1. Selection of an Efficient AAV Vector for Robust CNS Transgene Expression

Abstract: Adeno-associated virus (AAV) capsid libraries have generated improved transgene delivery vectors. We designed an AAV library construct, iTransduce, that combines a peptide library on the AAV9 capsid with a Cre cassette to enable sensitive detection of transgene expression. After only two selection rounds of the library delivered intravenously in transgenic mice carrying a Cre-inducible fluorescent protein, we flow sorted fluorescent cells from brain, and DNA sequencing revealed two dominant capsids. One of the capsids, termed AAV-F, mediated transgene expression in the brain cortex more than 65-fold (astrocytes) and 171-fold (neurons) higher than the parental AAV9. High transduction efficiency was sex-independent and sustained in two mouse strains (C57BL/6 and BALB/c), making it a highly useful capsid for CNS transduction of mice. 3. Multiple Stressors in North America: Perspectives for the New World

Abstract: Mexico, the United States, and Canada are home to a diverse array of river systems. As is the case for rivers across the globe, complex mixtures and interactions of stressors have fundamentally altered the structure and function of these ecosystems. Here, we summarize the intensity and extent of major regional stressors to North American rivers and their impacts on important physical, biological, and chemical properties. The large lakes ecoregion is impacted by broad-scale physical alterations of watercourses, which include both point and nonpoint pollution from industry, municipalities, and agriculture. The establishment of invasive aquatic species is another key stressor. The most pressing stressor in polar freshwaters is rapid anthropogenic climate change. Urbanization is a focal stressor in temperate coastal river habitats. Increasing agricultural production, fine-sediment pollution, and nutrient enrichment are major threats to temperate floodplain rivers. Stressors common across temperate upland river systems include increasing populations and urban areas, impoundments and surface-water abstraction, and land-use change. Point-source pollution, nonnative species, and climatic vulnerability are primary stressors affecting tropical and subtropical coastal rivers. Similarly, climate change and human activities that mitigate water scarcity such as impoundments, diversions, crop irrigation, and surface and groundwater withdrawals are principal stressors in xeric and endorheic river systems. Across ecoregions, these and other stressors can interact and cause surprising ecological responses. Further, the development and introduction of new contaminant sources (e.g., genetically modified crops, antibiotics, hormones, pharmaceuticals, and personal health care products) pose new challenges for river conservation.

Abstract: Recent evidence has strongly implicated the IL-1 family cytokines IL-36 alpha, IL-36 beta, and IL-36 gamma as key initiators of skin inflammation. Similar to the other members of the IL-1 family, IL-36 cytokines are expressed as inactive precursors and require proteolytic processing for activation; however, the responsible proteases are unknown. Here, we show that IL-36 alpha, IL-36 beta, and IL-36 gamma are activated differentially by the neutrophil granule-derived proteases cathepsin G, elastase, and proteinase-3, increasing their biological activity similar to 500-fold. Active IL-36 promoted a strong pro-inflammatory signature in primary keratinocytes and was sufficient to perturb skin differentiation in a reconstituted 3D human skin model, producing features resembling psoriasis. Furthermore, skin eluates from psoriasis patients displayed significantly elevated cathepsin G-like activity that was sufficient to activate IL-36 beta. These data identify neutrophil granule proteases as potent IL-36-activating enzymes, adding to our understanding of how neutrophils escalate inflammatory reactions. Inhibition of neutrophil-derived proteases may therefore have therapeutic benefits in psoriasis.

Abstract: African American women are at heightened risk for intimate partner violence (IPV) and its negative consequences, including health-compromising behaviors. Deliberate self-harm (DSH) is one clinically-relevant behavior that has been understudied among African American women generally and those with exposure to IPV in particular. To date, no studies have examined factors that may account for the relationship between IPV and DSH. Therefore, the goal of the present study was to examine the intercorrelations among IPV (physical, psychological, and sexual), PTSD, and DSH history and versatility, and the potentially mediating role of PTSD symptoms in the IPV-DSH relation. Participants were 197 African American community women currently experiencing IPV. Sixty participants (31%) reported a history of DSH. Among participants who reported DSH, there was an average endorsement of 2.3 unique forms of deliberate self-harm (i.e., DSH versatility). Significant positive associations were detected among physical IPV severity, psychological IPV severity, PTSD symptom severity, and DSH history and versatility. PTSD symptom severity mediated the relationships between physical and psychological IPV severity and DSH history and versatility. Results highlight the relevance of PTSD symptoms to DSH and suggest that treatments targeting PTSD symptoms may be useful in reducing DSH among IPV-exposed African American women.

Abstract: © 2015 Elsevier Ltd. Many species are undergoing rapid population declines, while other species have increased. Previous work has related population change to species traits to elucidate the drivers of population change. However, this assumes that these drivers operate consistently across habitats. We use national-scale monitoring data on UK bird abundance from 1994-2012 to calculate habitat-specific population trends, allowing us to evaluate whether the effect of species traits was consistent between habitats. Although we found significant interactions with habitat for traits relating to migratory behaviour, diet, nest site and habitat specialisation, the direction of these trait effects was generally consistent between habitats. This suggests that large-scale processes operating across habitats are responsible for many changes in bird populations, although processes operating within habitats can modulate the effect of these drivers. Despite this, differences in population trends between habitats remain when variation in population trends due to species identity is accounted for, indicating that some habitat effects do occur. By identifying the scale at which drivers of population change operate, it is possible to target conservation actions more directly. Population declines were most evident in woodland and urban habitats, and we suggest that these habitats should be the focus of increased research and conservation effort if declines evident in many bird species are to be reversed.

Abstract: Large-eddy simulation (LES) is a numerical technique for integrating spatially filtered equations of motion that describe high-Reynolds number time-evolving, three-dimensional turbulence. The spatial filtering cuts off the high frequency or small-scale part of the turbulence spectrum. Virtual turbulence generated by LES has been shown to be a surrogate for measurements of turbulent flow fields. LES is widely used for turbulence research and for applications. This article describes the LES technique, and reviews its contributions to and use in atmospheric sciences.

Abstract: This chapter presents the wildlife toxicity assessment (WTA) for pyrene that is formed from the incomplete combustion of organic chemicals. This WTA primarily addresses pyrene's toxicity to mammals, as data for amphibians and birds were limited, and data for reptiles were lacking. No oral acute, chronic, or reproductive studies were available. A single oral subchronic (13-week) study indicated limited toxicity of pyrene to mammals. The principal effect was kidney toxicity. In the developmental and reproduction studies using intraperitoneal injections, no adverse effects were observed at the doses tested. Pyrene shows a low potential for mutagenicity and carcinogenicity. Based on these limited data, the NOAEL-based TRVs for mammals for pyrene were estimated to be NOAEL of 6.25 mg/kg/day and LOAEL of 31.25 mg/kg/day with medium confidence. The limited data for birds and amphibians failed to provide adequate information with which to establish a NOAEL or LOAEL.

Abstract: Because of the linked nature of stream-riparian ecosystems, the highly adapted organisms and food webs that inhabit them, and the disproportionate contribution of natural resources from stream-riparian areas, the role of wildfire in these ecosystems may be essential to managing biodiversity and conservation across landscapes. The diverse array of organisms that inhabit stream-riparian ecosystems respond to wildfire across a gradient of fire severity. In addition, time since fire and annual timing of fires can interact with other sources of disturbance and life-history events to influence stream-riparian systems across levels of ecological organization. The degree of impact wildfire has also depends on spatial aspects, including extent and patchiness, as well as interactions with anthropogenic disturbance. For example, if characteristic stream-riparian continuity is disrupted or other natural features are impaired or lost as a result of human activities, detrimental effects can occur. In general, stream-riparian organisms are highly adapted to natural disturbance processes, and the occurrence of dynamic, mixed-severity fire regimes may be necessary to maintain ecological integrity and native biodiversity.

Abstract: Turbulent flows are ubiquitous, and as manifestations of one of the last outstanding unsolved problems of classical physics, they form today the focus of numerous investigations. In view of the very large number of modes that are excited, a variety of modeling techniques can be used in conjunction with state of the art numerical methods. A few of the issues that need to be addressed by models of turbulence, such as the presence of strong localized structures, the degree of nonlocality of nonlinear interactions, the slow return to isotropy and homogeneity, and the interactions between eddies and waves, are reviewed here; all implicate a large number of scales in interactions. Two specific modeling examples are given, one for waves and eddies in oceanic flows, and one for the generation of magnetic fields in planetary and stellar bodies, both using variants of Lagrangian-averaged methods. Finally, it is also argued that in order to understand geophysical turbulence, there is a strong need for combining modeling methods and sophisticated numerical techniques, such as high-accuracy adaptive mesh refinement.

Abstract: Publisher Summary Advances in the understanding of the molecular biology and pharmacology of nicotinic receptors (nAChRs) may provide targets for the development of novel and selective modulators of nAChRs in the brain. This contention is supported by the dissimilar behavioral effects observed following systemic administration of currently available nicotinic ligands. This chapter attempts to describe the biochemical and behavioral effects of (−)-nicotine, the classic Iigands [(−)-lobeline, (−)-cytisine, etc.], and the novel cholinergic channel modulators (ChCMs) [ABT-418, (±epibatidine, and GTS-21] in view of the latest molecular biological studies demonstrating the presence of multiple subunits comprising the nAChRs in the brain, ganglia, and neuromuscular junction. Some of these compounds selectively activate or block a particular nAChR subtype, and they will eventually help to define the potential physiological role of the nAChRs in the peripheral and central nervous systems. Increasing evidence suggests that the positive and negative effects of (−)-nicotine are mediated through its binding to the different subtypes in the brain and in the periphery. Based on this premise, ChCMs could selectively enhance central nAChR-mediated transmission without having the side-effect liabilities normally associated with (−)-nicotine (cardiovascular, gastrointestinal, abuse potential, and others). Identification of these ChCMs may represent a potential therapeutic strategy to ameliorate many of the CNS deficits accompanying Alzheimer's disease (AD) or other related disorders, as discussed in the chapter.

Abstract: Background: Neurobiological models of posttraumatic stress disorder (PTSD) implicate fear processing impairments in the maintenance of the disorder. Specific deficits in extinction recall, the retention of learned extinction, have been demonstrated. While deficient extinction recall, and the associated activation pattern of prefrontal and hippocampal regions, distinguishes individuals with PTSD from controls, research has not yet examined changes following treatment. We examined the behavioral and neural correlates of extinction recall before and after cognitive behavioral treatment of PTSD. Methods: Fifty-eight participants (30 with PTSD, 28 trauma-exposed matched controls) underwent a 2-day behavioral fear conditioning, extinction, and recall paradigm during functional magnetic resonance imaging (fMRI). The same procedures were repeated 10 weeks later, after PTSD patients had completed prolonged exposure treatment. We analyzed fMRI data from 32 subjects (16 PTSD; 16 controls) and skin conductance response (SCR) data from 33 subjects (16 PTSD; 17 controls). Neural activity during extinction recall, SCR, and PTSD symptoms were compared across groups and over time. Results: PTSD patients exhibited pre- to post-treatment reduction in rostral anterior cingulate cortex (rACC) activation during extinction recall, and increase in functional coherence between the rACC and the ventromedial prefrontal cortex (vmPFC) and subgenual anterior cingulate cortex (sgACC). Reduced PTSD symptom severity from pre- to post-treatment was significantly associated with reduced subgenual ACC and parahippocampal activation during this task. SCR during the extinction recall phase did not significantly change with treatment in the PTSD group, but change in SCR was associated with reduction in PTSD symptom severity. Conclusions: Prolonged exposure treatment appears to alter neural activation in PTSD patients during recall of fear extinction, and change in extinction recall (measured by SCR) is associated with symptom reduction. We discuss results in the context of neural systems involved in response to affective stimuli.

22. Velopharyngeal insufficiency after cleft palate repair in patients with isolated Robin sequence versus isolated cleft palate: A systematic review

Abstract: Background: Robin sequence (RS) is characterized by micrognathia, glossoptosis, and upper airway obstruction, and is often combined with a cleft palate. It is unclear whether RS negatively impacts the development of velopharyngeal incompetence (VPI) and attainable speech outcomes. This study systematically reviewed speech outcomes in patients with cleft and isolated RS (IRS) compared with only isolated cleft palate (ICP).Methods: A literature search following the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines was performed using the PubMed and EMBASE databases. Articles reporting speech outcomes following primary palatoplasty in patients with IRS only or IRS versus ICP were identified. Study characteristics and methods, primary- and VPI palatoplasty, speech measurements, and post-operative complications were collected. Primary outcomes included VPI and need for speech correcting surgery (SCS). Methodological quality was appraised using the methodological index for non-randomized studies (MINORS) criteria (range: 0-16 and 0-24).Results: Nineteen studies reported VPI event rates that varied between 14% and 88% for IRS and 0% and 62% for ICP. Five out of 8 studies (67%) comparing VPI event rates between IRS and ICP found no significant difference. SCS rates varied between 0% and 48% for IRS and 0% and 24% for ICP. Six out of 9 studies (67%) comparing SCS rates between IRS and ICP, found no significant difference. Combined VPI event rates were 36.1% for the IRS group and 26% for the ICP group, for SCS rates this was 20% for IRS and 13% for ICP.Conclusion: Most articles found no significant difference between the VPI and SCS rates indicating that speech outcomes might be similar in patients with IRS and ICP. To better compare these groups a standardized international protocol is needed.

23. Characteristics and Habits of Psychiatrists and Neurologists With High Occupational Well-Being: A Mixed Methods Study

Abstract: Objective: To identify the characteristics that distinguish occupationally well outliers (OWO), a subset of academic psychiatrists and neurologists with consistently high professional fulfillment and low burnout, from their counterparts with lower levels of occupational well-being. Participants and Methods: Participants included faculty physicians practicing psychiatry and neurology in academic medical centers affiliated with the Professional Well-being Academic Consortium. In this prospective, longitudinal study, a mixed qualitative and quantitative approach was used. Quantitative measures were administered to physicians in a longitudinal occupational well-being survey sponsored by the academic organizations where they work. Four organizations participated in the qualitative study. Psychiatrists and neurologists at these organizations who competed survey measures at 2 consecutive time points between 2019 and 2021 were invited to participate in an interview. Results: Of 410 (213 psychiatrists and 197 neurologists) who completed professional fulfillment and burnout measures at 2 time points, 84 (20.5%) met OWO criteria. Occupationally well outliers psychiatrists and neurologists had more favorable scores on hypothesized determinants of well-being (values alignment, perceived gratitude, supportive leadership, peer support, and control of schedule). Ultimately, 31 psychiatrists (25% of 124 invited) and 33 neurologists (18.5% of 178 invited) agreed to participate in an interview. Qualitatively, OWO physicians differed from all others in 3 thematic domains: development of life grounded in priorities, ability to shape day-to-day work context, and professional relationships that provide joy and support. Conclusion: A multilevel approach is necessary to promote optimal occupational well-being, targeting individual-level factors, organizational-level factors, and broader system-level factors.

Abstract: This Letter presents a differential cross-section measurement of Lund subjet multiplicities, suitable for testing current and future parton shower Monte Carlo algorithms. This measurement is made in dijet events in 140 fb−1 of s=13 TeV proton–proton collision data collected with the ATLAS detector at CERN's Large Hadron Collider. The data are unfolded to account for acceptance and detector-related effects, and are then compared with several Monte Carlo models and to recent resummed analytical calculations. The experimental precision achieved in the measurement allows tests of higher-order effects in QCD predictions. Most predictions fail to accurately describe the measured data, particularly at large values of jet transverse momentum accessible at the Large Hadron Collider, indicating the measurement's utility as an input to future parton shower developments and other studies probing fundamental properties of QCD and the production of hadronic final states up to the TeV-scale.

Abstract: The HEALthy Brain and Child Development (HBCD) Study, a multi-site prospective longitudinal cohort study, will examine human brain, cognitive, behavioral, social, and emotional development beginning prenatally and planned through early childhood. The longitudinal collection of biological samples from over 7000 birthing parents and their children within the HBCD study enables research on pre- and postnatal exposures (e.g., substance use, toxicants, nutrition), and biological processes (e.g., genetics, epigenetic signatures, proteins, metabolites) on neurobehavioral developmental outcomes. The following biosamples are collected from the birthing parent: 1) blood (i.e., whole blood, serum, plasma, buffy coat, and dried blood spots) during pregnancy, 2) nail clippings during pregnancy and one month postpartum, 3) urine during pregnancy, and 4) saliva during pregnancy and at in-person postnatal assessments. The following samples are collected from the child at in-person study assessments: 1) saliva, 2) stool, and 3) urine. Additionally, placenta tissue, cord blood, and cord tissue are collected by a subset of HBCD sites. Here, we describe the rationale for the collection of these biospecimens, their current and potential future uses, the collection protocol, and collection success rates during piloting. This information will assist research teams in the planning of future studies utilizing this collection of biological samples.

27. Perceptions around occupational mental well-being of community health workers and an intervention package for its promotion: A mixed-methods study in rural Chiapas, Mexico

Abstract: Background: The challenging working conditions experienced by community health workers (CHWs) have an impact on their mental health, as detected by the NGO Compañeros En Salud (CES) in rural Mexico. In response to this situation, CES designed through a participatory process a package of interventions to promote the mental well-being of CHWs, beginning implementation in 2021. The objective of the present study was to learn how CES CHWs' work affects their mental well-being and to evaluate the intervention package to promote CHWs’ mental well-being implemented by CES. Methods: In June–August 2023, 52 CHWs from the CES-supported communities participated in the study, responding to a survey and participating in 10 focus group discussions. Quantitative data were analyzed using statistical descriptive analysis and qualitative data using thematic analysis. Findings: Participants highlighted the impact on their communities as one of the main aspects of their job that contribute positively to their mental well-being, as well as the challenging work-life balance as one of the main aspects that contribute negatively. As for the interventions, most participants considered them significant and positive for their mental well-being, highlighting positive aspects such as the possibility of creating community with their peers or a feeling of self-efficacy. However, the access to interventions was uneven among participants and most interventions presented areas for improvement, such as the periodicity of psychological distress screening or the response time to material needs. Conclusions: Efforts to support CHW well-being in the areas they signal as needs can impact their experiences around work and their perceived well-being. Access to work materials, preparedness on clinical topics, and relationships with their teams are key areas that may have a bearing on CHWs' emotional and mental well-being. Interventions aimed at these areas can positively impact CHWs’ self-efficacy, their community with each other, and their interactions with patients.

Abstract: Gulf War Illness (GWI) is a disorder experienced by many veterans of the 1991 Gulf War, with symptoms including fatigue, chronic pain, respiratory and memory problems. Exposure to toxic chemicals during the war, such as oil well fire smoke, pesticides, physiological stress, and nerve agents, is thought to have triggered abnormal neuroinflammatory responses that contribute to GWI. Previous studies have examined the acute effects of combined physiological stress and chemical exposures using GWI rodent models and presented findings related to neuroinflammation and changes in diffusion magnetic resonance imaging (MRI) measures, suggesting a neuroimmune basis for GWI. In the current study, using ex vivo MRI, cytokine mRNA expression, and immunohistological analyses of brain tissues, we examined the brain structure and immune function of a chronic rat model of GWI. Our data showed that a combination of long-term corticosterone treatment (to mimic high physiological stress) and diisopropyl fluorophosphate exposure (to mimic sarin exposure) primed the response to subsequent systemic immune challenge with lipopolysaccharide resulting in elevations of multiple cytokine mRNAs, an increased activated glial population, and disrupted brain microstructure in the cingulate cortex and hippocampus compared to control groups. Our findings support the critical role of neuroinflammation, dysregulated glial activation, and their relationship to disrupted brain microstructural integrity in the pathophysiology of GWI and highlight the unique consequences of long-term combined exposures on brain biochemistry and structural connectivity.

Abstract: Collaborative project-based learning is one pedagogical approach which has the potential to play a meaningful role in synchronous online teaching but its use at second level, or high school, is under-explored. This paper describes the use, in an online setting, of the Bridge21 pedagogical model for collaborative project-based learning with secondary school students. It compares the experiences of a group (N = 119) of students (aged 15–16) who, in an Emergency Remote Teaching offered during Covid, experienced the model in an online, synchronous modality with that of a group (N = 253) who participated face-to-face in the following year.Two related research questions are addressed. (RQ1) Are the Bridge to College collaborative project-based learning workshops as effective, in terms of enhancing secondary school student confidence in Transversal Skills, when delivered in a synchronous online modality as they were when delivered face-to-face? (RQ2) If so, then what are the elements of the workshops which contribute to their effectiveness?Data on improvement in student confidence in Transversal Skills show that the online experience was on a par with face-to-face offerings. Student feedback suggests that the focus on collaborative work, and the structured approach to PBL, were key factors in the success of the model in the online setting. The modifications needed to adapt the model for use online were minimal. Thus, the contribution of the paper is to provide a model for synchronous online learning, for secondary schools, which preserves the benefits arising from face-to-face collaborative PBL.

30. Influence of the presence of RuO2 on the reactivity of Fe2O3 in the artificial photosynthesis reaction

Abstract: Surface Plasmonic Resonance (SPR) is the oscillation of free electrons on the surface of a metal or metallic particle upon irradiation with light of a certain frequency. The incorporation of Fe2O3 (a H2O oxidising photocatalyst) with plasmonic RuO2 nanoparticles to form a composite is studied. XRD results show that RuO2 is formed in the rutile phase while Fe2O3 is rhombohedral and also suggests doping of the Fe2O3 lattice with Ru atoms in the composite. UV-Vis spectroscopy shows that RuO2 exhibits a plasmon peak at 511 nm, and CO2-TPD experiments show that RuO2 adsorbs and desorbs CO2. TEM also shows that the RuO2 particles are spherical, as are the Fe2O3 particles with some irregular polyhedra present, too. The composite is a mixture of these two morphologies. The effect of composite formation on the activity of the materials in the artificial photosynthesis reaction is dramatic. Neither RuO2 nor Fe2O3 alone produce significant quantities of gaseous CH4 or CO products. However, the composite material produces both (as well as generating levels of unidentified adsorbed hydrocarbonaceous species). This reactivity is ascribed to the generation of a heterojunction in the composite material. It is suggested that the generation of holes in Fe2O3 is used to provide protons (from H2O oxidation), and the decay of an SPR response on RuO2 provides hot electrons, which together with the protons reduce CO2 to produce CH4, CO and adsorbed hydrocarbonaceous species.

Abstract: This paper illustrates a novel hybrid approach to analysis which offers unique insights into the experience of postnatal depression in British Pakistani-Muslim women, through the prioritising of voice. To do so, we combine thematic and dialogical analysis, using data from a study on motherhood and postnatal depression. Participants were part of the birth cohort ‘Born in Bradford's Better Start’. The rationale for developing a hybridised approach is discussed. The ensuing analyses highlight the role of internal voices and chronotope, which give a detailed insight into women's meaning making of psychological distress and motherhood during the postnatal period. We go onto discuss the strength and limitations of hybridising thematic-dialogical analysis in the context of health research.

Abstract: Background: A developing literature suggests that transcranial magnetic stimulation (TMS) can target anhedonia and suicidal ideation (SI), core symptoms of treatment-resistant depression (TRD). This present naturalistic study extends the existing literature by investigating the connection between changes in anhedonia and suicidal ideation (SI) related to transcranial magnetic stimulation (TMS), independent of any overall changes in depression. Methods: Pre and post treatment PHQ-9 and IDS-SR data were collected from 181 TRD patients who received dorsolateral prefrontal cortex ( dlPFC) TMS using the Figure-8 or H1-coil. Changes in overall depression symptoms, anhedonia, and SI were analyzed using chi square tests, repeated measure ANOVAS, and linear regression for repeated measures. Results: TMS yielded changes in overall depression symptoms (PHQ-9 Cohen’s d = 1.02; IDS-SR Cohen’s d = 1.05), with 23.9 % and 41.7 % of patients experiencing response as measured by IDS-SR and PHQ-9, respectively. TMS treatment was also associated with large changes in both anhedonia (d = 1.03) and SI (d = 0.88), which were similar in magnitude to changes in all other depression symptoms (d = 0.97). Importantly, changes in anhedonia predicted changes in SI, even after controlling for baseline depression severity and change in other depression symptoms. Limitations: The lack of a control arm and a neuroimaging measure temper mechanistic conclusion. Conclusion: Our results reinforce the effectiveness of TMS in TRD and provide new evidence that anhedonia and SI may belong to a broader symptom cluster potentially undergirded by a shared circuitry accessible to dlPFC TMS.

34. Reducing Postoperative Opioids in Pediatric Orthopaedics: An Updated Tiered Prescription Guideline with a 'Tier 0'

Abstract: Background: There is growing data on strategies to reduce opioid prescriptions in pediatric orthopaedics. Our institution implemented tiered guidelines for opioid discharge prescriptions in 2018 to standardize prescribing patterns across our entire pediatric orthopaedics practice. In 2022, we updated the tiered system with a goal to further minimize the quantity of opioids prescribed and eliminate opioids altogether for certain procedures. The aim of this project was to evaluate the effects of the updated tiered guidelines for discharge opioid prescribing following common pediatric orthopaedic procedures. Methods: A quality improvement project was conducted at a single academic institution. Guidelines for discharge opioid prescriptions were updated in January 2022 and established 5 tiers of increasing invasiveness for 28 procedures. The updated guidelines included a “Tier 0” which provided no opioids, reorganized surgical tiers based on prior data, and decreased the doses recommended for tiers. Opioid prescriptions were reported as oral morphine equivalents (OMEs). Univariate tests were performed to assess statistically significant differences before and after implementation of updated guideline eras, comparing 2020–2021 and 2022. Results: Overall, 747 patients (2020–2021, n = 500; 2022, n = 247) were included. The proportion of patients receiving discharge opioid prescriptions decreased from 89.8% to 80.6% (P 10 years (135 vs 113, P 0.05). Discharge prescriptions falling within overall guidelines increased after guideline implementation (83.3% vs 85.4%), though this was not statistically significant and guideline adherence was low among Tier 0 at 31%. Conclusions: Including “Tier 0” of procedures receiving no opioid prescriptions, reorganizing procedures within tiers, and decreasing the recommended doses to an already strict tiered guideline for discharge opioid prescriptions significantly decreased the quantity of opioids prescribed. We noted continued excellent overall adherence and, importantly, no overall increase in refills, with areas for improvement in Tier 0. Quality improvement projects can continue to reduce the amount of opioids prescribed following pediatric orthopaedic procedures. Key Concepts: (1) Updated tiered guidelines, including a “Tier 0” for discharge opioid prescriptions following pediatric orthopaedic procedures were effective in reducing the quantity of opioids provided at discharge. (2) A continued high adherence rate was observed with the updated guidelines, though areas for improvement include adherence to no opioids in Tier 0. (3) Refill rates remained similar after implementation of the updated guidelines. Abstract: Summary: Background: The COVID-19 global pandemic placed unprecedented pressure on cancer services, requiring new interim Systemic Anti-Cancer Treatments (SACT) options to mitigate risks to patients and maintain cancer services. In this study we analyse interim COVID-19 SACT therapy options recommended in England, evaluating the evidence supporting inclusion and delineating how these have been integrated into routine cancer care. Methods: We performed a retrospective analysis of interim Systemic Anti-Cancer Treatments endorsed by NHS England during the COVID-19 pandemic. Interim therapy options were compared to baseline (replacement) therapies by comparing data from the key pivotal trial(s) in terms of clinical efficacy and potential benefits (e.g., reduced immunosuppression or improved adverse effect profile) within the context of the pandemic. Furthermore, we evaluated the evolution of these interim SACT options, exploring if these have been integrated into current treatment pathways or are no longer accessible at the pandemic end. Findings: 31 interim oncology treatment options, across 36 indications, for solid cancers were endorsed by NHS England between March 2020 and August 2021. Interim therapies focused on the metastatic setting (83%; 30/36), allowing greater utilisation of immune checkpoint inhibitors (45%; 14/31) and targeted therapies (26%; 8/31), in place of cytotoxic chemotherapy. Overall, 36% (13/36) of therapies could not have efficacy compared with baseline treatments due to a paucity of evidence. For those which could, 39% (9/23) had superior efficacy (e.g., overall survival), 26% (6/23) had equivocal efficacy and 35% (8/23) lower efficacy. 53% (19/36) of interim therapies had better or equivocal toxicity profiles (when assessable), and/or were associated with reduced immunosuppression. Almost half (47%; 17/36) of interim therapies did not have UK market authorisation, being classified as ‘off label’ use. Analysing access to interim options at the end of the pandemic (May 2023) identified 19 (53% 19/36) interim options were fully available, and a further four (11% 4/36) therapies were partially available. Interpretation: Interim SACT options, introduced in England, across a range of solid cancers supported delivery of cancer services during the pandemic. Most interim therapies did not demonstrate superior efficacy, but provided other important benefits (e.g., reduced immunosuppression) in the context of the pandemic. Funding: None.

36. Effects of low-density development on stream biota: Evidence for biotic homogenization from an assemblage perspective

Abstract: Land use/land cover change from forested to developed land is a major threat to freshwater biodiversity globally. However, existing research has focused on high-density development in urban centers. We know less about how low-density development affects stream biodiversity, even though increases in low-density development have been documented across the globe and may be tied to the homogenization of stream biotic assemblages. Here, we investigated the diversity (alpha and beta) and assemblage composition of algal diatoms and fish over a ten-year period at three points in time (2000, 2005, 2010) in forested watersheds (n = 4) and watersheds experiencing increasing low-density development (“developing watersheds”; n = 4) in the southern Appalachian Mountains of western North Carolina, USA. We employed a repeated snapshot sampling approach for the data collection. In developing watersheds, we observed higher alpha diversity and different species assemblages for both diatoms and fishes compared to forested watersheds. These differences were attributed to the establishment and/or higher abundances of cosmopolitan diatoms (e.g., Achnanthidium rivulare Potapova and Ponader, Navicula spp., Nitzschia spp.) as well as fishes (e.g., Creek Chub [Semotilus atromaculatus] and Blacknose Dace [Rhinichthys atratulus]). Most endemic species persisted in developing reaches, but in lower abundances (e.g., diatoms: Meridion alansmithii Brant; fishes: Mottled Sculpin [Cottus bairdi]). Diatom beta diversity within developing reaches was lower compared to forested reaches, and lower in 2005 and 2010 compared to 2000. Diatom assemblage composition also changed over time in developing reaches but remained stable in forested reaches. In contrast, fish beta diversity showed no significant differences between watershed types or among years, suggesting that diatom assemblages may respond more quickly or to lower levels of watershed development than fish assemblages. We conclude that biotic assemblages in streams draining developing watersheds in southern Appalachia show evidence of homogenization, but not yet extirpation of endemic taxa. Our study demonstrates the importance of understanding the effects of low-density development on biodiversity in stream ecosystems around the world before native species are lost.

Abstract: A search for Higgs boson pair production via vector-boson fusion is performed in the Lorentz-boosted regime, where a Higgs boson candidate is reconstructed as a single large-radius jet, using 140 fb−1 of proton–proton collision data at s=13 TeV recorded by the ATLAS detector at the Large Hadron Collider. Only Higgs boson decays into bottom quark pairs are considered. The search is particularly sensitive to the quartic coupling between two vector bosons and two Higgs bosons relative to its Standard Model prediction, κ2V. This study constrains κ2V to 0.55

Abstract: We present the first case of a dupilumab-induced hyperinflammatory state in the setting of underlying eosinophilic esophagitis characterized by multisystem granulomatous inflammation. Although clinical trial data and subsequent real-world experience support dupilumab as a highly effective therapy for eosinophilic esophagitis, close monitoring for development of adverse symptoms following initiation remains paramount.

Methods: In a retrospective cohort study, administrative data from a large managed care organization was used to identify patients with confirmed SM, based on World Health Organization criteria. Demographic data, delay to diagnosis, disease progression, and health care resource utilization were examined. Results: Of 116 patients with confirmed SM, 77% had indolent SM, 2% had smoldering SM, 12% had SM with associated hematologic neoplasm, 9% had aggressive SM, and none had mast cell leukemia. In all, 5 patients were misclassified as having a less advanced SM subtype initially and 3 were completely undiagnosed (missed diagnosis). The average delay to diagnosis of SM was 58.3 plus or minus 73.1 months. In all, 18% of patients progressed from a nonadvanced form of SM (indolent or smoldering SM) to an advanced form of SM (aggressive SM, SM with associated hematologic neoplasm, or mast cell leukemia) over an average of 88.3 plus or minus 82.7 months. Patients with SM had increased health care utilization, including increases in their numbers of hospital admissions, emergency room visits, urgent care visits, and specialty provider visits, after diagnosis versus before. Conclusions: Rare diseases such as SM would benefit from increased understanding and awareness to improve diagnostic accuracy. Prospective studies are needed to better characterize this patient population and determine the type of follow-up needed to recognize advanced forms of SM so that appropriate treatment can be implemented.

40. Anterior and posterior thalamic volumes differentially correlate with memory, attention, and motor processes in HIV infection and alcohol use disorder comorbidity

Abstract: The thalamus, with its reciprocal connections to and from cortical, subcortical, and cerebellar regions, is a central active participant in multiple functional brain networks. Structural MRI studies measuring the entire thalamus without respect to its regional or nuclear divisions report volume shrinkage in diseases including HIV infection, alcohol use disorder (AUD), and their comorbidity (HIV+AUD). Here, we examined relations between thalamic subregions (anterior, ventral, medial, and posterior) and neuropsychological functions (attention/working memory, executive functioning, episodic memory, and motor skills). Volumes of thalamic subregions were derived from automatic segmentations of standard T1 weighted MRIs of 65 individuals with HIV, 189 with AUD, 80 with HIV+AUD comorbidity, and 141 healthy controls (CTRL). Total thalamic volume was smaller and cognitive and motor composite scores were lower in the three diagnostic groups relative to the CTRL group. The AUD and HIV+AUD groups had significantly smaller thalamic subregional volumes than the CTRL group. The HIV+AUD group had smaller anterior thalamic volume than the HIV-only group and smaller ventral thalamic volume than the AUD-only group. In the HIV+AUD group, memory scores correlated with anterior thalamic volumes, attention/working memory scores correlated with posterior and medial thalamic volumes, and motor skill scores correlated with posterior thalamic volumes. Exploratory analyses focused on the HIV+AUD group indicated that within the posterior thalamic region, the pulvinar and medial geniculate nuclei were related to attention/working memory scores, and the pulvinar was related to motor skills scores. This study is novel in locating volume deficits in specific thalamic subregions, in addition to the thalamus as a whole, in HIV, AUD, and their comorbidity and in identifying functional ramifications of these deficits. Taken together, this study highlights the relevance of thalamic subregional volume deficits to dissociable cognitive and motor processes.

41. Multi-dimensional predictors of first drinking initiation and regular drinking onset in adolescence: A prospective longitudinal study

Abstract: Early adolescent drinking onset is linked to myriad negative consequences. Using the National Consortium on Alcohol and NeuroDevelopment in Adolescence (NCANDA) baseline to year 8 data, this study (1) leveraged best subsets selection and Cox Proportional Hazards regressions to identify the most robust predictors of adolescent first and regular drinking onset, and (2) examined the clinical utility of drinking onset in forecasting later binge drinking and withdrawal effects. Baseline predictors included youth psychodevelopmental characteristics, cognition, brain structure, family, peer, and neighborhood domains. Participants (N=538) were alcohol-naïve at baseline. The strongest predictors of first and regular drinking onset were positive alcohol expectancies (Hazard Ratios [HRs]=1.67–1.87), easy home alcohol access (HRs=1.62–1.67), more parental solicitation (e.g., inquiring about activities; HRs=1.72–1.76), and less parental control and knowledge (HRs=.72–.73). Robust linear regressions showed earlier first and regular drinking onset predicted earlier transition into binge and regular binge drinking (βs=0.57–0.95). Zero-inflated Poisson regressions revealed that delayed first and regular drinking increased the likelihood (Incidence Rate Ratios [IRR]=1.62 and IRR=1.29, respectively) of never experiencing withdrawal. Findings identified behavioral and environmental factors predicting temporal paths to youthful drinking, dissociated first from regular drinking initiation, and revealed adverse sequelae of younger drinking initiation, supporting efforts to delay drinking onset.

Abstract: The freshwater pearl mussel, Margaritifera margaritifera is widely considered to be confined to fast-flowing, near-pristine rivers, and is highly endangered throughout its Holartic range. Here we describe a large population of some 150–200,000 individuals of the freshwater pearl mussel living in an oligotrophic Irish lake.Employing underwater photographic techniques and snorkel diving, mussels were found in two separate locations in the lake. In the first location, 200 m from the shore, mussels were found at a maximum density of 43 m−2, in a largely random distribution in the sandy bed substrate of the lake, up to 9.5 m deep. In the second shoreline location, mussels were observed in dense aggregations up to 118 individuals m−2, in sandy pockets in between boulders and bedrock, in waters 1–2 m deep. In both locations, mussels were found associated with macrophytes – the charophyte Nitella sp. in the deeper location and the quillwort Isoetes lacustris in the shallower shoreline location. Juvenile mussels (>1.5 cm length) were observed lying exposed on the sediment surface in the shallow littoral location, indicating that recruitment occurs in this population, although no estimates of juvenile density were obtained.The discovery of a large population of lentic Margaritifera margaritifera has significant implications for the species’ conservation and indicates that there may be substantial lentic populations of the species elsewhere. The environmental envelope of the species is clearly much wider than thought and our discovery challenges assumptions about the fundamental feeding and reproductive biology of this endangered species.

Abstract: Background: : Biobehavioral interventions including relaxation, distraction, and mindfulness meditation exercises have been shown to decrease perioperative stress, anxiety, and pain. Our aims were to 1) quantify pediatric surgical patient-reported pre-operative exposure to and post-operative use of biobehavioral interventions; 2) understand barriers and facilitators to incorporation of biobehavioral interventions into clinical practice; and 3) evaluate associated patient-reported outcomes. Methods: : We conducted an embedded mixed-methods study with a quantitative focus. Data were collected as part of the 18-hospital ENhanced Recovery In CHildren Undergoing Surgery (ENRICH-US) clinical trial for children, ages 10–18 years, undergoing elective gastrointestinal surgery. Patients/caregivers were surveyed about preoperative exposure to and postoperative use of biobehavioral interventions. Four semi-structured group interviews with 20 pediatric surgery providers were conducted. Outcomes included pain-related functional disability, health-related quality of life (HRQoL), and perioperative nervousness. Results: : 41 % (n = 67) of 164 enrolled patients/caregivers reported preoperative exposure to and 71 % (n = 117) reported postoperative use of a biobehavioral intervention(s). Barriers to incorporation of biobehavioral interventions included lack of standardized workflows, clinician knowledge, and resources. Potential facilitators included media and peer-counseling. After adjusting for individual and hospital characteristics, those who reported using a biobehavioral intervention(s) were 70 % less likely to report worsened postoperative nervousness (95 % CI 0.10–0.91; p = 0.03). Reported use of a biobehavioral intervention(s) was not found to be associated with pain-related functional disability or HRQoL. Conclusions: : Use of a biobehavioral intervention(s) may stabilize postoperative nervousness of children undergoing surgery. There is a need for redesign of clinical workflows and clinician training to facilitate integration of biobehavioral interventions.

The growing understanding of the role of MHC antigens in transplantation and immune system recognition has resulted in considerable interest in the functional and structural characteristics of these cell-surface glycoproteins. Monoclonal antibody affinity chromatography has proved to be a rapid and efficient means of purifying relatively large amounts of both class I and class II murine MHC antigens. There is a successfully use of four monoclonal antibodies as affinity reagents. The same general approach is applicable to the purification of a variety of MHC antigens using different monoclonal antibodies. The antigens purified in this way retain serological and biological activity and prove to be useful for studying the structure and function of these molecules.

Abstract: Publisher Summary Heparin-binding growth factors have been purified from tumors as well as normal tissue such as brain, pituitary, hypothalamus, retina, and cartilage. This chapter analyzes most of the endothelial cell growth factors by heparin affinity chromatography. Two assays are used to monitor growth factor activity. The growth factors described in this chapter stimulate both DNA synthesis in 3T3 cells and the proliferation of capillary endothelial (CE) cells. Measurement of the stimulation of DNA synthesis in 3T3 cells is a relatively nonspecific assay that will detect many growth factors such as fibroblast growth factor (FGF), platelet-derived growth factor (PDGF), and epidermal growth factor (EGF). This assay is relatively simple and rapid and allows screening of many samples such as is generated by column chromatography. The CE cell proliferation assay is much more specific—for example, PDGF and EGF do not stimulate CE cell proliferation.

Abstract: Publisher Summary This chapter dicsusses the general features of scientific inference and explains the way inference has been discussed by archaeologists. It presents an explication of current problems in the various areas: (1) the justification of archaeological conclusions, (2) the concepts of archaeological evidence, and (3) the undeveloped relations between archaeological theory and archaeological evidence. The chapter defines these problems and explains the way to resolve them. To resolve many of the problems surrounding uncertain links between inference and evidence, archaeologists should develop a rigorous model that specifies the way information about the past is transmitted to the present via material remains. The chapter presents the structure of such a model. However, establishing relevant data to support conclusions about past phenomena is only part of the overall process of generating archaeological knowledge. Some of the processes responsible for producing the material remains that archaeologists manipulate in assembling evidence to support claims about past phenomena are described in the chapter.

Abstract: Publisher Summary This chapter discusses the purification of cartilage-derived growth factors (CDGFs) from a variety of cartilage sources by using affinity chromatography on heparin-Sepharose. The chapter discusses the extraction procedure of CDGF from cartilage. CDGF has a marked affinity for heparin. As a result, CDGF enriched by cation-exchange chromatography on Bio-Rex 70 can be purified to homogeneity by affinity chromatography on immobilized heparin. An alternative to the two-step purification scheme is the method of recycling CDGF on heparin-Sepharose two to three times. The strong affinity of CDGF to heparin makes it possible to produce a highly purified preparation of CDGF by using repeated chromatography on columns of heparin-Sepharose alone without previous cation-exchange chromatography.

Abstract: Background: Anti-Program-Death-1 (PD-1) is a standard adjuvant therapy for patients with resected melanoma. We hypothesized that there are discrepancies in survival, recurrence pattern and toxicity to adjuvant PD-1 between different ethnicities and melanoma subtypes. Objective: We performed a multicenter cohort study incorporating 6 independent institutions in Australia, China, Japan, and the United States. The primary outcomes were recurrence free survival (RFS) and overall survival (OS). Secondary outcomes were disease recurrence patterns and toxicities. Results: In total 534 patients were included. East-Asian/Hispanic/African reported significantly poorer RFS/OS. Nonacral cutaneous or melanoma of unknown primary reported the best RFS/OS, followed by acral, and mucosal was the poorest. Within the nonacral cutaneous or melanoma of unknown primary subtypes, East-Asian/Hispanic/African reported significantly poorer RFS/OS than Caucasian. In the multivariate analysis incorporating ethnicity/melanoma-subtype/age/sex/stage/lactate dehydrogenase/BRAF (v-Raf murine sarcoma viral oncogene homolog B)-mutation/adjuvant radiotherapy, East-Asian/Hispanic/African had independently significantly poorer outcomes (RFS: HR, 1.71; 95% CI, 1.19-2.44 and OS: HR, 2.34; 95% CI, 1.39-3.95), as was mucosal subtype (RFS: HR, 3.25; 95% CI, 2.04-5.17 and OS: HR, 3.20; 95% CI, 1.68-6.08). Mucosal melanoma was an independent risk factor for distant metastasis, especially liver metastasis. East-Asian/Hispanic/African had significantly lower incidence of gastrointestinal/musculoskeletal/respiratory/other-rare-type-toxicities; but higher incidences of liver toxicities. Limitations: A retrospective study. Conclusions: Ethnicity and melanoma subtype are associated with survival and recurrence pattern in melanoma patients treated with adjuvant anti-PD-1. Toxicity profile differs by ethnicity and may require a precision toxicity surveillance strategy.
