Since the development of the artificial intelligence (AI), several applications have been proposed. Among these, the intersection of AI and medicine has sparked a wave of innovation, revolutionizing various aspects of healthcare delivery, diagnosis, and treatment. A review of the current literature was performed to evaluate the possible applications of ChatGPT (OpenAI, San Francisco, CA, USA) in the dermatological field. A total of 20 manuscripts were collected in the present review, reporting several potential applications of ChatGPT in dermatology, ranging from clinical practice to patients' support. The convergence of ChatGPT and dermatology represents a compelling synergy that transcends traditional boundaries of healthcare delivery. As AI continues to evolve and permeate every facet of medicine, ChatGPT stands at the forefront of innovation, empowering patients and clinicians alike with its conversational prowess and knowledge dissemination capabilities.

Background: Suppurative hidradenitis (HS) is a chronic, inflammatory skin disease of the hair follicle unit. Adalimumab (ADA), an anti-tumor necrosis factor (TNF) alpha, is the only FDA-approved biologic available for the management of HS. TNF-α can also affect glucose and lipid metabolism, promoting insulin resistance and obesity by negatively regulating irisin, a new adipomyokine., Methods: A total of 17 HS patients were enrolled in the study. Blood samples were collected from all patients at baseline and week-16. Plasma irisin levels were detected by ELISA assay., Results: Plasma irisin levels were significantly increased after 16 weeks of ADA therapy in HS patients compared to baseline. Interestingly, plasma irisin levels correlated with clinical response., Conclusions: The link between skin inflammatory diseases and metabolic disorders has aroused great interest in order to research new biomarkers able to early identify metabolic comorbidities. Among these emerging biomarkers, irisin is one of the most recently discovered. We examined a group of patients affected by moderate-severe HS treated with anti-TNF-α, demonstrating for the first time how a therapy able to block an inflammatory cytokine can also affect the metabolic profile by modifying levels of irisin.

Background: Acne is a chronic inflammatory disease of the pilosebaceous unit resulting from different cofactors. The alteration of the skin microbiome has recently been revealed to play a role in acne pathogenesis. Concerns with side effects of available systemic treatment for acne resulted in a greater focus on topical therapies, such as topical azelaic acid which showed to be an effective and safe treatment option for acne. The aim of our study was to evaluate the efficacy of a new treatment protocol for acne based on an oral supplement composed of biotin and 3 strains of lactic ferments combined with a topical gel composed of azelaic acid, hydroxypinacolone retinoate, and α-hydroxy acids., Methods: An Italian single-center interventional study was performed enrolling patients suffering from mild-to-moderate-acne. Patients were treated with a supplement based on biotin and 3 strains of lactic ferments, combined with a topical gel product (azelaic-acid, hydroxypinacolone retinoate, and α-hydroxy acids). All enrolled patients were scheduled for a total of 2 visits, a baseline visit (V0) and a follow-up visit after 60 days of treatment (V1)., Results: A total of 30 patients were enrolled in the study. Between V0 (baseline) and V1 (60 days), there was a reduction of 37.4% in the GAGS Score, 40.7% in the SEBUTAPEtm Score, and 18% in the TEWL Score, and an increment of 44% in the T-Blue Test Score. No cases of serious AEs were reported in our experience., Conclusions: Our results confirmed the promising therapeutic role of a probiotic supplement associated with topical therapy in the treatment of mild to moderate acne.

Background: Psoriatic arthritis (PsA) is a chronic inflammatory disease that can produce disabling joint symptoms, adding significantly to the physical and psychosocial burden of psoriasis. Early detection is important to allow the development of an appropriate care plan to delay the onset of PsA and maximize the patient's quality of life. Our aim was to present the criteria, based on evidence and expert opinion, for a multidisciplinary approach to the management of PsA in a patient already characterized by skin manifestation., Methods: An expert panel from the principal psoriatic care units of the Campania region of Italy met to discuss their mutual experience of the multidisciplinary approach to the management of psoriatic disease and to describe an integrated dermatologic/rheumatologic approach focused on the early diagnosis, management, and treatment of PsA., Results: Two types of consultation modalities were considered most relevant to the care of patients with psoriatic disease in Italy: the parallel approach and the face-to-face care unit approach. Screening criteria for multidisciplinary care unit admission were described, with dermatologists, as the primary managers of the majority of patients with psoriasis, playing a critical role in introducing patients early on to therapy., Conclusions: An integrated management approach may enhance patient care by ensuring early diagnosis and treatment, with the potential to achieve better outcomes for both skin and musculoskeletal manifestations of psoriasis. The multidisciplinary care unit model is an effective and satisfying collaborative approach, not only optimizing outcomes and satisfaction for the patient but strengthening collaboration between the specialties.

Background: Several reports have previously suggested that oligomineral water may have a beneficial immunomodulatory role in skin physiology. However, molecular, and cellular mechanisms through which oligo-elements act in cutaneous trophism have not yet been fully clarified. Among the external stimuli that affect the skin, ultraviolet (UV) radiation, which is frequently encountered in everyday life, is a major environmental factor of skin damage. Keratinocytes are the major target of UV, and they play a key role in a first line of body defenses. Accumulating evidence suggests that UVB irradiation induces nuclear DNA damage, membrane destruction, resulting in apoptosis and skin inflammation. The aim of this study was to investigate the anti-inflammatory, antioxidant, antiapoptotic effects of Rocchetta ® oligomineral (Co.Ge.Di. International SpA, Rome, Italy) water in UVB-irradiated immortalized human keratinocytes., Methods: HaCaT UVB-irradiated was cultured with increasing concentrations of Rocchetta ® oligomineral water. To evaluate the anti-inflammatory properties gene expression of TNF, IL1β, IL6, COX2 and Caspase1 was performed. Moreover, the antiapoptotic effects were evaluated through gene expression of GADD45, Caspase3 and RIPK3. Finally, we evaluated the antioxidant activity of Rocchetta ® oligomineral water by measuring total ROS/RNS and superoxide production as markers of oxidative stress after UVB irradiation., Results: Our findings have shown that Rocchetta ® oligomineral water is well tolerated by the cells and displays anti-inflammatory, antioxidant and antiapoptotic proprieties when used prior keratinocyte UVB irradiation., Conclusions: Our results highlight a possible protective role of Rocchetta oligomineral water in modulating the cutaneous inflammatory response to external triggers and injuries.

Fertility is a function of the body that is often overlooked as a site for the expression of the side effects of certain drugs. With the approval of new drugs with a totally innovative mechanism of action, the risk assessment on fertility both in male and female is more difficult. This is particularly true in psoriasis, an invalidating inflammatory skin disease. The estimated prevalence of psoriasis in adults ranged from 0.51% to 11.43%, and in children from 0% to 1.37%, with frequent diagnosis in young patients of childbearing age. With the increasing use of new, predominantly immunosuppressive or biologic drugs for psoriasis, questions frequently arise in clinical practice as to their safety in men and women wishing to procreate. Both psoriatic patients and their physicians are concerned about adverse effects of the disease and its treatment on their future fertility, causing additional concerns in the therapeutic management of these patients. Among antipsoriatic drugs, conventional therapies are mainly involved in the onset of infertility in both sexes, exerting in some cases toxic effects against reproductive organs. Conversely, biologic agents appear to improve male and female fertility especially when gonadal impairment is related to inflammatory phenomena. There is a lack of review articles of commonly used medications in psoriasis with respect to their potential effects on fertility. The aim of this paper was to provide a practical guide for both dermatologist and endocrinologist in therapeutic management of psoriatic patients of childbearing age, considering the impact of prescribed drugs on their current and future fertility.
