Background: Several techniques have been described to repair giant omphaloceles. Component Separation Technique (CST) is a surgical technique first reported in adults to repair large abdominal wall defects. The purpose of this study is to describe our experience using this technique in repairing large ventral hernias secondary to giant omphaloceles in children. Methods: Nine patients with giant omphalocele were treated. The abdominal wall was reconstructed using CST, based on the enlargement of the abdominal wall by separate mobilization of the muscular layers. Patients were monitored for complications during admission, postoperative complications, and all patients were seen for follow-up. Results: Component separation technique was performed at a median age of 7.2 years. The median diameter of the hernia was 109.8 cm2. There was no mortality. Minor complications were seen (infection, skin necrosis, and hematoma). There were no recurrences after an average follow-up of 3.3 years (2 - 4 years). Conclusions: Component separation technique is a valuable method for repairing large ventral hernias as a consequence of giant omphaloceles in children. Our series supports the use of CST in children, in a wide age range, from newborns for primary closure to adolescence in cases of giant ventral hernias secondary to omphaloceles with multiple previous surgeries. Computational fluid dynamics (CFD) is a recognised modelling tool for simulating pollutant dispersion in urban street canyons. These studies require accurate street geometry, sufficient modelling experience and access to software to provide reliable outputs. Parked cars are known to act as a passive barrier and affects pollutant dispersion in an urban street canyon. This modelling study examines the impact of accurate geometrical detailing and suitable mesh sizing on air pollution concentrations measured at street level. Mean steady state concentrations of carbon monoxide (CO) were measured in a reference scenario (without parked cars) and three parked car scenarios. A comparison of the results indicated that individual rectangular blocks and generic car scenarios overestimated pollutant concentrations on the footpaths by up to 25% when compared to the results from the five distinct car designs scenario, with the parked cars acting as a passive barrier in each scenario under different wind conditions. The most notable finding demonstrated how the five distinct car designs scenario presented an increase in CO concentrations on the leeward footpath as opposed to a reduction in the results in the other coarser geometrically detailed scenarios. The parked cars trapped pollutants in each scenario with low wind speeds for perpendicular wind conditions, as higher concentrations were measured on the footpaths. A further examination of pollutant decay rates demonstrated how the geometrical detailing of the parked cars also impacted upon the duration taken for pollutants to escape from the footpath zones. These findings demonstrate how parked cars affects pollutant dispersion and concentrations measured on the footpaths at street level, and the adoption of a suitable meshing scheme is vital to capture results in the zone of interest in an urban street canyon., (Copyright © 2018 Elsevier B.V. Purpose: To evaluate the relationship between age and race on the receipt of definitive therapy among men with high-risk prostate cancer (CaP)., Methods: We used the Surveillance, Epidemiology and End Results Program to identify 62,644 men with high-risk CaP (PSA >20 or Gleason 8-10 or stage ≥cT3a) diagnosed from 2004 to 2010. Multivariable logistic regression analysis modeled the interaction between age and race and its association with receipt of definitive therapy on 57,674 patients (47,879 white men; 9,795 African American [AA] men) with complete data on the covariates of interest., Results: Among men age ≥70, AA men had a higher risk of CaP-specific mortality (PCSM) compared to white men after adjusting for sociodemographic and prostate cancer-specific factors (Adjusted HR 1.20; 95% CI 1.02-1.38; P=0.02). Nevertheless, a significant interaction between race and age was found (Pinteraction=0.01), such that the adjusted odds of receiving definitive treatment for AA vs. white was 0.67 (95% CI 0.62-0.73; P<0.001) among men age <70, but was 0.60 (95% CI 0.55-0.66; P<0.001) among men age ≥70, suggesting increased racial disparity in the receipt of definitive treatment among older men., Conclusion: AA men with high-risk CaP are less likely to receive definitive therapy than white men. This disparity is significantly larger among men age ≥70, despite excess PCSM among AA men in this group. With a rapidly expanding population of older minority men, this disparity should be urgently addressed to prevent increasing disparities in cancer care., (Copyright © 2014 Elsevier Inc. Tensiomyography is a non-invasive method of neuromuscular assessment used to measure muscle action characteristics, muscle tone, and muscle fiber type, and provides information on acute and chronic responses of muscle to different training loads. The aims of the present study were: to analyse differences in muscle response and mechanical characteristics of two major muscles of the lower extremity in a large group of Spanish soccer players according to playing position, and to provide group norms against which clinical findings may be compared. Data were collected from 78 professional soccer players (age 26.6 ± 4.4 years; height: 179.2 ± 5.3 cm; body mass: 75.8 ± 5.3 kg). Tensiomyography was recorded from the rectus femoris (RF) and biceps femoris (BF) muscles after 2 days without take part in any strenuous exercise or training. Five tensiomyographic parameters were analyzed: maximal displacement (D(m)), contraction time (T(c)), sustain time (T(s)), delay time (T(d)), and half-relaxation time (T(r)). A good to excellent intra-session reliability was found for all contractile parameters (ICC ranged from 0.78 to 0.95). No significant differences between players of any position were observed in absolute values of BF. However, significant differences were observed for T(c), T(r) and T(s) between the different playing positions on RF (P < 0.05, effect size ranged from 1.3 to 1.6). Professional soccer players showed muscles with ability to rapidly generate force during contractions. The neuromuscular profile provided could help in identifying the normative data that are important for the different positions in order to optimize the training and recovery process of each individual player., (Copyright © 2012 Elsevier Ltd. Initiation of transcription mediated by RNA polymerase II requires a number of transcription factors among which TFIID is the major core promoter recognition factor. TFIID is composed of highly conserved factors which include the TATA-binding protein (TBP) and about 14 TBP-associated factors (TAFs). Recently, the complete Arabidopsis TAF family has been identified. To obtain functional information about Arabidopsis TAFs, we analyzed a T-DNA insertion mutant for AtTAF6. Segregation analysis showed that plants homozygous for the mutant allele were never found, indicating that inhibition of the AtTAF6 function is lethal. Genetic experiments also revealed that the male gametophyte was affected by the attaf6 mutation since significant reduced transmission of the mutant allele through the male gametophyte was observed. Detailed histological and morphological analysis showed that the T-DNA insertion in AtTAF6 specifically affects pollen tube growth, indicating that the transcriptional regulation of only a specific subset of genes is controlled by this basal transcription factor.
