Aim: To generate a substantive theory for understanding the phenomenon of nursing home care for older people in Taiwan., Background: Taiwanese culture shows great respect for older people and older people are traditionally cared for at home by their families. However, the older population in Taiwan is rapidly increasing and this demographic shift, together with various socio-economic changes, has resulted in nursing homes becoming a new and significant care option., Design: A grounded theory approach was used to study the residents and relatives from three nursing homes in Taiwan., Methods: Formal and informal interviews and participant observation data were collected over two months in each nursing home. Forty nursing home residents and 20 of their relatives were recruited. The data were analysed using the constant comparative method and involved the use of theoretical memos and theoretical sampling procedures., Results: The study found that nursing home care for older people in Taiwan is understood to be a process of forced choice, involving three stages; namely, 'becoming a problem', 'making a forced choice' and 'coping with the forced choice'., Conclusion: Taiwan is in a state of conflict with regard to providing care for older people, a situation in which the influence of traditional cultural and that of industrialization exist side by side. The consequence of having to relocate older people into nursing homes means that both parties are the victims of this choice., Relevance to Clinical Practice: This conceptual explanation helps us to understand how the Taiwanese respond to the issue of care for older people and how they resolve their main concerns related to it. Subsequently, it is hoped that this will help health care practitioners to provide care more effectively to meet the needs of the Taiwanese with the aim of enhancing the standards of care for older people.

This paper outlines a recent PhD study that explored factors underpinning decisions of people with schizophrenia regarding neuroleptic medicine taking. Relevant studies tend to be within the world view of psychiatry, resting on traditional concepts of compliance and insight. These studies examined schizophrenia outwith the context of other illnesses taking a snap shot view of medicine taking decision making, thus de-emphasizing its process. The chosen method of grounded theory (Glaser and Strauss 1967) overcomes these deficiencies. Stage 1 data arose from patient interviews, patients with asthma, epilepsy and schizophrenia contributed (n = 45), carers also sometimes contributed (n = 11), all three conditions being episodic disorders vulnerable to environmental stressors. Stage 2 develops from stage 1, mental health workers involved in the care of people with schizophrenia were interviewed, plus one asthma specialist and one epilepsy specialist (n = 16). Their views were compared to perspectives arising from the patient interviews and were included in the data, out of which an explanatory typology emerged. The three parts of the typology comprise the study's major categories and are labelled as: direct-reactive, deferential-compliant, and active discerning and optimizing. Active discerning and optimizing consists of two stages; experimental-reflective and consolidation. The core category of responsive resolution contains themes which run through all major categories.

Background: This research outlines some preliminary findings emerging from a grounded theory investigation into the 'meaning of spirituality'. These initial results raise some important questions about the terminology and language that nurses use regarding the term spirituality. It seems that many of the policy directives and statutory guidelines make two major assumptions regarding 'spirituality'. Firstly, patients and nurses are aware and understand the concept, and secondly, patients may require their spiritual needs to be met. These preliminary findings suggest that a dichotomy is emerging between professional assumption and patient expectation regarding the meaning of spirituality., Aim: The study had one broad research aim, to gain a deeper insight into how patients, nurses, and people from the major world religions understand the concept of 'spirituality'., Design: A qualitative research design was used involving a grounded theory method of inquiry. It was felt that this qualitative method would aid the investigation of this subjective dimension of peoples' existence, enabling existing theoretical constructs and arguments to be tested., Methods: The constant comparative method was used throughout the data collection and analysis. Analysis was undertaken at two levels, 'overview analysis' and 'line-by-line analysis'. This enabled the creation of categories and central themes., Results: Constant comparative analysis resulted in the formation of several categories and central themes. Two categories presented and discussed in detail are 'definitions of spirituality', and 'diverse perceptions of spirituality'., Conclusion: It would seem that there is now an urgent need for nursing to evaluate and perhaps adjust its vision regarding what constitutes spirituality. Such an approach may serve to reduce the gap between policy and public expectation., Relevance to Clinical Practice: It seems that there may be no 'precise' terminology associated with the language used to define spirituality, raising possible implications for nursing practice and nurse education.

Patients with schizophrenia relapse because of inadequate levels of medicine taking. Therefore, it seems logical to learn about the factors underpinning their medicine-taking decisions. Further research is urgently needed to explore this area and thereby to refine models of practice, to promote therapeutic interactions with medicine. Existing research tends to share three common deficits. * It is grounded in the worldview of the psychiatrist not the patient. * It studies the influence of symptoms on compliance behaviour in isolation from other potential variables. * It overlooks the potential influence of the chronicity experience in general by examining schizophrenia outwith the context of other enduring illnesses. Overall these deficits may combine to distort the influence of schizophrenic symptoms on medicine decision making and justify a coercive rather than an empowering approach. A pernicious self-fulfilling spiral may be contributing to the problem of relapse in schizophrenia. Potentially the mental health nurse has a role in addressing this problem.

Recent manometric and radiological studies suggest that the upper oesophageal sphincter has poor compliance in patients with a pharyngeal (Zenker's) diverticulum. To test the hypothesis that this phenomenon is related to structural changes within the cricopharyngeus muscle we examined, histologically, muscle strips from 14 patients with a Zenker's diverticulum and compared them with control tissue obtained at autopsy from 10 non-dysphagic individuals. The cricopharyngeus muscle from patients and controls differed from inferior constrictor muscle by virtue of type 1 fibre predominance and greater fibre size variability. Ragged red fibres and nemaline bodies are a normal finding in the cricopharyngeus. Marked differences were observed in the cricopharyngeus muscle of Zenker's patients which demonstrated fibro-adipose tissue replacement and fibre degeneration. It is concluded that these structural changes may account for the observed diminished upper oesophageal sphincter opening and dysphagia in patients with Zenker's diverticulum.
