The new trend in urban stormwater management is to preserve natural hydrological conditions by addressing changes from urbanization. The software Chaak: Urban Hydrological Impact Modeling is a new hydroinformatics tool designed for sustainable modeling, simulation, and analysis of stormwater sewer networks. It emphasizes two key aspects: enhancing planning for Sustainable Urban Drainage Systems (SUDS) to restore natural hydrological conditions and providing an alternative to conventional storm drainage designs. Additionally, it encourages global scientific collaboration to quickly advance the tool's capabilities through structured development and quality control.

Abstract

The proton drip-line nucleus 17Ne is considered a good candidate for a Borromean two-proton halo with a 15O + p + p structure. Angular distributions of the elastic scattering and inclusive 15O production for a 136 MeV 17Ne beam incident on a 208Pb target were measured for the first time at the SPIRAL1 facility, GANIL. Use of the GLORIA detector array allowed high-resolution data over a wide angular range from 20∘ up to 95∘ in the laboratory frame to be obtained. The elastic scattering angular distribution shows similarities with those for both 6He and 20Ne at equivalent collision energies with respect to the corresponding Coulomb barriers, exhibiting the suppression of the Coulomb rainbow peak characteristic of strong coupling. Optical model and coupled channel fits suggest that this is due to a combination of coupling to low-lying quadrupole resonances and Coulomb dipole coupling to the low-lying continuum, although their relative importance depends on the relevant B(E2) values which remain to be firmly determined.

3. TEN-YEAR FOLLOW-UP OF MSSA TREATMENT PRESCRIPTION BEHAVIOR AND POST-PRESCRIPTION AMS INTERVENTION IMPACT

Abstract

Intro: Methicillin susceptibility prevails within clinically relevant Staphylococcus aureus strains. This study aimed to describe proportions of Methicillin- Susceptible S. aureus (MSSA), prescription behavior for its treatment, and impact of a post-prescription antimicrobial stewardship (AMS) strategy within two private tertiary care Mexican hospitals over 10 years. Methods: Patients older than 14 years with an S. aureus culture-proven infection between 2011 to 2020 were included. Disease was classified as monomicrobial or polymicrobial; strains were classified as MSSA if Cefoxitin susceptible. Spectrum Score was calculated according to the participants’ antibiotic regimen at: admission, 72 hours after culture results, and end-of- therapy. During the last two years analyzed, post-prescription review with feedback to prescribers as an AMS strategy. MSSA rates were compared within both time periods. Findings: We identified 1322 clinically relevant S. aureus strains isolated from 2011 to 2020. A total of 817 specimens were MSSA (61.8%), while only 505 demonstrated methicillin resistance(38.2%). During the antimicrobial stewardship period, MSSA proportion increased by 21.6% compared to the pre- intervention period(78.9% vs 57.3%). In patients with monomicrobial MSSA infections, mean decrease Spectrum Score was lower at end of therapy in comparison to that calculated 72 hours after culture results(1.3 vs 0.2 points). The latter didn't seem to differ when contrasting the Spectrum score between the period before(0.3 points) and after(0.2 points) the AMS intervention. The three most commonly used antimicrobials at admission, 72 hours after culture results, and end of therapy were respectively: Ceftriaxone, Meropenem, Vancomycin; Clindamycin, Vancomycin, Linezolid; and Trimethoprim/Sulfamethoxazole, Clindamycin, Ciprofloxacin. Conclusion: MSSA caused more infections in comparison to methicillin-resistant strains in our studied population. Despite the poor antimicrobial de-escalation in MSSA infections, evidenced by the high Spectrum scores, methicillin resistance has decreased over time. Post prescription interventions have not proven to be effective at these private hospitals and AMS strategies should be expanded to continue avoiding antibiotic resistance.

4. Second litter syndrome in Iberian pig breed: factors influencing the performance

Abstract

Second litter syndrome (SLS) consists of a loss of prolificacy in the second parity (P2), when a sow presents the same or lower results for litter size than in the first parity (P1). This syndrome has been reported for modern prolific breeds but has not been studied for rustic breeds. The objectives of this study are to determine how and to what degree Iberian sows (a low productivity breed recently raised on intensive farms) are affected by SLS; to establish a target and reference levels; and to assess the factors influencing the performance. Analysed data correspond to 66 Spanish farms with a total of 126 140 Iberian sows. The average Iberian sow prolificacy in P1 was 8.91 total born (TB) and 8.47 born alive (BA) piglets, whereas in P2, it decreased by −0.05 TB and −0.01 BA piglets, suggesting some general incidence of SLS. At the sow level, 56.63% did not improve prolificacy in terms of BA piglets in P2, and 16.98% had a clear decrease in prolificacy, losing ≥3 BA piglets in P2. Within herds, a mean of 57.75% of sows showed SLS, with an evident decrease in the number of BA piglets in P2. The plausible target for the Iberian farm’s prolificacy comes from the quartile of farms with the lowest percentage of SLS sows within the farms with the highest prolificacy between P1 and P2 (mean of 8.77 BA). So, in this subset of farms (N = 17), 47.3% of sows improved their prolificacy in P2 (i.e. did not show SLS). Hence, half the sows could be expected to show SLS even on farms with a good performance. Finally, this study brings out the main factors reducing P2 prolificacy through SLS in the Iberian breed: later age at first farrowing, long first lactation length, medium weaning to conception interval and large litter size in P1. In conclusion, improving the reproductive performance of Iberian farms requires reducing the percentage of sows with SLS, paying special attention to those risk factors. The knowledge derived from this study can provide references for comparing and establishing objectives of performance on Iberian sow farms which can be used for other robust breeds.

5. Assessing heat tolerance through productive vs physiological indicators. Data from dairy sheep under on-farm conditions

Abstract

The search for criteria that allow the quantification of the level of thermotolerance of an animal is a major challenge in animal production. Different criteria have been proposed to date, mainly the use of routine milk recording and weather information or the collection of physiological measures related with heat stress. This study aimed at quantifying the association between indicators of heat tolerance derived from productive and physiological traits. For this purpose, two physiological traits, rectal temperature (RT) and respiratory rate (RR), and nine productive traits (milk yield, fat, protein and lactose yields and contents, casein and urea contents) were measured from June to September of 2018 in three flocks of Manchega sheep. A total of 462 lactating ewes participated in the study. Air temperature (Ta), relative humidity (RH) and associated temperature and humidity index (THI) were recorded inside the barn and also obtained from the closest weather station from the national meteorological network, and used to produce several measurements of heat load on animals. Based on the results of fits for quadratic and cubic regressions on the alternative heat load measures, the cubic regression on Ta and THI obtained inside the barn at time of recording yielded the best fit for physiological and productive parameters. The use of weather information taken from the official weather station closest to the farm also produced similar estimates and could be considered as a good alternative when on-farm meteorological data are not available. Two-trait random regression models that involved individual intercept and slope of response to heat load were used to obtain correlations between basal levels and heat tolerance within and across traits. Estimated correlations showed that animals with smaller vs larger basal levels of RT and RR tend to be more vs less heat tolerant (correlations up to 0.46) and that slopes of increase for RR and RT under heat stress were highly correlated (0.82). Estimated correlations between tolerance criteria from production vs physiology were up to −0.5 (between milk yield and RT), indicating that animals that show less increase in body temperature also tend to show a smaller decrease in production under heat stress. However, because of the non-unity correlation between the two types of indicators of heat tolerance, both sources of information, productive and physiological ought to be taken into account to ensure the long-term sustainability of selection programmes aiming at improving productive levels when heat stress is a concerning issue.

6. Measuring farmers' attitude towards breeding tools: the Livestock Breeding Attitude Scale

Abstract

Under-use of genetic improvement tools and low participation in breeding programmes are key drivers of breeding programmes under-performance. Both aspects are heavily influenced by farmers attitudes which, to date, have not been analysed in an objective and systematic manner. A key factor constraining the implementation of attitudinal studies towards livestock breeding tools is the lack of a reference scale for measuring attitudes. In this research, we provide the livestock breeding sector with such a reference measure. We developed the scale following the standardized psychometric methodologies and statistical tools. Then, as a case study, we used the scale to explore the attitudes of beef and dairy sheep farmers in Australia, New Zealand and Spain and analysed farmer and farming system factors related to those attitudes. Fourteen sheep and beef breed associations facilitated the implementation of a survey of 547 farmers, generating data that was used for the scale evaluation. The relationship between attitudinal factors and farmer and farming system factors was analysed using generalized linear models across and within breeds. The results suggest that the 8-item definitive scale we have developed is appropriate to measure farmer attitudes. We found that attitudes towards genetic improvement tools have two components; i) traditional selection and ii) genetic and genomic selection combined. This means that positive attitudes towards traditional phenotypic selection do not necessarily imply a negative attitude towards genetic and genomic selection tools. Farmer attitudes varied greatly not only across the studied breeds, species and countries, but also within them. High-educated farmers of business-oriented farms tend to have the most negative attitude towards traditional selection. However, attitudes towards genetic and genomic selection tools could not be linked to these factors. Finally, we found that the breed raised had a large effect on farmer attitude. These findings may help in the evolution of breeding programmes by identifying both the farmers most inclined to uptake breeding innovations in the early stages of its establishment and the farmers who would be more reluctant to participate in such programmes, thus informing where to focus extension efforts.

Abstract

Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P

Abstract

This study investigated the potential application of genomic selection under a multi-breed scheme in the Spanish autochthonous beef cattle populations using a simulation study that replicates the structure of linkage disequilibrium obtained from a sample of 25 triplets of sire/dam/offspring per population and using the BovineHD Beadchip. Purebred and combined reference sets were used for the genomic evaluation and several scenarios of different genetic architecture of the trait were investigated. The single-breed evaluations yielded the highest within-breed accuracies. Across breed accuracies were found low but positive on average confirming the genetic connectedness between the populations. If the same genotyping effort is split in several populations, the accuracies were lower when compared with single-breed evaluation, but showed a small advantage over small-sized purebred reference sets over the accuracies of subsequent generations. Besides, the genetic architecture of the trait did not show any relevant effect on the accuracy with the exception of rare variants, which yielded slightly lower results and higher loss of predictive ability over the generations.

Abstract

The Spanish local beef cattle breeds have most likely common origin followed by a process of differentiation. This particular historical evolution has most probably left detectable signatures in the genome. The objective of this study was to identify genomic regions associated with differentiation processes in seven Spanish autochthonous populations (Asturiana de los Valles (AV), Avileña-Negra Ibérica (ANI), Bruna dels Pirineus (BP), Morucha (Mo), Pirenaica (Pi), Retinta (Re) and Rubia Gallega (RG)). The BovineHD 777K BeadChip was used on 342 individuals (AV, n=50; ANI, n=48; BP, n=50; Mo, n=50; Pi, n=48; Re, n=48; RG, n=48) chosen to be as unrelated as possible. We calculated the fixation index (FST) and performed a Bayesian analysis named SelEstim. The output of both procedures was very similar, although the Bayesian analysis provided a richer inference and allowed us to calculate significance thresholds by generating a pseudo-observed data set from the estimated posterior distributions. We identified a very large number of genomic regions, but when a very restrictive significance threshold was applied these regions were reduced to only 10. Among them, four regions can be highlighted because they comprised a large number of single nucleotide polymorphisms and showed extremely high signals (Kullback–Leiber divergence (KLD)>6). They are located in BTA 2 (5 575 950 to 10 152 228 base pairs (bp)), BTA 5 (17 596 734 to 18 850 702 bp), BTA 6 (37 853 912 to 39 441 548 bp) and BTA 18 (13 345 515 to 15 243 838 bp) and harbor, among others, the MSTN (Myostatin), KIT-LG (KIT Ligand), LAP3 (leucine aminopeptidase 3), NAPCG (non-SMC condensing I complex, subunit G), LCORL (ligand dependent nuclear receptor corepressor-like) and MC1R (Melanocortin 1 receptor) genes. Knowledge on these genomic regions allows to identify potential targets of recent selection and helps to define potential candidate genes associated with traits of interest, such as coat color, muscle development, fertility, growth, carcass and immunological response.

Abstract

SWOT (Strengths, Weaknesses, Opportunities and Threats) analysis is a tool widely used to help in decision making in complex systems. It suits to exploring the issues and measures related to the conservation and development of local breeds, as it allows the integration of many driving factors influencing breed dynamics. We developed a quantified SWOT method as a decision-making tool for identification and ranking of conservation and development strategies of local breeds, and applied it to a set of 13 cattle breeds of six European countries. The method has four steps: definition of the system, identification and grouping of the driving factors, quantification of the importance of driving factors and identification and prioritization of the strategies. The factors were determined following a multi-stakeholder approach and grouped with a three-level structure. Animal genetic resources expert groups ranked the factors, and a quantification process was implemented to identify and prioritize strategies. The proposed SWOT methodology allows analyzing the dynamics of local cattle breeds in a structured and systematic way. It is a flexible tool developed to assist different stakeholders in defining the strategies and actions. The quantification process allows the comparison of the driving factors and the prioritization of the strategies for the conservation and development of local cattle breeds. We identified 99 factors across the breeds. Although the situation is very heterogeneous, the future of these breeds may be promising. The most important strengths and weaknesses were related to production systems and farmers. The most important opportunities were found in marketing new products, whereas the most relevant threats were found in selling the current products. The across-breed strategies utility decreased as they gained specificity. Therefore, the strategies at European level should focus on general aspects and be flexible enough to be adapted to the country and breed specificities.

11. Second litter syndrome in Iberian pig breed: factors influencing the performance

Abstract

Second litter syndrome (SLS) consists of a loss of prolificacy in the second parity (P2), when a sow presents the same or lower results for litter size than in the first parity (P1). This syndrome has been reported for modern prolific breeds but has not been studied for rustic breeds. The objectives of this study are to determine how and to what degree Iberian sows (a low productivity breed recently raised on intensive farms) are affected by SLS; to establish a target and reference levels; and to assess the factors influencing the performance. Analysed data correspond to 66 Spanish farms with a total of 126 140 Iberian sows. The average Iberian sow prolificacy in P1 was 8.91 total born (TB) and 8.47 born alive (BA) piglets, whereas in P2, it decreased by −0.05 TB and −0.01 BA piglets, suggesting some general incidence of SLS. At the sow level, 56.63% did not improve prolificacy in terms of BA piglets in P2, and 16.98% had a clear decrease in prolificacy, losing ≥3 BA piglets in P2. Within herds, a mean of 57.75% of sows showed SLS, with an evident decrease in the number of BA piglets in P2. The plausible target for the Iberian farm’s prolificacy comes from the quartile of farms with the lowest percentage of SLS sows within the farms with the highest prolificacy between P1 and P2 (mean of 8.77 BA). So, in this subset of farms (N = 17), 47.3% of sows improved their prolificacy in P2 (i.e. did not show SLS). Hence, half the sows could be expected to show SLS even on farms with a good performance. Finally, this study brings out the main factors reducing P2 prolificacy through SLS in the Iberian breed: later age at first farrowing, long first lactation length, medium weaning to conception interval and large litter size in P1. In conclusion, improving the reproductive performance of Iberian farms requires reducing the percentage of sows with SLS, paying special attention to those risk factors. Abstract

We analyzed the influence of the stratification process in the vertical distribution of larval fish in a microtidal coastal Mediterranean zone. By applying a Self Organizing Maps (SOM) technique, we could analyze a complex dataset accounting for non-linear processes. The analysis integrated multivariate data on larval fish and environmental parameters in two depth strata through two-time components (nictimeral and fortnightly through the main spawning seasons), and considered size-based information. Although causal relationships cannot be constructed, the use of SOM analyses enabled the description of the whole system evolution overcoming the constraints of linear approaches in complex multivariate datasets with multiple dependencies in the data. 13. Are farmers motivated to select for heat tolerance? Linking attitudinal factors, perceived climate change impacts, and social trust to farmers' breeding desires

Abstract

ABSTRACT: This study provides an understanding of dairy farmers' willingness to include heat tolerance in breeding goals and the modulating effect of sociopsychological factors and farm profile. A survey instrument including a choice experiment was designed to specifically address the trade-off between heat tolerance and milk production level. A total of 122 farmers across cattle, goat, and sheep farms were surveyed face-to-face. The results of the experiment show that most farmers perceive that heat stress and climate change are increasingly important problems, and that farming communities should invest more in generating knowledge and resources on mitigation strategies. However, we found limited initial support for selection for heat tolerance. This attitude changed when farmers were presented with objective information on the benefits and limitations of the different breeding choices, after which most farmers supported selection for heat tolerance, but only if doing so would compromise milk production gains to a small extent. Our results show that farmers' selection choices are driven by the interactions between heat stress risk perception, attitudes toward breeding tools, social trust, the species reared, and farm production level. In general, farmers willing to support selection of heat-tolerant animals are those with positive attitudes toward genetic values and genomic information and a strong perception of climate change and heat stress impacts on farms. On the contrary, negative support for selection for heat tolerance is found among farmers with high milk production levels; high trust in farming magazines, livestock farmers' associations, and veterinarians; and low trust in environmental and animalist groups.

Abstract

Background: The diversity of patients' symptomatology among people seeking treatment on community-based mental health services poses significant challenges to traditional models of care. Recent approaches favor identifying transdiagnostic factors that allow a better understanding of patient heterogeneity and designing more effective and quality interventions. This study examines the heterogeneity of patients with internalizing symptoms based on profiles identified with cognitive and motivational control variables. Differences between these profiles on dimensional measures of psychopathology and quality of life are examined. Methods: 263 patients were selected by non-probabilistic sampling procedures on mental health services in the province of Huelva (Spain). A latent class analysis on the standardized scale scores of The Behavioral Inhibition/Behavioral Activation System Scales and the Effortful Control Scale of the Adult Temperament Questionnaire Short-Form was conducted. Profiles were compared on the scores of the Inventory of Depression and Anxiety Symptoms-II, the WHO Disability Assessment Schedule II, and the Health Assessment Questionnaire SF-36. Results: The four latent profile solution is the one that showed the best fit indicators and substantive interpretability, with a kappa of 0.94 in the cross-validation procedure with 75% of the sample. No sex differences were found between the profiles (χ32 5.17, p = .160). Profiles #1 and #3, both characterized by an imbalance between low activation and high inhibition, had lower well-being, lower functionality, and quality of life. When comparing profile #2 (featuring the highest inhibitory control) lower scores on most internalizing scales are observed, specially claustrophobia, social anxiety, panic mania. Profile #4 (low control, high activation, and high inhibition) showed greater scores on both mania and euphoria and lower scores on emotional role. Conclusions: We identified four distinctive profiles that had overly increased behavioral inhibition (as expected in internalizing disorders) and differed in the degree of imbalance between inhibition and activation systems, and between motivational systems and top-down cognitive control. The profile characterized by high activation and reduced cognitive (inhibitory) control was the one showing greater mood-related symptoms and lower levels of quality of life. These profiles could be generated by treatment providers to guide clinical management in an evidence-based manner.

Abstract

In recent decades, non-structural exposed brick facades have been widely used in Spain and other European and American countries. However, when they are partially supported by floor slabs, to give the image of a continuous element, they present problems that can lead to risks such as collapse and detachment of ceramic pieces. In this article, twenty-two affected buildings located in several Spanish cities were analyzed to determine the parameters that induce risk situations, and to identify the causes of each type of damage. The aim was to verify, using data extracted from real behavior of the facades under analysis, the fragility of most analytical models that have been adopted, in order to avoid anomalies. Consequently, the aim was also to obtain a more reliable starting point for future diagnosis and, when necessary, repair of such vulnerable face brick facades.

Abstract

To our knowledge, this is the first reported case of renal cell carcinoma in kidney horseshoe diagnosed in the second trimester of pregnancy. We performed open radical nephrectomy when the pregnancy was completed. Kidney cancer is rare during pregnancy and the symptoms can be mimic urinary infection. The diagnosis and its management can be a challenge.

Resumen: Introducción: Los niños afectados de inmunodeficiencias primarias presentan infecciones graves y mayor prevalencia de manifestaciones autoinmunitarias, alergias y enfermedad linfoproliferativa. El trasplante alogénico de precursores hematopoyéticos ha sido el único tratamiento curativo durante décadas. Pacientes y métodos: Pacientes con inmunodeficiencias primarias que recibieron trasplante alogénico de precursores hematopoyéticos desde 1985 hasta 2011, recogidos en el Registro Nacional del Grupo Español para Trasplante de Médula Ósea en Niños. Resultados: Ciento cincuenta y nueve niños recibieron un total de 173 trasplantes, 97 por inmunodeficiencia combinada grave, 30 por enfermedades de disregulación inmunitaria, 25 por síndrome de Wiskott-Aldrich y 21 por defectos de número y/o función de los fagocitos.La mediana de edad al diagnóstico fue de 6 meses (17 días-168 meses) y de 12 meses (1 mes-189 meses) al trasplante.Los donantes fueron hermano HLA idéntico en 30 (19%), donante familiar alternativo en 40 (25%) y donante no emparentado en 89 (56%). La fuente de progenitores fue médula ósea en 68 (43%), sangre de cordón umbilical en 52 (33%) y sangre periférica en 39 (24%).Permanecen vivos 98 niños (61,6%), 57 (35,9%) fallecieron. La supervivencia libre de enfermedad a los 10 años fue del 63, el 90% para los pacientes trasplantados de hermano HLA idéntico, el 36% para los trasplantados de un donante familiar alternativo y el 66 para los trasplantados de donante no emparentado. Conclusiones: Los mejores resultados se obtienen con un hermano HLA idéntico, cuando no se dispone de este, otros donantes deben ser considerados. Abstract: Introduction: Children with primary immunodeficiency have severe life-threatening infections and a higher prevalence of autoimmune problems, allergy and lymphoproliferative disorders. Allogenic hematopoietic stem cell transplantation has been the only potentially curative option. Patients and methods: Patients with primary immunodeficiency underwent allogenic stem cell transplantation in the period 1985-2011, and registered in the Spanish Working Party for Bone Marrow Transplantation in Children. Results: One hundred and fifty nine patients underwent 173 allogenic stem cell transplantations, of whom 97 had severe combined immunodeficiency, 30 with immune dysregulation disorders, 25 Wiskott-Aldrich syndrome, and 21 phagocyte disorders.The median patient age at diagnosis was 6 months (range: 17 days – 168 months) and the median patient age at transplant was 12 months (range: 1 month – 189 months).The donors were 30 (19%) identical siblings, 40 (25%) alternative family donors, and 89 (56%) unrelated donors. The source of stem cells was bone marrow in 68 (43%), cord blood in 52 (33%), and peripheral blood in 39 (24%).Ninety eight (61.6%) are alive, 57 (35.9%) died. Event-free survival at 10 years was 63%, with 90% for children transplanted from identical siblings, 36% for those transplanted from alternative family donors, and 66% for those transplanted from unrelated donors. Conclusions: The best results have been obtained with identical siblings, but other options may be considered.

Abstract

This study analyzed the haplotype diversity along the genome of seven Spanish Beef Cattle populations within regions of 500 kb using the information provided by the BovineHD Beadchip. The results of the analysis pointed out a strong variability of the haplotype diversity across the genome, which is greatly conserved across populations. This strong concordance between populations suggests that the reasons behind it are intrinsic to the structure of the bovine genome and caused probably by the mutation or recombination rate. Nevertheless, some of the genomic regions with very large haplotype diversity are also due of genome assembly errors. © 2017 Elsevier B.V.

Abstract

The extraction of plant constituents is essential to isolate biologically active compounds, aimed to understand their role on the treatment of diabetes.This study was designed to explore the preliminary phytochemical and physicochemical analysis of Carica papaya L., Caricaceae, leaf, and further evaluation of its hypoglycemic effect on diabetic rats. C. papaya leaves were extracted using chloroform, n-hexane or ethanol. For each extract a phytochemical screening was performed. The tests were conducted in triplicate and the qualitative and quantitative determination of the various metabolites was done using analytical standards proposed by Mexican Herbal Pharmacopoeia. The chloroform extract, containing steroids and quinones as major components, was chosen to study C. papaya biological effects. The chloroform extract was evaporated to dryness, and doses 0, 31, 62, 125 mg/kg were orally administered in 300 μl polyethylene glycol to diabetic rats; and 0 and 62 mg/kg to non-diabetic rats. After a 20-day treatment with the chloroform extract, the animals were sacrificed and blood was obtained for biochemical studies. The main effect observed was a decrease in serum glucose, triglycerides and transaminases in diabetic rats after the administration of C. papaya chloroform extract. These results confirm the potential beneficial action of C. papaya to treat the symptoms of diabetic patients. 21. Resultados del trasplante de progenitores hematopoyéticos en hemoglobinopatías: talasemia maior y enfermedad drepanocítica

Abstract

Resumen: Introducción: La prevalencia de las hemoglobinopatías en nuestro medio ha aumentado como consecuencia de los flujos migratorios.La talasemia maior cursa con anemia hemolítica crónica y necesidad de transfusiones regulares desde el año de vida. La enfermedad drepanocítica cursa con anemia, vasculopatía y daño orgánico progresivo. En ambas, la esperanza de vida está disminuida. El trasplante alogénico de progenitores hematopoyéticos es una opción de curación para estos pacientes. Pacientes: Diecisiete pacientes recibieron un trasplante alogénico de progenitores hematopoyéticos: 14 afectados de talasemia maior y 3 de enfermedad drepanocítica. Resultados: Los donantes fueron en los pacientes con talasemia maior 9 hermanos HLA-idénticos, 2 progenitores con una diferencia antigénica HLA y 3 donantes no emparentados, y en aquellos con enfermedad drepanocítica, 3 hermanos HLA-idénticos. La fuente fue la médula ósea en todos, excepto uno.La mediana de edad al trasplante de progenitores hematopoyéticos fue de 6 años (intervalo: 1-16) en los niños con talasemia maior y doce años (intervalo: 8-15) en los niños con enfermedad drepanocítica.Se confirmó injerto medular en todos los pacientes. Dos con talasemia maior presentaron fallo de injerto secundario, precisando nuevamente soporte transfusional. Trece pacientes presentaron quimerismo completo y 2 quimerismo mixto, todos con normalización de la cifra de hemoglobina y sin requerimiento de transfusiones. Los pacientes con enfermedad drepanocítica no presentaron más episodios vasooclusivos y la funciones pulmonar y cerebral en aquellos pacientes que presentaban afectación en el momento del trasplante se estabilizaron. Tres pacientes con talasemia maior desarrollaron enfermedad injerto contra huésped crónica y 5, hipogonadismo hipogonadotropo. Conclusiones: Nuestra experiencia confirma que el trasplante alogénico de progenitores hematopoyéticos de hermano HLA idéntico es una buena opción en el tratamiento de la talasemia maior y la enfermedad drepanocítica. En el caso de la talasemia maior, el trasplante de donante no emparentado es una opción terapéutica en centros especializados, ya que, a pesar de los buenos resultados presentados, la morbimortalidad de este procedimiento puede ser elevada, frente a la alternativa de un tratamiento médico no curativo pero con expectativas de supervivencia prolongada. En el caso de la enfermedad drepanocítica, el trasplante de donante no emparentado todavía está en fases preliminares de investigación. Abstract: Background: The prevalence of hemoglobinopathies in Spain is increasing as a result of immigration.Thalassemia major presents with chronic hemolytic anemia that requires regular red blood cell transfusions within the first year of life. Patients with sickle cell disease suffer from chronic anemia, vasculopathy and progressive damage in almost any organ. There is decreased life expectancy in both conditions. Allogeneic hematopoietic stem cell transplantation represents the only potentially curative option. Patients: Seventeen patients (fourteen thalassemia major, and three sickle cell disease) underwent allogeneic hematopoietic stem cell transplantations. Results: In the thalassemia group, nine donors were HLA-geno-identical siblings, two were partially matched related donors (one HLA allele mismatch), and three unrelated donors. All three patients with sickle cell disease were transplanted from HLA-geno-identical siblings. The source of stem cells was bone marrow in sixteen cases.Median patient age at transplant was six years (range: 1–16) in the thalassemia group, and twelve years (range: 8–15) in the sickle cell disease group.The graft was successful in all patients. Secondary graft rejection was observed in two thalassemia patients rendering them dependent on blood transfusions. Complete chimerism was observed in thirteen patients and, although mixed chimerism occurred in two, with all of them showing normal hemoglobin levels after transplantation and not requiring further transfusion support. Patients affected by sickle cell disease did not present with new vaso-occlusive crises, and stabilization of pulmonary and neurological function was observed. Chronic graft-versus-host disease was detected in three patients affected by thalassemia, and hypogonadotrophic hypogonadism in five patients. Conclusions: We conclude that for thalassemia major and sickle cell disease, allogenic hematopoietic stem cell transplantation from HLA-geno-identical siblings offers a high probability of complication-free survival. Despite good results, morbidity and mortality associated with transplantation from unrelated donors is a risk that might be considered, in contrast to a non-curative medical treatment that offers a long term survival. For thalassemia major groups it could be an option, but not for sickle cell disease, which is still in the investigational phase.

Abstract

Resumen: Introducción: Hasta el momento se han descrito más de 200 inmunodeficiencias primarias (IDP) diagnosticándose un 60% en la edad pediátrica. Un diagnóstico y tratamiento precoces mejoran significativamente el pronóstico de estos pacientes. Objetivo: Análisis de los pacientes afectos de IDP diagnosticados en un centro de referencia durante 10 años. Pacientes y métodos: Revisión retrospectiva y análisis de las características clínicas, epidemiológicas, resultados de laboratorio, tratamiento administrado y curso evolutivo de las mismas. Resultados: Ciento ochenta y nueve pacientes fueron diagnosticados-controlados en este periodo de tiempo, siendo el déficit predominante de anticuerpos el diagnóstico más frecuente. En nuestra serie, la clínica de presentación al diagnóstico fue: infecciones respiratorias de repetición en pacientes con déficit selectivo de IgA e inmunodeficiencia común variable (IDCV), retraso ponderoestatural e infecciones oportunistas (principalmente virus) en pacientes con inmunodeficiencia combinada grave (IDCG), abscesos cutáneos (Staphylococcus aureus, Serratia spp.) y neumonía (Aspergillus spp., Rhodococcus equi) en la enfermedad granulomatosa crónica, cardiopatía y fenotipo compatible en el síndrome de deleción 22q11, abscesos cutáneos y ectima gangrenoso en la neutropenia congénita grave e infecciones oportunistas y sepsis (Pseudomonas aeruginosa) en niños con agammaglobulinemia ligada al cromosoma X (ALX). Se describen manifestaciones linfoproliferativas con mayor frecuencia en pacientes con IDCV y ninguna manifestación compatible con un proceso maligno. Uno de los pacientes con ALX desarrolló una encefalitis crónica.Todos los pacientes con IDCV y ALX reciben tratamiento sustitutivo con gammaglobulina inespecífica (8 vía intravenosa y 14 (desde 2006) vía subcutánea) y todos los pacientes con IDCG, excepto 2, recibieron un trasplante de progenitores hematopoyéticos. La evolución de todos ellos fue buena excepto 8 IDCG (2 pre y 6 post-trasplante), 3 síndromes de Wiskott-Aldrich, 1 síndrome de DiGeorge completo, 1 enfermedad granulomatosa crónica y 1 ataxia-telangiectasia que fallecieron durante el seguimiento. Conclusiones: La mayoría de los pacientes incluidos en esta serie se presentaron clínicamente con las manifestaciones habitualmente descritas en la literatura, por lo que el conocimiento básico de estas entidades por parte del pediatra de primaria y la colaboración con hospitales de referencia con experiencia en las IDP, debe permitir un diagnóstico precoz de un número importante de IDP facilitando instaurar un tratamiento y un seguimiento adecuados y por lo tanto mejorar el pronóstico de estos pacientes. Abstract: Introduction: More than 200 primary immunodeficiencies (PID) have been described and about 60% present during childhood. Early diagnosis and treatment have been shown to improve patient outcome. Aim: Analysis of patients with a PID diagnosed in a paediatric tertiary care hospital-referral centre over a period of 10 years. Patients and methods: Medical records of all paediatric patients followed up in our unit were retrospectively reviewed. Clinical and epidemiological features, laboratory tests, therapy and outcome were analysed. Results: One hundred and eighty nine patients were followed up in this period of time. Antibody disorders were the most common diagnosis. In our series, clinical presentation at diagnosis were: recurrent respiratory infections in selective IgA deficiency and common variable immunodeficiency (CVID) patients, failure to thrive and opportunistic infections (mainly viral infections) in patients with severe combined immunodeficiency (SCID), skin abscesses (Staphylococcus aureus, Serratia spp.) and complicated pneumonia (Aspergillus spp., Rhodococcus equi) in chronic granulomatous disease, congenital heart disease and consistent phenotype in 22q11 deletion syndrome, skin abscesses and ecthyma gangrenosum in severe congenital neutropenia and opportunistic infections and sepsis (Pseudomonas aeruginosa) in children with X-linked agammaglobulinaemia (XLA). Lymphoproliferative disorders were common in CVID. No malignancies were observed during this period. One patient with XLA developed chronic encephalitis.All patients with CVID and XLA were receiving immunoglobulin replacement therapy (8 intravenous and 14 (since 2006) subcutaneous route) and in all but two SCID patients, stem cell transplantation was performed. Outcome was good in most of them except 8 SCID (2 prior and 6 after transplantation), 3 Wiskott-Aldrich syndrome, 1 complete DiGeorge, 1 chronic granulomatous disease and 1 ataxia-telangiectasia patients who died during follow-up. Conclusion: The vast majority of patients included in this series presented with typical clinical features; therefore, basic knowledge of these entities in primary care and collaboration with hospital referral centres should allow a large number of PID in children to be diagnosed at an early stage, leading to proper treatment and monitoring, and therefore improvement of patient prognosis.

Abstract

Introducción: El trasplante de progenitores hematopoyéticos (TPH) de donante familiar compatible es el tratamiento de elección en la aplasia medular adquirida (AMA) grave en la infancia. Se presenta la experiencia de Grupo Español para el Trasplante de Médula Ósea en Niños en esta enfermedad a lo largo del período cronológico 1982-2004. Pacientes y métodos: Recibieron un trasplante 62 pacientes con una mediana de edad de 10 años. En el período 1982-1990 lo recibieron 18 pacientes y en el período 1991-2004, 44. El régimen de acondicionamiento varió según el período cronológico; en el primero se utilizó preferentemente la asociación de radioterapia y ciclofosfamida (72 % de los casos) y en el segundo ciclofosfamida con o sin globulina antitimocitaria (62 %). La profilaxis de enfermedad injerto contra huésped más utilizada fue la ciclosporina (57/62 pacientes). Resultados: Un total de 51 pacientes están vivos y en remisión completa de su aplasia con períodos de observación de entre 24 y 289 meses (mediana de 127 meses). La probabilidad de supervivencia actuarial libre de eventos a 5 años es del 82%. Dicha supervivencia se incrementó del 61 al 90 % entre los dos períodos analizados. Un total de 11 pacientes fallecieron por fracaso o pérdida del injerto (3), enfermedad injerto contra huésped aguda o crónica asociada a infecciones (4) o fallo multiorgánico (4). El análisis univariante evidenció dos factores con valor predictivo para la supervivencia: el intervalo diagnóstico/trasplante y el período cronológico en que se efectuó (en ambos, p = 0,03). Conclusiones: Esta experiencia confirma que el trasplante de progenitores hematopoyéticos de donante familiar compatible es el tratamiento de elección para la aplasia medular grave adquirida, con un porcentaje de supervivencia libre de episodios del 90 % en la actualidad. : Introduction: Allogeneic haematopoietic stem-cell transplantation is the treatment of choice for acquired aplastic anaemia in children. Experience with this approach from Spanish Working Party for Bone Marrow Transplantation in Children in two sequential time periods (1982-1990 and 1991-2004) is reported. Patients and methods: Sixty two consecutive patients with a median age of 10 years were transplanted; 18 in the 1982-1990 period and 44 in the 1991-2004 period. Conditioning regimen consisted mainly of irradiation and cyclophosphamide in the first period (72 % of patients) and cyclophosphamide ± anti-thymocyte globulin (62 %) in the second. Graft versus host disease prophylaxis consisted of cyclosporine in most patients (57/62). Results: Fifty one patients are alive and disease-free at a median follow-up of 127 months. Five years probability of event-free survival is 82 %. The survival increased from 61 % to 91 % during the two time periods. Eleven patients died from graft failure or rejection (3), acute or chronic graft versus host disease and infection (4) or multi-organ failure (4). Univariate analysis identified two significant prognostic factors: interval diagnostic/transplant and time period of transplant (for both p = 0.03). Conclusions: This experience corroborates that allogeneic haematopoietic stem-cell transplantation is the best treatment for severe acquired aplastic anaemia, with a current disease – free survival of 90 % of patients.

Abstract

Antecedentes: Existe cierta variabilidad de práctica clínica en España sobre las intervenciones preventivas para evitar la infección respiratoria infantil por el virus respiratorio sincitial (VRS), entre las que se incluyen medidas higiénicas e inmunoprofilaxis intramuscular con palivizumab. Esta tarea implica varias especialidades pediátricas, que pueden tener distinta percepción de la magnitud del problema y distinto criterio profesional sobre las actuaciones apropiadas. Objetivos: Desarrollar un consenso multidisciplinar español sobre la prevención de la infección por VRS, con participación de las sociedades científicas pediátricas implicadas: Sociedad Española de Neonatología (SEN), Sociedad Española de Cardiología Pediátrica y Cardiopatías Congénitas (SECPCC), Sociedad Española de Neumología Pediátrica (SENP) y Sociedad Española de Hematología y Oncología Pediátrica (SEHOP). Métodos: Consenso Delphi modificado en dos rondas. El estudio se efectuó en cuatro fases: 1) constitución de un comité científico multidisciplinar, a propuesta de las sociedades científicas participantes, para la revisión bibliográfica y formulación de las recomendaciones a debate; 2) constitución de un panel experto con 77 representantes de las especialidades; 3) encuesta postal en dos rondas con procesamiento intermedio de opiniones e informe a los panelistas, y 4) discusión de resultados en sesión presencial del comité científico. Resultados: Se consensúan 48 de las 57 recomendaciones preventivas analizadas. En las nueve cuestiones restantes no se consigue acuerdo, bien por disparidad de opinión profesional, bien por falta de criterio establecido en la mayoría de los expertos. Sólo en tres recomendaciones, la opinión de los expertos se asocia a la especialidad de procedencia. Conclusiones: Se presenta una lista de recomendaciones profilácticas anti-VRS, cualificadas según el grado de acuerdo profesional en que se sustentan, que pueden considerarse vigentes hasta la aparición de nueva información científica que indique su revisión. : Background: A certain degree of feasibility exists in Spanish clinical practice with respect to interventions performed to prevent paediatric respiratory infection by RSV, including hygienic measures and intramuscular immunoprophylaxis with palivizumab. This task involves different paediatric specialties that may have a different perception of the magnitude of the problem and different professional criteria regarding the most appropriate actions. Objectives: To develop Spanish multidisciplinary consensus on preventing infection by RSV with the participation of the paediatric scientific societies involved (SNS, SSPC, SSPP and SSHPO). Methods: Delphi Consensus modified in two rounds was used. The study was conducted in four phases: 1) constitution of a multidisciplinary Scientific Committee at the recommendation of the scientific entities participating in the study, for bibliographic review and submission of the recommendations to discussion; 2) constitution of an Expert Panel with 77 speciality representatives; 3) postal survey organised in two rounds and intermediate processing of opinions and issuing of a report for the panellists, and 4) discussion of the results in a meeting of the Scientific Committee. Results: Consensus was reached on 48 of the 57 preventive recommendations analysed. With respect to the 9 remaining issues, no consensus was reached, due to differences in professional opinion and the absence of established criteria among the majority of the experts. Only in 3 recommendations was the opinion of the experts associated with the speciality of origin. Conclusions: A list of anti-RSV prophylactic recommendations was submitted, rated in accordance with the degree of professional consensus on which they were based. These can be considered valid until such time as new scientific information emerges that warrants a review thereof.

25. Síndrome mieloproliferativo transitorio neonatal en ausencia de síndrome de Down

Abstract

El síndrome mieloproliferativo transitorio o leucemia transitoria neonatal es una entidad que suele asociarse a pacientes afectados de síndrome de Down. Se resuelve de manera espontánea en 4 o 5 meses, salvo en el 25% de los casos en que puede desarrollarse posteriormente una leucemia megacarioblástica aguda, o bien un síndrome mielodisplásico. Rara vez se ha descrito sin anomalías constitucionales y aún menos en gemelos. Se presentan 3 pacientes, fenotípicamente normales, afectados por esta enfermedad. Uno de ellos fue diagnosticado por presentar un síndrome de “blueberry muffin baby”. El estudio anatomopatológico de las lesiones cutáneas orientó el diagnóstico. Los otros eran dos trillizas monocoriales. Su hermana bicorial con las dos anteriores no presentó alteraciones hematológicas. En todos se descartó la cromosomopatía constitucional. Sólo recibieron tratamiento de soporte sin quimioterapia. La evolución fue favorable, desapareciendo la blastosis medular y periférica en 4 o 5meses. Con un período de seguimiento de 18 y 19 meses no han presentado nueva patología hematológica. Se debe ser cauto antes de iniciar la quimioterapia en estos pacientes. Se comenta el diagnóstico diferencial con la leucemia congénita y las implicaciones pronósticas y terapéuticas que conlleva. : Transient neonatal leukemia or transient neonatal myeloproliferative disorder is commonly associated with Down syndrome. It usually resolves spontaneously in 4–5 months. However, 25% of patients will subsequently develop acute megakaryoblastic leukemia or myelodysplastic syndrome. It has seldom been described without constitutional anomalies and is even less frequent in twins. We present three phenotypically normal patients with this disorder. One of them was diagnosed because he presented blueberry muffin syndrome. Diagnosis was guided by pathological examination of the skin lesions. The other two patients were monochorionic triplets. Their bichorionic sister presented no hematological disorders. Constitutional chromosomal abnormalities were ruled out in all three patients. They received support treatment only without chemotherapy. The clinical course was favorable with disappearance of marrow and peripheral blastosis in 4–5 months. Follow-up of 18 and 19 months has not revealed any hematological disorders. Caution must be exercised before initiating chemotherapy in these patients. We discuss the differential diagnosis with congenital leukemia and the prognostic and therapeutic implications that this entails.

Abstract

[Resumen] Introducción: La evaluación cognitiva es una herramienta fundamental en los centros de mayores a la hora de planificar la atención e individualizar la intervención. La Sociedade Galega de Xerontoloxía e Xeriatría ha constituido un Grupo de Trabajo sobre Evaluación Cognitiva y Neuropsicológica con el objetivo de estudiar los instrumentos empleados e identificar los principales retos en esta área. Material y métodos: Se realizó un estudio exploratorio a través de encuesta en línea a los socios de la Sociedade Galega de Xerontoloxía e Xeriatría entre diciembre de 2013 y enero de 2014. Participaron 49 profesionales, de los cuales 26 eran profesionales de la psicología y 10 de la medicina. Resultados: Se encuentra un consenso en el uso del MMSE como instrumento de cribado y del GDS como escala global de la demencia. Existe mayor variabilidad en las baterías generales para la valoración de las demencias (CAMCOG-R, ADAS-Cog, Test Barcelona abreviado) y en el uso de pruebas específicas y de baterías de deterioro severo. Conclusiones: Se sugiere un riesgo de polarización del sector, en el que un porcentaje elevado de profesionales emplearía unos pocos instrumentos generales y un porcentaje peque˜no un número elevado de instrumentos específicos. Se subraya la necesidad de un consenso que facilite el desempe˜no, objetive las necesidades formativas, maximice la relación entre evaluación e intervención cognitiva y proporcione herramientas para valorar el cambio. [Abstract] Introduction: Cognitive assessment is a fundamental tool in centres for the elderly when planning individualized care and intervention. The Galician Society of Gerontology and Geriatrics (Sociedade Galega of Xerontoloxía and Xeriatría) has formed a Working Group on Cognitive and Neuropsychological Assessment, with the aim of studying the tools used and to identify the main challenges in this area. Material and methods: An exploratory study was conducted via an online questionnaire with Sociedade Galega of Xerontoloxía and Xeriatría members between December 2013 and January 2014. It was completed by 49 professionals, of whom 26 were professionals in psychology and 10 medical doctors. Results: Consensus was found on the use of the MMSE as a screening tool, as well as the Global Deterioration Scale for dementia. There is a wide variation in the choice of general batteries of tests for the assessment of dementia (CAMCOG-R, ADAS-Cog, abbreviated Barcelona Test), and of the use of specific tests and a severe impairment battery. Conclusions: The risk of bias in the sector is suggested, with a high percentage of general practitioners using a few tools, and a small percentage using a large number of specific tools. The need for a consensus is stressed in order to facilitate assessment, objectify training needs, maximize the relationship between assessment and cognitive intervention, and provide tools to assess change.

Abstract

Cyclotriphosphazene is used as a sacrificial solid-state template to synthesize a range of Ag and Pd nanoparticles with diverse geometries by thermal treatment using MLn/N3P3(O2C12H8)3 mixtures. The Pd and Ag nanoparticles are synthesized by solid-state pyrolysis of AgPPh3[CF3SO3]/N3P3(O2C12H8)3 and PdCl2/N3P3(O2C12H8)3 mixtures with molar relationships of 1:1, 1:5 and 1:10 respectively, in air and at 800 °C. The morphology of the as-prepared nanoparticles is found to depend on the molar ratio of the precursor mixture, the preparation method and of the nature of the metal. Ag and Pd, microcrystals were thermally grown on Si from the respective 1:1 precursors while that metal foams were grown from 1:5 ratios precursors on SiO2 wafers. High resolution transmission electron microscopy investigations reveal in most cases small crystals of Pd. HRSTEM measurements indicate that the formation of the Pd and Ag nanoparticles occurs through a phase demixing and dewetting mechanism. This approach has potential to be a useful and facile method to prepare metallic nanoparticles without requiring solutions or surfactants for application in electronic, catalytic and sensor materials and devices.

Abstract

The Cherenkov Telescope Array (CTA) is a new observatory for very high-energy (VHE) gamma rays. CTA has ambitions science goals, for which it is necessary to achieve full-sky coverage, to improve the sensitivity by about an order of magnitude, to span about four decades of energy, from a few tens of GeV to above 100 TeV with enhanced angular and energy resolutions over existing VHE gamma-ray observatories. An international collaboration has formed with more than 1000 members from 27 countries in Europe, Asia, Africa and North and South America. In 2010 the CTA Consortium completed a Design Study and started a three-year Preparatory Phase which leads to production readiness of CTA in 2014. Herramienta de interés para el control y la gestión de pesquerías, The regulation of minimum legal size(MLS) of catches is a tool widely applied in the management of fisheries resources, although the MLS does not always coincide with the length at first maturit(LFM). The optimization of this management tool requires a series of quality control in fish markets and transportation. A software application has been developed to make the control of the landings of several target species easier and faster. In order to test and make this tool operational,six species of commercial interest were selected: four species of fish hand two species of bivalves. It is proposed to estimate the proportion of illegal specimens in the studied lot from the proportion of illegal individuals found in the samples taken from this lot.The input data for the application are the minimum legal size(MLS) of the species and the total length(TL)of each specimen sampled. Resumen: Las levaduras del género Candida son comensales ubicuos, que pueden causar infección oportunista en cualquier localización del organismo; la fuente de infección puede ser tanto endógena como exógena. La candidiasis invasiva engloba entidades distintas que van desde la candidemia o invasión limitada al torrente circulatorio a la candidiasis diseminada o infección multiorgánica. La candidemia constituye la tercera causa de infección del torrente circulatorio en la IN y la cuarta de todas las infecciones. También constituye la IFI más frecuente en el paciente crítico no neutropénico, habiendo sufrido un incremento muy notable en los últimos 20 años, debido tanto a una mayor supervivencia de los pacientes críticamente enfermos como a una mayor complejidad de los procedimientos diagnóstico-terapéuticos y quirúrgicos. Su incidencia en lactantes, según revisiones recientes, se sitúa en 38,8 casos/100.000 menores de 1 año. La especie más frecuentemente implicada en infecciones invasoras sigue siendo Candida albicans, aunque en los últimos años existe un incremento de infecciones causadas por especies distintas: es relevante la aparición de Candida krusei y Candida glabrata y el incremento de candidemia por Candida parapsilosis asociada principalmente al manejo de catéteres intravenosos centrales, especialmente en unidades neonatales.La mortalidad global de la candidiasis invasiva es elevada, llegando a cifras del 20 al 44% a los 30 días en determinadas series que incluyen a pacientes pediátricos.Este documento recoge una revisión actualizada sobre incidencia, epidemiología, diagnóstico, tratamiento y evolución, de la infección invasiva por Candida spp. en el paciente pediátrico. Abstract: Candida yeasts are ubiquitous commensals, which can cause opportunistic infection in any location of the body. The source of infection may be both endogenous and exogenous. Invasive candidiasis encompasses different entities ranging from invasive candidiasis to disseminated multiorgan infection. Invasive candidiasis is the third leading cause of nosocomial bloodstream infection and the fourth of all nosocomial infections. It is also the most common invasive fungal infection in non-neutropenic critically ill patients, with a remarkable increase in the last 20 years owing to the increased survival of these patients and to more complex diagnostic, therapeutic and surgical procedures. Its incidence in infants, according to recent reviews, stands at 38.8 cases/100,000 children younger than 1 year. Candida albicans remains the most frequent isolate in invasive infections, although infections caused by other species have risen in the last years, such as C. kruzsei, C. glabrata and C. parapsilosis; the latter causing invasive candidiasis mainly associated with central venous catheter management, especially in neonatal units. The overall mortality of invasive candidiasis is high, with 30-day mortality reaching 20-44% in some series involving paediatric patients.This report provides an update on incidence, epidemiology, clinical presentation, diagnosis, treatment and outcome of invasive infection by Candida spp. in the paediatric patient.

Diagnostic and therapeutic advances in recent years have significantly improved the prognosis for these patients.It is therefore important that the pediatrician has a clinical practice guideline with recommendations for diagnosis and treatment of this disease that leads to the appropriate intervention.

Current production systems of finfish aquaculture, and in particular intensive farming systems, can cause welfare problems leading not only to poor condition of the fish but also to a decrease in product quality. Adding structural environmental enrichment (EE) to bare rearing environments may improve the welfare of certain cultured fish. In this study we experimentally demonstrate the positive effects of adding structural EE on rearing environments of juvenile seabream (Sparus aurata). Fish maintained for 35 days with EE showed less aggression and interactions with the net pen, and lower erosion of pectoral and caudal fins, compared to fish kept in bare conditions (non-enriched, NE). In addition, EE modified the horizontal distribution of fish in the experimental cage, increasing the use of the inner areas. Non-significant effects of EE were observed on fish body condition and growth, and on brain monoamines levels and mortality. Nevertheless, this work highlights the potential use of structural EE to improve welfare of juvenile seabream, which might be feasible to apply at larger-commercial scale. Abstract

Livestock breed-related public good functions are often used to justify support for endangered breed conservation despite the fact that little is known about such non-market values. We show how stated preference techniques can be used to assess the non-market values that people place on livestock breeds. Through the application of a case study choice experiment survey in Zamora province, Spain, the total economic value (TEV) of the threatened Alistana–Sanabresa (AS) cattle breed was investigated. An analysis of the relative importance of the non-market components of its TEV and an assessment of the socio-economic variables that influence people’s valuation of such components is used to inform conservation strategy design. Overall, the findings reveal that the AS breed had significant non-market values associated with it and that the value that respondents placed on each specific public good function also varied significantly. Functions related with indirect use cultural and existence values were much more highly valued than landscape maintenance values. These high cultural and existence values (totalling over 80% of TEV) suggest that an AS in situ conservation strategy will be required to secure such values. As part of such a strategy, incentive mechanisms will be needed to permit farmers to capture some of these public good values and thus be able to afford to maintain breed population numbers at socially desirable levels. One such mechanism could be related to the development of breed-related agritourism initiatives, with a view to enhancing private good values and providing an important addition to continued direct support. Where linked with cultural dimensions, niche product market development, including through improving AS breed-related product quality and brand recognition may also have a role to play as part of such an overall conservation and use strategy. We conclude that livestock breed conservation strategies with the highest potential to maximise societal welfare would be those that secure the breed-related functions that people value most, with appropriate in situ conservation interventions and strategies being identified accordingly.

Objective: Nurses inevitably encounter patients who require care aimed at limiting therapeutic effort (LTE), even though many of them are not prepared to provide support to individuals with terminal illnesses and their families. One of the contexts in which the LTE is considered is the intensive care unit (ICU). This review is to describe the competencies for the execution of a nursing professional role in the LTE in the ICU. Method: An integrative review of the literature published between the years 2010 and 2023. The search was carried out in five databases: Medline, Wiley Online Library, SciELO, ScienceDirect, and Web of Science. The Critical Appraisal Skills Programme in Spanish was used as the template for study evaluation. The methodology of the Oxford Center for Evidence-Based Medicine (CEBM) was used to assess the level of evidence and the degree of recommendation. Result: A total of 25 articles in a wide range of studies were included. The findings suggest that the competencies for LTE in the ICU are direct patient care, family-centered care, and the role of the nurse within the team. However, more high-quality studies are needed to confirm these conclusions. Three categories were identified: (a) competencies as defender agent between the patient, his family, and the interdisciplinary team; (b) competencies for decision-making in limiting the therapeutic effort; and (c) competencies for nursing therapeutic interventions at the end of life. Conclusion: The competencies of the nursing professionals who work in the adult ICU in the LTE are essential to the patient’s quality of life, dignity of their death, and comprehensive family support for coping with grief.

Background: Peru reports higher levels than other countries in Latin America of resistance to antimicrobials among Gram-positive and Gram-negative bacteria, however data on antibiotic use in Peru are scarce. This study aims to estimate the prevalence and quality of antibiotic prescription in hospitalized patients and to determine the antibiotic susceptibility rates of bacteria causing key bacterial infections. Methods: We carried out a point prevalence survey of antibiotic prescription at ten public hospitals in nine regions of Peru. Data was collected from patients hospitalized during a 3-week period, with details about antibiotic use, patient information, and antimicrobial susceptibility. Results: 1620 patient charts were reviewed; in 924 cases antibiotics were prescribed (57.0 %, range 45.9–78.9 %). Most of the antibiotics (74.2 %) were prescribed as empirical treatment, only 4.4 % as targeted treatment. For 9.5 % of cases the reason for antibiotic use was unknown. Cephalosporins were the most prescribed (30.0 %), followed by carbapenems (11.3 %). Ninety-four blood cultures were positive for bacterial growth, 48.8 % of the Staphylococcus aureus were methicillin-resistant, among Escherichia coli and Klebsiella pneumoniae, 51.7 % and 72.7 % were resistant to third-generation cephalosporins (3GC), 3.4 % and 18.2 % were resistant to carbapenems, respectively. Among bacteria isolated from urine cultures (n = 639), 43.9 % of E. coli and 49.2 % of K. pneumoniae were resistant to 3GC, and 0.9 % of E. coli and 3.2 % of K. pneumoniae were resistant to meropenem. Conclusions: The overall proportion of hospitalized patients receiving antibiotics in hospitals from different regions in Peru was high, with only a small proportion receiving targeted treatment. Cephalosporins and carbapenems were the most frequently prescribed antibiotics, reflecting high resistance rates against 3GC and carbapenems in Enterobacterales isolated from blood and urine.

Background: Recent data about clinical features, triggers and management of anaphylaxis in Latin America is lacking. Objective: To provide updated and extended data on anaphylaxis in this region. Method: An online questionnaire was used, with 67 allergy units involved from 12 Latin-American countries and Spain. Among data recorded, demographic information, clinical features, severity, triggering agents, and treatment were received. Results: Eight hundred and seventeen anaphylactic reactions were recorded. No difference in severity, regardless of pre-existing allergy or asthma history was found. Drug induced anaphylaxis (DIA) was most frequent (40.6%), followed by food induced anaphylaxis (FIA) (32.9%) and venom induced anaphylaxis (VIA) (12%). FIA and VIA were more common in children-adolescents. Non-steroidal anti-inflammatory drugs (NSAIDs) and beta-lactam antibiotics (BLA) were the most frequent drugs involved. Milk (61.1% of FIA) and egg (15.4% of FIA) in children, and shellfish (25.5% of FIA), fresh fruits (14.2% of FIA), and fish (11.3% of FIA) in adults were the most common FIA triggers. Abstract

Advanced LIGO and Advanced Virgo are monitoring the sky and collecting gravitational-wave strain data with sufficient sensitivity to detect signals routinely. In this paper we describe the data recorded by these instruments during their first and second observing runs. The main data products are gravitational-wave strain time series sampled at 16384 Hz. The datasets that include this strain measurement can be freely accessed through the Gravitational Wave Open Science Center at http://gw-openscience.org, together with data-quality information essential for the analysis of LIGO and Virgo data, documentation, tutorials, and supporting software.

Abstract

Basement membranes (BMs) are specialized extracellular matrices required for tissue organization and organ formation. We study the role of laminin and its integrin receptor in the regulation of tissue migration during Drosophila oogenesis. Egg production in Drosophila involves the collective migration of follicle cells (FCs) over the BM to shape the mature egg. We show that laminin content in the BM increases with time, whereas integrin amounts in FCs do not vary significantly. Manipulation of integrin and laminin levels reveals that a dynamic balance of integrin-laminin amounts determines the onset and speed of FC migration. Thus, the interplay of ligand-receptor levels regulates tissue migration in vivo. Laminin depletion also affects the ultrastructure and biophysical properties of the BM and results in anterior-posterior misorientation of developing follicles. Laminin emerges as a key player in the regulation of collective cell migration, tissue stiffness, and the organization of anterior-posterior polarity in Drosophila.

Abstract

Background and aims. Steroid-related hepatotoxicity has become one of the most relevant causes of drug induced liver cholestasis. Some patients do not improve after standard medical treatment (SMT) and may therefore require other approaches, like extracorporeal liver support.Material and methods. We report four cases of patients with pruritus, abnormal liver function tests and biopsy-proven anabolic steroid-induced cholestasis who were unresponsive to SMT. They underwent treatment with albumin dialysis (Molecular Adsorbent Recirculating System -MARS®-). A minimum of two MARS sessions were performed.Results. After MARS® procedure, patients’ symptoms improved, as well as liver function tests, thus avoiding liver transplantation.Conclusion. Albumin dialysis appears as a valuable therapeutic option for the management of anabolic steroid-induced cholestasis in patients that are unresponsive to SMT.

Abstract

Background & Aims: Renal dysfunction is prevalent in advanced chronic liver disease (aCLD) and is associated to liver-related death (LRD). This makes a reliable evaluation of renal function (RF) a crucial aspect. RF can be estimated by formulas or measured by gold standard method. Estimated RF is not reliable in aCLD. However, there is a lack of information on the reliability of formulas in the prediction of LRD., Methods: We analysed a cohort of patients with aCLD in whom RF was measured by the plasma clearance of iohexol (mGFR) and estimated (eGFR) by formulas: MDRD, CKD-EPI, Royal Free Hospital (RFHC), GRAIL and Mindikoglu-eGFR. LRD was defined as death from hepatic causes. Multivariable analysis was used to evaluate association of mGFR or eGFR with LRD., Results: 161 patients were evaluated, with median follow-up of 28 months, 58 died from LRD. In overall group mGFR (OR 0.99; p = 0.022) and formulas: CKD-EPI (OR 0.98; p = 0.044), GRAIL (OR 0.98; p = 0.038) was associated with LRD. In patients with normal creatinine levels (≤ 1.1 mg/dL), mGFR (OR 0.99; p = 0.031) was whereas any formula was not associated with LRD., Conclusions: eGFR appears as an unreliable method for predicting LRDs in aCLD, especially in those with lower creatinine levels. Abstract

Stem cell transplantation into seminiferous tubules of recipient testis could become a tool for fertility restoration, genetic improvement, or conservation of endangered species. Spermatogonial stem cells (SSCs) are primary candidates for transplantation; however, limited abundance, complexity for isolation and culture, and lack of specific markers have limited their use. Mesenchymal stromal/stem cells (MSCs) are multipotent progenitors that are simple to isolate and culture and possess specific markers for identification, and immune evasive and migratory capacities. The objective of the present study was to evaluate the potential for survival and colonization in seminiferous tubules of two different concentrations of bovine fetal adipose tissue-derived MSCs (AT-MSCs), native of pre-induced, and to compare the fate of bovine adult peripheral blood-derived MSCs (PB-MSCs) and SSCs after allogenic transplantation in testis of recipient bulls. In experiment 1, AT-MSCs at two concentrations (1x10 7 and 2x10 7 ; n = 3) or pre-exposed to 2 μM testosterone and 1 μM retinoic acid (RA) for 14 days (n = 5) were evaluated. In experiment 2, adult PB-MSCs and SSCs (4x10 7  cells each) pre-exposed to Sertoli cell conditioned media (SCs/CM; n = 4) for 14 days were compared. Each cell type was separately labelled with PKH26 and then transplanted into testes of 8-month-old recipient bulls. Four weeks (Exp. 1) and two weeks (Exp. 2) after transplantation, testicular tissue was processed for confocal microscopy detection of PKH26-positive cells. Mean number of PKH26-positive cells were higher (P < 0.05) in testis transplanted with 2x10 7 AT-MSCs in the proximal (6.7 ± 3.7) and medial (6.6 ± 3.2) sections compared to testis transplanted with 1x10 7 AT-MSCs (proximal: 1.9 ± 1; medial: 1.9 ± 1) sections or pre-induced AT-MSCs (proximal: 4.7 ± 5.6; medial: 3.8 ± 4.1). In Exp. 2, mean number of PKH26-positive SSCs in medial testicular section (22.5 ± 1.3) were higher (P < 0.05) compared to respective section in PB-MSCs group (17 ± 4.2). Thus, in vivo data indicates that a higher number of transplanted AT-MSCs resulted in more cells surviving and colonizing seminiferous tubules; however, pre-induction with testosterone and RA did not improve these capacities. 47. Mammalian placental explants: A tool for studying host-parasite interactions and placental biology.

Abstract

The placenta plays a critical role in host-pathogen interactions. Thus, ex vivo infection of mammalian placental explants is an excellent and simple method to study the mechanisms of cellular and tissue invasion by different pathogens in different mammalian species. These explants can be maintained in culture for several days, preserving the tissue architecture and resembling in-utero conditions under more physiological conditions than their isolated counterparts in isolated cell culture models. In addition, placental explants not only allow us to study how the placenta responds and defends itself against various infections but also provide a versatile platform for advancing our understanding of placental biology and the immune response. Furthermore, they serve as powerful tools for drug discovery, facilitating the screening of potential therapeutics for placental infections and for the identification of diagnostic markers. This review highlights the utility of mammalian placental explants in studying the host-pathogen interaction of two relevant protozoan parasites, Trypanosoma cruzi, the causative agent of Chagas disease, and Toxoplasma gondii, the etiological agent of Toxoplasmosis. Abstract

Introduction: The knowledge of BRCA status offers a chance to evaluate the role of the intraperitoneal route in patients selected by biomolecular profiles after primary cytoreduction surgery in advanced ovarian cancer., Materials and Methods: We performed a retrospective, multicenter study to assess oncological outcomes depending on adjuvant treatment (intraperitoneal [IP] vs intravenous [IV]) and BRCA status (BRCA1/2 mutated vs. BRCA wild type [WT]). The primary endpoint was to determine progression-free survival. The secondary objectives were overall survival and toxicity., Results: A total of 288 women from eight centers were included: 177 in the IP arm and 111 in the IV arm, grouped into four arms according to BRCA1/2 status. Significantly better PFS was observed in BRCA1/2-mutated patients with IP chemotherapy (HR: 0.35; 95% CI, 0.16-0.75, p = 0.007), which was not present in BRCA1/2-mutated patients with IV chemotherapy (HR: 0.65; 95% CI, 0.37-1.12, p = 0.14). Significantly better OS was also observed in IP chemotherapy (HR: 0.17; 95% CI, 0.06-043, p < 0.0001), but was not present in IV chemotherapy in relation with BRCA mutation (HR: 0.52; 95% CI, 0.22-1.27, p = 0.15). For BRCA WT patients, worse survival was observed regardless of the adjuvant route used. The IP route was more toxic compared to the IV route, but toxicity was equivalent at the long-term follow-up., Conclusion: This retrospective study suggests that BRCA status can help to offer an individualized, systematic treatment after optimal primary surgery for advanced ovarian cancer, but is limited by the small sample size. Abstract

Gastrointestinal Stromal Tumors (GIST) are the most frequent mesenchymal neoplasia of the digestive tract. Genomic alterations in KIT, PDFGRA, SDH, and BRAF genes are essential in GIST oncogenesis. Therefore, the mutations in these genes have demonstrated clinical implications. Tumors with deletions in KIT-exon 11 or duplications in exon 9 are associated with a worse prognosis. In contrast, KIT-exon 11 substitutions and duplications are associated with a better clinical outcome. Moreover, mutations in Kit exon 9 and 11 are actionable, due to their response to imatinib, while mutations in PDGFRA respond to sunitinib and/or avapritinib. Although, molecular testing on tissue samples is effective; it is invasive, requires adequate amounts of tissue, and a long experimental process is needed for results. In contrast, liquid biopsy has been proposed as a simple and non-invasive method to test biomarkers in cancer. The most common molecule analyzed by liquid biopsy is circulating tumor DNA (ctDNA). GISTs ctDNA testing has been demonstrated to be effective in identifying known and novel KIT mutations that were not detected using traditional tissue DNA testing and have been useful in determining progression risk and response to TKI therapy. This allows the clinician to have an accurate picture of the genetic changes of the tumor over time. Abstract

In this work, an innovative green strategy has been developed for the analysis of twenty-seven endocrine disruptors, including bisphenols, alkylphenols and alkylphenol ethoxylates, phthalic acid esters and one adipate in tropical beverages. For this purpose, nine natural hydrophobic deep eutectic solvents based on the terpenoids camphor, thymol and menthol at different molar ratios were investigated for the first time as extractants for the liquid-liquid microextraction of the target analytes from coconut waters and Aloe Vera drinks. A mixture of camphor:thymol at molar ratio 1:2 (n/n) was selected as extraction solvent. Determination of the target analytes was carried out by ultra-high performance liquid chromatography coupled to tandem mass spectrometry. After optimisation of the determination and extraction conditions, the methodology was validated achieving good results in terms of linearity, as well as recovery values in the range 75-111% and limits of quantification from 0.137 to 10.08 μg/L.
