Background The purpose of this study was to determine whether implementation of regional guidelines improves the management of acute myocardial infarction with ST-elevation (STEMI), in terms of reperfusion decision and orientation. Program The editing of local guidelines in a hospital or a network of care is recognised as a useful approach, but rarely evaluated. The RESCUe network has defined and implemented a guideline shared by 30 hospitals to improve MI care pathway and transfer of STEMI. To evaluate the guideline9s impact, the emergency doctors involved in the network included in a prospective register all suspected MI entering the prehospital care pathway, to analyse their route since the call to the centre 15. Recent data were compared to those collected before the guideline dissemination. The acute phase indicators spotting good practices as defined by French National Authority for Health were evaluated: rate and time to reperfusion, rate of direct referral to transfer to interventional cardiology center (ICC), rate of patients receiving anti-platelet therapy, analgesia and Heparin, in hospital mortality. Results Between February 2008 and May 2009, 568 patients with less than 24 h STEMI at the acute phase were managed by the 18 mobile intensive care units (MICU) and the 8 ICC of the zone covered by RESCUe network. Their data were compared to those of 544 patients managed between January 2004 and December 2005, using the same inclusion criteria and analysis. The exhaustivity not evaluated in 2005, was 95% in 2009. The comparison of the results between 2005 and 2009 shows: a significant improvement (p a significant reduction (p an in hospital mortality rate of 6%. Discussion The establishment of regional guideline allowed improving STEMI care pathway since the call to the 15 Center to the reperfusion for patients managed by mobile intensive care units. The access to reperfusion therapy and recommended treatments for the patients managed within this care pathway has significantly improved between 2005 and 2009, according to guidelines, with a privileged referral to the interventional cardiology center. Conclusion The establishment of a regional guideline with measurement of its implementation brings together health professionals and significantly improve the acute care pathway for MI management, for patients benefit. Guideline and clinical practice indicators are efficient tools (i) to implement good practices according to guidelines (ii) to evaluate, to improve and to follow-up professional practices. Objectif, contexte Evaluer l9impact de l9application d9un referentiel regional sur l9amelioration de la prise en charge de l9infarctus du myocarde avec sus-decalage du segment ST, en terme de strategies de reperfusion et d9orientation des patients. Programme La definition et la mise en place de referentiels dans un etablissement ou un reseau de soins est une demarche reconnue comme utile, mais rarement evaluee. Le reseau RESCUe a defini et mis en place un referentiel commun a ses 30 etablissements membres pour optimiser la prise en charge et les transferts des infarctus du myocarde avec sus-decalage du segment ST. Pour en evaluer l9impact, les urgentistes adherents a RESCUe ont inclus dans un registre prospectif toutes les suspicions d9infarctus aigus du myocarde (IDM) pris en charge en pre-hospitalier sur la zone sanitaire du reseau afin d9analyser le parcours depuis l9appel au Centre 15. Les donnees recentes ont ete comparees avec les donnees anterieures a la diffusion du referentiel. Les indicateurs phares des bonnes pratiques a la phase aigue tels que publies par la HAS ont ete evalues: taux et delai de reperfusion, taux de transfert direct en Centre de Cardiologie Interventionnelle (CCI), taux de traitements par antiagregants plaquettaires et antalgiques, mortalite hospitaliere. Resultats Entre fevrier 2008 et mai 2009, 568 patients presentant un IDM avec sus-decalage de ST a la phase aigue de moins de 24 heures ont ete inclus par les 18 SMUR et les 8 CCI de la zone sanitaire du reseau. Ces donnees ont ete comparees a celles des 544 patients pris en charge en prehospitalier entre janvier 2004 et decembre 2005, sur les memes criteres d9inclusion et d9analyse. L9exhaustivite non evaluee en 2005, etait de 95% en 2009. L9acces a la reperfusion et aux traitements recommandes des patients passant par cette filiere a significativement progresse entre 2005 et 2009 en conformite avec les recommandations, avec un transfert direct preferentiel en CCI. Conclusion La mise en place d9un referentiel avec mesure de sa mise en œuvre federe les professionnels de sante et ameliore significativement la filiere aigue de prise en charge de l9infarctus du myocarde au benefice des patients. Le referentiel et les indicateurs de pratique clinique, sont des outils efficaces (i) pour implementer les bonnes pratiques en accord avec les recommandations (ii) pour evaluer, ameliorer et suivre les pratiques professionnelles.
