Background: There is a lack of literature on the role of splenectomy in hemolytic anemias due to glucose-6-phosphate isomerase (GPI) deficiency. GPI deficiency is a rare red blood cell (RBC) enzymopathy of the glycolytic pathway. Most present with severe disease requiring frequent transfusions. In this study, we report the effect of splenectomy in the world's largest cohort of GPI deficiency patients. The study aimed to describe the clinical, mutational, and laboratory parameters of patients with GPI deficiency. A comparison of the transfusion requirement pre- and post-splenectomy is also included. Material and methods: The ambispective study was performed from 2017 to 2023. Patients with congenital non-spherocytic hemolytic anemia (CNSHA) were screened for GPI deficiency. Detailed history, including demographic, clinical data, and transfusion details, were noted. Hematological parameters and RBC enzyme activity were estimated using spectrophotometry. The genetic study was done using restriction fragment length polymorphism, and confirmation was obtained through Sanger's sequencing. Patients were followed up after splenectomy. Results: Eighteen patients were diagnosed with GPI deficiency. About 3/4th (14/18; 77.7 %) had significant hepatosplenomegaly. Median serum ferritin levels were 890 ng/ml. Seven patients were on oral iron chelation. The nutritional status assessed as per the Indian Academy of Pediatrics growth charts revealed significant growth retardation. All the patients had severe anemia (mean Hb: 6.4 g/dl) and macrocytosis (mean MCV: 129.2 fl). Laboratory features of hemolysis were evident with reticulocytosis, raised serum lactate dehydrogenase, and indirect bilirubin. The mean GPI enzyme activity was 28.75 IU/g Hb.Ten (55.5 %) patients underwent splenectomy at a median age of 7 years. Five remain transfusion-free post-splenectomy at a median follow-up of 54 months. Other 5 had a significant reduction in transfusion requirement post-splenectomy (p A(p.Arg347His) homozygous mutation. Conclusion: The study demonstrates the benefit of splenectomy in patients with GPI deficiency.

A powerful fault detection and diagnosis (FDD) system plays a pivotal role in achieving operational excellence by maximizing system performance, optimizing maintenance strategies, and ensuring the longevity and resilience of process plants. In the context of FDD for multivariate sensor data, this study presents an improved FDD approach using graph-based neural networks. This graph neural network uses an adjacency matrix developed by extracting the expert domain knowledge and topological information of the multi-sensor system. This additional graph representation of the system is incorporated along with multivariate sensor data to capture the spatial and temporal information in neural networks efficiently. In this regard, we propose and evaluate: 1) A Graph Auto Encoder (GAE) based fault detection strategy and 2) An Attention-based Spatial Temporal Graph Convolution Network (ASTGCN) based fault diagnosis methodology. By leveraging the additional knowledge in the form of graphs, the GAE captures the complex relationships and dependencies among sensors, enabling effective anomaly detection, which identifies abnormal patterns and deviations from normal behavior, thus indicating potential faults in the system. The ASTGCN incorporates attention mechanisms to selectively focus on relevant sensor nodes and capture their spatial and temporal dependencies for fault diagnosis. The effectiveness of the proposed FDD approach is demonstrated using the benchmark Tennessee Eastman Process (TEP) problem. The results show that the proposed approaches outperform traditional methods and highlight the importance of leveraging graph-based knowledge for FDD in complex systems.

Existing literature has portrayed numerous challenges that healthcare workers (HCWs) faced during the COVID-19 pandemic, such as heightened risks of transmission against the scarcity of protective equipment, burgeoning workload, and emotional distress, to name a few. However, most studies explored HCWs' experiences at the individual level rather than examining the collective responses. Exploring these experiences could reveal the social-cultural locality of the pandemic while identifying the system constraints in public health emergencies. As part of a mixed-method study on COVID-19 pandemic impacts, we analysed qualitative interview data with 129 HCWs and health-related staff to explore their experiences during the pandemic between 2020 and 2021 in Vietnam, Indonesia, and Nepal. Using Bahers' sociological framework, Community of Fate, we describe five themes reflecting the formation of a community of HCWs and the social cohesion underlying their efforts to survive hardship. The first three themes characterise the HCW community of fate, including (1) Recognition of extreme work-related danger, (2) physical and figurative closures where HCWs restrict themselves from the outside world, (3) chronic ordeals with overwhelming workload and responsibilities, encompassing recurrent mental health challenges. Against such extreme hardship, cohesive bonding and social resilience are reflected through two additional themes: (4) a mutual sense of moral and professional duty to protect communities, (5) the vertical and horizontal convergence among HCWs across levels and among government departments. We discuss these HCWs’ challenges in relation to systemic vulnerabilities while advocating for increasing investment in public health and collaboration across government sectors to prepare for emergency situations.

Dye-sensitized solar cells (DSSC) are considered as one of the most promising photovoltaic technologies, while exploring alternatives to traditional silicon-based solar cells. Dye sensitizer is a very important component of DSSC and the use of natural dyes in these cells is a promising development for this technology, as it has non-hazardous impact on the environment. This work reports the new natural dye extracted from Brassica Napus flower petals and used as sensitizer in TiO2 based DSSC. UV–Vis spectra showed the presence of carotenoid and flavonoid pigments along with broad absorption in dye loaded TiO2. FITR revealed functional groups in the Brassica Napus dye extract. Mass spectroscopy was used to find major constituents present in extract. With the natural Brassica Napus sensitizer and TiO2, photovoltaic performance results viz. conversion efficiency (ɳ%) 0.062, short circuit current density (Jsc) 0.45 mA/cm2, open circuit voltage (Voc) 0.260 V and fill factor (FF) 0.52 were observed. Further, HOMO-LUMO levels of constituents present in Brassica Napus have been measured via CV measurements and compared with relative energetic obtained by computational studies. The transport characteristics revealed from EIS studies were used to rationalize the observed photo-voltaic performance of the DSSC.

Capping agents are widely employed to stabilize the synthesized nanoparticles. To address the problems associated with the chemical capping agents and agglomeration of nanoparticles in the chemical and physical methods, green synthesis methods based upon the usage of biodegradable, well-dispersed capping agents have widely been used. In this work, TiO2 was synthesized by simple sol–gel method and a green capping agent, potato starch has been used. Potato starch consist of two basic biopolymers i.e. amylose (∼20 %) and amylopectin (∼80 %), that binds through the functional groups viz. hydroxyl, carboxylic or amino groups with the metal cations to prevent agglomeration of the growing NPs. The structure and morphology of the synthesized TiO2 were characterized by FTIR and XRD, where the XRD patterns confirmed the anatase phase of TiO2. From the Tauc’s plot, the band-gap of un-capped and differently capped TiO2 was obtained. Potato starch-capped TiO2 coated with the Ru dye (N719) revealed photoelectric conversion efficiency of 2.58 % under illumination of 100 mW/cm2, that was much upgraded compared to the performance with the un-capped TiO2 (1.84 %). The EIS studies revealed the kinetic parameters. The illustrated results showed that potato starch-capped TiO2 have greater potential in enhancing the DSSC efficiency.

Background: cancer is one of the foremost life-threatening diseases and is managed by various strategies like surgery, radiotherapy, and chemotherapy. Among all, chemotherapy is the most preferential strategy for cancer management but faces certain limitations. Phytochemicals (PCs) have been considered potential chemotherapeutics and chemopreventive agents but their clinical application can be questionable because of their poor bioavailability and stability. Purpose: phytosomes are promising carriers to deliver anticancer PCs to the target site with improved bioavailability and stability owing to their unique properties. This comprehensive review discusses the recent advances in phytosomes loaded with anticancer PCs (covering the period of 2013-December 2023) to overcome the limitations of PCs. Study design and methodology: initially, this review covers the anticancer PCs that are under clinical trials along with the PCs that are FDA-approved (especially for cancer management). Here, we have excluded the PCs that are (a) under an experimental/preclinical or investigational stage; (b) possess activity other than the anticancer; (c) exhibit anticancer activity in combination with other anticancer drugs; and (d) FDA-approved PC-based drugs with activity other than the anticancer. Further, this article mainly emphasizes the clinical application of phytosomes loaded with anticancer PCs to overcome the limitations. Additionally, phytosomes under clinical trials for the management of cancer, their challenges, and future directions are also discussed. Results and conclusion: phytosomes are promising and versatile alternatives to boost the anticancer PCs’ therapeutic efficacy, bioavailability, and specific targeting owing to their distinctive structural composition, which is similar to the composition of cell membranes enabling them close to the physiological systems. Moreover, these vesicular systems can also increase the oral bioavailability of polar PCs, protect them from physical and chemical degradation, and facilitate the targeting efficiency of anticancer PCs through the stimuli-responsive and sustained release of PCs. Therefore, this review will help the researchers to develop a phytochemical-based drug delivery system for the successful management of cancer.

Introduction: Some specific personality factors in patients of bipolar disorder are potential risk factors for developing comorbid substance use disorder. Comorbid substance use disorder is one of the most common factors associated with non-adherence to treatment in bipolar disorder patients leading to poor quality of life. We compared all three variables in bipolar disorder-1patients with or without substance use disorder. Methods: It is a cross-sectional study done on 90 patients of Bipolar disorder-1 diagnosed as per DSM-5, 45 without and 45 with comorbid substance use disorder. Personality factors, non-adherence to medication and quality of life were assessed and compared between both the groups using standardized tools and appropriate statistical analysis methods. Results: There was a significant difference between the groups in terms of distribution of socio-economic status (Bias Corrected Cramer'sV = 0.31 moderate association), gender (Bias Corrected Cramer'sV = 0.37 moderate association), distribution of 16 personality factor B (Cramer's V = 0.28) and factor G (χ2 = 6.222, p = 0.045) (Cramer's V = 0.26). Limitations: Small sample size and a cross-sectional study design. Conclusion: SUD and NSUD group differed on 16 PF factor B and G which are factors for reasoning and Rule-Consciousness respectively although on regression analysis this difference was not significant for factor G. In binary logistic regression multivariable model, an average score (as compared to low score) of personality factor B was found to significantly increase the ODDS of substance use. SUD group had more severity and greater number of episodes, had lower level of education, belonged to low socio-economic status.

This study investigates the utilization of snow persistence (SP) as a metric to assess snow cover patterns in the Upper Indus-Ganga-Brahmaputra (UIGB) river basins, an area notably susceptible to climate change. The objectives of the study are to establish seasonal snow persistence, calculate seasonal snow lines, and quantify the temporal trend of snow lines. It also underscores the importance of accounting for climate zones, elevation, and seasonal variations when investigating snow persistence. Furthermore, the study estimates the snow line elevation using the Regional Snow Line Elevation (RSLE) approach with remote sensing data. Over a span of 19 years (2003–2021), the analysis of SP% in UIGB basins indicates a notable reduction in the mean annual permanent snow zone area, decreasing by -2.61 %, -2.37 %, and -4.18 % from winter (DJF) to summer (JJA). The results uncover considerable fluctuations in snow cover across various climate zones and seasons. The impact of warming is clearly visible on snow cover, instigating a transition from permanent and seasonal snow zones towards intermittent and minimal or no snow zones. The seasonal analysis of the snow line from 2003 to 2021 illustrates fluctuation in the snow line for UIGB basins, from 2349 ± 117 m to 4802 ± 51.7 m, 3003 ± 286 m to 4871 ± 64.4 m, and 3238 ± 127 m to 5047 ± 73 m between winter and summer seasons, with the most significant fluctuation occurring in the Brahmaputra basin. A substantial increase in Snow Line Elevation (SLE) is observed for Ganga (6.67 m yr−1), and Brahmaputra (7.26 m yr−1) at the conclusion of the melting season. These findings offer invaluable insights for formulating targeted mitigation strategies in the snow accumulation regions of each river basin. They equip us with the ability to respond effectively to the spatiotemporal variability of snow cover caused by climate change.

Effective water resource management is essential in large metropolitan cities. Digital Twins (DT), supported by IIoT and machine learning technologies, provide opportunities for real-time prediction and optimization for effective decision-making in water distribution systems. A framework for the digital twin of the Water Distribution Network (WDN) is developed in this paper to achieve higher operational efficiency using ‘WNTR’, the Python-based library of EPANET. All computational experiments and methods were validated on the benchmark hydraulic C-TOWN network (Ostfeld et al., 2011). The hydraulic parameters and quality parameters of the DT model for the water network were calibrated using the Differential Evolution (DE) algorithm. The calibrated DT served as a real-time proxy to generate simulation data, which is used for two different applications in large-scale water networks: (i) Disinfectant dosage regulation task using booster stations and (ii) pipe leakage localization task. The calibrated DT was utilized to estimate the optimal disinfectant dosing rates, ensuring water quality control within an acceptable range using optimization. The results highlight the effectiveness of the neural network and real-time optimization strategy to achieve the optimal dosing rate. For the leakage localization task, the Graph Convolution Networks (GCN) based neural network trained on the DT was found to predict leakage location very accurately.

The field of nanotechnology has revolutionised global attempts to prevent, treat, and eradicate infectious diseases in the foreseen future. Nanovaccines have proven to be a valuable pawn in this novel technology. Nanovaccines are made up of nanoparticles that are associated with or prepared with components that can stimulate the host's immune system. In addition to their delivery capabilities, the nanocarriers have been demonstrated to possess intrinsic adjuvant properties, working as immune cell stimulators. Thus, nanovaccines have the potential to promote rapid as well as long-lasting humoral and cellular immunity. The nanovaccines have several possible benefits, including site-specific antigen delivery, increased antigen bioavailability, and a diminished adverse effect profile. To avail these benefits, several nanoparticle-based vaccines are being developed, including virus-like particles, liposomes, polymeric nanoparticles, nanogels, lipid nanoparticles, emulsion vaccines, exomes, and inorganic nanoparticles. Inspired by their distinctive properties, researchers are working on the development of nanovaccines for a variety of applications, such as cancer immunotherapy and infectious diseases. Although a few challenges still need to be overcome, such as modulation of the nanoparticle pharmacokinetics to avoid rapid elimination from the bloodstream by the reticuloendothelial system, The future prospects of this technology are also assuring, with multiple options such as personalised vaccines, needle-free formulations, and combination nanovaccines with several promising candidates.

Hydrophobic coatings basically decrease the wettability of surface due to the presence of nanostructures on it. In this research work, nanozirconia sol was prepared by the sol-gel method and further hydrophobically modified using the hexamethyldisilazane (HMDS). Prepared sol was coated on the glass substrate and then analysed for its wetting behaviour. Three samples with different HMDS ratios mixed to Zirconia sol were coated, and analysed for surface morphological parameters through the atomic force microscopy and field emission scanning electron microscopy techniques. The wetting behaviour was analysed by studying the water contact angle and surface free energy of the samples. Here the transmittance was measured by transmittance metre at regular intervals for 30 days. Functional group determination was done by Fourier transform infrared spectrometer that confirmed the formation of Zirconium nanoparticles. Maximum contact angle of 101.2° was observed that showcased the hydrophobic properties. Hydrophobic coating was then successfully prepared, and sample with 2:1 ratio of Zirconium nanoparticles and HMDS showed the best hydrophobic nature amongst all the coatings prepared in this investigation.

Grain germination is a process involving numerous factors that influence the biochemical processes inside the plant cells. This review covered the abiotic factors that lead to the germination and significantly impact the nutritional properties and digestion behavior of rice grains. The macro- and micro-nutrients can be changed depending on the intensity of the applied variables during germination. For instance, germination time can increase the protein content in the grain and concurrently reduce its protein digestibility. In most cases, the number of bioactive compounds present in rice grains are increased regardless of germination conditions. Germination can promote the complexation of nutrients and thus negatively interfere with the digestibility of macronutrients. This review highlighted the influence of the germination process on the nutritional quality of rice grains, providing information about the germination conditions and their impacts on the anabolic and catabolic reactions of the grain, emphasizing the health benefits.

Congenital cytomegalovirus (CMV) infection is rampant viral disease, one of the commonest non-genetic cause for neurodevelopmental delay which is often under diagnosed. Leukemoid reaction, Hemophagocytic Lymphohistiocytosis (HLH) and autoimmune cytopenias are rare hematological manifestations of CMV infection. There is paucity of literature regarding hematological manifestation in CMV infection. Here we present four cases of CMV infection with different hematological findings where early diagnosis and prompt intervention resulted in good outcome.

Hepatocellular carcinoma (HCC) frequently unfolds under an inflammatory condition, which is a hub for a plethora of cytokines. A better understanding of the cytokine functions and their contributions to disease development is key to design of future therapeutic strategies and reduction of global HCC burden. In this context, one of the major cytokines present in the HCC tumour milieu is the transforming growth factor-β (TGF-β). One of its classical functions involve facilitation of epithelial to mesenchymal transition (EMT), in tumour cells, promoting an invasive phenotype. In spite of its clinical relevance, the cellular events associated with TGF-β-induced EMT and its molecular regulation is poorly elucidated. Therefore, as part of this study, we treated HCC cells with TGF-β and characterized the cellular processes associated with EMT. Interestingly, EMT triggered by TGF-β was found to be associated with cytostasis and altered cellular metabolism. TGF-β resulted in down-regulation of cell cycle-associated transcripts, like Cyclin A2 (CCNA2), and metabolic genes, like Glutamic-oxaloacetic transaminase 1 (GOT1) through epigenetic silencing. An overall increase in total histone repressive mark (H3K27me3) associated with a specific enrichment of H3K27me3 at the upstream promoter region of CCNA2 and GOT1 was observed after TGF-β exposure, leading to their down-regulation. Importantly, TGF-β-downstream signalling mediator- SMAD and chromatin repressive complex member-enhancer of zeste homolog 2 (EZH2) were found to co-immunoprecipitate and were required for the above effects. Overall, our findings reflect that HCC cells undergoing EMT, attain cytostasis and modulate metabolic demands to efficiently facilitate the EMT differentiation switch, and these events are regulated at the epigenomic level through TGF-β-mediated signalling. Our results provide better understanding of cellular invasive features which can lead to development of novel therapeutic strategies.

Purpose: To describe the use of a tarsoconjunctival pedicle flap for the repair of scleral melt secondary to treatment of conjunctival melanoma. Observation: A 67-year-old woman developed progressive scleromalacia after multiple treatments for an American Joint Committee on Cancer cT2d category conjunctival melanoma. Prior to referral, she underwent synchronous topical chemotherapy (interferon, 5-fluorouracil, mitomycin). Then, incomplete tumor regression led to excision with adjuvant cryotherapy. Lastly, systemic metastasis treated with systemic immunotherapy provided durable remission. However, her multiple treatments (e.g., topical chemotherapy, resection, cryotherapy) were associated with progressive nasal bulbar scleromalacia treated by conjunctival advancement and amniotic membrane grafts. Sclera reinforcement was achieved after a tarsoconjunctival flap was affixed to the eye to cover, and thus vascularize the scleral defect. The tarsoconjunctival flap provided 5 years of tectonic support. Conclusions and Importance: Tarsoconjunctival pedicle flaps can provide scleral integrity for a patient with progressive scleral melting.

ABSTRACT: Objectives: This study aimed to develop and evaluate a novel air-dried high-resolution melt (HRM) assay to detect eight major extended-spectrum β-lactamase (ESBL) (blaSHV and blaCTX-M groups 1 and 9) and carbapenemase (blaNDM, blaIMP, blaKPC, blaVIM and blaOXA-48-like) genes that confer resistance to cephalosporins and carbapenems. Methods: The assay was evaluated using 439 DNA samples extracted from bacterial isolates from Nepal, Malawi and the UK and 390 clinical isolates from Nepal with known antimicrobial susceptibility. Assay reproducibility was evaluated across five different real-time quantitative PCR (qPCR) instruments [Rotor-Gene® Q, QuantStudioTM 5, CFX96, LightCycler® 480 and Magnetic Induction Cycler (Mic)]. Assay stability was also assessed under different storage temperatures (6.2 ± 0.9°C, 20.4 ± 0.7°C and 29.7 ± 1.4°C) at six time points over 8 months. Results: The sensitivity and specificity (with 95% confidence intervals) for detecting ESBL and carbapenemase genes was 94.7% (92.5–96.5%) and 99.2% (98.8–99.5%) compared with the reference gel-based PCR and sequencing and 98.3% (97.0–99.3%) and 98.5% (98.0–98.9%) compared with the original HRM wet PCR mix format. Overall agreement was 91.1% (90.0–92.9%) when predicting phenotypic resistance to cefotaxime and meropenem among Enterobacteriaceae isolates. We observed almost perfect inter-machine reproducibility of the air-dried HRM assay, and no loss of sensitivity occurred under all storage conditions and time points. Conclusion: We present a ready-to-use air-dried HRM PCR assay that offers an easy, thermostable, fast and accurate tool for the detection of ESBL and carbapenemase genes in DNA samples to improve antimicrobial resistance detection.

Introduction: The nucleoside analogue reverse-transcriptase inhibitor Abacavir has potent antiviral activity against HIV; however, 5–8% patients develop hypersensitivity reactions within six weeks of treatment. The presence of the HLA-B*57:01 allele is strongly associated with the risk of Abacavir-associated hypersensitivity reaction (ABC-HSR). Prevalence of HLA-B*57:01 allele varies in different populations. This observational study was performed to determine the prevalence of HLA-B*57:01 allele in the population of Eastern India which included both HIV-positive and HIV-negative subjects. Methods: We screened 406 subjects attending the ART (antiretroviral treatment) centre and linked ICTC (Integrated counselling and testing centre), however 6 samples did not have adequate DNA. HLA-B*57:01 typing was done using direct sequence specific primer PCR (SSP-PCR). All PCR positive samples were sequenced using Sanger's method. Results: The prevalence of HLA-B*57:01 genotype in our cohort was 12.25% (49/400). Prevalence among men was 13.3% (33/248) and female was 10.5% (16/152). The prevalence was similar in HIV-positive subjects (13.5%) and in HIV-negative subjects (11%). Conclusions: Prevalence of HLA-B*57:01 in our eastern Indian population was high. Therefore, screening for HLA-B*57:01 before ABC administration would be useful to prevent ABC-HSR.

Purpose: Radiation has been used in the treatment of retinoblastoma. Herein, we present the novel use of palladium-103 plaque brachytherapy as primary treatment. Observation: An 8-year-old asymptomatic girl presented was found to have a solitary peripheral retinoblastoma in her right eye. She was treated with primary palladium-103 plaque brachytherapy (47.4 Gray over 5 consecutive days). A secondary, vitreous hemorrhage noted 46 months after irradiation was successfully controlled by laser tumor-demarcation. With 19-years follow up, there has been no clinical scleropathy, or local tumor recurrence. The eye yields 20/20 vision and there has been no systemic metastasis. Conclusion and importance: Palladium-103 plaque brachytherapy successfully controlled retinoblastoma, while preserving the globe, vision, and life.

Present study aimed to assess the alterations in neurotransmitter expression and its association with Blood Lead Level (BLL) and neurobehavioral pattern in children. 72 school going children were recruited. Blood lead levels were determined by Atomic Absorption Spectrophotometer. Neurobehavioral state was assessed by means of population specific scale i.e. CPMS (Childhood Psychopathological measurement Schedule). Serum serotonin and dopamine were estimated by ELISA, receptor and transporter gene expressions were assessed by quantitative real time PCR. Significant positive correlation was observed between Total CPMS score (i.e. adverse neurobehaviour) and BLL. Further, serum serotonin levels and dopamine receptor expression showed a negative and positive association with BLL, respectively. In similarity, serum serotonin levels showed a negative correlation and dopamine receptor expression had a significant positive correlation with total CPMS score. Environmental exposure to Lead (Pb) may result in significant alterations in the neurotransmitter levels which may be associated with neurobehavioral changes in the children exposed to Pb.

Imidacloprid, chlorpyrifos, profenofos and cypermethrin are most commonly applied pesticides on green leafy vegetables. After a survey conducted to know the pesticide management practices in Sagar, India, a procedure based on hybrid micellar liquid chromatography coupled to a photodiode array detector has been developed and validated to monitor imidacloprid, chlorpyrifos, profenofos and cypermethrin content in green leafy vegetables. The method was validated following the guideline of SANTE/11,312/2021 in terms of: selectivity, linearity (r2> 0.998), limit of quantification (0.09–0.25 mg/Kg, depending on the analyte under investigation), precision (

Objectives: India shares largest burden of congenital birth defects (CBDs) in the world. There is paucity of data on the CBDs in our region of the country. Methods: This was a hospital based observational study to find incidence, risk factors and outcome of CBDs among live neonates. Results: Of 7,075 babies, 92 had 124 CBDs, making the incidence 1.3%. The system predominantly involved was musculoskeletal system (52.2%) followed by central nervous system (28.3%), genitourinary system (26.1%). Among the single CBDs, congenital talipes equino varus (27.4%) was the commonest followed by spina bifida (11.2%) and cleft palate (6.4%). The significant factors associated with CBD were no antenatal visits (p = 0.041), TORCH and/or VDRL positivity (p = 0.023), gestational diabetes mellitus (GDM) (p = 0.007), hypertension (p = 0.090), consanguinity (p = 0.03), no intake of folic acid (p = 0.015). Of these, factors found significant on logistic regression were GDM (OR 3.44; p = 0.018), maternal hypertension (OR 4.88; p = 0.011), consanguinity (OR 13.55; p = 0.028) and no intake of iron/folic acid (OR 5.53; p = 0.001). Of the 92 neonates with CBD, 44 (47.8%) were discharged, 28 (30.5%) expired, 20 (21.7%) left against medical advice. Conclusion: Incidence of CBD was comparable to previous Indian studies. Consanguinity, no intake of folic acid, maternal hypertension and GDM were associated risk factors. About half neonates with CBD expired or left against medical advice. This shows dismal outcome of neonates with CBD in this region.

Medical images are watermarked with patient details to impose patient identification in tele-medicine applications. Fundus images comprise of fine features like blood vessels, macula and optic disks. Conventional watermarking methods may tamper crucial information in this case. Even minutest change can distort fine features and affect final decision. ROI (Region of Interest) selection approach also fails here as fine features of fundus images are evenly distributed and it’s almost impossible to properly define Region of Interest (ROI) and Region of Non-Interest (RONI) for medical practitioner. Hence arises an imperative necessity of a watermarking system which does not tamper the fundus images and results in no loss of medical information. This paper proposes a zero-watermarking system which uses singular value coefficients in Wavelet domain for generation of unique identification code from fundus images which is strategically combined with patient ID to generate watermark. Proposed technique suits to the specific requirements of fundus images and optimizes the conflicting requirements of traditional watermarking. The technique aims at unique identification, authentication and integrity verification of medical images. Experimental results reveal the robustness of the system against some common image processing attacks and indicate its suitability for secure exchange of medical images for tele-ophthalmological applications and for storage in large distributed medical databases.

Microbial fuel cells (MFCs) that generate bioelectricity from biodegradable waste have received considerable attention from biologists. Fungi play a significant role as both anodic and cathodic catalysts in MFCs. Saccharomyces cerevisiae is a fungus with an ability to transfer electrons through mediators such as methylene blue (MB), neutral red (NR) or even without a mediator. This unique role of fungal cells in exocellular electron transfer (EET) and their interactions with electrodes hold a lot of promise in areas such as wastewater treatment where yeast cell-based MFCs can be used. The present article highlights the physico-chemical factors affecting the performance of fungal-mediated MFCs in terms of power output and degradation of organic pollutants, along with the challenges associated with fungal MFCs. In addition, to this comparative assessment of fungal-mediated bio-electrochemical systems, their development, possible applications and potential challenges are also discussed.

Summary: Background: Typhoid fever is a major public health problem in low-resource settings. Vaccination can help curb the disease and might reduce transmission. We have previously reported an interim analysis of the efficacy of typhoid conjugate vaccine (TCV) in Nepali children. Here we report the final results after 2 years of follow-up. Methods: We did a participant-masked and observer-masked individually randomised trial in Lalitpur, Nepal, in which 20 019 children aged 9 months to younger than 16 years were randomly assigned in a 1:1 ratio to receive a single dose of TCV (Typbar TCV, Bharat Biotech International, India) or capsular group A meningococcal conjugate vaccine (MenA). Participants were followed up until April 9, 2020. The primary outcome was blood culture-confirmed typhoid fever. Cases were captured via passive surveillance and active telephone surveillance followed by medical record review. The trial is registered at ISRCTN registry, ISRCTN43385161 and is ongoing. Findings: From Nov 20, 2017, to April 9, 2018, of 20 119 children screened, 20 019 participants were randomly assigned to receive TCV or MenA vaccine. There were 75 cases of blood culture-confirmed typhoid fever included in the analysis (13 in the TCV group and 62 in the MenA group) over the 2-year period. The present work mainly highlights the simultaneous detection of four skin whitening agents i.e. hydroquinone (HQ), resorcinol (RS), catechol (CC) and 3,3′-dichlorobenzidine (DCB) in facial creams and body lotion. Among these, the first three are positional isomers of dihydroxybenzene so simultaneous separation is difficult with the conventional reverse-phase high-performance liquid chromatographic technique (RP-HPLC). The selected skin whitening agents were detected in facial cream and body lotion using micellar liquid chromatography coupled to a photodiode array detector (MLC-PDA). In the present study, optimization of the method was accomplished using response surface methodology (RSM) with central composite design (CCD). The second-order polynomial model for predicting the optimal chromatographic run time was evaluated by the analysis of variance (ANOVA) and 3D response surface plots for the interactions between three variables were constructed. Three experimental parameters which were chosen as independent variables were; surfactant concentration (SDS), the volume percentage of organic modifier (OM) and pH of the mobile phase. The second-order polynomial model for predicting the optimal chromatographic run time was evaluated by the analysis of variance (ANOVA) and 3D response surface plots for the interactions between three variables were constructed. The optimized mobile phase was 0.15 M SDS and 7% 1-butanol, buffered at pH 7 with 0.01 M NaH2PO4. The chromatographic run time for simultaneous determination of selected analytes was 7.5 min. The correlation coefficient (r2) values were satisfactory between 0.998–0.999 over the linear concentration range. Limits of detection (LODs) and the limits of quantification (LOQs) for the four skin whitening agents were in the range of 0.05–0.07 μg/mg and 0.11–0.14 μg/mg, respectively. Trueness (98.4–102.7%) and precision (< 4.3%) were acceptable. The developed method was fast, cost-effective, and green which could easily analyze complex matrices (facial creams, body lotion) without any pretreatment other than filtration. The results indicated that the MLC-PDA method proved to be more suitable for the simultaneous separation of selected positional isomers.

Cyamopsis tetragonoloba and Prosopis cineraria are two legumes of the semi-arid region of Indian subcontinent which are unexplored with respect to their medicinal potential. Moreover, there is considerable lack in the comparative analysis of the biological properties of crude and enriched fractions obtained from the pods and seeds. Therefore, this study aims in investigating the effect of purification on the antioxidant and anticancerous activities of the extracts from the two legumes. This is the first study to purify an enriched methanolic fraction using Amberlite XAD7HP column chromatography followed by analysis using Thin Layer Chromatography. This matrix provided an economic and time efficient isolation of flavonoids and isoflavonoids from the seeds and pods of the above mentioned legumes. In addition, antioxidant activity carried out using DPPH assay showed that purification process did not contributed to enhanced antioxidant potential. However, inverse results were obtained during anticancerous activity assay on Huh-7 cell lines.

Introduction: Target delineation variability is a significant technical impediment in multi-institutional trials which employ intensity modulated radiotherapy (IMRT), as there is a real potential for clinically meaningful variances that can impact the outcomes in clinical trials. The goal of this study is to determine the variability of target delineation among participants from different institutions as part of Southwest Oncology Group (SWOG) Radiotherapy Committee’s multi-institutional in-silico quality assurance study in patients with Pancoast tumors as a “dry run” for trial implementation. Methods: CT simulation scans were acquired from four patients with Pancoast tumor. Two patients had simulation 4D-CT and FDG-FDG PET-CT while two patients had 3D-CT and FDG-FDG PET-CT. Seventeen SWOG-affiliated physicians independently delineated target volumes defined as gross primary and nodal tumor volumes (GTV_P & GTV_N), clinical target volume (CTV), and planning target volume (PTV).Six board-certified thoracic radiation oncologists were designated as the ‘Experts’ for this study. Their delineations were used to create a simultaneous truth and performance level estimation (STAPLE) contours using ADMIRE software (Elekta AB, Sweden 2017). Individual participants’ contours were then compared with Experts’ STAPLE contours. Results: When compared to the Experts’ STAPLE, GTV_P had the best agreement among all participants, while GTV_N showed the lowest agreement among all participants. There were no statistically significant differences in all studied parameters for all TVs for cases with 4D-CT versus cases with 3D-CT simulation scans. Conclusions: High degree of inter-observer variation was noted for all target volume except for GTV_P, unveiling potentials for protocol modification for subsequent clinically meaningful improvement in target definition. Various similarity indices exist that can be used to guide multi-institutional radiotherapy delineation QA credentialing. The recent advancements in the synthetic polymer chemistry have resulted in synthesis of potentially distinct conducting polymers that have found a wide range of applications from industry to biology. Among various conducting polymers, poly (diaminonaphthalene) (PDAN) and its derivatives have emerged as promising materials in constructing bio/chemical sensors and shown potential in quantitative and qualitative assays of a variety of bioactive molecules. It is, however, surprising that in spite of having unusual physicochemical properties of this typical polymer and its monomers, no comprehensive information about their sensing properties are available in the literature either as review articles or monographs. The present review article highlights a broad scenario of diaminonaphthalene (DAN) and its polymer-based nanostructured bio/chemical sensors providing insights into their functioning, structural composition, fabrication through various polymerization routes, and eventual applications in sensing different type of biochemically active molecules. Moreover, PDAN based Dual Gate Organic Field Effect Transistor has been modeled and substantially reviewed for their applications in sensing the biomolecules. The underlying present challenges in the area of PDAN based biochemical sensors and their future prospects have been critically examined and several issues to be addressed have also been raised.

At the end of December 2019, Wuhan City became the epicenter of the highly contagious virus known as the novel coronavirus. Now that mid-2020 has already passed, almost every country is adversely affected by Corona Virus Disease (COVID-19). The routine activities of people of all ages are overturned, which has led to a shift in the trends of waste created by households, streets, and most importantly, medical facilities and quarantine centers. Compulsive use of personal protection equipment such as masks, gloves, sanitizers, etcetera by the frontline workers from the medical sector, banks, daily need stores, waste collection industries, etc. and the use of masks by every common man stepping out has skewed the trend of waste generation to a different direction. Recently, the replacement of single-use plastic was accepted by the masses, and the pandemic suddenly rebounded to the previous situation, it is expected to be worse in the long run. Another secondary outcome is reduced waste collection and recycling due to lockdown, leading to a pile-up of wastes. But several nations are adopting strategies to break the transmission chain of the virus by trying to minimize human contact. The study discusses the effect of COVID-19 on the generation, recycling, and disposal of solid waste. A brief collection of different countries’ efforts to restrict the transmission of virus through solid waste is also discussed.

Background: The study was carried out to assess the genetic variability present in ashwagandha and to examine the nature of associations of various traits to the root yield of the plant. Methods: Fifty-three diverse genetic stocks of ashwagandha (Withania somnifera) were evaluated for 14 quantitative characteristics. Analysis of variance, correlation, and path coefficient analysis were performed using the mean data of 2 years. Results: Analysis of variance revealed that the genotypes differed significantly for all characteristics studied. High heritability in conjunction with high genetic advance was observed for fresh root weight, 12 deoxywithastramonolide in roots, and plant height, which indicated that selection could be effective for these traits. Dry root weight has a tight linkage with plant height and fresh root weight. Further, in path coefficient analysis, fresh root weight, total alkaloid (%) in leaves, and 12 deoxywithastramonolide (%) in roots had the highest positive direct effect on dry root weight. Conclusion: Therefore, these characteristics can be exploited to improve dry root weight in ashwagandha genotypes and there is also scope for the selection of promising and specific chemotypes (based on the alkaloid content) from the present germplasm. The pacemaker properties of the suprachiasmatic nucleus (SCN) circadian clock are shaped by mechanisms that influence the expression and behavior of clock proteins. Here, we reveal that G-protein-coupled receptor kinase 2 (GRK2) modulates the period, amplitude, and entrainment characteristics of the SCN. Grk2-deficient mice show phase-dependent alterations in light-induced entrainment, slower recovery from jetlag, and longer behavioral rhythms. Grk2 ablation perturbs intrinsic rhythmic properties of the SCN, increasing amplitude and decreasing period. At the cellular level, GRK2 suppresses the transcription of the mPeriod1 gene and the trafficking of PERIOD1 and PERIOD2 proteins to the nucleus. Moreover, GRK2 can physically interact with PERIOD1/2 and promote PERIOD2 phosphorylation at Ser545, effects that may underlie its ability to regulate PERIOD1/2 trafficking. Together, our findings identify GRK2 as an important modulator of circadian clock speed, amplitude, and entrainment by controlling PERIOD at the transcriptional and post-translational levels.

Three new terpenic constituents characterized as 3,7,11,15-tetramethyl hexadecanyl 6′-methyl hept-5′-enyl ether (phytyl iso-octyl ether) (1), 2,6,10,14,18-pentamethylnonadecanoyl-β-d-glucopyranosyl-(2 → 1)-β-d-glucopyranosyl-(2 → 1)-β-d-glucopyranoside (procerasesterterpenoyl triglucoside) (3) and 3,7,11,15 tetramethylhexadecanoyl-β-d-glucopyranosyl-(2 → 1)-β-d-glucopyranosyl-(2 → 1)-β-d-glucopyranosyl(2 → 1)-β-d-glucofuranoside (dihydrophytoyl tetraglucoside) (4) have been isolated for the first time from the roots of Calotropis procera (Ait.) R. Br. (Asclepiadaceae) along with the known compound 1,2-dihexadecanoyl-3-phosphatyl glycerol (2). The structures of all these phytoconstituents have been established by spectral data analysis and chemical reactions.
