Background: Dementia involves a progressive decline in many functional areas. Policy and practice guidelines should cover the entire course of the disease from early detection to the end-of-life. The present study aimed to evaluate the contents of national dementia strategies with a focus on palliative care content., Methods: We employed qualitative content analyses. Sixteen national dementia strategies from 14 countries were reviewed. Using open coding, the contents were compared to the domains and recommendations of the palliative care in dementia white paper of the European Association for Palliative Care (EAPC)., Results: Although palliative care was not explicitly referred to in eight of the 14 countries and only to a limited extent in three countries, a number of domains from the EAPC white paper were well represented, including "person-centered care, communication, and shared decision making"; "continuity of care"; and "family care and involvement." Three countries that referred to palliative care did so explicitly, with two domains being well represented: "education of the health care team"; and "societal and ethical issues." The strategies all lacked reference to the domain of "prognostication and timely recognition of dying" and to spiritual caregiving., Conclusions: National dementia strategies cover part of the recent definition of palliative care in dementia, although they do not frequently label these references as "palliative care." In view of the growing numbers of people dying with dementia, preparation for the last phase of life should be added to national strategies.

The degradation of the traditional cultural landscape due to abandonment of agricultural management is perceived as a serious problem in different parts of the world. Rising consciousness concerning this issue in Japan led to the formation of numerous voluntary civil farming programs. This paper presents a multi-method case study conducted in Japan (Ōyamasenmaida, Chiba prefecture) about a highly relevant rural–urban cooperation, where landholders lease out their rice terraces to city dwellers to grow their own rice under the intensive instruction and well-organized support by local farmers and other local experts. The activity is known as ‘Tanada Ownership’ (tanada means rice terrace). It is spread over the country and promises, in contrast to the short-term individualistic European models, long-term rural–urban relations, the valorization of local knowledge and natural resources as well as the maintenance of the rice-terrace landscapes in several regions of Japan. The particular goal of this research is the investigation of the ‘how’ and ‘why’ of the system and the motivation of its participants. The Japanese approach is compared with similar European initiatives and conclusions focus on the particularities of the Japanese Ownership System and its transferability to the European context. Despite the innovativeness and popularity of the Ownership System, scientific studies are relatively scarce and none of them published are in English. This article therefore presents an original and important contribution to the scientific community, as it provides insights into the Tanada Ownership System and puts it into an international context by comparing it with European initiatives of voluntary farm work. In this article Erika Fischer-Lichte argues that, since the beginning of the twentieth century, in different parts of the world, modern theatre was invented by way of interweaving cultures in performance. Different examples from the early twentieth century and the 1990s show how theatre acted as a laboratory for testing and experiencing the potential of cultural diversity. An innovative performance aesthetics enabled the exploration of the emergence, stabilization, and destabilization of cultural identity, merging aesthetics with politics. Erika Fischer-Lichte is Professor of Theatre Studies at the Freie Universität in Berlin. She has published widely in the fields of the theory and history of the theatre, and is the author of numerous books, including, in English, Theatre, Sacrifice, Ritual: Exploring Forms of Political Theatre (2005) and The Transformative Power of Performance: a New Aesthetics (2008). In 2008 she established a new research programme, 'Interweaving Cultures in Performance', at the Freie Universität. As the baby boomer generation ages, the need for laws to enhance quality of life for the elderly and meet the increasing demand for family caregivers will continue to grow. This paper reviews the national family leave laws of nine major OECD countries (Canada, Denmark, France, Germany, Italy, Japan, Netherlands, Spain, and the United Kingdom) and provides a state-by-state analysis within the U.S. We find that the U.S. has the least generous family leave laws among the nine OECD countries. With the exception of two states (California and New Jersey), the U.S. federal Family Medical Leave Act of 1993 provides no right to paid family leave for eldercare. We survey the current evidence from the literature on how paid leave can impact family caregivers' employment and health outcomes, gender equality, and economic arguments for and against such laws. We argue that a generous and flexible family leave law, financed through social insurance, would not only be equitable, but also financially sustainable. [ABSTRACT FROM AUTHOR]

BackgroundIn this global study we sought to estimate the degree to which a family member might feel embarrassed when a close relative is suffering from an alcohol, drug, or mental health condition (ADMC) versus a general medical condition (GMC). To date, most studies have considered embarrassment and stigma in society and internalized by the afflicted individual but have not assessed family embarrassment in a large-scale study.MethodIn 16 sites of the World Mental Health Surveys (WMHS), standardized assessments were completed including items on family embarrassment. Site matching was used to constrain local socially shared determinants of stigma-related feelings, enabling a conditional logistic regression model that estimates the embarrassment close relatives may hold in relation to family members affected by an ADMC, a GMC, or both conditions.ResultsThere was a statistically robust association such that subgroups with an ADMC-affected relative were more likely to feel embarrassed compared to subgroups with a relative affected by a GMC (p < 0.001), even with covariate adjustments for age and sex.ConclusionsThe pattern of evidence from this research is consistent with conceptual models for interventions that target individual- and family-level stigma-related feelings of embarrassment as possible obstacles to effective early intervention and treatment for an ADMC. Macro-level interventions are under way but micro-level interventions may also be required among family members, along with care for each person with an ADMC. [ABSTRACT FROM PUBLISHER]

Background: Over the past two decades, scores of articles and several books have been published calling for more attention to the prevention of eating disorders, but less than two dozen prevention intervention studies have been conducted to date., Methods: This paper reports the results of a systematic review of 20 empirical intervention studies on eating disorders prevention, discussing the data accumulated on what has and has not been effective. Beyond a description of study design and findings, this paper devotes special attention to the theoretical orientations of the studies and their implied assumptions about preventive strategies., Results: Though there has been a compelling interest in social, political and economic factors influencing the incidence of disordered eating, little of this interest has been carried over into the prevention end of eating disorders research. Most studies reported thus far have been designed to target and measure change principally on the individual level, to the exclusion of considering leverage points for intervention in the larger social environment., Conclusions: This paper concludes with a recommendation for new attention to a model of proactive primary prevention targeted at environmental change and cross-disciplinary collaboration to achieve a reduction in the incidence of eating disorders.
