Abstract

Purpose: To identify genetic risk factors contributing to central corneal thickness (CCT) in individuals from South India, a population with a high prevalence of ocular disorders., Methods: One hundred ninety-five individuals from 15 large South Indian pedigrees were genotyped using the Omni2.5 bead array. Family-based association for CCT was conducted using the score test in MERLIN., Results: Genome-wide association study (GWAS) identified strongest association for single nucleotide polymorphisms (SNPs) in the first intron of WNT7B and CCT (top SNP rs9330813; β = -0.57, 95% confidence interval [CI]: -0.78 to -0.36; P = 1.7 × 10-7). We further investigated rs9330813 in a Latino cohort and four independent European cohorts. A meta-analysis of these data sets demonstrated statistically significant association between rs9330813 and CCT (β = -3.94, 95% CI: -5.23 to -2.66; P = 1.7 × 10-9). WNT7B SNPs located in the same genomic region that includes rs9330813 have previously been associated with CCT in Latinos but with other ocular quantitative traits related to myopia (corneal curvature and axial length) in a Japanese population (rs10453441 and rs200329677). To evaluate the specificity of the observed WNT7B association with CCT in the South Indian families, we completed an ocular phenome-wide association study (PheWAS) for the top WNT7B SNPs using 45 ocular traits measured in these same families including corneal curvature and axial length. The ocular PheWAS results indicate that in the South Indian families WNT7B SNPs are primarily associated with CCT., Conclusions: The results indicate robust evidence for association between WNT7B SNPs and CCT in South Indian pedigrees, and suggest that WNT7B SNPs can have population-specific effects on ocular quantitative traits.

41. Factors influencing the output of rural cataract surgical facilities in China: the SHARP study.

Abstract

Purpose: To identify factors associated prospectively with increased cataract surgical rate (CSR) in rural Chinese hospitals., Methods: Annual cataract surgical output was obtained at baseline and 24 months later from operating room records at 42 rural, county-level hospitals. Total local CSR (cases/million population/y), and proportion of CSR from hospital and local competitors were calculated from government records. Hospital administrators completed questionnaires providing demographic and professional information, and annual clinic and outreach screening volume. Independent cataract surgeons provided clinical information and videotapes of cases for grading by two masked experts using the Ophthalmology Surgical Competency Assessment Rubric (OSCAR). Uncorrected vision was recorded for 10 consecutive cataract cases at each facility, and 10 randomly-identified patients completed hospital satisfaction questionnaires. Total value of international nongovernmental development organization (INGDO) investment in the previous three years and demographic information on hospital catchment areas were obtained. Main outcome was 2-year percentage change in hospital CSR., Results: Among the 42 hospitals (median catchment population 530,000, median hospital CSR 643), 78.6% (33/42) were receiving INGDO support. Median change in hospital CSR (interquartile range) was 33.3% (-6.25%, 72.3%). Predictors of greater increase in CSR included higher INGDO investment (P = 0.02, simple model), reducing patient dissatisfaction (P = 0.03, simple model), and more outreach patient screening (P = 0.002, simple and multiple model)., Conclusions: Outreach cataract screening was the strongest predictor of increased surgical output. Government and INGDO investment in screening may be most likely to enhance output of county hospitals, a major goal of China's Blindness Prevention Plan., (Copyright 2015 The Association for Research in Vision and Ophthalmology, Inc.)

Abstract

Purpose: To assess the performance of two approaches to visual acuity testing in a group of nursing home residents with cognitive impairment. The study was a cross-sectional comparison of the effectiveness of two tests of visual acuity., Methods: Nursing home residents participating in a clinical trial were tested with both recognition acuity charts and grating acuity cards (Teller) by masked observers., Results: Of the nursing home residents (n = 656) who participated in the study, 86% could respond to visual acuity testing in at least one eye. Eighty-four percent were testable using Teller cards versus 73% who were testable by Early-Treatment Diabetic Retinopathy Study (ETDRS) charts or Lea symbol charts. Forty-one percent of individuals with MiniMental Status Examination (MMSE) scores lower than 10 were testable by recognition acuity, whereas 61% were testable with grating acuity cards. Grating acuity correlated well with recognition acuity (R = 0.79; 95% CI, 0.75-0.98, intraclass correlation coefficient [ICC]). The correlation was slightly lower in individuals with decreased MMSE scores. Although grating acuity was one line better than recognition acuity on average and median acuities were the same, 24% of individuals had results that differed by three or more lines., Conclusions: Teller acuity cards can effectively test the vision in cognitively impaired individuals who are not testable by conventional means. Grating acuity results correlated well with those of recognition acuity, although differences of three or more lines were not uncommon. Wider use of grating acuity testing allows a more complete assessment of visual function in the cognitively impaired elderly.

Abstract

Purpose: To determine which measures of the binocular visual field correlate best with the patient's assessment of vision., Methods: Esterman binocular visual field testing and four other binocular visual field tests (designated peripheral 20 dB [p20], peripheral 22 dB [p22], central 24 dB [c24] and central 26 dB [c26]) were performed in 101 patients with glaucoma or suspected glaucoma. Scores from these five tests, as well as binocular visual field scores calculated from monocular testing (best-location summation and probability summation), were correlated with performance on the National Eye Institute's Visual Function Questionnaire (VFQ)-25 and Short-Form (SF)-36 quality of life instruments, as well as with the linear rating scale utility test., Results: The mean percentage of correct responses was 87%, 69%, 59%, 78%, and 71% for the Esterman, p20, p22, c24, and c26 tests, respectively. The distribution of scores was much broader for the p20 and p22 tests than for the Esterman test. The mean decibels for the binocular visual fields calculated from the monocular visual fields were 21.5 +/- 7.7 dB for the best-location algorithm and 25.1 +/- 6.7 dB for the probability-summation algorithm. The binocular visual field score calculated with the best-location algorithm correlated better with the overall, general vision, distance activities, and peripheral vision domains of theVFQ-25 (partial correlation coefficients of 0.48, 0.48, 0.49, and 0.51, respectively) than did the probability-summation algorithm and all five binocular visual field tests. The best-location algorithm also had the strongest correlation with the linear rating scale utility test (partial correlation coefficient, 0.40)., Conclusions: In this sample of clinic-based patients with glaucoma or suspected glaucoma, a global score derived from a combination of two monocular fields correlated better with patient assessment of vision than did the Esterman and four novel binocular visual field tests.
