There is a shortage of radiologists in the Pacific Island nations. To help 'fill the gap', a project was conducted between 2004 and 2006 aimed at upskilling radiographers from South Pacific countries in basic radiological interpretation of plain film radiographs. It was intended that the radiographers would act as a resource, identifying and describing abnormalities to the local doctors and nurses, thus reducing the risk of misdiagnosis. A total of 23 participants, including one doctor and three health-care assistants, attended a 2-week course in 2004 or 2005. The course covered common abnormalities of the musculoskeletal system, chest and abdomen. Although they found the course challenging, most participants performed satisfactorily. Follow-up visits to the radiographers and their nominated, medically qualified mentors emphasized interprofessional, sociocultural and workload challenges experienced by some of the radiographers in implementing their extended role. Most of them, however, had found ways to integrate their new knowledge and skills into their clinical practice. Thirteen radiographers attended a further 1-week refresher course in 2006, showing their commitment to continued learning.

Intrathoracic extramedullary haematopoiesis occurring as a complication of osteopetrosis is uncommon. The combination of classic plain film and CT findings should suggest the diagnosis, which can be non-invasively confirmed with bone marrow scintigraphy.

The peritoneal cavity is a potential space that is divided by the peritoneal reflections into various complex subspaces. It can be involved in many disease processes including developmental, inflammatory, neoplastic and traumatic conditions. Computed tomography is highly sensitive and consistent in detecting peritoneal pathology. This pictorial essay aims to emphasize and illustrate the CT features of the spectrum of peritoneal diseases.

Acute pulmonary schistosomiasis occurs in non-immune hosts, particularly visitors to regions where Schistosoma species are endemic. While radiological findings of acute pulmonary schistosomiasis might resemble neoplastic or granulomatous disease, an appropriate travel history might suggest the diagnosis. With rising popularity of travel to Africa and South-East Asia, the incidence of acute pulmonary schistosomiasis can be expected to increase. We describe a case of acute pulmonary schistosomiasis occurring in an Australian returning from Lake Malawi, Sub-Saharan Africa.

Transjugular intrahepatic portosystemic shunts (TIPS) have recently been used to manage the portal hypertensive complications of the Budd-Chiari syndrome. We report this application of TIPS (to our knowledge the first such application in Australia) in a young man with an excellent result and no major complications. This treatment offers an alternative to portacaval shunt surgery and has the advantage of bypassing a stenosed or compressed inferior vena cava. Additionally, the procedure does not complicate liver transplantation surgery if this is indicated at a later date.

The variable magnetic resonance imaging appearances of central nervous system haemorrhage, both intra- and extra-axial, are described. These will vary with the type of image contrast (T1 or T2 weighting), the nature of the imaging sequence (spin-echo or gradient-echo) and the time from onset of haemorrhage. Magnetic resonance imaging is a useful technique for imaging haemorrhage in the central nervous system as it yields temporal information about haematoma development, and it is the only non-invasive means of imaging intraspinal haemorrhage. It is, however, limited in the imaging of haematomas within 24 h of onset and in subarachnoid haemorrhage where computed tomography is the investigation of choice.

A case of primary oxalosis and hyperoxaluria in a 16 year old boy is presented. The skeletal appearances in this disorder result from the deposition of oxalate crystals in the bone marrow. Prior to the advent of renal dialysis and transplantation these patients invariably died at a young age.

The radiological findings in five patients with pelvic soft tissue neoplasms directly involving the sacroiliac joint, are described. All patients had Computed Tomography (CT) examinations, two of the patients also having Magnetic Resonance Imaging (MRI). The role of imaging in this uncommon entity is discussed as well as the importance of making this diagnosis, thereby excluding unilateral sacroiliitis. The therapeutic implications of this diagnosis relate to local neural involvement, especially the sciatic nerve, and the fact that involvement of the sacroiliac joint by tumors significantly compromises chances of a successful surgical outcome. The role of MR in this condition is not yet certain, but it may prove to be the method of choice in view of its excellent depiction of skeletal neoplasms.

MRI at 0.3T and CT with myelographic contrast (CTM) were compared in the retrospective evaluation of 35 patients investigated for the development of new neurological symptoms following longstanding spinal cord injury. Compared with MRI, CTM was relatively accurate for the demonstration of spinal cord compression, but failed to identify 23% of patients with spinal cord atrophy, and 43% of patients with post-traumatic syrinx formation. However, 5 patients had unsatisfactory MR imaging, either due to motion or metallic artifact, and in 3 of these, CTM demonstrated a syrinx. Although MRI is the method of choice in the investigation of this problem, CTM may still be required for patients with an unsatisfactory MR examination. Magnetic Resonance (MR) imaging is now an established technique for imaging the spine, with accurate depiction of the spinal cord, as well as the adjacent soft tissues (1, 2). However, the cost of this technique, and its as yet limited availability in Australasia, has resulted in the necessity to demonstrate its superiority over other imaging modalities for any specific clinical problem (3). One of the major areas of impact of MR has been in the investigation of the problem of acute neurological deterioration in patients with past spinal trauma (4, 5, 6). Some of these patients will have treatable causes of deterioration, either a post-traumatic syrinx, or spinal cord compression (6), and MR can be used to image these conditions (7), which, until recently, were investigated with computed tomography with myelographic contrast medium (CTM), (8, 9).(ABSTRACT TRUNCATED AT 250 WORDS)

Magnetic Resonance Imaging (MRI) at 0.3T and Computed Tomography (CT) were compared in the retrospective evaluation of 34 patients with acute spinal cord injury. MRI was highly accurate in the imaging of vertebral body fracture, and spondylitic changes, and is the method of choice for imaging ligament injury, traumatic disc protrusion and spinal cord compression. It was also useful for the identification of subtle subluxations in the sagittal plane. CT remains the method of choice for imaging neural arch fractures. MRI at 0.3T is a valid technique for assessing patients with acute spinal trauma.

Computed tomography (CT) and ultrasound (US) appearances of diffuse and focal fatty infiltration of the liver (FIL) are well recognized. We have recently seen 10 cases with "fat spared" areas in FIL presenting as pseudo tumours of the liver. Characteristic appearances of fat free areas in FIL which help differentiate these areas from other focal liver lesions include (i) location in the medial segment of the left lobe of the liver, (ii) absence of mass effect on surrounding vessels and liver tissue, and (iii) presence of typical changes of FIL elsewhere in the liver on CT or US examination.
