absorbance at 254 nm. The method was linear in the range of 0.2-10 microg/mL, and the coefficients of variation were <10%. Using this technique, we measured the intracellular accumulation of cisplatin in cancer cells and in tumours of mice receiving treatment with cisplatin and evaluated the pharmacokinetics of cisplatin in nu/nu mice after intraperitoneal (i.p.) administration. The method proved to be adequate for measuring cisplatin both in vitro and in vivo and could be suitable for studies of cisplatin pharmacokinetics in animal models.

The interactions of anti-cancer drugs with blood constituents, particularly with serum albumin (HSA) may have a major influence on drug pharmacology and efficacy. In the present work the binding of paclitaxel (trade name Taxol) to human serum albumin and its effect on cisplatin and adriamycin interactions has been investigated through UV/visible, CD, fluorescence spectroscopy and the inductively couplet plasma atomic emission spectroscopy method. Displacement studies with use of bilirubin, as a competitive agent provided relevant information about the location of the binding site in HSA as well as the possible multidrug interactions.

Cisplatin is a cytotoxic platinum compound, used in the treatment of several solid tumors. Cisplatin and to a greater extent its hydrolysis product monohydrated cisplatin are responsible for side-effects like nephrotoxicity. A sensitive, accurate and precise method was developed to simultaneously determine cisplatin and monohydrated cisplatin in plasma. The compounds were separated by high-performance liquid chromatography and quantified by off-line furnace atomic absorption spectrophotometry. The linear ranges for cisplatin and monohydrated cisplatin in deproteinized plasma were 60-600 and 87.5-700 nM, respectively. From plasma, the mean recovery of cisplatin was 83.2% and that of monohydrated cisplatin 79.1%. The lower limits of quantification of cisplatin and monohydrated cisplatin in deproteinized plasma were 60 and 87.5 nM, respectively. Over the whole calibration range, the within- and between-day accuracy of intact cisplatin ranged from 100.7 to 111.4 and 94.8-102.0%, respectively. The within- and between-day accuracy of monohydrated cisplatin ranged from 107.1 to 113.3 and 101.4-104.9%, respectively. The within-day and between-day precision of cisplatin ranged from 3.4 to 11.5 and 7.3-10.3%, respectively. For monohydrated cisplatin, the within-day and between-day precision ranged from 3.7 to 6.2 and 5.6-7.9%, respectively. Currently, the developed assay has been implemented in pharmacokinetic studies of patients treated with cisplatin alone or in combination with other drugs.

The three platinum derivatives currently available share many pharmacokinetic and pharmacodynamic (PK-PD) properties but present also some distinct characteristics, due to their structural differences. They result in different systemic PK-PD and metabolic behaviour and toxicity profile. Oxaliplatin is quickly transformed into dach-platinum, the active metabolite, by loosing oxalate chain. Eighty to eighty-eight per cent of platinum are bound to proteins, as for cisplatin, whereas carboplatin is less reactive. Cisplatin and oxaliplatin active metabolites, i.e. monoaquo platin and dach-platin quickly react with small proteins with sulfhydryl groups, such as glutathione, cysteine and methionine, and then with high molecular weight proteins, such as albumin and gammaglobulins through covalent link. Thus, their terminal half lives are long, about ten days, but no platinum accumulation has been reported in plasma with oxaliplatin, whereas after cisplatin administration, both total and ultrafiltrable platinum progressively accumulate in plasma. This difference may play a role in the lack of oxaliplatin nephrotoxicity and its more delayed and reversible neurotoxicity. On the other hand, carboplatin is more stable, less bound to proteins and is largely excreted inchanged in urine. This can explain that it passes more easily through the blood brain barrier. Erythrocytes represent an important deep compartment, especially for oxaliplatin, a little bit less for cisplatin. Oxaliplatin is trapped in erythrocytes through a covalent binding to globin. There, its half life is identical to that of erythrocytes. According to certain authors, this trapping would be involved in the incidence of anemia. On the contrary, carboplatin is quickly extruded from erythrocytes. The three derivatives kinetics in plasma present a wide interindividual variability, resulting in differences in term of toxicity and efficacy. For the three of them, plasma clearance is correlated to creatinine clearance, but only carboplatin dosage can be individually adjusted, based on creatinine clearance measurement, thanks to its simple renal excretion, due to exclusive glomerular filtration, and after Calvert's, Egorin's and Chatelut's population kinetics studies. Cisplatin renal excretion is more complex, combining reabsorption and secretion processes. Therefore, individual dosage adjustment needs platinum concentration measurement in plasma, but there is no general agreement on the platinum species to measure, ultrafiltrable or bound. Oxaliplatin is too recent in clinical practice and still lacks of PK-PD data. These characteristics can help us for a better knowledge of the three platinum derivatives clinical properties, both in term of kinetics, behaviour and toxicity.

Cisplatin (cis-dichlorodiammine platinum II) is one of the most effective antitumor agents to date. Its usefulness is limited, however, by toxicity to healthy tissues, most notably, its nephrotoxicity. To maximize the chemotherapeutic potential of cisplatin and minimize its adverse effects, it is imperative to monitor formation of cisplatin:DNA adducts throughout treatment. We developed a novel, highly sensitive, SINE (Short Interspersed DNA Element)-mediated. PCR-based assay for detection of cisplatin adducts in vitro and in vivo, in DNA from mouse blood cells. The assay relies on the abundance, dispersion and conservation of SINEs in mammalian genomes. The B1 elements at a copy number of 50,000-80,000 are the most abundant SINEs in the mouse genome. Due to their strong sequence conservation, primers complementary to the B1 consensus sequence anneal to the majority of their targets in the genome and allow simultaneous amplification of long random segments of genomic DNA. Thus, in conjunction with the fact that cisplatin adducts block the progression of thermostable polymerase, B1 element-anchored PCR makes a sensitive tool for assessing the overall integrity of the transcribed regions in the mouse genome. The high sensitivity of the assay allows detection of DNA damage at the low cisplatin dosage of 1-8 mg/kg that is considered as sub-chemotherapeutic in experimental animal models. The sensitivity range therefore, makes this assay suitable for the development of predictive correlation for both, the efficacy of treatment as well as induction of nephrotoxicity.

Purpose: To determine possible schedule dependent pharmacokinetic and pharmacodynamic interactions between gemcitabine (2',2'-difluorodeoxycytidine, dFdC) and cisplatin (cis-diammine-dichloroplatinum, CDDP) in patients with advanced stage solid tumors in a phase I trial., Patients and Methods: A total of 33 patients with advanced stage solid tumors were treated with gemcitabine (30-min infusion, 800 mg/m2) and cisplatin (one-hour infusion, 50 mg/m2). Sixteen patients had a four-hour interval between gemcitabine (days 1, 8, 15) and cisplatin (days 1 and 8), followed by the reverse schedule and seventeen patients had a 24-hour interval between gemcitabine (days 1, 8, 15) and cisplatin (days 2 and 9), followed by the reverse schedule. Gemcitabine and cisplatin pharmacokinetics were measured in plasma and white blood cells (WBC), isolated from blood samples taken at several time points after the start of treatment., Results: A four-hour time interval between both agents did not reveal major differences in plasma pharmacokinetics of gemcitabine, dFdU (deaminated gemcitabine) and platinum (Pt), and of gemcitabine-triphosphate (dFdCTP) accumulation and Pt-DNA adduct formation in WBC between the two different sequences of gemcitabine and cisplatin. In the patients treated with the 24-hour interval, cisplatin before gemcitabine did not significantly change peak gemcitabine levels and the AUC of plasma dFdU, but tended to increase dFdCTP AUC in WBC 1.5-fold (P < 0.06). Gemcitabine before cisplatin decreased the plasma AUC of Pt 2.1-fold (P = 0.03). No significant differences in Pt-DNA adduct levels in WBC were found, although gemcitabine before cisplatin tended to increase the 24-hour retention of Pt-DNA adducts. Creatinine clearance on day 28 was related to the peak plasma levels of total Pt (linear regression coefficient (r) = 0.47, P = 0.02, n = 26). Furthermore, the increase in the Pt-GG to Pt-AG ratio 24 hours after cisplatin treatment was related to the overall response of patients (r = 0.89, P < 0.01, n = 8)., Conclusions: Of all schedules the treatment of patients with cisplatin 24 hours before gemcitabine led to the highest dFdCTP accumulation in WBC and total Pt levels in plasma. These characteristics formed the basis for further investigation of this schedule in a phase II clinical study.

In water, the antineoplastic drug cisplatin, cis-[PtCl2(NH3)2] (1) hydrolyses slowly to the aqua complexes cis-[Pt(NH3)2Cl(H2O)]+ (2) and, to a small extent, cis-[Pt(NH3)2(H2O)2]2+ (3), which are thought to play an important role in the metabolism of cisplatin. HPLC is a useful technique for monitoring 2 and 3, but only if the components of the mobile phase used in the reverse phase HPLC technique are unreactive toward these aqua complexes under the conditions of the experiment. 15N Nuclear magnetic resonance (NMR) with samples highly enriched (>98%) in 15N has been used to check the reactivity of 2 and 3 toward substances commonly used as components of the mobile phase. The results reported herein indicate that acetonitrile, often used as an organic modifier, reacts readily with 2 and 3. Methanol, also commonly employed, is much less reactive. Carboxylic acids RCO2H (R = CH3, H, CF3), which are frequently used to adjust pH of the mobile phase, also react readily with 2 and 3. Trifluoromethanesulfonic acid ("triflic acid"), CF3SO3H, is unreactive. Neither hexanesulfonic acid nor sodium dodecyl sulfate (SDS), used as "ion-pairing agents", reacts significantly with 2 or 3 under the experimental conditions, but SDS gives better peak separation. Commercial SDS must, however, be purified from chloride contamination. From our studies, optimal conditions for HPLC separation of 1, 2, and 3, with a C18 stationary phase at 37 degrees C, require an aqueous mobile phase with 3% v/v methanol, 0.05 mM SDS, and pH 2.5 (adjusted with triflic acid). This technique was then used to measure levels of 1, 2, and 3 in ultrafiltered serum after incubation for various times with cisplatin at 37 degrees C.

The plasma disposition kinetics and tissue distribution of platinum was evaluated following intravenous bolus administration to CD1 immune-competent mice of cisplatin, cisplatin conjugated to anti-CEA monoclonal antibody A5B7 via a carboxymethyl dextran (CMdextran) carrier molecule, and cisplatin coupled to the CMdextran in the absence of antibody. In addition, the in vivo characteristics of 125I-labeled A5B7 were compared with and without conjugation to CMdextran. Conjugation of cisplatin [clearance (CL = 0.62 mL/min/g, volume of distribution at steady-state (Vdss) = 16 mL/g] to CMdextran restricted its tissue distribution (Vdss = 0.43 mL/g) and reduced its systemic clearance (CL = 0.055 mL/min/g). Subsequent conjugation of the complex to A5B7 further reduced both its distribution (Vdss = 0.20 mL/g) and clearance (CL = 0.016 mL/min/g). Clearance of A5B7 (CL = 0.002 mL/min/g) was increased by conjugation to CMdextran (CL = 0.014 mL/min/g); tissue distribution was unchanged. A5B7-CMdextran-cisplatin was relatively stable in plasma and other tissues, except the liver. The extent of distribution of platinum into tissues (lung, liver, muscle, kidney) was markedly influenced by conjugation, with the influence being greatest for unmodified cisplatin and least for the A5B7-CMdextran conjugate. However, the time courses of tissue distribution, expressed in mean residence time scales, were similar, implying a common mechanism controlling tissue uptake.

An high-performance liquid chromatographic (HPLC) method has been developed for the determination of cisplatin, based on precolumn derivatization of platinum(II) with bis(salicylaldehyde)tetramethylethylenediimine, extraction in chloroform and elution from a 3 microm Hypersil ODS column with methanol-acetonitrile-water as mobile phase and detection at 254 nm. Copper(II), iron(II), nickel(II), palladium(II), dioxouranium(IV) separated completely and did not affect the determination of platinum(II). The method was applied for the determination of cisplatin as platinum(II) in a pharmaceutical preparation and in blood samples of cancer patients after infusion of cisplatin.

This study was undertaken to demonstrate that alginates could form a complex with cisplatin and that the pharmacokinetics and nephrotoxicity of cisplatin could be altered by this complexation. The complexes were prepared with alginates of mean molecular weights of 10000 (AL-1) and 40000 (AL-2). The plasma clearances of cisplatin-alginate complexes were significantly reduced when compared with the drug alone. Urinary excretion of platinum (Pt) was increased when animals were dosed with the cisplatin-AL-1 complex, but not with the cisplatin-AL-2 complex. Renal clearance of cisplatin, when administered alone, was more rapid during the first 2 h than the rest of the study period (96 h) and was higher than its plasma clearance, which is consistent with irreversible binding of cisplatin to plasma protein. On the other hand, renal clearance of cisplatin-alginate complexes was not highly time dependent, and the values were more similar to those of plasma clearance. Complexation of cisplatin with alginates, especially AL-1, inhibited the accumulation of Pt in the kidneys and reduced blood urea nitrogen elevation by cisplatin. The in vitro antitumor activities against U937, P388, and cisplatin-resistant P388 (P388/cisplatin) cells were similar among cisplatin and its alginate complexes. These studies indicated that the pharmacokinetics and nephrotoxicity of cisplatin could be altered through complexation with alginate by (1) reducing cisplatin clearance, (2) inhibition of Pt accumulation of in the kidneys, and (3) reduction of apparent nephrotoxicity without decrease in in vitro antitumor activity.

A post-column derivatization method has been developed for the determination of cisplatin and its monohydrated form. Cisplatin was isolated on a strong anion-exchange column, while a strong cation-exchange column was used for the monohydrated complex. Diethyldithiocarbamate was used as reagent and the influence of temperature, pH and methanol content on the yield of derivative was investigated. The reaction was quantitative using a packed-bed reactor with a surrounding temperature of 115 degrees C and a mobile phase consisting of 0.125 M succinic acid-sodium hydroxide buffer pH 5.2 and methanol (2:3, v/v). The resulting complex, Pt(DDTC)2, was monitored photometrically at 344 nm. The precision of the determination was 11.5% (C.V.) at an injected amount of 20 ng (n = 12) for monoaqua and 8.0% (C.V.) at 9 ng (n = 10) for cisplatin. The method was used to evaluate the plasma concentration of cisplatin and its monohydrated form in a patient.

Background: Although cisplatin pharmacokinetics is well documented in patients with various degrees of renal dysfunction, no information is available concerning cisplatin administration to anephric patients. Since anephric patients may sometimes need cisplatin therapy, it is therefore of importance to define therapeutic guidelines for cisplatin administration in this patient population., Patient and Methods: Cisplatin was administered to an anephric patient (bilateral nephrectomy) requiring cisplatin therapy for a metastatic carcinoma of the urothelium. A test dose of 12 mg (7.5 mg/m2) of cisplatin was first administered as a 1 hour infusion in order to determine the patient's pharmacokinetic parameters. Filterable and total platinum levels were determined by flameless atomic absorption spectrophotometry. Haemodialysis was started 30 min before the beginning of the cisplatin infusion and was maintained for 4 h thereafter., Results: Under haemodialysis, filterable and total platinum pharmacokinetics after the test dose were comparable with a patient with normal renal function, i.e. with peak plasma concentrations of 126 ng/ml and 166 ng/ml for the filterable and the total platinum, respectively. The area under the curves (AUC) were 154 ng.h/ml for the filterable and 11486 ng.h/ml for the total platinum. The terminal half-lives of filterable and total platinum were 0.42 h and 101 h, respectively. Based on the test dose platinum pharmacokinetics, a therapeutic dose of 100 mg (63 mg/m2) of cisplatin was administered. Following the therapeutic dose, peak plasma concentrations reached 1,120 ng/ml for the filterable and 1,280 ng/ml for the total platinum. The AUCs were 1,609 and 65,556 ng.h/ml for the filterable and the total platinum, respectively, as expected from the predicted AUCs obtained from the test dose pharmacokinetics. The terminal half-lives of filterable and total platinum were similar to the ones observed after the test dose, i.e. 0.36 h and 86 h, respectively. Although the patient died of rapidly progressive hepatic failure, the feasibility of the test dose-guided cisplatin administration in an anephric patient is demonstrated., Conclusion: This approach may be helpful in monitoring cisplatin therapy in similar cases requiring cisplatin administration.

Chemotherapy using cis dichlorodiamine platinum (CDDP) in doses of 30 and 60 mg/m2 was carried out in 6 patients who had adenocarcinoma of the ovary, that had not been treated before, 15 to 120 minutes before surgery was carried out. The results of doses of platin given on the blood levels are demonstrated. A slight and progressive increase in the plasma levels of CDDP with peaks between 0.6 and 1.95 of total platinum and 0.22 mcg/ml of free platin were found. The tissue levels in the region of the ovaries were found to be between 0.16 and 0.94 mcg/mg. There was a stepwise increase in the levels found in the pathological peritoneal cover of between 0.42 and 31.2 mcg/mg. This dropped as time went by; and then led to change in accordance with the intra-peritoneal concentrations. This study shows that it is possible when giving drugs intraperitoneally to avoid high plasma peaks and to obtain good levels of concentration of cisplatin in the peritoneum.

Wistar rats (five in each group) were given either 1 mg/kg of free cisplatin, 1 or 2 mg/kg of liposomal encapsulated cisplatin, or saline solution intraperitoneally biweekly for 15 injections. Rats in the free drug group showed significantly less weight gain; two rats died during the study. At necropsy, the free cisplatin--treated rats showed gross and microscopic evidence of peritoneal fibrosis that was not detected in any of the remaining groups. The free cisplatin--treated rats showed serum and histologic evidence of renal damage; all five rats had moderate or severe acute tubular necrosis. No renal abnormalities were detected in rats that received 1 mg/kg, and only focal or mild changes were found in rats that received 2 mg/kg of the liposomal preparation. Neurotoxicity, as determined by nerve conduction and inclined plane studies, developed in rats treated with free and liposomal cisplatin. These results are encouraging and warrant further investigation.

The platinum complex cis-diamminedichloroplatinum(II)(cisplatin or CDDP), which is used successfully to treat various kinds of tumour, can be determined in human plasma ultrafiltrate using liquid chromatography with reductive electrochemical detection (LC-ED). Polarographic analyses of other platinum-containing drugs have been carried out, and the results indicate that some of them might be good candidates for detection using the ED method. cis-Dichloro-trans-dihydroxo-cis-bis-(isopropylamine)platinum(IV) (CHIP or JM-9), diammine(1,1-cyclobutanedicarboxylato)platinum(II) (CBDCA or JM-8), and tetrachloro(trans-1,2-diaminocyclohexane)platinum(IV) (TCDCP), which are under evaluation for antitumour properties, have been investigated by this method. The results suggest that LC-ED may be a suitable technique for the determination of CHIP and TCDCP. The separation of cationic hydrolysis products from the neutral parent complex (CDDP) was carried out on reversed-phase columns, modified with alkylsulphonic acid ion-pair reagents. The rate of disappearance of CDDP in various media at 37.0 degrees C was studied using this method. These results are in good agreement with those determined by other investigators using different methods. In addition, the monoaqua hydrolysis product was detected after incubation of CDDP with plasma ultrafiltrate and 100 mM sodium chloride. Atomic absorption spectrophotometry with electrothermal atomisation was used to determine the total platinum content in eluate fractions. This aided the identification of platinum-containing peaks in the LC-ED profile, and was also used to measure the percentage platinum recovered from the column.

A technically simple, rapid and sensitive high performance liquid chromatographic assay for cis-dichlorodiammineplatinum (II) in human plasma ultrafiltrate and urine is described. The drug was chelated by exchange with diethyldithiocarbamate and extracted into chloroform. Nickel (II) was used as an internal standard which allows correction for the matrix effects observed with previous chromatographic and spectrometric methods. Chromatography was performed on a mu-Bondapak CN column and the eluent measured spectrophotometrically at 254 nm. Precision and reproducibility were both excellent and the detection limit was less than 50 ng/ml using only 1 ml of biological fluid.

A reversed-phase ion-pair chromatographic method with on-line radioactivity detection for the simultaneous determination of 195mPt-labelled cisplatin and related platinum complexes has been developed. With this system a good resolution of various radiolabelled platinum complexes can be achieved. The detection limit of the radioactivity detector is 10 ng of cisplatin (specific activity of 15 MBq/mg cisplatin) per millilitre of urine or plasma ultrafiltrate. The detector response is independent of both the chemical structure of the platinum complexes and the matrix composition of the samples. This method may serve as a reference system for other high-performance liquid chromatographic systems with less specific and sensitive detectors.

cis-Diamminedichloroplatinum(II) (DDP) was measured in plasma ultrafiltrate following derivatization with sodium diethyldithiocarbamate (DDTC) by quantitation against a nickel chloride internal standard. A chloroform extract containing the Pt(DDTC)2 and Ni(DDTC)2 complexes was separated by reversed-phase high-performance liquid chromatography on a C18 radial compression column. The complex was eluted with methanol/water, 4/1, at a flow rate of 1.5 ml/min, and was detected at 254 nm. The limit of sensitivity was 0.1 microgram/ml DDP in the ultrafiltrate. This analytical approach was validated by comparison to graphite furnace atomic absorption spectrophotometric determinations of duplicate samples. There was clearly a component of the ultrafiltrable platinum present that was resistant to derivatization by DDTC. Evidence is presented that this component, presumably Pt(II) complexed with endogenous small molecules, is non cytotoxic and, hence, that this method may be selective for "active Pt(II)." This method offers an advantage over atomic absorption determination of total platinum in ultrafiltrate which does not discriminate between active and inactive forms, and over off-line FAA detection of parent DDP in HPLC eluates which ignores other active forms. Using this technique we have measured the pharmacokinetics of DDTC-reactive Pt(II) in humans after either i.v. infusion or infusion of DDP into the peritoneal cavity of patients with ovarian carcinoma.

The present review on the quantification of cytostatic drugs has mainly been focussed on chromatographic techniques. Special attention has been paid to the precautions that have to be taken into account to ensure the selectivity and accuracy of the various methods. The various cytostatics that have been dealt with are: alkylating agents, antimetabolites, vinca alkaloids, antibiotics, cis-diamminedichloroplatinum, podophyllotoxine derivatives, and nitrosoureas.

The reactions of cis- and trans-dichlorodiammineplatinum(II) (cis- and trans-DDP) with albumin and two plasma proteinase inhibitors were compared. Reaction with alpha 2-macroglobulin (alpha 2M) resulted in subunit crosslinking and loss of proteinase binding activity. The reaction also modified a receptor recognition site present on each alpha 2M subunit. While more trans-DDP was incorporated into alpha 2M than cis-DDP, cis-DDP was more effective at blocking receptor recognition, alpha 1-proteinase inhibitor was also inactivated by reaction with either cis- or trans-DDP. These reactions resulted in binding of platinum to methionine-358 at the reactive center of this inhibitor. Trans-DDP, however, was less selective and also bound to the single cysteine residue (Cys-232) of alpha 1PI. Reaction of albumin with cis-DDP resulted in incorporation of about 1 mol platinum per mol protein, and this platinum modified the single cysteine (Cys-34) in the molecule. Albumin incorporated twice as much trans-DDP, but the binding did not involve cysteine-34. In general, reactions of cis-DDP with proteins appear to be more selective than those observed for modification with the trans isomer.

Incubation of dialyzed rat serum with cisplatin at a concentration of 908 ppm results in binding of platinum to the proteins and electrophoretic evidence of protein alterations. Intravenous administration of protein-bound platinum(cis), containing the same platinum content as an 8.5-mg/kg dose of cisplatin, fails to produce elevated serum BUN and creatinine levels as do equivalent doses of 'free' cisplatin in 0.9% saline. Light microscopy of the right side kidneys of rats receiving protein-bound platinum(cis) revealed no obvious pathology while the kidneys of rats receiving 'free' cisplatin showed consistent pathological alterations including hydropic degeneration and pyknotic nuclei. These observations suggest that a protein-bound form of platinum will be unlikely to contribute to the renal toxicity observed during cisplatin chemotherapy.

A thin-layer gold/mercury electrode and a hanging mercury drop electrode are compared as part of an evaluation of liquid chromatography (LC) with electrochemical detection (ED) for the determination of platinum species in human plasma ultrafiltrate. The platinum species, derived from an aged aqueous solution of the antineoplastic agent cis-diamminedichloroplatinum(II) (CDDP), can be separated by ion-pair chromatography. Variation of a number of parameters is described along with the limitations and advantages of each kind of electrode system. We have used our LC-ED technique to separate CDDP from its hydrolysis products and other non-platinum-containing species in human plasma ultrafiltrate with a detection limit of 62 ng/ml (ppb).

The antitumoral agent cis-diamminedichloroplatinum (II) was administered at doses of 40 mg m-2 body surface area daily for 5 days via continuous i.v. infusion in association with etoposide (VP-16-213). The Pt concentration in serum up to 30 days from the beginning of the therapy was monitored by inductively-coupled plasma emission spectrometry. Results lead to two main conclusions: the analytical technique employed is suitable for measurements of Pt in biological fluids with the necessary precision (0.95-2.5%), accuracy (recovery 98.5-101.7%) and detection power (0.002-0.004 mg/l); there were effective Pt plasma concentrations for a greater length of time (with peak value 2.0 mg/l towards the end of treatment) than those achieved by other therapies so far adopted. On the other hand, toxic side effects, in particular gastrointestinal toxicity, myelosuppression and nephrotoxicity, were found to be not worse than those generally caused by the administration of the chemotherapeutic compound at lower doses. Both aspects were deeded to be essential prerequisites for better exploiting the drug's effectiveness.

195mPt-cis-Dichlorodiammine platinum(II) (195mPt-cisplatin) binds in vitro essentially irreversibly to bovine serum albumin (BSA) and rat plasma proteins at a 195mPt-cisplatin/protein molar ratio of 1:1 or less. The binding to rat serum proteins shows no apparent specificity, with albumin retaining the greatest fraction of the label (40-55%). The binding is neither instantaneous nor linear with time. Upon intravenous administration to rats of a rat serum protein bound 195mPt-cisplatin, the label remains bound to the proteins while in the blood. The label is cleared from the blood faster than a relatively native 131I-labeled human serum albumin (HSA) and accumulates primarily in the liver. The blood disappearance and organ distribution of the rat serum protein bound 195mPt-cisplatin is consistent with a modified protein. Approx. 11% of the injected dose was excreted via the urine in 24 h in a form which has a different chromatographic mobility than free 195mPt-cisplatin. Since little intact protein is lost in the urine, the excreted label probably represents the product(s) of metabolism of the rat serum bound 195mPt-cisplatin. Due to the apparent irreversibility (both in vivo and in vitro) of the protein/195mPt-cisplatin complex, it is unlikely that the protein bound fraction of the administered 'free' drug will serve as a therapeutically useful 'drug resevoir'.

A model of human advanced ovarian cancer was made by intra-abdominal inoculation of a cloned ovarian adenocarcinoma cell line of the Sprague-Dawley rat (ROT68/C1) into isologous newborn rats. Intra-abdominal tumors and tumors metastatic to the lung developed in 100% of the animals within 3 weeks after inoculation. With use of this model the intraperitoneal and intravenous routes of cisplatin (cis-diamminedichloroplatinum) administration were compared with regard to both the pharmacokinetics and the antitumor activity. After 2 hours of administration the serum cisplatin values were greater following use of the intraperitoneal route than with use of the intravenous route. Cisplatin values in the intra-abdominal tumor tissues were greater after the intraperitoneal route of administration than the intravenous route, and the growth was suppressed more prominently after intraperitoneal administration. No difference in drug values in the kidney tissues was found between the two administration routes. Thus the intraperitoneal route of cisplatin administration seems to be much more effective against advanced ovarian cancer confined to intra-abdominal cavity than does the intravenous route.

The applicability of reversed-phase ion-pair chromatography with on-line inductively coupled plasma atomic emission spectrometric detection was investigated for the analysis of cisplatin, its hydrolysis products and two methionine-platinum complexes in aqueous solutions. The detector response for cisplatin was linear over three orders of magnitude. The detection limit was found to be 3.5 ng, corresponding to a concentration detection limit of 35 ng/ml. The ion-pair system developed was applied to the analysis of (bio)transformation products originating from cisplatin in human and rat plasma in vitro and in vivo. In vitro, a number of platinum-containing species are formed, two of which show the same retention as the mono- and diaqua hydrolysis products of cisplatin. Preliminary results for in vivo experiments indicate cisplatin to be the predominant species in rat plasma.
