Abstract

The prevalence of asthma in Australian children is one of the highest in the world, as reported by the International Study of Asthma and Allergies in Childhood.1 Poorly controlled asthma has an increased risk of future exacerbations, emergency attendances and poor quality of life, and is a leading cause of school absenteeism and poor academic outcomes. We sought to examine the prevalence and control of asthma in Australian adolescents. From 2011 to 2014 a population-based, cross-sectional, stratified, cluster sampling of primary and secondary school students in greater metropolitan Melbourne was used to recruit a sample of students aged 10–14 years as part of the SchoolNuts study. Of the 229 schools contacted, 117 (51.1%) participated in the study. Among the 20 965 eligible students in the participating schools, a questionnaire which included questions on asthma and asthma control was completed by 9663 (46.1%) students.2 The prevalence of self-reported asthma ever was 26.9% (95% CI 26.0 to 27.8) and self-reported current asthma …

Abstract

IntroductionNeoadjuvant chemotherapy for breast cancer treatment is prescribed to facilitate surgery and provide confirmation of drug-sensitive disease, and the achievement of pathological complete response (pCR) predicts improved long-term outcomes. Docosahexaenoic acid (DHA) has been shown to reduce tumour growth in preclinical models when combined with chemotherapy and is known to beneficially modulate systemic immune function. The purpose of this trial is to investigate the benefit of DHA supplementation in combination with neoadjuvant chemotherapy in patients with breast cancer.Methods and analysisThis is a double-blind, phase II, randomised controlled trial of 52 women prescribed neoadjuvant chemotherapy to test if DHA supplementation enhances chemotherapy efficacy. The DHA supplementation group will take 4.4 g/day DHA orally, and the placebo group will take an equal fat supplement of vegetable oil. The primary outcome will be change in Ki67 labelling index from prechemotherapy core needle biopsy to definitive surgical specimen. The secondary endpoints include assessment of (1) DHA plasma phospholipid content; (2) systemic immune cell types, plasma cytokines and inflammatory markers; (3) tumour markers for apoptosis and tumour infiltrating lymphocytes; (4) rate of pCR in breast and in axillary nodes; (5) frequency of grade 3 and 4 chemotherapy-associated toxicities; and (6) patient-perceived quality of life. The trial has 81% power to detect a significant between-group difference in Ki67 index with a two-sided t-test of less than 0.0497, and accounts for 10% dropout rate.Ethics and disseminationThis study has full approval from the Health Research Ethics Board of Alberta - Cancer Committee (Protocol #: HREBA.CC-18-0381). We expect to present the findings of this study to the scientific community in peer-reviewed journals and at conferences. The results of this study will provide evidence for supplementing with DHA during neoadjuvant chemotherapy treatment for breast cancer.Trial registration numberNCT03831178

Abstract

IntroductionActive surveillance (AS) is the preferred primary treatment strategy for men with low-risk clinically localised prostate cancer (PCa); however, the majority of these men still receive radical treatment within 10 years due to disease progression and/or fear of cancer progression. Interventions designed to suppress tumour growth, mitigate fear of cancer progression and precondition men for impending radical treatments are an unmet clinical need. Exercise has been shown to delay the progression of prostate tumours in animal models, improve physical and functional health and manage psychological outcomes in cancer patients; however, these outcomes have not been demonstrated in PCa patients undergoing AS.Methods and analysisThis phase II randomised controlled trial will randomise 66 men undergoing AS to either an exercise group or a usual care group. The exercise group will perform a 12-week, supervised, high-intensity interval training programme, consisting of 3 sessions/week for 28–40 min/session. The primary outcome will be cardiorespiratory fitness. Secondary outcomes will include immunosurveillance and cancer-related biomarkers, psychosocial outcomes including fear of cancer progression and quality of life and physical function. Exploratory outcomes will include clinical indicators of disease progression. The trial has 80% power to detect a significant between-group difference in VO2peakof 3.5 mL/kg/min with a two-tailed alpha level Ethics and disseminationThe study has received full ethical approval from the Health Research Ethics Board of Alberta – Cancer Committee (Protocol Number: HREBA.CC-17–0248). Abstract

Background: Transgenic mice expressing the dominant interfering p193 protein in cardiomyocytes (MHC-1152stop mice) exhibit an induction of cell cycle activity and altered remodelling after experimental myocardial infarction (MI). Objective: To determine whether the altered remodelling results in improved cardiac function in the MHC-1152stop mice after MI, as compared with non-transgenic mice. Methods: MHC-1152stop mice and non-transgenic littermates were subjected to experimental MI via permanent occlusion of the coronary artery. Infarct size was determined at 24 h and at 4 weeks after MI, and left ventricular pressure-volume measurements were performed at 4 weeks after MI in infarcted and sham-operated animals. Results: Infarct size in MHC-1152stop mice and non-transgenic littermates was not statistically different at 24 h after MI, as measured by tetrazolium staining. Abstract

Objective: To describe the later health status of newborn infants who received extracorporeal membrane oxygenation (ECMO) for acute respiratory failure in the era after the UK ECMO trial. Design: Prospective follow up study of newborn infants who received ECMO at a single centre between January 1997 and January 2001. Setting: Departments of ECMO and Paediatric Intensive Care, University Hospitals of Leicester. Patients: All babies who received ECMO within 14 days of birth. Interventions: Neurodevelopment screening using the schedule for growing skills-II (SGS-II) assessment tool. Main outcome measures: Survival at 12 months of age by disease and functional development at follow up. Results: A total of 145 neonates received ECMO for treatment of respiratory failure. Of these, 108 (75%) were alive at 1 year of age. There were no deaths in children treated for respiratory failure secondary to meconium aspiration syndrome (73/145). Ninety three (86% of survivors) infants attended a follow up visit at 11–19 months postnatal age. Eighty two were classed as normal, seven as having “impairment”, and four as having “severe disability”. Conclusions: Most newborn infants with acute respiratory failure treated with ECMO will have a normal neurodevelopment screening assessment at 11–19 months of postnatal age. There is no evidence to suggest that changes in neonatal practice since the UK ECMO trial have led to changes in outcome of infants undergoing ECMO therapy.

Abstract

Objective The objective of the current study was to compare the responses to different ex vivo immunogenic challenges between immune cells derived from metabolically healthy subjects with obesity and subjects with obesity and type 2 diabetes. Research design and methods We recruited 10 metabolically healthy subjects with obesity (Edmonton Obesity Staging System (EOSS) stage 0) and 9 subjects with obesity and type 2 diabetes (EOSS stage 2) aged between 21 years and 70 years and matched for body mass index. Peripheral blood mononuclear cells (PBMCs) were isolated and immune cell phenotypes and ex vivo cytokine production after phytohaemagglutinin (PHA, a T cell mitogen) stimulation were determined. Abstract

Objectives:To explore the relation between receipt of sex education and experience of first intercourse. Subjects and design:The national survey of sexual attitudes and lifestyles is based on a sample of 18876 respondents aged 16-59, randomly selected from the Post Office9s small-user postcode address file. Data were collected between May 1990 and November 1991 by personal interviews combining a self administered questionnaire with a face to face interview. Main outcome measures: Age at first intercourse, use of contraception at first intercourse, actual and preferred source of sex education (including school based lessons). Results: Median age at first intercourse fell by four years for women and three years for men over the past four decades, to 17 for both men and women aged 16-19 at the time of interview. Of those respondents for whom school was the main source of information about sexual matters, men were less likely, and women no more likely, to have had intercourse before the age of 16 than were those citing other main sources, such as friends and the media. Both men and women were more likely to have used some method of contraception. In multivariate analysis, these effects remained after controlling for the effect of current age, educational attainment, and religious affiliation. Conclusions: These data provide no evidence to support the concern that provision of school sex education might hasten the onset of sexual experience. These findings have important implications for the provision of sexual health education and highlight the need to carry out prospective and randomised studies of the impact of sex education.

Abstract

The case of a 14 year old girl is reported, referred to Child Psychiatry with tearfulness and possible auditory hallucinations. Closer questioning revealed a history of low energy, sleeping excessively without refreshment, exertional dyspnoea, and poor growth. Psychologically, there was no evidence of low mood or negative cognitions despite the inexplicable tearfulness. Detailed enquiry revealed the “hallucinations” to be “whooshing” noises in her ears precipitated by standing. She was found to have a haemoglobin level of 55 g/l, attributed to a combination of poor diet and menorrhagia. Periodic, anaemic, cerebral hypoxia could be proposed to be the root of most, if not all, of the symptoms, illustrating the importance of marrying physical and psychological history taking with suitable investigations for an eminently treatable condition.

An unusual rod-like structure enclosed within a vacuole is described as occurring in enlarged glial cells associated with the lesions encountered in an uncommonly acute case of multiple sclerosis apparently heralded by an attack of `viral encephalitis'. Similar bodies were not found in a variety of other enlarged glial cells. An encapsulated `grape-fruit' like structure was also seen.

Paralysis in man and the domestic fowl caused by “T o CP” (tri-cresyl phosphate, tri-tolyl phosphate or TTP) is reviewed, and the search for a non-toxic plasticizer derived either from TTP or tri-xylenyl phosphate (tri-dimethylphenyl phosphate or TXP) is described. It had been previously shown that the ortho -tolyl esters are toxic but we found that their removal does not make the mixture non-toxic. The meta - and para -tolyl esters were known to be free from toxic effects and the 2: 4 and 2: 5 dimethylphenyl esters were equally shown to be non-toxic except in massive doses. We confirmed that the tri- ortho -ethylphenyl ester was non-toxic, but found the mono- and di- ortho -ethylphenyl phosphates to be highly toxic. We showed that TTP made from phenols with all the ortho -cresol and ortho -ethylphenol removed is non-toxic. With TXP, however, elimination of all ortho -cresol and ortho -ethylphenol from the raw materials does not result in a non-toxic plasticizer. Tri- ortho - n -propylphenyl phosphate was shown to non-toxic but esters containing one or two ortho -propylphenyl groups are toxic. The toxicity of the ortho -tolyl, ortho -ethylphenyl and ortho -propylphenyl groups is much reduced in the presence of the 3: 5-dimethylphenyl group. This influence is greatest in mono- ortho -propylphenyl-di-3: 5-dimethylphenyl phosphate which is non-toxic in five consecutive daily doses of 500 mg./kg. body weight. The removal of all ortho -cresol, ortho -ethylphenol and ortho -propylphenol from the raw material results in a non-toxic product which we termed non-toxic plasticizer “NTP”. This has been fed in various doses to a variety of mammals and a large number of chickens in short term tests without toxic effects. Our experiments showed that tri- meta -ethylphenyl phosphate is non-toxic but tri- para -ethylphenyl phosphate is highly toxic. The mixture from which NTP is made contains 10% of para -ethylphenol. Based on chemical statistics the tri- para -ethylphenyl ester can be present only in minute quantities, and we have shown that esters containing one or two para -ethylphenyl groups are non-toxic. An ester with a methyl group in the ortho , and an ethyl group in the para position in the same benzene nucleus (tri-2-methyl-4-ethylphenyl phosphate) we showed, surprisingly, to be non-toxic.

Spinal fluid lactate dehydrogenase (L.D.H.) isoenzyme patterns have been investigated in normal subjects, in those with non-destructive nervous disease, and in those with destructive lesions in the nervous system. The spinal fluid from organic disease in general showed a significantly increased level of L.D.H. 2 and L.D.H. 3 so that the combined values of L.D.H. 2 and L.D.H. 3 might perhaps be of value in determining the presence of destructive nervous lesions.

Abstract

Lymphocytes from patients with multiple sclerosis are sensitized to carcinoembryonic antigen (C.E.A.) in almost the same degree as are those from subjects with cancer. During pregnancy and the early puerperium there is an even higher degree of cellular sensitization to C.E.A. Patients with neurological diseases other than multiple sclerosis also show significant sensitization to C.E.A. though of less degree than do those with multiple sclerosis. Sensitization to C.E.A. is thus not specifically characteristic of cancer. The unexpected sensitization in multiple sclerosis may be associated with the pronounced degree of gliosis found in that condition.

Abstract

Studies of lymphocyte sensitization in patients with asthma showed that the intrinsic and extrinsic forms of the disease fell into two distinct groups. Intrinsic disease showed a general sensitization to a number of non-specific antigens, while the extrinsic form had only slight elevation above normal values. These findings suggest that intrinsic asthma results from a defect of general immunity, whereas extrinsic asthma is a specific sensitization.

Abstract

The work of Hellem (1960) and of Gaarder, Jonsen, Laland, Hellem, and Owren (1961) has made it clear that red blood cells are a potent source of adenosine diphosphate of consider able importance in determining the adhesiveness of platelets in vitro. Caspary (1965) showed that the presence of even 2% haematocrit red cells was enough to confer a normal degree of stickiness on platelets suspended in plasma. The implication of this finding, brought out in correspondence which followed the publication of the latter paper (Caspary, 1966), is that platelet stickiness might depend in some measure on the readi ness with which erythrocytes yield adenosine diphosphate, and a similar suggestion has more recently been made by Harrison and Mitchell (1966) and Hampton and Mitchell (1966). Atten tion had been drawn to abnormality in size of red blood cells in multiple sclerosis by Plum and Fog (1959), and the present work indicates that they are also more osmotically fragile.

Abstract

Objectives: To determine the effect of intravenous regional guanethidine on post-traumatic algodystrophy., Methods: Ten of 20 consecutive patients with algodystrophy after Colles' fracture were treated with serial intravenous regional guanethidine blockades and the other 10 were treated with physiotherapy alone. Patients were assessed before and after each block and then monthly over a six month period., Results: The treated patients had subjective and objective improvements in the assessed features of algodystrophy; treatment resulted in a significant reduction in finger tenderness at five and six months., Conclusions: Guanethidine blockade induces short term benefits in the symptoms of algodystrophy but improves finger tenderness only in the long term.
