Background: This study aimed to determine physicians' perceptions of the extent of suboptimal insulin dosing and the barriers and solutions to optimal dosing in people with diabetes (PwD) treated with insulin.Methods: A cross-sectional online survey was conducted in four countries with primary care physicians and endocrinologists treating PwD using insulin pens, which included 53 questions on physicians' characteristics and their perceptions of the behaviors of PwD in relation to insulin dosing routines, unmet needs and potential solutions. Analyses were descriptive.Results: Of the 160 physicians (80 primary care physicians, 80 specialists) surveyed in Spain, 58.1% were male and 88.8% had been qualified to practice for more than five years. Most physicians (> 65%) indicated that 0– 30% of PwD missed or skipped, mistimed, or miscalculated an insulin dose in the last 30 days. Common reasons for these actions were that PwD forgot, were out of their normal routine, were too busy or distracted, or were unsure of how much insulin to take. To optimize insulin dosing, over 75% of physicians considered it very helpful for PwD to have real-time insulin dosing calculation guidance, mobile app reminders, a device automatically recording glucose measurements and/or insulin, having insulin and glucose data in one place, and having the time for more meaningful conversations about insulin dosing routines.Conclusion: According to physicians' perspectives, suboptimal insulin dosing remains common among PwD. This survey highlights the need for integrated and automated insulin dosing support to manage the complexity of insulin treatment, improve communications between PwD and physicians, and ultimately improve outcomes for PwD.Plain Language Summary: In Spain, more than 50% of people with diabetes (PwD) do not reach glycemic targets (glycated hemoglobin [HbA1C] under 7%). Introduction: Diabetes mellitus is one of the most public health challenges of the twenty-first century. Globally, 382 million people had diabetes by the year 2013. Purpose: The purpose of this study was to determine the prevalence of diabetes mellitus and identify its associated factors at public health institutions in Addis Ababa. Patients and Methods: An institution-based cross-sectional study was carried out from June to July 2016. A total of 758 participants were selected using a multistage sampling technique. Data were collected with a structured interviewer-administered questionnaire; a WHO STEPwise approach of NCDs risk factors identification, and the collected data were checked for completeness immediately following data collection and the filled questionnaires were entered into Epi–Info 3.5.1, and then exported to SPSS 23 for further analysis. Descriptive statistics such as mean, percentages, standard deviation, and ranges were determined. To identify factors associated with diabetes mellitus, binary logistics regression was used. Results: The overall prevalence of diabetes mellitus was 14.8%, with a sex-specific prevalence of 18.35% and 16.62% for males and females, respectively. Older age participants had higher risks of developing diabetes mellitus than younger age individuals. Alcohol drinkers had more risks of developing diabetes mellitus than non-alcohol drinkers. Participants with plasma HDL-C ≥ 40mg/dl were more likely to develop diabetes mellitus than those with < 40mg/dl. Participants with a higher level of plasma triglyceride ≥ 130mg/dl were found to be more exposed to the risks of developing diabetes mellitus than study participants with a low level of triglycerides. Conclusion: A higher prevalence of diabetes mellitus was observed in Addis Ababa public health institutions. Factors such as age, alcohol drinking, HDL, triglycerides, and vagarious physical activity were associated with diabetes mellitus. Background: Type 1 diabetes mellitus (T1DM) is one of the most common chronic diseases in children that may be due to micro or macrovascular complications. Diabetic renal disease or nephropathy is a common complication of DM, clinically silent and the only detectable abnormality due to the presence of microalbuminuria. Subjects and methods: This study was a case–control study. Participants were classified into two groups. The first group included 40 children with T1DM and the second group included 30 matched healthy controls. Serum apelin (APLN), chemerin, cholesterol, and triglycerides (TG) levels were measured for each case. Also, albumin/creatinine ratio was analyzed in random urine sample. Results: Comparison between T1DM patients and controls revealed that serum apelin, chemerin, cholesterol, TG levels, and albuminuria were significantly increased in cases compared to their controls. Significant positive correlations were found between HbA1c% and albuminuria for APLN and chemerin in the diabetic group. Whereas significant negative correlations were found between apelin and glomerular filtration rate (GFR). Conclusion: Increased levels of serum apelin and chemerin in T1DM patients may be considered as promising adipokines for the development of diabetic complication. [ABSTRACT FROM AUTHOR]

Objective: We analyzed the concordance between two methods for measuring treatment adherence (TA) and studied the determinants of TA in patients with type 2 diabetes mellitus. Methods: We conducted a cross-sectional descriptive study in a primary care center, involving 320 diabetic patients. TA was measured using the Haynes-Sackett (H-S) adherence test during the patient interview and based on pharmacy refill data. TA was calculated globally and by drug groups (antihypertensive, lipid-lowering, and antidiabetic drugs). Results: Poor TA as measured by the H-S test was observed in 11.2% of the patients. Based on pharmacy refill data, there was a poor global TA rate of 30.3%, which was 33.3%, 26.6%, and 34.2% for oral antidiabetic, antihypertensive, and lipid-lowering drugs, respectively. Concordance between the two methods was poor. There was no relationship between the degree of disease control and TA as measured by the H-S test. Good TA measured based on pharmacy refill data for antidiabetic and antihypertensive drugs was associated with lower glycosylated hemoglobin and diastolic blood pressure values, respectively. Patients with good global TA showed lower glycosylated hemoglobin, diastolic blood pressure, and low-density lipoprotein cholesterol values. The multivariate analysis found good oral antidiabetic adherence to be associated to free pharmacy service; good antihypertensive drug adherence to the existence of comorbidities; and good lipid-lowering drug adherence to a history of ischemic heart disease, and a more experienced physician and/or female physician. Conclusion: Concordance between the two methods in assessing TA was low. Approximately one-third of the patients with type 2 diabetes mellitus presented poor TA in relation to antihypertensive, lipid-lowering, and antidiabetic medication. An improved TA was associated with a better control of the studied parameters. Comorbidities, such as ischemic heart disease and access to free pharmacy service, were identified as determinants of good TA. [ABSTRACT FROM AUTHOR]
