Background: Fetal MRI is increasingly performed at 3 T. Nonetheless, safety concerns persist regarding potential increased risk of intrauterine growth restriction from in-utero 3-T MRI exposure. Objective: To compare neonatal anthropometric measurements between newborns who underwent 3-T fetal MRI, newborns who underwent 1.5-T fetal MRI, and newborns without in utero MRI exposure. Methods: This single-center retrospective study included gravid patients who underwent fetal ultrasound and possible 1.5-T or 3-T fetal MRI within 10 days from January 2017 to January 2022. For each included patient who also underwent 3-T fetal MRI, one matched patient who also underwent 1.5-T MRI and two matched patients without in-utero MRI exposure, were randomly selected. Matching was based on gestational age per the fetal ultrasound. Neonatal anthropometric characteristics were compared among groups. Results: The final sample included 416 patients (mean age, 32±5 years), 104 in the 3-T MRI group, 104 in the 1.5-T MRI group, and 208 in the MRI-unexposed group. Mean gestational age at the time of fetal ultrasound used for matching was 27 weeks 2 days in the 3-T group, 25 weeks 2 days in the 1.5-T group, and 26 weeks 0 days in the MRI-unexposed group (p=.07). The distribution of indications for fetal MRI was not significantly different between the 3-T and 1.5-T groups (p=.62). Mean gestational age at delivery was 37 weeks 5 days in the 3-T group, 38 weeks 0 day in the 1.5-T group, and 38 weeks 2 days in the unexposed group (p=.51). No significant difference (p=.09) was observed among groups in mean neonatal weight (3-T: 3120±753 g; 1.5-T: 3104±704 g; unexposed: 2967±614 g); neonatal weight percentile (3-T: 45±27; 1.5-T: 42±26; MRI: 41±24); neonatal head circumference (3-T: 34±3 cm; 1.5-T: 34±3 cm; unexposed: 34±2 cm), or neonatal head circumference percentile (3-T: 48±29; 1.5-T: 42±23; unexposed: 43±30). Conclusion: There were no significant differences in neonatal anthropometric measurements among newborns who underwent in-utero 3-T MRI, newborns who underwent in-utero 1.5-T MRI, and newborns without in-utero MRI exposure. Clinical Impact: The results support the safety of 3-T MRI with respect to growth of the developing fetus.

BACKGROUND. Anesthetic exposure in children may impact long-term neurocognitive outcomes. Therefore, minimizing pediatric MRI scan time in children under anesthesia and the associated anesthetic exposure is necessary. OBJECTIVE. The purpose of this study was to evaluate pediatric MRI scan time as a predictor of total propofol dose, considering imaging and clinical characteristics as covariates. METHODS. Electronic health records were retrospectively searched to identify MRI examinations performed from 2016 to 2019 in patients 0-18 years old who received propofol anesthetic. Brain; brain and spine; brain and abdomen; and brain, head, and neck MRI examinations were included. Demographic, clinical, and imaging data were extracted for each examination, including anesthesia maintenance phase time, MRI scan time, and normalized propofol dose. MRI scan time and propofol dose were compared between groups using a t test. A multiple linear regression with backward selection (threshold, p < .05) was used to evaluate MRI scan time as a predictor of total propofol dose, adjusting for sex, age, time between scan and study end, body part, American Society of Anesthesiologists (ASA) classification, diagnosis, magnet strength, and IV contrast medium administration as covariates. RESULTS. A total of 501 examinations performed in 426 patients (172 girls, 254 boys; mean age, 6.55 ± 4.59 [SD] years) were included. Single body part examinations were shorter than multiple body part examinations (mean, 52.7 ± 18.4 vs 89.3 ± 26.4 minutes) and required less propofol (mean, 17.7 ± 5.7 vs 26.1 ± 7.7 mg/kg; all p < .001). Among single body part examinations, a higher ASA classification, oncologic diagnosis, 1.5-T magnet, and IV contrast medium administration were associated with longer MRI scan times (all p ≤ .009) and higher propofol exposure (all p ≤ .005). In multivariable analysis, greater propofol exposure was predicted by MRI scan time (mean dose per minute of examination, 0.178 mg/kg; 95% CI, 0.155-0.200; p < .001), multiple body part examination ( p = .04), and IV contrast medium administration ( p = .048); lower exposure was predicted by 3-T magnet ( p = .04). CONCLUSION. Anesthetic exposure during pediatric MRI can be quantified and predicted based on imaging and clinical variables. CLINICAL IMPACT. This study serves as a valuable baseline for future efforts to reduce anesthetic doses and scan times in pediatric MRI.

OBJECTIVE. Whole-body MRI is a valuable tool in the surveillance of cancer predisposition syndromes (CPSs). Because it allows wide-FOV imaging without ionizing radiation, whole-body MRI is ideal in pediatric patients, enabling efficient assessment of different organ systems for multifocal disease. This article summarizes the use of whole-body MRI in pediatric patients with CPSs for earlier detection of malignancy, provides evidence where available, and offers guidance where lacking because of the rarity of CPSs. Protocol modifications and technique performance in specific CPSs are also considered. CONCLUSION. Whole-body MRI is the preferred imaging modality for surveillance of pediatric patients with CPSs, and the growing literature supports its importance in presymptomatic cancer detection.

OBJECTIVE. The purpose of this study was to assess if dual-source dual-energy CT (DS-DECT) can be used with lower radiation doses and contrast material volumes than single-energy CT (SECT) in children and young adults. MATERIALS AND METHODS. This retrospective study included 85 consecutive children and young adults (age range, 1 month old to 19 years old; 81 male, 70 female) who underwent contrast-enhanced DS-DECT of the chest ( n = 41) or the abdomen and pelvis ( n = 44) on second- or third-generation dual-source CT scanners (Somatom Flash or Force, Siemens Healthineers) for clinically indicated reasons. We included 66 age-, sex-, body region-, and weight-matched patients who underwent SECT on the same scanner. Patients were scanned with either SECT (with automatic exposure control using both CARE kV [Siemens Healthineers] and CARE Dose 4D [Siemens Healthineers]) or DS-DECT (with CARE Dose 4D). Two pediatric radiologists assessed clinical indications, radiologic findings, image quality, and any study limitations (noise or artifacts). Patient demographics (age, sex, weight), scan parameters (tube voltage, tube current-time product, pitch, section thickness), CT dose descriptors (volume CT dose index, dose-length product, size-specific dose estimate [SSDE]), and contrast material volume were recorded. Descriptive statistics, paired t test, and Cohen kappa test were performed. RESULTS. Mean patient ages and weights ± SD in DS-DECT (10 ± 6 years old, 38 ± 23 kg) and SECT (11 ± 7 years old, 43 ± 29 kg) groups were not significantly different ( p > 0.05). Respective SSDEs for chest DS-DECT (4.0 ± 2.1 mGy), chest SECT (6.1 ± 4.4 mGy), abdomen-pelvis DS-DECT (5.0 ± 5.0 mGy), and abdomen-pelvis SECT (8.3 ± 4.0 mGy) were significantly different ( p = 0.003-0.005). Contrast material volume for DS-DECT examinations was 19-22% lower compared with the weight- and body region-matched scans obtained with SECT. Image quality of DECT was acceptable in all patients. CONCLUSION. In children and young adults, chest and abdomen-pelvis DS-DECT enables substantial radiation dose and contrast volume reductions compared with weight- and region-matched SECT.

OBJECTIVE. The objective of this article is to review strategies to reduce the use of gadolinium-based contrast agents for pediatric abdominal MRI. CONCLUSION. Alternative contrast agents that do not contain gadolinium and unenhanced pediatric abdominal MRI protocols have shown clinical utility. Sequences such as DWI and new multicontrast MRI pulse sequences offer promise for tissue characterization without IV contrast agents. Patients requiring repeat MRI to evaluate for change in focal disease can be monitored with unenhanced abdominal MRI.

Objective: The purpose of this study was to investigate the MR enterographic findings that best correlate with mucosal healing assessed with ileocolonoscopy., Materials and Methods: Patients with Crohn disease who underwent two ileocolonoscopic examinations and also underwent MR enterography close in time to the second endoscopic examination were included in a retrospective study. Two pediatric gastroenterologists blinded to the imaging findings reviewed the endoscopic examinations to assess for mucosal healing, defined as resolution of inflammation within a bowel segment at subsequent ileocolonoscopy. Two radiologists blinded to endoscopic and clinical data interpreted the MR enterographic images. Sensitivity, specificity, and accuracy for mucosal healing were calculated for several imaging features., Results: A total of 30 patients (15 female patients, 15 male patients; age range, 8-24 years; mean, 17.2 ± 3.2 years) with pediatric-onset Crohn disease were examined. The average time between MR enterography and the second ileocolonoscopic examination was 12.7 ± 7.9 days. A total of 202 bowel segments from the terminal ileum to rectum were evaluated in the 60 ileocolonoscopic examinations. Forty-four bowel segments exhibited mucosal healing, and 37 segments exhibited persistent inflammation. At imaging, the MR index of activity score in mucosal healing segments was 6.6 ± 3.4, compared with 13.7 ± 9.7 in segments without mucosal healing (p = 0.0001). The average bowel wall thickness in healing segments was 2.7 ± 0.9 mm compared with 4.7 ± 3.1 mm in persistently inflamed segments (p = 0.0004). An MR index of activity score less than 8 had the highest accuracy for mucosal healing (accuracy, 74%; sensitivity, 84%; specificity, 62%; p < 0.0001). Mucosal hyperenhancement (72%, 98%, 41%), mesenteric hypervascularity (72%, 98%, 41%), bowel wall edema (72%, 93%, 46%), and bowel wall thickness less than 4 mm (72%, 84%, 57%) were also strongly associated with mucosal healing (p < 0.0003)., Conclusion: In this study MR enterography was accurate for assessing mucosal healing, an important therapeutic endpoint in pediatric patients with Crohn disease.

Objective: Splenic lesions are commonly encountered and are often incidental in nature. Benign splenic vascular neoplasms include hemangioma, hamartoma, lymphangioma, extra-medullary hematopoiesis (EMH), and sclerosing angiomatoid nodular transformation (SANT). Uncommonly encountered entities of the spleen include focal EMH, focal myeloma, angiomyolipoma, and SANT. Primary splenic angiosarcoma is the most common malignant nonhematolymphoid malignancy of the spleen. Lymphoma, myeloma, and metastases are the other malignant entities involving the spleen. The clinical presentation, key imaging findings, and associations of benign, neoplastic, and malignant diseases that can involve the spleen will be discussed., Conclusion: Radiologists can use multimodality imaging to diagnose entities involving the spleen by recognizing key imaging features and considering patient characteristics. However, biopsy may be warranted for definitive diagnosis when imaging findings are nonspecific.

Objective: The purpose of this study was to classify noncystic splenic lesions detected on imaging in young patients (0-30 years) and to determine the optimal imaging workup for such lesions., Materials and Methods: This study was conducted at three academic institutions by performing a database search of radiology reports (2002-2011) to identify patients with noncystic splenic lesions. Medical records were then searched to identify radiology examination indications, clinical follow-up, and lesion changes on subsequent imaging. All lesions had either definitive diagnosis (histopathology or laboratory results consistent with infectious cause) or lesion stability more than 2 years consistent with a benign cause., Results: Benign (n = 32), benign indeterminate (n = 7), and malignant (n = 14) lesions were identified in 53 patients (26 males and 27 females; mean age, 19 years; age range, 1 month-30 years). Lesions were initially detected on the following imaging modalities: CT (n = 27), ultrasound (n = 12), MRI (n = 6), and PET/CT (n = 8). A total of 14 patients underwent MRI for lesion characterization, and 12 underwent PET/CT. MRI permitted definitive characterization of benign lesions in 10 of 14 (72%) patients, whereas PET/CT was used to diagnose nine of nine (100%) malignant splenic lesions and helped exclude malignancy in two of three benign lesions., Conclusion: Contrast-enhanced MRI is recommended for imaging workup of noncystic splenic lesions discovered in young patients because it can enable definitive diagnosis of most benign lesions. Lesions with indeterminate MRI features can be followed-up with ultrasound or CT. PET or PET/CT is recommended for patients with clinical evidence of malignancy but is less helpful for characterization of isolated splenic lesions.

Objective: The objective of our study was to determine whether voiding cystourethrography (VCUG) fluoroscopy time is related to the training level of the performing radiologist., Materials and Methods: VCUG reports with normal findings from 2008 to 2011 at one institution were retrospectively reviewed. The average fluoroscopy time was calculated for first-year radiology residents, senior radiology residents, pediatric radiology fellows, and attending pediatric radiologists. The average fluoroscopy time was also calculated for radiologist sex, patient sex, and patient age group. The analysis of variance was used to evaluate differences in average fluoroscopy times between training levels of radiologists, patient age groups, and patient sexes., Results: We reviewed 784 VCUG reports with normal findings: 340 (43.4%) were performed by first-year residents; 181 (23%), by senior residents; 161 (20.5%), by fellows; and 102 (13%), by attending pediatric radiologists. The overall average fluoroscopy time was 1.86 minutes (SD ± 0.98). The attending pediatric radiologists had the shortest average fluoroscopy time (1.63 ± 0.92 minutes), significantly shorter than senior residents (1.96 ± 1.09 minutes; p = 0.0070) and fellows (1.91 ± 0.85 minutes; p = 0.0255). There was no significant difference between attending radiologists and first-year residents (1.85 ± 1.00 minutes; p = 0.0550). The male-to-female ratio of radiologists was 54% versus 46%, with identical average fluoroscopy times: male radiologists, 1.86 ± 1.05 minutes; female radiologists, 1.86 ± 0.90 minutes. There was no significant difference in average fluoroscopy times among patient age groups: 1.93, 1.76, and 1.78 minutes, respectively, for groups A (0-1 years), B (> 1 to ≤ 5 years), and C (> 5 years) (p = 0.1750, 0.4605, 0.6303). The average fluoroscopy time for male patients (2.02 ± 1.00 minutes) was significantly longer than that for female patients (1.71 ± 0.95 minutes; p < 0.0001)., Conclusion: Attending pediatric radiologists have the shortest fluoroscopy time; the differences between their average time compared with the average times of pediatric radiology fellows and of senior radiology residents were statistically significant. The average fluoroscopy time is longer for male patients than for female patients.

Objective: The goal of this review is to examine the current imaging literature and develop basic imaging guidelines for evaluation of children with inflammatory bowel disease (IBD). The three following typical clinical scenarios in the imaging evaluation of IBD are considered: patient with an initial diagnosis of suspected IBD, the goals being to determine disease extent and severity and to differentiate Crohn disease from ulcerative colitis; patient with known IBD presenting with new acute symptoms (fever, peritonitis, leukocytosis) requiring urgent evaluation; and patient with known IBD presenting with nonacute symptomatic recurrence (abdominal pain, diarrhea), the goals being to assess the efficacy of the current treatment and to evaluate the possible need for additional medical or surgical intervention., Conclusion: Imaging of pediatric patients with IBD must balance considerations of diagnostic accuracy against concerns about patient exposure to ionizing radiation and tolerance of the imaging technique. The imaging modality chosen depends on the clinical presentation and expected pathologic finding.

Objective: The objectives of this study were prospective evaluation of MR enterographic accuracy for detecting Crohn disease imaging features in pediatric patients, compared with a CT reference standard, as well as determination of MR enterographic accuracy for detecting active bowel inflammation and fibrosis using a histologic reference standard., Subjects and Methods: The study group for this blinded prospective study included 21 pediatric subjects with known Crohn disease scheduled for clinical CT and histologic bowel sampling for symptomatic exacerbation. All subjects and their parents gave informed consent to also undergo MR enterography. CT and MR enterography examinations were independently reviewed by two radiologists and were scored for Crohn disease features. All bowel histology specimens were reviewed by a single pathologist for the presence of active mucosal inflammation and mural fibrosis, followed by correlation of imaging and histologic findings., Results: All 21 subjects underwent MR enterography and histologic sampling, 18 of whom also underwent CT. MR enterography had high sensitivity for detecting Crohn disease imaging features (e.g., bowel wall thickening, mesenteric inflammation, lymphadenopathy, fistula, and abscess) compared with CT, with individual sensitivity values ranging from 85.1% to 100%. Of a total of 53 abnormal bowel segments with correlation of MRI and histologic findings, MR enterography showed 86.7% accuracy (90.0% sensitivity and 82.6% specificity) for detecting active inflammation (p < 0.001). The accuracy of MR enterography for detecting mural fibrosis overall was 64.9%, compared with histology, but increased to 83.3% (p < 0.05) for detecting fibrosis without superimposed active inflammation., Conclusion: MR enterography can substitute for CT as the first-line imaging modality in pediatric patients with Crohn disease, on the basis of its ability to detect intestinal pathologic abnormalities in both small and large bowel as well as extraintestinal disease manifestations. Additionally, MR enterography provides an accurate noninvasive assessment of Crohn disease activity and mural fibrosis and can aid in formulating treatment strategies for symptomatic patients and assessing therapy response.

Objective: The purposes of this study were to determine the frequency with which percutaneous abdominopelvic abscess drainage catheters must be replaced because of inadequate drainage, to assess the effect of percutaneous catheter exchange on clinical outcome, and to determine the predictors of clinical success after catheter exchange., Materials and Methods: A database of interventional radiology procedures performed at a single tertiary care hospital from 2001 to 2006 was searched to identify the cases of patients who underwent percutaneous catheter drainage of abdominal or pelvic abscesses that was followed by exchange of the drainage catheter at a later date. The electronic medical records and imaging studies of these patients were retrospectively reviewed to determine the abscess characteristics, details of drainage catheter manipulation, and clinical outcome., Results: Among the 3,027 percutaneous abscess drainage catheters placed, 82 were exchanged because of lack of improvement (imaging evidence of undrained fluid and persistent fever and leukocytosis), for an overall frequency of catheter exchange of 2.7% of abscesses in 3.7% of patients. The success rate of catheter replacement, defined as resolution of the fluid collection without open surgical drainage, was 76.8% (63/82). Prognostic factors favorably influencing the clinical success of catheter exchange included a larger number of drainage catheter sideholes, absence of a fistula, low residual abscess volume after initial catheter drainage, and low CT attenuation of abscess fluid., Conclusion: In cases of persistent abscess despite adequate catheter placement, percutaneous catheter exchange over a wire is useful for salvage after the large majority of primary failures. In addition, performance of CT and fluoroscopic catheter injection before catheter exchange facilitates recognition of the predictors of a likely response to catheter exchange.
