Background: Investigating movement strategies that can be utilized to avoid pain-provocation could enhance the management of low back pain episodes., Objective: To assess the effect of visual cues at different heights on the kinematics of sit-to-stand movements, as well as perceived difficulty and pain levels., Design: Cross-over design comparing individuals with low back pain to healthy controls., Methods: 26 asymptomatic controls and 15 individuals with chronic, recurrent low back pain performed 5 sets of 5 sit-to-stand movements. High, middle, and low visual cues were used during sets 2-4. Spinal sagittal plane range of motion, peak spinal flexion and extension angles, and trunk centre of mass velocity were obtained from kinematic data., Results: The low cue led to significantly more head and lumbar spine flexion, while the high cue led to significantly more head and thoracic spine extension and increased thoracic spine range of motion. The low back pain group demonstrated a significantly lower vertical trunk centre of mass velocity than the control group during the high cue trials. There was a significant association between higher perceived difficulty scores and lower trunk centre of mass velocity for the low back pain group. Pain scores were not significantly different between cue conditions., Conclusion: Visual cues can be used to temporarily change the spinal kinematics of sit-to-stand movements in people with and without low back pain. Background: The large number of articulating joints within the spinal column provides an abundance of options to control its movement. However, the ability of individuals to consciously manipulate these movement options is poorly understood., Objectives: To determine if short-term training can improve the ability to consciously dissociate motion between the pelvis and thorax during repetitive pelvic tilting movements., Design: Cross-over design with young healthy individuals., Method: Seventeen participants performed trials consisting of 35 continuous lift/lowers followed by 35 continuous anterior/posterior pelvic tilts while spine kinematics were recorded. Participants then underwent a 20-min training protocol designed to improve the control of pelvic motion and in particular the dissociation of pelvic and trunk motion. Post-training, the continuous pelvic tilt and lift/lower trials were repeated. Thorax-pelvis movement coordination was analyzed via vector coding and lumbar spine local dynamic stability was analyzed via Lyapunov exponents. Participants were grouped as being either high or low skill movers based on their ability to perform the pre-training pelvic tilt movements., Results: The low skill movement group demonstrated statistically significant increases in the time spent using in-phase pelvic dominant (p = 0.028) and anti-phase pelvic dominant (p = 0.043) coordination patterns during the pelvic tilt movements after the completion of the training protocol. The high skill movement group showed no differences in their movement patterns post-training., Conclusions: Short-term training, targeted to improve the ability to dissociate pelvic from thorax motion, had a beneficial effect on the group of individuals who initially lacked skill performing the pelvic tilting task., (Copyright © 2024 Elsevier Ltd. Introduction: Passive mechanical properties of the paraspinal muscles are important to the biomechanical functioning of the spine. In most computational models, the same biomechanical properties are assumed for each paraspinal muscle group, while cross-sectional area or fatty infiltration in these muscles have been reported to differ between the vertebral levels. Two important properties for musculoskeletal modeling are the slack sarcomere length and the tangent modulus. This study aimed to investigate the effect of vertebral level on these biomechanical properties of paraspinal muscles in a rat model., Methods: The left paraspinal muscles of 13 Sprague-Dawley rats were exposed under anesthesia. Six muscle biopsies were collected from each rat: three from multifidus (one per each of the L1, L3, and L5 levels) and similarly three from longissimus. Each biopsy was cut into two halves. From one half, two to three single muscle fibers and two to six muscle fiber bundles (14 ± 7 fibers surrounded in their connective tissue) were extracted and mechanically tested in a passive state. From the resulting stress-strain data, tangent modulus was calculated as the slope of the tangent at 30% strain and slack sarcomere length (beyond which passive force starts to develop) was recorded. The other half of each biopsy, which represented the muscle at the fascicle level, was snap frozen, sectioned, stained for Collagen I and its area fraction was measured. To evaluate the effect of spinal level on these biomechanical properties of multifidus and longissimus, one-way repeated measures ANOVA (p < 0.05) was performed for tangent modulus and slack sarcomere length, while for collagen I content linear mixed-models analysis was adopted., Results: In total, 192 fibers and 262 fiber bundles were mechanically tested. For both muscle groups, no significant difference in tangent modulus of the single fibers was detected between the three spinal levels (p = 0.9 for multifidus and p = 0.08 for longissimus). Similarly, the tangent modulus values for the fiber bundles were not significantly different between the three spinal levels (p = 0.13 for multifidus and p = 0.49 for longissimus). In both muscle groups, the slack sarcomere lengths were not different among the spinal levels except for multifidus fibers (p = 0.02). Collagen I area fraction in muscle fascicles averaged 6.8% for multifidus and 5.3% for longissimus and was not different between the spinal levels., Discussion: The results of this study highlighted that the tangent modulus, slack sarcomere length, and collagen I content of the lumbar paraspinal muscles are independent of spinal level. Abdominal bracing is a voluntary method of increasing spine stiffness to restrict spine displacement. Previous investigations of abdominal bracing have measured effects on whole lumbar motion; however, how this effect is distributed across the lumbar spine is unknown. Therefore, this study was designed to test the influence of abdominal bracing on spine intersegmental (T9/T10 to L5/S1) flexion, measured via skin surface markers, in response to sudden loading perturbations applied through the hands in 16 young healthy participants. Abdominal and back muscle activation responses were also measured. The results demonstrated that abdominal bracing significantly reduced sagittal plane motion at intersegmental levels T12/L1 to L4/L5, by 45% (0.74 degrees) at L4/L5 to 94% (0.71 degrees) at L1/L2 compared to control. L5/S1 experienced a 50% (0.36 degrees) reduction, but this was not statistically significant. Additionally, abdominal bracing resulted in greater baseline activation of all abdominal and back muscles, but did not affect onset times or response magnitudes of any of the back muscles acting counter to the perturbation. As the largest and most superficial organ, the skin is well positioned for receiving sensory information from the environment. It is conceivable that changes in posture could result in deformations of the skin and subsequent changes in skin material properties. Specifically, the ankle and metatarsophalangeal joints have the capability to undergo large postural alterations with the potential to induce large structural deformations in the skin of the foot. The purpose of this study was to determine the extent to which alterations in foot posture may influence measures of foot sole and dorsum skin stretch, hardness, and thickness in vivo. Ten young and healthy individuals were tested while three static foot postures (plantar flexion, neutral and dorsiflexion) were maintained passively. Skin stretch deformation was quantified across each posture using an 11 × 4 point matrix of 3D kinematic markers affixed to the skin of the foot sole and dorsum. Skin hardness was assessed across each posture at specific locations of the foot sole (1st metatarsal, 5th metatarsal, medial arch, lateral arch and heel) and foot dorsum (proximal, middle and distal) using a handheld Shore durometer. Skin (epidermal + dermal) thickness was measured in each posture from the same test locations using ultrasound images obtained for the foot sole and dorsum. In the plantar flexion ankle posture, the foot sole skin was observed to relax/retract on average (± standard errorr of the mean (SEM) by 9 ± 2% to become both 20 ± 6% softer and 10 ± 6% thicker. In this posture, the foot dorsum skin stretched on average by 7 ± 2% resulting in 84 ± 8% harder and 5 ± 4% thinner skin. In the dorsiflexion ankle posture, the skin of the foot sole was observed to stretch on average by 5 ± 1% to become both 20 ± 8% harder and 4 ± 7% thinner. In this posture, the skin of the foot dorsum relaxed/retracted on average by 9 ± 1% resulting in the skin becoming 27 ± 12% softer and 7 ± 5% thicker. Notably, all of the sites responded with movement in a similar direction, but each site responded to a variable extent. Importantly, it was clear that the majority of skin structural deformation of the foot sole occurred within the 1st metatarsal, 5th metatarsal, and medial arch regions, while deformation was more evenly distributed across regions of the foot dorsum. The results suggest there is location specificity in the retraction and stretch characteristics of the foot skin. While not tested directly, this may suggest that local stretch distributions could be in part due to the underlying dermal and hypodermal structures in these foot regions. With these observed changes in the mechanical structure of the foot sole and dorsum skin tissue matrix, it is possible that corresponding posture-dependent changes in cutaneous mechanoreceptor activation may be present., (Copyright © 2019 Elsevier Ltd. This study was designed to assess the utility of using a measure of dynamic spine stability in an unfatigued, rested state as a predictor of dynamic spine stability in a challenged, fatigued state. Participants completed three trials (Day 1: Rested, Fatigued; Day 2: Recovery) during which the dynamic stability of the spine was assessed over 30 repeated flexion/extension motions using maximum finite-cycle Lyapunov exponents. Multiple sets of dynamic trunk extensions were performed to fatigue the trunk extensor muscles. Across the sample population, an increase in dynamic spine stability when fatigued was observed, as well as a moderate correlation between the level of dynamic stability when rested and a stabilizing response when fatigued. Further analysis of the data on a person-by-person basis revealed three distinct responses in which participants either stabilized, destabilized or had no change in dynamic spine stability when fatigued. Therefore, the mean response of the sample population did not adequately represent the true, meaningful response of individuals within the population. These results illustrate the importance of considering individualized responses when examining dynamic stability measures, and provide preliminary evidence that suggests that individual injury risk cannot be completely captured by measures taken in an unchallenged, rested state., (Copyright © 2018 Elsevier Ltd. As the most superficial organ, the skin is the most accessible sensory system interfacing one's body and environment. With changes in posture, it is possible that the skin may undergo large deformations resulting in changes in its structural properties. The purpose of the current work was to determine the influence of spine posture on measures of trunk dorsum skin deformation, thickness and hardness in vivo. 28 young and healthy individuals were assessed while holding three static, supported spine postures (prone neutral, spine extension, and spine flexion). Skin stretch deformation was measured across each posture using an 11×9 dot matrix of 3D kinematic markers affixed to the skin of the back. Skin thickness (epidermis + dermis) was quantified using ultrasound images obtained from specific spinal levels (L4, L2, T12 and T10). Skin hardness was measured at the same specific spinal levels using a handheld Shore durometer. During the spine extension posture it was observed that the skin of the trunk dorsum relaxed/retracted on average by 12% to become both ~17% thicker and ~39% softer compared to the neutral prone posture. During the spine flexion posture the skin of the trunk dorsum stretched on average by 38% to become both ~19% thinner and ~106% harder. Additionally, in the spine flexion posture it was clear that the majority of the skin deformation occurred within the lumbar region, while deformation was more evenly distributed in the extension posture. Results also suggest that stretch distributions could be in large part determined by the intervertebral distribution of the overall spine angle, as well as in part due to the local hypodermal/subcutaneous fat and fascia distribution. With such large structural deformations of the skin of the trunk dorsum, it is suggested that corresponding changes in skin tactile sensitivity (e.g. perceptual thresholds of force necessary to elicit a sensation of touch) may be present, and should be investigated through future work. These data will aid in the development of computer models to investigate skin structural deformation (e.g. finite element models or kinematic models that track skeletal structures by accounting for soft tissue artifacts) as well as further applications pertaining to skin sensitivity, and tactile biofeedback across various body postures., (Copyright © 2016 Elsevier Ltd.
