1. Mepolizumab treatment in a patient with previous liver transplantation: One year follow-up

Abstract

Severe Asthma (SA) is characterized by inadequate disease control despite maximal inhalation therapy. In November 2021, a 68-years old female patient presented at our facility referring a worsening in her asthma-related symptoms and a high exacerbations rate. She reported a liver transplantation in 2015 and was in treatment with tacrolimus. We started treatment with Mepolizumab 100 mg once every 28 days and monitored her lung function as well as her lymphocytes subsets. After one year follow-up, the patient had a substantial improvement in lung function, exacerbation rate, daily OCS intake dose and no variation in the blood concentration of tacrolimus.

Abstract

Microalgae biomass have been used as a source of nutrient-rich food, feed, and health-promoting compounds. Among the several commercially available microalgae, Haematococcus pluvialis is the most abundant source of natural astaxanthin (3,3′-dihydroxy-β,β-carotene-4,4′-dione), which is considered as “super antioxidant.” Therefore natural astaxanthin produced by H. pluvialis has a higher antioxidant capacity than the synthetic sources, which reduces oxidative stress and free radicals and helps the human body to maintain a healthy state. However, H. pluvialis contains astaxanthin inside the cells, which need to be extracted using nontoxic extraction technologies for different application. Among the different available extraction technologies, supercritical fluid extraction (SFE) is a modern technology with increasing applications in pharmaceutical and nutraceutical sector. The most frequently employed supercritical solvent in food and natural product processing is carbon dioxide (CO2) due to its low critical temperature and pressure while considered as nontoxic extraction solvent. Therefore, nowadays, supercritical carbon dioxide (SC-CO2) is considered as a new substitute for complete extraction of natural compounds from different matrices. However, the extraction efficiency and purity of astaxanthin in the extract are influenced by different operative conditions such as the extraction pressure, temperature, time, and use of cosolvent. Hence the optimum operative condition of SC-CO2 extraction could enhance the extraction yields of astaxanthin and of its purity the extract. Therefore present chapter summarizes the effects of several extraction parameters on SC-CO2 extraction of astaxanthin from H. pluvialis, which could serve as benchmark for future development of SC-CO2 extraction technology and its commercial implementation in pharmaceutical and nutraceutical sector.

Abstract

Highlights • The drainage pattern of the pulmonary veins presents a wide range of anatomical variations. • Identification of anatomical variations in pulmonary veins can help avoid intraoperative complications. • A careful dissection during VATS lobectomy is essential to avoid intraoperative complications., Introduction The anatomical abnormalities in pulmonary veins can have a serious impact on pulmonary resections. Presentation of case We report the case of a 70-year-old woman undergoing VATS right upper lobectomy for the treatment of non-small cell lung cancer. During subcarinal dissection, an anomalous vein draining from the superior segment of the right lower lobe into the left atrium and passing behind the bronchus intermedius was incidentally discovered. The patient had, in addition to the inferior pulmonary vein formed by the confluence of superior and common basal veins, a supernumerary vessel identified as: accessory right V6. Retrospective review of preoperative enhanced chest computed tomography confirmed the pulmonary vascular anomaly. Discussion/Conclusion A careful dissection during pulmonary resections can help to recognize variations of the pulmonary veins, avoiding unexpected intraoperative complications.

Abstract

This chapter is dedicated to an overview of the technological options to produce in an efficient way power from biomass gasification products. This is a very important task as it tries to couple two different aspects: the benefits coming from biomass gasification, in terms of efficiency and environmentally friendly operations, with the new trends of bio syngas energetic valorization. The final goal is to explore all the potential steps for an integrated process able to valorize fully the energy content of biomass in a wide range of potential sustainable energy vectors: from pure hydrogen to electrical energy via-fuel cells or high-performance internal combustion engines. More attention is paid to the effect of processing gas coming out from gasification of biomasses that exhibits some specific characteristics such as a wide variability on hydrocarbon composition or the presence of inorganic undesired compounds.

Abstract

Biogas is one of the most important biofuels that can be produced by biomass owing to its simple and consolidated process. Until the past decade it was used for power production, but because of the direction of European Union policy issues that have drawn attention to the possibilities offered by upgrading biogas to biomethane, currently there are the important conditions for the dynamic growth of this industry. The aim of this chapter is to underline the end uses of biogas compared with its upgrade, with a focus on the gains and limitations of using polymeric membranes for biomethane purification.
