Vitamin D provides calcium (Ca) -phosphorus (P) metabolism and bone mineralization between bone, kidney and intestine together with parathyroid hormone and calcitonin. Nutritional rickets are caused by defective chondrocyte and osteoid mineralization in growth plaques due to vitamin D and / or calcium deficiency. In our country, free 400 IU / day vitamin D support is provided to all infants for the first year since 2005 by the Ministry of Health. Although infants of immigrant families in our country are given vitamin D support, families cannot communicate with healthcare professionals in primary health care centers, so they do not know at what dose and until when to use this supportive treatment, and can use it in under-doses or for a short time. In this article, we wanted to contribute to the literature by presenting the patient who was treated with the diagnosis of nutritional rickets and to draw attention to the problems that may occur due to the lack of communication with immigrant families., D vitamini, parathormon ve kalsitonin ile birlikte kemik, böbrek ve bağırsak arasındaki kalsiyum (Ca)-fosfor(P) metobolizmasını ve kemik mineralizasyonunu sağlamaktadır. Nütrisyonel rikets, D vitamini ve/veya kalsiyum eksikliğine bağlı olarak büyüme plaklarında kusurlu kondrosit ve osteoid mineralizasyonundan kaynaklanır. Ülkemizde Sağlık Bakanlığı tarafından 2005 yılından itibaren ilk bir yıl tüm süt çocuklarına ücretsiz 400 IU/gün D vitamini desteği yapılmaktadır. Ülkemizde bulunan göçmen aile bebeklerine D vitamini desteği verilmesine rağmen, aileler birinci basamak sağlık merkezlerindeki görevli sağlık çalışanları ile iletişim kuramadığı için bu destek tedaviyi hangi dozda ve ne zamana kadar kullanması gerektiğini bilmemekte, eksik dozda veya kısa süreli kullanabilmektedir. Biz bu makalede nutrisyonel rikets tanısı ile tedavi verilen hastayı sunarak literatüre katkı yapmak ve göçmen aileler ile iletişim eksikliğine bağlı oluşabilecek sorunlara dikkat çekmek istedik.

Amaç: Bu çalışmada, hastanemiz erişkin ve çocuk endokrin polikliniklerinde diabetes mellitus (DM) ve obezite tanısı almış hastalarda, üriner sistem enfeksiyon etkeni olarak izole edilen patojenlerin dağılımı, bu patojenlerin antimikrobiyallere direnç oranları, glukozüri ve piyüri varlığının araştırılması amaçlanmıştır.Gereç ve Yöntemler: Ocak 2012-Şubat 2014 tarihleri arasında Düzce Üniversitesi Tıp Fakültesi Hastanesi Erişkin ve Çocuk Endokrin polikliniklerinde DM ve obezite tanısı almış 107 hastaya ait kayıtlar geriye dönük olarak incelenmiştir. Bu hastalardan alınan idrar örneklerinin biyokimyasal/mikrobiyolojik laboratuvar sonuçları değerlendirilmiştir. Verilerin istatistiksel değerlendirmesinde SPSS 22.0 programı ile Fisher Exact ve Pearson Chi-Square testleri kullanılmıştır.Bulgular: Hastaların 75 (%70)’i çocuk, 32 (%30)’si erişkin; 84 (%78,5)’ü kadın, 23 (%21,5)’ü erkek, yaş ortalaması 25,1±23 idi. Diabetes mellitus ve obezite tanısı olan hasta sayısı sırasıyla 76 (%71,0) ve 31 (%29,0) idi. İdrar kültürlerinin 28 (%26)’inde (DM tanılı hastaların %30,2’si ve obezite tanılı hastaların %16,1’inde) bakteri üremesi olmuştur. Etken olarak en sık E. coli saptanmıştır. Piyüri ve glikozüri olan hastalarda daha fazla bakteri üremesi saptanmış olup, piyüri-bakteri üremesi arasında istatistiksel olarak anlamlı ilişki bulunmuşken, glikozüri ile anlamlı ilişki tespit edilmemiştir.Sonuç: Glukozürinin, her ne kadar enfeksiyon riskini artırsa da tek başına belirleyici bir parametre olamayacağı ancak hastaların mevcut risk göz önünde tutularak değerlendirilmeleri gerektiği; ayrıca piyüri varlığı ile idrar yolu enfeksiyonu arasında kuvvetli bir ilişki olduğu görülmüştür. Ancak tam idrar tetkiki sonucuna göre ampirik tedaviye başlamak yerine idrar kültüründe izole edilen etkene ve duyarlılık profiline göre antibiyotik tedavisinin başlanmasının uygun olacağı düşünülmüştür., Aim: In this study, it was aimed to investigate the distribution of isolated pathogens, the resistance rates of these pathogens to antimicrobials, presence of glycosuria and pyuria in patients with diabetes mellitus (DM) and obesity in adult and pediatric endocrine outpatient clinics with urinary system infections.Material and Methods: Between January 2012 and February 2014, records of 107 patients with DM and obesity in Duzce University Medical Faculty Hospital Adult and Pediatric Endocrine outpatient clinics were retrospectively reviewed. Biochemical/microbiological laboratory results of urine specimens taken from these patients were evaluated. Fisher Exact and Pearson Chi-Square tests were used with statistical evaluation of SPSS 22.0 program.Results: Thirty-two (70%) of the patients were children, 75 (30%) are adults; 84 (78.5%) were female, 23 (21.5%) were male, mean of ages 25.1±23. Number of patients with diabetes and obesity were 76 (71.0%) and 31 (29.0%), respectively. Bacterial growth occurred 28 (26%) of urine cultures (23 (30.2%) of patients with DM, 5 (16.1%) of patients with obesity). E. coli was detected most frequently as causative agent. In patients with pyuria and glycosuria, more bacterial growth were detected and statistically significant correlation was found between pyuria and bacterial growth, but no significant relationship was found with glycosuria.Conclusion: Although glycosuria may increase the risk of infection, it may not be a determining parameter alone, but patients should be assessed considering the current risk; it has also been found that there is a strong relationship between the presence of the pyuria and urinary tract infection. However, it is considered appropriate to start antibiotic treatment according to the causative agent and sensitivity profile isolated in urine culture rather than to start empirical treatment according to the complete urine analysis result.

Turnersendromu (TS), kadınlarda ikinci X kromozomunun kısmi veya tam kaybı ya da Xkromozomunun yapısal anomalileri sonucu oluşan ve boy kısalığı, lenfödem,kardiyak anomaliler, primer over yetmezliği ve nörokognitif problemler gibiklinik bulgular ile karakterize bir hastalıktır. Prevalansı 1/2000-1/2500 canlıdişi doğum olarak ortaya konmuştur. Hastaların TS tanısı almaları geniş yaşaralıklarında olabilmektedir. TS’luhastalar farklı karyotip kompozisyonları göstermekte bu da hastalarda farklıklinik manifestasyonlara neden olmaktadır. Yaptığımız çalışmadasitogenetik analizi sonuçlarına göre 9 hastada 45,X (%45), 1 hastada 46,X,der(X),t(X,X)(p.11.2;q22)/45,X (%5), 1 hastada 45,X/46,X,del(X)(p.11.2)(%5), 1 hastada 45,X/47,XXX (%5), 4 hastada 45,X/46,XX (%20), 2 hastada46,X,i(X)(q10)/45,X (%10), 1 hastada 46,X,i(X)(q10) (%5) ve 1 hastada46,X,del(X)(p21) (%5) kromozomal kompozisyon tespit edildi.Turner sendromlu olgularımızın karyotip kompozisyonlarını, başvuru şikayetlerini,antropometrik verileriyle birlikte literatüre katkı amaçlı sunduk., Turner syndrome (TS) is the result of partial or complete lossof the second X chromosome in women or structural anomalies of the Xchromosome, and is characterized by clinical manifestations such as shortstature, lymphedema, cardiac anomalies, primer over failure and neurocognitiveproblems. The prevalence is 1/2000-1/2500 live born female children. Patientswith Turner syndrome can be diagnosed at a wide age of range. Patients with TSshow different karyotype compositions, which cause different clinicalmanifestations in cases. According to the results of cytogenetic analysis inour study, it was detected that 9 patients were 45, X (45%), 1 patient was 46,X,der(X),t(X,X)(p.11.2;q22)/45,X (%5), 1 patient was45,X/46,X,del(X)(p.11.2) (%5), 1 patient was 45,X/47,XXX (%5), 4 patients were45,X/46,XX (%20), 2 patients were 46,X,i(X)(q10)/45,X (%10), 1 patient was46,X,i(X)(q10) (%5),1 patient was 46,X,del(X)(p21) (%5) chromosomal composition. We have presented our patients with Turner syndrometogether with the reasons for application complaints and anthropometric dataswith karyotype compositions for contribution to the literature.

The SRD5A2 gene that encodes SRD5A2 enzyme is placed on chromosome 2p23. The coding region of the gene includes 5 exons that are translated into a 254-amino acid protein. To evaluate SRD5A2 gene that encodes SRD5A2 enzyme is placed on chromosome 2p23 a ccompany with biotidinase deficiency in case with ambiguous external genitalia. We investigated a case with ambiguous external genitalia for cytogenetic and gene mutation analaysis. Gene mutation analaysis and cytogenetic analaysis were performed according to a standard DNA sequencing method based on PCR and the present international standard nomenclature (ISCN), respectively. A Novel p.Arg179Ser (c.537 G>T) heterozygotes mutation on exon 3 of SRD5A2 gene accompany with biotidinase deficiency was detected. The chromosomal analaysis result is 46, XY. This is the first case with biotinidase deficiency and novel R179S p.Arg179Ser (c.537 G>T) mutation of the SRD5A2 gene, which cause 5-alpha reductase deficiency.
