The presence of a femoral hip stem changes local mechanical signals inside the surrounding bone. In this study we examined the hypothesis that the eventual loss of bone can be estimated from the initial patterns of elastic energy deviation, as determined in FE models of the intact bone and the operated femur. For that purpose two hypothetical relations between elastic energy reduction and resorption were investigated. Their estimates of bone loss were compared to the results of iterative computer simulations. Two kinds of FE model were used, and in each stem stiffness and remodeling threshold (a measure of 'biological reactivity') were varied. Provided that reasonable values of the remodeling threshold are assumed and that the stem is firmly bonded to the bone, we found that the difference between direct estimates and simulation models was 4 percent of bone loss. It is therefore concluded that initial patterns of elastic energy deviation give a reasonable indication of expected bone loss.

The designer of a cementless hip stem in total hip replacement must find a balance between two conflicting demands. On the one hand, a stiff stem shields the surrounding bone from mechanical loading (stress shielding), which may lead to bone loss, particularly around the proximal part of the stem. Reducing the stem stiffness decreases the amount of stress shielding and hence the amount of bone loss. However, this measure inevitably promotes higher proximal interface stresses and thereby increases the risk of proximal interface failure. The designer's task therefore is to optimize the stem stiffness in order to find the best compromise in the conflict. Yet, a better compromise might be found when the stem material was nonhomogeneous, in other words when an arbitrary distribution of the elastic properties inside the stem was allowed. The number of conceivable designs would increase enormously, making the designer's task almost impossible. In the present paper, we develop a numerical design optimization method to determine the optimal stiffness characteristics for a hip stem. A finite element program is coupled with a numerical optimization method, thus producing a design optimization scheme. The scheme minimizes the probability for interface failure while limiting the amount of bone loss, by adapting the parameters describing the nonhomogeneous elastic modulus distribution. As an example, a simplified model of a hip stem is made, whose modulus distribution is optimized. Assuming equal long-term bone loss, the maximum interface stress can be reduced by over 50 percent when compared to a homogeneous flexible stem, thus demonstrating the value of the new approach.

Damage accumulation in living tissues occurs when the rate of damage formation is greater than the rate of damage repair. For very large increases in the loading rate of bones, this can result in 'stress fractures' due to the growth and coalescence of fatigue related microdamage. At lower increases of loading rates, the damage accumulation process is halted because there is time for adaptive bone remodeling to occur in response to the new load. However, it is not known if there is a relationship between microdamage and bone remodeling per se. One hypothesis for the control of bone remodeling is that osteocytes sense strains and mediate osteoblastic and osteoclastic activity. The purpose of this study was to investigate whether damage generates strains which may trigger bone remodeling. If this were true, then accumulative damage would cause adaptive bone remodeling. This study applies the methods of finite element analysis to determine the effect of observed damage mechanisms on the proposed sensors of remodeling in Haversian bone. Individual lamellae are modeled and osteocyte-lacunae are included in a generalized plane strain geometric representation. It is predicted that microdamage alters the local deformation behavior around lacunae, and that the changes increase as microdamage accumulates. Hence, if damage accumulates in a bone, it could be sensed as a change in strain at a microstructural level. The results give theoretical support to the experimental studies that have shown a correlation between microdamage and the initiation of resorption as a first step in bone remodeling.

Due to both its shape and its structural architecture, the mechanics of the pelvic bone are complex. In Finite Element (FE) models, these aspects have often been (over) simplified, sometimes leading to conclusions which did not bear out in reality. The purpose of this study was to develop a more realistic FE model of the pelvic bone. This not only implies that the model has to be three-dimensional, but also that the thickness of the conical shell and the density distribution of the trabecular bone throughout the pelvic bone have to be incorporated in the model in a realistic way. For this purpose, quantitative measurements were performed on computer tomography scans of several pelvic bones, after which the measured quantities were allocated to each element of the mesh individually. To validate this FE model, two fresh pelvic bones were fitted with strain gages and loaded in a testing machine. Stresses calculated from the strain data of this experiment were compared to the results of a simulation with the developed pelvic FE model.

Bone atrophy caused by stress-shielding may cause serious complications for the long-term fixation of hip stems. In particular, uncemented total hip arthroplasty is threatened by this problem, because the stems are usually larger and, as a consequence, stiffer than those of cemented implants. In the present study, the effects of fit and bonding characteristics of femoral hip stems were investigated, using the (nonlinear) finite element method in combination with adaptive bone remodeling theory to predict the bone density distribution in a bone or bone/implant configuration. Unknown parameters used in the theory, such as a reference equilibrium loading stimulus and a threshold (dead) zone of this stimulus, were established (triggered) by using the method to predict the density distributions in the natural femur and around fully coated uncemented implants. The computer simulation method can provide long term predictions of remodeling patterns around various implant configurations. Several cases were analyzed, whereby the coating conditions (fully, partly, or noncoated) and the fit characteristics (press fitted or overreamed) were varied. The computer predictions showed that partly coating can only significantly reduce bone atrophy relative to fully coated stems, when the coating is applied at a small region at the utmost proximal part of the stem. For smooth press-fit stems the predicted amount of bone loss (35 percent in the proximal medial region) was less than for a one-third proximally coated or a fully coated stem (50 to 54 percent predicted bone loss in the proximal medial region). The results showed that overreaming the femoral canal in the press fit case can have important effects. Distal overreaming gave reduced proximal atrophy. Proximal overreaming (or undersizing) resulted in a distal jam of the stem, a proximal 'stress-bypass,' and dramatic proximal bone loss (up to 90 percent).

The introduction of finite element analysis (FEA) into orthopaedic biomechanics allowed continuum structural analysis of bone and bone-implant composites of complicated shapes (Huiskes and Chao, J. Biomechanics, Vol. 16, 1983, pp. 385-409). However, besides having complicated shapes, musculoskeletal tissues are hierarchical composites with multiple structural levels that adapt to their mechanical environment. Mechanical adaptation influences the success of many orthopaedic treatments, especially total joint replacements. Recent advances in FEA applications have begun to address questions concerning the optimality of bone structure, the processes of bone remodeling, the mechanics of soft hydrated tissues, and the mechanics of tissues down to the microstructural and cell levels. Advances in each of these areas, which have brought FEA from a continuum stress analysis tool to a tool which plays an ever-increasing role in the scientific understanding of tissue structure, adaptation, and the optimal design of orthopaedic implants, are reviewed.

In mathematical knee-joint models, the ligaments are usually represented by straight-line elements, connecting the insertions of the femur and tibia. Such a model may not be valid if a ligament is bent in its course over bony-surfaces, particularly not if the resulting redirection of the ligament force has a considerable effect on the laxity or motion characteristics of the knee-joint model. In the present study, a model for wrapping of a ligament around bone was incorporated in a three-dimensional mathematical model of the human knee. The bony edge was described by a curved line on which the contact point of the line element representing a ligament bundle was located. Frictionless contact between the ligament bundle and the bone was assumed. This model was applied to the medial collateral ligament (MCL) interacting with the bony edge of the tibia. It was found that, in comparison with the original model without bony interactions, the bony edge redirected the ligament force of the MCL in such a way that it counterbalanced valgus moments on the tibia more effectively. The effect of the bony interaction with the MCL on the internal-external rotation laxity, however, was negligible.

On the basis of earlier reported data on the in vitro kinematics of passive knee-joint motions of four knee specimens, the length changes of ligament fiber bundles were determined by using the points of insertion on the tibia and femur. The kinematic data and the insertions of the ligaments were obtained by using Roentgenstereophotogrammetry. Different fiber bundles of the anterior and posterior cruciate ligaments and the medial and lateral collateral ligaments were identified. On the basis of an assumption for the maximal strain of each ligament fiber bundle during the experiments, the minimal recruitment length and the probability of recruitment were defined and determined. The motions covered the range from extension to 95 degrees flexion and the loading conditions included internal or external moments of 3 Nm and anterior or posterior forces of 30 N. The ligament length and recruitment patterns were found to be consistent for some ligament bundles and less consistent for other ligament bundles. The most posterior bundle of each ligament was recruited in extension and the lower flexion angles, whereas the anterior bundle was recruited for the higher flexion angles. External rotation generally recruited the collateral ligaments, while internal rotation recruited the cruciate ligaments. However, the anterior bundle of the posterior cruciate ligament was recruited with external rotation at the higher flexion angles. At the lower flexion angles, the anterior cruciate and the lateral collateral ligaments were recruited with an anterior force. The recruitment of the posterior cruciate ligament with a posterior force showed that neither its most anterior nor its most posterior bundle was recruited at the lower flexion angles. Hence, the posterior restraint must have been provided by the intermediate fiber bundles, which were not considered in the experiment. At the higher flexion angles, the anterior bundles of the anterior cruciate ligament and the posterior cruciate ligament were found to be recruited with anterior and posterior forces, respectively. The minimal recruitment length and the recruitment probability of ligament fiber bundles are useful parameters for the evaluation of ligament length changes in those experiments where no other method can be used to determine the zero strain lengths, ligament strains and tensions.

In biomechanical joint-motion analyses, the continuous motion to be studied is often approximated by a sequence of finite displacements, and the Finite Helical Axis (FHA) or "screw axis" for each displacement is estimated from position measurements on a number of anatomical or artificial landmarks. When FHA parameters are directly determined from raw (noisy) displacement data, both the position and the direction of the FHA are ill-determined, in particular when the sequential displacement steps are small. This implies, that under certain conditions, the continuous pathways of joint motions cannot be adequately described. The purpose of the present experimental study is to investigate the applicability of smoothing (or filtering) techniques, in those cases where FHA parameters are ill-determined. Two different quintic-spline smoothing methods were used to analyze the motion data obtained with Roentgenstereophotogrammetry in two experiments. One concerning carpal motions in a wrist-joint specimen, and one relative to a kinematic laboratory model, in which the axis positions are a priori known. The smoothed and non-smoothed FHA parameter errors were compared. The influences of the number of samples and the size of the sampling interval (displacement step) were investigated, as were the effects of equidistant and nonequidistant sampling conditions and noise invariance.
