It is common in biomedical research to run case-control studies involving high-dimensional predictors, with the main goal being detection of the sparse subset of predictors having a significant association with disease. Usual analyses rely on independent screening, considering each predictor one at a time, or in some cases on logistic regression assuming no interactions. We propose a fundamentally different approach based on a nonparametric Bayesian low rank tensor factorization model for the retrospective likelihood. Our model allows a very flexible structure in characterizing the distribution of multivariate variables as unknown and without any linear assumptions as in logistic regression. Predictors are excluded only if they have no impact on disease risk, either directly or through interactions with other predictors. Hence, we obtain an omnibus approach for screening for important predictors. Computation relies on an efficient Gibbs sampler. Infants born preterm or small for gestational age have elevated rates of morbidity and mortality. Using birth certificate records in Texas from 2002 to 2004 and Environmental Protection Agency air pollution estimates, we relate the quantile functions of birth weight and gestational age to ozone exposure and multiple predictors, including parental age, race, and education level. We introduce a semi-parametric Bayesian quantile approach that models the full quantile function rather than just a few quantile levels. Our multilevel quantile function model establishes relationships between birth weight and the predictors separately for each week of gestational age and between gestational age and the predictors separately across Texas Public Health Regions. We permit these relationships to vary nonlinearly across gestational age, spatial domain and quantile level and we unite them in a hierarchical model via a basis expansion on the regression coefficients that preserves interpretability. Very low birth weight is a primary concern, so we leverage extreme value theory to supplement our model in the tail of the distribution. Gestational ages are recorded in completed weeks of gestation (integer-valued), so we present methodology for modeling quantile functions of discrete response data. In a simulation study we show that pooling information across gestational age and quantile level substantially reduces MSE of predictor effects. We find that ozone is negatively associated with the lower tail of gestational age in south Texas and across the distribution of birth weight for high gestational ages. A real-time surveillance method is developed with emphasis on rapid and accurate detection of emerging outbreaks. We develop a model with relatively weak assumptions regarding the latent processes generating the observed data, ensuring a robust prediction of the spatiotemporal incidence surface. Estimation occurs via a local linear fitting combined with day-of-week effects, where spatial smoothing is handled by a novel distance metric that adjusts for population density. Detection of emerging outbreaks is carried out via residual analysis. Both daily residuals and AR model-based detrended residuals are used for detecting abnormalities in the data given that either a large daily residual or an increasing temporal trend in the residuals signals a potential outbreak, with the threshold for statistical significance determined using a resampling approach., (© 2011, The International Biometric Society.)

In this article, we develop a latent class model with class probabilities that depend on subject-specific covariates. One of our major goals is to identify important predictors of latent classes. We consider methodology that allows estimation of latent classes while allowing for variable selection uncertainty. We propose a Bayesian variable selection approach and implement a stochastic search Gibbs sampler for posterior computation to obtain model-averaged estimates of quantities of interest such as marginal inclusion probabilities of predictors. Physical activity has many well-documented health benefits for cardiovascular fitness and weight control. For pregnant women, the American College of Obstetricians and Gynecologists currently recommends 30 minutes of moderate exercise on most, if not all, days; however, very few pregnant women achieve this level of activity. Traditionally, studies have focused on examining individual or interpersonal factors to identify predictors of physical activity. There is a renewed interest in whether characteristics of the physical environment in which we live and work may also influence physical activity levels. We consider one of the first studies of pregnant women that examines the impact of characteristics of the built environment on physical activity levels. Using a socioecologic framework, we study the associations between physical activity and several factors including personal characteristics, meteorological/air quality variables, and neighborhood characteristics for pregnant women in four counties of North Carolina. We simultaneously analyze six types of physical activity and investigate cross-dependencies between these activity types. Exploratory analysis suggests that the associations are different in different regions. Therefore, we use a multivariate regression model with spatially varying regression coefficients. This model includes a regression parameter for each covariate at each spatial location. For our data with many predictors, some form of dimension reduction is clearly needed. We introduce a Bayesian variable selection procedure to identify subsets of important variables. Our stochastic search algorithm determines the probabilities that each covariate's effect is null, non-null but constant across space, and spatially varying. We found that individual-level covariates had a greater influence on women's activity levels than neighborhood environmental characteristics, and some individual-level covariates had spatially varying associations with the activity levels of pregnant women., (© 2009, The International Biometric Society.)

Fetal growth restriction is a leading cause of perinatal morbidity and mortality that could be reduced if high-risk infants are identified early in pregnancy. We propose a Bayesian model for aggregating 18 longitudinal ultrasound measurements of fetal size and blood flow into three underlying, continuous latent factors. Our procedure is more flexible than typical latent variable methods in that we relax the normality assumptions by allowing the latent factors to follow finite mixture distributions. Using mixture distributions also permits us to cluster individuals with similar observed characteristics and identify latent classes of subjects who are more likely to be growth or blood flow restricted during pregnancy. We also use our latent variable mixture distribution model to identify a clinically meaningful latent class of subjects with low birth weight and early gestational age. We then examine the association of latent classes of intrauterine growth restriction with latent classes of birth outcomes as well as observed maternal covariates including fetal gender and maternal race, parity, body mass index, and height. Our methods identified a latent class of subjects who have increased blood flow restriction and below average intrauterine size during pregnancy. These subjects were more likely to be growth restricted at birth than a class of individuals with typical size and blood flow.

Summary: Deciding which predictor effects may vary across subjects is a difficult issue. Standard model selection criteria and test procedures are often inappropriate for comparing models with different numbers of random effects due to constraints on the parameter space of the variance components. Testing on the boundary of the parameter space changes the asymptotic distribution of some classical test statistics and causes problems in approximating Bayes factors. We propose a simple approach for testing random effects in the linear mixed model using Bayes factors. We scale each random effect to the residual variance and introduce a parameter that controls the relative contribution of each random effect free of the scale of the data. We integrate out the random effects and the variance components using closed-form solutions. The resulting integrals needed to calculate the Bayes factor are low-dimensional integrals lacking variance components and can be efficiently approximated with Laplace's method. We propose a default prior distribution on the parameter controlling the contribution of each random effect and conduct simulations to show that our method has good properties for model selection problems. Finally, we illustrate our methods on data from a clinical trial of patients with bipolar disorder and on data from an environmental study of water disinfection by-products and male reproductive outcomes.

An individual's health condition can affect the frequency and intensity of episodes that can occur repeatedly and that may be related to an event time of interest. For example, bleeding episodes during pregnancy may indicate problems predictive of preterm delivery. Motivated by this application, we propose a joint model for a multiple episode process and an event time. The frequency of occurrence and severity of the episodes are characterized by a latent variable model, which allows an individual's episode intensity to change dynamically over time. This latent episode intensity is then incorporated as a predictor in a discrete time model for the terminating event. Time-varying coefficients are used to distinguish among effects earlier versus later in gestation. Formulating the model within a Bayesian framework, prior distributions are chosen so that conditional posterior distributions are conjugate after data augmentation. Posterior computation proceeds via an efficient Gibbs sampling algorithm. The methods are illustrated using bleeding episode and gestational length data from a pregnancy study.

Researchers often measure stress using questionnaire data on the occurrence of potentially stress-inducing life events and the strength of reaction to these events, characterized as negative or positive and assigned an ordinal ranking. In studying the health effects of stress, one needs to obtain measures of an individual's negative and positive stress levels to be used as predictors. Motivated by data of this type, we propose a latent variable model, which is characterized by event-specific negative and positive reaction scores. If the positive reaction score dominates the negative reaction score for an event, then the individual's reported response to that event will be positive, with an ordinal ranking determined by the value of the score. Measures of overall positive and negative stress can be obtained by summing the reactivity scores across the events that occur for an individual. By incorporating these measures as predictors in a regression model and fitting the stress and outcome models jointly using Bayesian methods, inferences can be conducted without the need to assume known weights for the different events. We propose an MCMC algorithm for posterior computation and apply the approach to study the effects of stress on preterm delivery.

In studying the relationship between an ordered categorical predictor and an event time, it is standard practice to include dichotomous indicators of the different levels of the predictor in a Cox model. One can then use a multiple degree-of-freedom score or partial likelihood ratio test for hypothesis testing. Often, interest focuses on comparing the null hypothesis of no difference to an order-restricted alternative, such as a monotone increase across levels of a predictor. This article proposes a Bayesian approach for addressing hypotheses of this type. We reparameterize the Cox model in terms of a cumulative product of parameters having conjugate prior densities, consisting of mixtures of point masses at one, and truncated gamma densities. Due to the structure of the model, posterior computation can proceed via a simple and efficient Gibbs sampling algorithm. Posterior probabilities for the global null hypothesis and subhypotheses, comparing the hazards for specific groups, can be calculated directly from the output of a single Gibbs chain. The approach allows for level sets across which a predictor has no effect. Generalizations to multiple predictors are described, and the method is applied to a study of emergency medical treatment for stroke.

We present a method for estimating the parameters in random effects models for survival data when covariates are subject to missingness. Our method is more general than the usual frailty model as it accommodates a wide range of distributions for the random effects, which are included as an offset in the linear predictor in a manner analogous to that used in generalized linear mixed models. We propose using a Monte Carlo EM algorithm along with the Gibbs sampler to obtain parameter estimates. This method is useful in reducing the bias that may be incurred using complete-case methods in this setting. The methodology is applied to data from Eastern Cooperative Oncology Group melanoma clinical trials in which observations were believed to be clustered and several tumor characteristics were not always observed.
