The financialization of everyday life has received considerable attention since the 2008 global financial crisis. Financialization is thought to have created active financial subjects through the ability to participate in mainstream financial services. While the lived experience of these mainstream financial subjects has been the subject of close scrutiny, the experiences of financial subjects at the financial fringe have been rarely considered. In the UK, for example, the introduction of High-Cost, Short-Term Credit [HCSTC] or payday loan regulation was designed to protect vulnerable people from accessing unaffordable credit. Exploring the impact of HCSTC regulation is important due to the dramatic decline of the high-cost credit market which helped meet essential needs in an era of austerity. As such, the paper examines the impact of the HCSTC regulation on sixty-four financially marginalized individuals in the UK that are unable to access payday loans. First, we identify the range of socioeconomic strategies that individuals employ to manage their finances to create a typology of financial subjectivity at the financial fringe. Second, we demonstrate how the temporal and precarious nature of financial inclusion at the financial fringe adds nuance to existing debates of the everyday lived experience of financialization. The mouth may be presented and understood in different ways, be subject to judgement by others and, as we age, may intrude on everyday life due to problems that affect oral health. However, research that considers older people's experiences concerning their mouths and teeth is limited. This paper reports on qualitative research with 43 people in England and Scotland, aged 65–91, exploring the significance of the mouth over the lifecourse. It uses the concept of 'mouth talk' to explore narratives of maintaining, losing and replacing teeth. Participants engaged in 'mouth talk' to downplay the impact of the mouth, demonstrate socially appropriate ageing, and distance themselves from 'real' old age by retaining a moral identity and sense of self. They also found means to challenge dominant discourses of ageing in how they spoke about missing teeth. Referring to Leder's notion of 'dys-appearance' and Gilleard and Higgs' work on the social imaginary of the fourth age, the study illustrates the ways in which 'mouth talk' can contribute to sustaining a sense of self in later life, presenting the ageing mouth, with and without teeth, as an absent presence. It also argues for the importance of listening to stories of the mouth in order to expand understanding of people's approaches to oral health in older age. [ABSTRACT FROM AUTHOR]

The Anti-Social Behaviour Order (ASBO) is one of the best known measures used to tackle anti-social behaviour. In keeping with the popular conception, the order is frequently used against young people. Of all ASBOs issued in England and Wales up to the end of 2005, roughly 40% were imposed on under-18s. This paper begins with a brief outline of the three principles at the heart of the celebrated Scottish children’s hearings system. With reference to these principles, and to the provisions which govern the use of the order against 12–15 year olds north of the border, the paper then discusses five areas of concern about the use of ASBOs against young people in England and Wales: the readiness to resort to ASBOs; the forum for ASBO applications; the terms of ASBOs; publicising the details of ASBOs; and custodial net-widening. Governments around the world have sought to promote school choice, not just in order to improve educational outcomes, but also because such choice is believed to be intrinsically valuable: parents are believed to want to choice and to feel empowered by it. This article empirically evaluates the intrinsic value of school choice, comparing the attitudes and experiences of parents in England (where expanding choice is an explicit policy goal) and Scotland (where policymakers tend to play down choice), combining an online survey with in-depth interviews. While the overwhelming majority of parents in both countries express a desire for some school choice, only a minority want choice primarily for intrinsic reasons. Rather, most believe it is necessary to avoid negative outcomes for their children. Moreover, while parents in England tend to say they have more choice than their Scottish counterparts, they are no more satisfied with the level of choice that they have. Indeed, they tend to be more cynical, fatalistic and disempowered. Based on the British experience, school choice policies have not been successful in promoting intrinsic value. The rebellion against Charles I's authority that began in Edinburgh in 1637 involved the Scots in successive invasions of England and armed intervention in Ireland. Historians have almost universally taken a negative view of Scottish involvement in these wars, because it has been assumed that the Scottish political leadership sacrificed all other considerations in order to pursue an unrealistic religious crusade. This article suggests that aspects of the Anglo-Scottish relationship need to be reappraised. Using estimates of English payments to the Scots during the 1640s, it will be argued that the Scottish leadership made pragmatic political decisions based on a practical appreciation of the country's military and fiscal capacity. Substantial payouts from the English parliament enabled the Scottish parliamentary regime to engage in military and diplomatic activities that the country could not otherwise have afforded. The 1643 treaty that brought the Scots into the English Civil War on the side of parliament contrasts favourably with the 1647 Engagement in support of the king. It will be shown that, although the English parliament did not honour all of its obligations to the Scots, it does not automatically follow that the alliance was a failure in financial terms. The article presents a response to a paper by Mok et al. on the suicide rate for men and women in Wales, Scotland and England. The suicide rate in Scotland and Northern Ireland is higher than those for Wales. The paper also shows that there is a high incidence of frowning in Scotland, which is attributed to the presence of 90% standing freshwater in all of Great Britain.

Using quantitative and qualitative research designs, respectively, two studies investigated why countries make different health technology assessment (HTA) drug reimbursement recommendations. Building on these, the objective of this study was to (a) develop a conceptual framework integrating the factors explaining these decisions, (b) explore their relationship and (c) assess if they are congruent, complementary or discrepant. A parallel convergent mixed methods design was used. Countries included in both previous studies were selected (England, Sweden, Scotland and France). A conceptual framework that integrated and organised the factors explaining the decisions from the two studies was developed. Relationships between factors were explored and illustrated through case studies. The framework distinguishes macro-level factors from micro-level ones. Only two of the factors common to both studies were congruent, while two others reached discrepant conclusions (stakeholder input and external review of the evidence processes). The remaining factors identified within one or both studies were complementary. Bringing together these findings contributed to generating a more complete picture of why countries make different HTA recommendations. Results were mostly complementary, explaining and enhancing each other. Public involvement in service change has been identified as a key facilitator of health care transformation (Foley et al., 2017) but little is known about how health policy influences whether and how organisations involve the public in change processes. This qualitative study compares policy and practice for involving the public in major service changes across the UK's four health systems (England, Northern Ireland, Wales and Scotland). We analysed policy documents, and conducted interviews with officials, stakeholders, NHS staff and public campaigners (total number of interviewees = 47). Involving the public in major service change was acknowledged as a policy challenge in all four systems. Despite ostensible similarities, there were some clear differences between the four health systems' processes for involving patients and the public in major changes to health services. The extent of central Government oversight, the prescriptiveness of Government guidance, the role for intermediary bodies and arrangements for independent scrutiny of contentious decisions all vary. We analyse how health policy in the four systems has used 'sticks' and 'sermons' to promote particular approaches, and conclude that both policy and the wider system context within which health care organisations try to effect change are significant, and understudied aspect of contemporary practice. Geographical disparities in health outcomes have been evident across the UK for decades. Recent analysis on the dietary differences between Scotland and England that might go some way to explain these health differences is limited. This study aimed to assess whether, and to what degree, aspects of diet and nutrition differ between Scottish and English populations, specifically between those with similar household incomes. A period of 12 years of UK food purchase data (2001–2012) were pooled and used to estimate household-level consumption data for Scotland and England. Population mean food consumption and nutrient intakes were estimated, adjusting for known confounders (year, age of household reference person, age they left full-time education and income). Comparison was also made within equivalised income quintiles. Analysis showed that the foods and nutrients that should be increased in the diet (highlighted in the Scottish Dietary Goals) were lower in Scotland than in England (e.g. fruit and vegetables 267 g/d; 99 % CI 259, 274 v. 298 g/d; 99 % CI 296, 301), P<0·001). Similarly, foods and drinks linked with poor health outcomes were higher in Scotland. These regional inequalities in diet were even more pronounced in the lower-income groups (e.g. red and processed meat consumption in the lowest-income quintile was 65 g/d; 99 % CI 61, 69 in Scotland v. 58 g/day; 99 % CI 57, 60 in England, P<0·001, but similar in the highest-income quintile (58 g/d; 99 % CI 54, 61 v. 59 g/d; 99 % CI 58, 60, respectively). A poorer diet in Scotland compared with England, particularly among disadvantaged groups, may contribute to differences in excess mortality between countries. [ABSTRACT FROM AUTHOR]

Social engagement has powerful effects on wellbeing, but variation in individual engagement throughout the lifecourse is wide. The trajectories may differ by gender and be affected by socio-economic status (SES). However, long-term development of social engagement is little studied and the effect of childhood SES on later-life social engagement remains obscure. We aimed to describe the social engagement development from childhood to middle age by gender and test the effect of childhood SES on middle age social engagement. Data (N=16,440, 51.3% male) are drawn from the on-going National Child Development Study, following British babies born in 1958. Social engagement was measured by social activities, voluntary work and social contacts, with follow-ups at age 11, 16, 23 and 50. SES was measured by father's occupational social class and tenure status. Structural equation modelling suggested inter-individual stability in social engagement, showing that development of social engagement started in childhood and increased social engagement in middle age through adolescence and early adulthood. Longitudinal effects were detected within and across the social engagement domains. Lower childhood SES was significantly related to a lower level of voluntary work and social activity in middle age, but to higher levels of social contacts. Although stability in social engagement is moderate over the lifecourse, variation within and across the different social engagement domains is shaped by differences in childhood SES. BackgroundAn unhealthy body mass index (BMI) has been associated with depression but the direction of association is uncertain. Our aim was to estimate the co-morbidity and direction of association between BMI and depressive symptoms at several ages, from childhood to mid-adulthood.MethodThe data were from 18 558 individuals born in 1 week in March 1958, in England, Scotland and Wales, with follow-up at ages 7, 11, 16, 23, 33, 42, 45 and 50 years. Depression (scores ⩾90th percentile) was identified from child/adolescent (teacher questionnaires) and adult (self-complete questionnaires and clinical interview) measures. BMI (kg/m2) measured in child/adolescence and adulthood was classified as underweight, normal, overweight or obese.ResultsIn cross-sectional analyses, obesity and underweight (not overweight) from 11 to 45 years were associated respectively with 1.3–2.1 and 1.5–2.3 times the risk of depression compared with normal weight. Using the time-lagged generalized estimating equation (GEE) approach, we tested (a) whether underweight or obesity at prior ages (7 to 45 years) predicted subsequent risk of depression (11 to 50 years), adjusting for baseline depression; and (b) whether depression at prior ages (7 to 42 years) predicted subsequent risk of underweight or obesity (11 to 45 years), adjusting for baseline BMI. In longitudinal analyses, underweight predicted subsequent depression in both sexes [odds ratio (OR) 1.25, 95% confidence interval (CI) 1.11–1.40] and depression predicted subsequent underweight in males only (OR 1.84, 95% CI 1.52–2.23). Obesity predicted subsequent depressive symptoms in females only (OR 1.34, 95% CI 1.14–1.56), but depression did not predict obesity.ConclusionsClinicians should consider screening routinely for depression patients with unhealthy BMI, namely underweight and obesity, and vice versa. Background. We aimed to elucidate early antecedents of suicide including possible mediation by early child development. Method. Using the 1958 birth cohort, based on British births in March 1958, individuals were followed up to adulthood. We used data collected at birth and at age 7 years from various informants. Suicides occurring up to 31 May 2009 were identified from linked national death certificates. Multivariate Cox proportional hazard models were used to investigate risk factors. Results. Altogether 12399 participants (n=44 suicides) had complete data. The strongest prenatal risk factors for suicide were: birth order, with risk increasing in later-born children [ptrend =0.063, adjusted hazard ratio (HR)], e.g. for fourth- or later-born children [HR=2.27, 95% confidence interval (CI) 0.90-5.75]; young maternal age (HR=1.18, 95% CI 0.34-4.13 for ⩽ 19 years and HR=0.41, 95% CI 0.19-0.91 for >29 years, ptrend=0.034); and low (<2.5 kg) birth weight (HR=2.48, 95% CI 1.03-5.95). The strongest risk factors at 7 years were externalizing problems in males (HR=2.96, 95% CI 1.03-8.47, Ptrend"0.050) and number of emotional adversities (i.e. parental death, neglected appearance, domestic tension, institutional care, contact with social services, parental divorce/separation and bullying) for which there was a graded association with risk of suicide (ptrend = 0.033); the highest (HR=3.12, 95% CI 1.01-9.62) was for persons with three or more adversities. Conclusions. Risk factors recorded at birth and at 7 years may influence an individual's long-term risk of suicide, suggesting that trajectories leading to suicide have roots in early life. Some factors are amenable to intervention, but for others a better understanding of causal mechanisms may provide new insights for intervention to reduce suicide risk. [ABSTRACT FROM AUTHOR]

Background. Severe youth antisocial behaviour has been associated with increased risk of premature mortality in high-risk samples for many years, and some evidence now points to similar effects in representative samples. We set out to assess the prospective association between adolescent conduct problems and premature mortality in a population-based sample of men and women followed to the age of 65 years. Method. A total of 4158 members of the Medical Research Council National Survey of Health and Development (the British 1946 birth cohort) were assessed for conduct problems at the ages of 13 and 15 years. Follow-up to the age of 65 years via the UK National Health Service Central Register provided data on date and cause of death. Results. Dimensional measures of teacher-rated adolescent conduct problems were associated with increased hazards of death from cardiovascular disease by the age of 65 years in men [hazard ratio (HR) 1.17, 95% confidence interval (CI) 1.04-1.32], and of all-cause and cancer mortality by the age of 65 years in women (all-cause HR 1.16, 95% CI 1.07-1.25). Adjustment for childhood cognition and family social class did little to attenuate these risks. Adolescent conduct problems were not associated with increased risks of unnatural/substance-related deaths in men or women in this representative sample. Conclusions. Whereas previous studies of high-risk delinquent or offender samples have highlighted increased risks of unnatural and alcohol- or substance abuse-related deaths in early adulthood, we found marked differences in mortality risk from other causes emerging later in the life course among women as well as men. BackgroundA multi-centre, four-arm trial (the PACE trial) found that rehabilitative cognitive behaviour therapy (CBT) and graded exercise therapy (GET) were more effective treatments for chronic fatigue syndrome (CFS) than specialist medical care (SMC) alone, when each was added to SMC, and more effective than adaptive pacing therapy (APT) when added to SMC. In this study we compared how many participants recovered after each treatment.MethodWe defined recovery operationally using multiple criteria, and compared the proportions of participants meeting each individual criterion along with two composite criteria, defined as (a) recovery in the context of the trial and (b) clinical recovery from the current episode of the illness, however defined, 52 weeks after randomization. We used logistic regression modelling to compare treatments.ResultsThe percentages (number/total) meeting trial criteria for recovery were 22% (32/143) after CBT, 22% (32/143) after GET, 8% (12/149) after APT and 7% (11/150) after SMC. Similar proportions met criteria for clinical recovery. The odds ratio (OR) for trial recovery after CBT was 3.36 [95% confidence interval (CI) 1.64–6.88] and for GET 3.38 (95% CI 1.65–6.93), when compared to APT, and after CBT 3.69 (95% CI 1.77–7.69) and GET 3.71 (95% CI 1.78–7.74), when compared to SMC (p values ⩽0.001 for all comparisons). There was no significant difference between APT and SMC. Similar proportions recovered in trial subgroups meeting different definitions of the illness.ConclusionsThis study confirms that recovery from CFS is possible, and that CBT and GET are the therapies most likely to lead to recovery. Graphical displays of investigations are increasingly used in clinical care. Summaries of medical records for research or clinical review purposes can generate unmanageably large amounts of data, which may be helpfully summarised and displayed using timelines. During a prospective study of cancer care in primary care, care timelines were generated in Microsoft Visio, using data collected retrospectively from general practice records. Data from primary and secondary care consultations were included. Thirteen timelines were created, which proved valuable in summarising and analysing the data concerning the cases studied. Timelines provide a clear, concise way of displaying large amounts of diverse data, although some selectivity is required to facilitate interpretation. Generation of timelines in the software was time consuming: if they could be automatically generated within clinical IT systems, they would enable clinicians to generate useful summaries of care of complex cases, facilitating care reviews. Eighteenth-century architect-builders were a small group, but important for understanding the market strategies of knowledge-based experts in an age of rapid growth in technical information before the creation of modern professions. This article confronts a teleological historiography of emerging professionalization. It is focused on Robert Mylne and several of his contemporaries in Edinburgh and London, including a number of successful London-based Scots who were active as architects, builders, engineers, and surveyors, and self-styled in all these areas when it suited them. It supplies an account of what it took for building experts to establish themselves and flourish in big cities and the ways in which such experts navigated, controlled, and accommodated an environment of unregulated expertise that largely suited contemporary practitioners. BackgroundThe hormone ‘cortisol’ has been associated with cognitive deficits in older ages, and also with childhood cognition. The extent to which the associations of cortisol with cognitive deficits in later life reflect associations with childhood cognition ability is unclear. This study aimed to assess associations between adult cortisol levels and subsequent cognitive functions, while considering childhood cognition and other lifetime covariates.MethodData are from the 1958 British Birth Cohort. Two morning salivary cortisol samples were obtained at 45 years: 45 min after waking (t1) and 3 h later (t2). Standardized tests assessing immediate and delayed verbal memory, verbal fluency and speed of processing were administered at 50 years. Information on cortisol, cognitive outcomes and covariates [e.g. birthweight, lifetime socio-economic position (SEP), education, smoking and drinking habits, body mass index (BMI), menopausal status, and depression/anxiety] was obtained for 4655 participants.ResultsWorse immediate and delayed verbal memory and verbal fluency at 50 years were predicted by elevated t2 cortisol at 45 years. For instance, for 1 standard deviation (s.d.) increase in t2 cortisol, individuals scored −0.05 s.d. lower on verbal memory and fluency tests. Childhood cognition explained about 30% of these associations, but associations with adult cognition remained.ConclusionsThis study suggests that higher cortisol levels in late morning at 45 years are associated with poorer verbal memory and fluency at 50 years, with a contribution from childhood cognition to these associations. Late Carboniferous red-beds, < 700 m thick, at outcrop and in the subsurface of the Canonbie Coalfield can be assigned to the Warwickshire Group. They are preserved within the axial part of the Solway Syncline and are divisible into the Eskbank Wood, Canonbie Bridge Sandstone and Becklees Sandstone formations. Sedimentation largely took place on a well-drained alluvial plain, characterized mainly by early, primary oxidation of the strata. Large, northerly-flowing braided river systems were common, with overbank and floodplain fines deposited lateral to the channels; soils formed during intervals of low sediment aggradation. The Canonbie succession includes some of the youngest Carboniferous rocks preserved in the UK. Correlation of the Eskbank Wood Formation is equivocal, but using petrographical, heavy mineral, zircon age dating and palaeocurrent data, the Canonbie Bridge Sandstone Formation can be unambiguously correlated with the Halesowen Formation ofWarwickshire, the Pennant Sandstone Formation of SouthWales and the offshore Boulton Formation. This suggests that southerly-derived detritus travelled considerable distances from the Variscan highlands of Brittany and/or central Germany across the southern North Sea and UK areas, to a position some hundreds of kilometres north of that previously recognized. The Becklees Sandstone Formation has much in common with the Salop Formation of the English Midlands. It appears to have no preserved equivalent elsewhere in the UK or in the UK sector of the southern North Sea but resembles stratigraphically higher parts of the southern North Sea succession seen in the Dutch sector. A scheme of grouped lithostratigraphical units ('beds') proposed for the English Upper Jurassic Kimmeridge Clay Formation has been claimed to be also chronostratigraphical, but some of the resulting time-correlations conflict with those of the standard chronozonation based on ammonite biostratigraphy. Review of some critical ammonite species reaffirms the validity of the ammonite zonal scheme and shows that mismatching of lithologies (facies-correlations) has led to incorrect time-correlations. Because the numbering scheme of 'beds' was based on correlations of attenuated successions, it is on too coarse a time-scale to identify many non-sequences, and its usefulness as a chronostratigraphical tool is questioned. Evidence suggests that at least some calcareous concretions in the Kimmeridge Clay formed at shallow depths, which is relevant to discussions of the succession in terms of basin analysis. The sixteenth-century English Reformer John Hooper's views on the biblical idea of magisterial office and the application of God's law to the whole of the Christian community had a profound influence in England and Scotland. It is also clear that Hooper assimilated much of the German Reformer Heinrich Bullinger's theologico-political federalism, and played an important role in the reception of Bullinger's thought in sixteenth- and seventeenth-century Britain. This paper presents evidence for the involvement of herring gulls (Larus argentatus) as vectors in the recent outbreaks of Salmonella montevideo in sheep and cattle in Scotland and suggests that the transfer can take place over considerable distances. The breeding area in Scotland of herring gulls which overwinter in N.E. England is remarkably similar to the geographical distribution of the outbreaks. This pattern, together with the feeding behaviour of herring gulls on farmland, the presence of S. montevideo in herring gulls just before their departure from the wintering area and the timing of the return just before the peak of outbreaks are all circumstantial evidence implicating this gull in the outbreaks. The rapid return of these gulls to their breeding areas means that S. montevideo can be transported long distances in one day and raises the possibility that the original source of S. montevideo could have been in N.E. England rather than in Scotland.
