Objective: Non-operative management of abdominal injuries has recently become more common. Especially nonoperative treatment of blunt abdominal trauma is gaining wide acceptance. In this study, the efficacy of nonoperative treatment in abdominal trauma (blunt penetrating) is discussed. Material and Methods: All patients who received treatment due to abdominal trauma from November 2008 to January 2013 were retrospectively analyzed. The demographic characteristics, type of injury, injured organ, type of treatment (operative vs. nonoperative) and mortality data were evaluated. Results: The study includes 115 patients treated for abdominal trauma in our department. The mechanism of trauma was stab wounds in 60%, blunt abdominal trauma in 23.5% and gunshot wounds in 16.5%. Forty-two patients (36.5%) were operated for hemodynamic instability and/or peritonitis on admission. The remaining 63.5% of patients (n=73) were treated nonoperatively, 10 of whom required laparotomy during follow-up. The remaining 63 patients were treated with non-operative management. The success rate for non-operative treatment was 86.3% and there was no difference in terms of the types of injuries. The mortality rate was 4.3% (n= 5) in the whole series, but there were no deaths among the patients who had received non-operative treatment. In the whole patient group 54.2% (n=63) were treated nonoperatively. Conclusion: Nonoperative treatment in abdominal trauma is safe and effective. Patients with clinical stability and normal physical examination findings can be treated nonoperatively with close monitoring.

PubMed ID: 30129492 Objective: To assess visceral adiposity index (VAI) as a sign of cardio-vascular diseases (CVD) in hypothyroidism patients and to compare CVD risks in overt or subclinical hypothyroidism patients. Materials and methods: Sixty-eight hypothyroidism patients (29 with overt and 39 with subclinical hypothyroidism) and 33 age- and gender-matched control patients were included. VAI levels were calculated with the following formula: (waist circumference (WC)/[36.58+(1.89xbody mass index (BMI))])x[(triglyceride (TG) (mmol/L)/0.81)x(1.52xhigh-density lipoprotein cholesterol (HDL-cholesterol) (mmol/L))] and (WC/[39.68+(1.88xBMI)])x[(TG (mmol/L)/1.03)x(1.31xHDL-cholesterol (mmol/L))], respectively. Results: While body weight (p < 0.01), BMI (p < 0.01), TG and VAI levels (p < 0.01) were higher in hypothyroidism patients than controls, HDL-cholesterol levels were lower (p = 0.02). When patients were divided to groups as subclinical (n = 39) and overt hypothyroidism (n = 29) and compared with each other and controls (n = 33), body weight (p = 0.02 and p = 0.02, respectively), BMI (p = 0.01 and p < 0.01, respectively) and TG (p < 0.01 and p = 0.03, respectively) were higher in overt and subclinical hypothyroidism groups than controls. HDL-cholesterol was lower only in the group with overt hypothyroidism than controls (p = 0.01). Although found similar to each other in overt and subclinical hypothyroidism groups, VAI levels were observed to be higher in both groups than controls (p < 0.01 and p = 0.02, respectively). In correlation analysis, a positive correlation was determined between thyroid stimulating hormone (TSH), BMI and VAI levels (p = 0.03 and p < 0.01, respectively). Conclusions: Due to the association between increased VAI levels, and metabolic syndrome and CVDs, we consider that several measures should be promptly taken to decrease these risk factors, and further studies with a larger sample size should be performed. © 2018 National Medical Association

Objectives: The purpose of this study is to compare the adults diagnosed and not diagnosed with anxiety disorder in terms of positive psychotherapy formation. Methods: Participants of the study are 100 people diagnosed with anxiety disorder, and another 100 people who do not have any mental disorder. Positive Psychotherapy and Family Therapy Inventory, and Beck Anxiety Inventory were used to collect data. T-test and multivariate regression analysis were conducted for data analysis. Results: Regarding primary aptitudes, there are significant differences in terms of patience, relationship, love, and faith aptitudes. In study, a significant difference was found in terms of orderliness, cleanliness, kindness, honesty, obedience, justice and loyalty among the secondary and non-diagnosed groups. Diagnosed individuals have been found to use order, cleanliness, reliability, obedience, justice and loyalty more than non-diagnosed individuals. It is seen that individuals who have anxiety disorder are more patient than relatives without diagnosis are and use relational, love and faith ability more. It has been found that for the individuals diag-nosed with anxiety disorder, all of primary aptitudes explains 32% of the total variance and all of secondary aptitudes explains approximately 30% of the total variance. It has been found that for non-diagnosed individuals all of primary aptitudes explains approximately 15% of the total variance and all of secondary aptitudes explains approximately 13% of the total variance. Discussion: Conducting similar studies in the future on individuals who are at different developmental periods, such as adolescence and old age, may be useful. In addition, various intervention programs can be performed using the perspective of positive psychotherapy for anxiety disorder treatment.

Gunumuzde pilonidal sinusun edinsel hastalik oldugu genel kabul gormektedir. Akdeniz ulkelerinde yuksek siklik gosterir (2). Turk askerleri arasinda %8,8 oraninda pilonidal sinus bulunmustur (3). Insan sagligi ve ulkemizde cok uzerinde durulmayan ekonomik yuk ve isgucu kaybi bakimindan %8,8 onemli bir orandir.
