Objective: To evaluate the outcome and complications of removal of silicone oil after pars plana vitrectomy., Study Design: Case series., Place and Duration of Study: Layton Rahmatullah Benevolent Trust (L.R.B.T), Free Base Eye Hospital, Karachi, from February 2008 to January 2011., Methodology: Ninety five eyes of 95 patients with a history of undergoing three-port pars plana vitrectomy were included in this study that subsequently underwent removal of silicone oil. Silicone oil was removed after ophthalmoscopically determining retina attachment or when the duration of silicone oil tamponade was atleast of 6 months. Patients were followed for a period of 12 months., Results: Retinal re-detachment was seen in 19 (20%) out of 95 eyes, vitreous haemorrhage in 2 (2.1%) out of 95 eyes, corneal decompensation in 6 (6.3%) out of 95 eyes, hypotony in 7 (7.3%) out of 95 eyes, phthisis bulbi in 2 (2.1%) out of 95 eyes and lens opacification in 9 (9.4%) out of 95 eyes., Conclusion: In this study, silicone oil removal resulted in various complications among which retinal re-detachment was the most frequent.

Parotidectomy is a common procedure usually done for a parotid mass necessitating a histological diagnosis. Operation is normally performed under General anesthesia with a nerve stimulator to facilitate facial nerve stimulation. We describe a new technique with reports of three cases, making total parotidectomy under local anesthesia possible. The ascending cervical branch of cervical plexus and the auriculotemporal nerve were anesthetized by bupivacaine 0.25% (2mg/kg) and lignocaine with adrenaline 7 mg/kg. Effective onset of anesthesia was within 15-25 minutes and the operations lasted between 2-3 hours without any complications. This offers advantage in high-risk patients where general anesthesia is contraindicated. The facial nerve can be easily identified with on command movements by the patient rendering the use of nerve stimulator or injection of the dye superfluous. This technique makes total parotidectomy an outpatient procedure and facilitates an early discharge.

We report a rare case of low grade sarcoma of scrotal wall. A male child aged 2 1/2 years presented with history of scrotal lymphoedema involving the left side of scrotum, slowly progressive since birth. We planned scrotectomy but the patient was lost to follow-up. During intervening period, left inguinal omentopexy was done elsewhere. The patient again presented 3 years later with multiple swellings in scrotum. We performed almost complete scrotectomy with reconstruction of the scrotal defect using scrotal remnant as a stretchable musculocutaneous flap. A third of the residual scrotum could be expanded to resurface the entire scrotum. On histopathology, low grade malignant peripheral nerve sheath tumor of grade-I was confirmed. Post-operative follow-up showed no residual disease.

A case of a 55 years old lady with tuberculosis of the tongue is reported. Patient presented with non-healing ulcer in dorsum of the tongue for one year. Initial biopsy and histopathological examination revealed non-specific inflammation. After surgery, biopsy report proved primary tuberculosis of tongue. Anti-tubercular therapy for nine months showed uneventful recovery of the patient.
