Objective: To present a rare case of Struma ovarii with thyrotoxic storm, review the medical literature, and present the latest trends in management.Case Report: A 58 year old unmarried postmenopausal lady presented with abdominal pain, and on evaluation was found to have large abdominopelvic multilobulated lesion. She was febrile and had tachycardia on admission, and was subsequently found to have thyrotoxic storm.Conclusion Large Struma ovarii with torsion presenting with thyrotoxic storm is rare. Successful management of this case with a multidisciplinary approach was gratifying.
