Objective: The objective of this study was to assess the efficacy of a newly developed warm-up programme ('11+ Kids') regarding its potential to reduce injuries in children's football., Methods: Children's football teams (under 9 years, under 11 years, and under 13 years age groups) from Switzerland, Germany, the Czech Republic and the Netherlands were invited. Clubs were randomised to an intervention group and a control group, and followed for one season. The intervention group replaced their usual warm-up by '11+ Kids', while the control group warmed up as usual. The primary outcome was the overall risk of football-related injuries. Secondary outcomes were the risks of severe and lower extremity injuries. We calculated hazard ratios using extended Cox models, and performed a compliance analysis., Results: In total, 292,749 h of football exposure of 3895 players were recorded. The mean age of players was 10.8 (standard deviation 1.4) years. During the study period, 374 (intervention group = 139; control group = 235) injuries occurred. The overall injury rate in the intervention group was reduced by 48% compared with the control group (hazard ratio 0.52; 95% confidence interval 0.32-0.86). Severe (74% reduction, hazard ratio 0.26; 95% confidence interval 0.10-0.64) and lower extremity injuries (55% reduction, hazard ratio 0.45; 95% confidence interval 0.24-0.84) were also reduced. Injury incidence decreased with increasing compliance., Conclusion: '11+ Kids' is efficacious in reducing injuries in children's football. We observed considerable effects for overall, severe and lower extremity injuries. The programme should be performed at least once per week to profit from an injury preventive effect. However, two sessions per week can be recommended to further increase the protective benefit., Trial Registration: ClinicalTrials.gov identifier: NCT02222025.

Background: Mindfulness as a present-oriented form of mental training affects cognitive processes and is increasingly considered meaningful for sport psychological training approaches. However, few intervention studies have examined the effects of mindfulness practice on physiological and psychological performance surrogates or on performance outcomes in sports., Objective: The aim of the present meta-analytical review was to examine the effects of mindfulness practice or mindfulness-based interventions on physiological and psychological performance surrogates and on performance outcomes in sports in athletes over 15 years of age., Data Sources: A structured literature search was conducted in six electronic databases (CINAHL, EMBASE, ISI Web of Knowledge, PsycINFO, MEDLINE and SPORTDiscus). The following search terms were used with Boolean conjunction: (mindful* OR meditat* OR yoga) AND (sport* OR train* OR exercis* OR intervent* OR perform* OR capacity OR skill*) AND (health* OR adult* OR athlete*)., Study Selection: Randomized and non-randomized controlled studies that compared mindfulness practice techniques as an intervention with an inactive control or a control that followed another psychological training program in healthy sportive participants were screened for eligibility., Data Extraction: Eligibility and study quality [Physiotherapy Evidence Database (PEDro)] scales were independently assessed by two researchers. A third independent researcher was consulted to achieve final consensus in case of disagreement between both researchers. Standardized mean differences (SMDs) were calculated as weighted Hedges' g and served as the main outcomes in comparing mindfulness practice versus control. Statistical analyses were conducted using a random-effects inverse-variance model., Results: Nine trials of fair study quality (mean PEDro score 5.4, standard deviation 1.1) with 290 healthy sportive participants (athletics, cyclists, dart throwers, hammer throwers, hockey players, hurdlers, judo fighters, rugby players, middle-distance runners, long-distance runners, shooters, sprinters, volleyball players) were included. Intervention time varied from 4 weeks to over 2 years. The practice frequency lasted from twice daily to just once a week, and the mean session time covered 50-60 min. In favor of mindfulness practice compared with the control condition, large effects with narrow confidence limits and low heterogeneity were found for mindfulness scores [SMD 1.03, 90% confidence interval (CI) 0.67-1.40, p < 0.001, I 2  = 17%]. Physiological performance indices depicted wide confidence limits accompanied with very large heterogeneity. However, the effect sizes remained very large, with confidence limits that did not overlap zero (SMD 3.62, 90% CI 0.03-7.21, p = 0.10, I 2  = 98%). Moderate to large effects were observed for both psychological performance surrogates (SMD 0.72, 90% CI 0.46-0.98, p < 0.001, I 2  = 14%) and performance outcomes in shooting and dart throwing (SMD 1.35, 90% CI 0.61-2.09, p = 0.003, I 2  = 82%)., Conclusions: Mindfulness practice consistently and beneficially modulates mindfulness scores. Furthermore, physiological and psychological surrogates improved to a meaningful extent following mindfulness practice, as well as performance outcomes in shooting and dart throwing. It seems reasonable to consider mindfulness practice strategies as a regular complementary mental skills training approach for athletes, at least in precision sports; however, more high-quality, randomized, controlled trials on mindfulness practice and performance improvements in diverse sport settings are needed.

Background: Adequate static and dynamic balance performance is an important prerequisite during daily and sporting life. Various traditional and innovative balance training concepts have been suggested to improve postural control or neuromuscular fall risk profiles over recent years. Whether slackline training (balancing over narrow nylon ribbons) serves as an appropriate training strategy to improve static and dynamic balance performance is as yet unclear., Objective: The aim was to examine the occurrence and magnitude of effects of slackline training compared with an inactive control condition on static and dynamic balance performance parameters in children, adults and seniors., Data Sources: Five biomedical and psychological databases (CINAHL, EMBASE, ISI Web of Knowledge, PubMed, SPORTDiscus) were screened using the following search terms with Boolean conjunctions: (slacklin* OR slack-lin* OR tight rop* OR tightrop* OR Slackline-based OR line-based OR slackrop* OR slack-rop* OR floppy wir* OR rop* balanc* OR ropedanc* OR rope-danc*) STUDY SELECTION: Randomized and non-randomized controlled trials that applied slackline training as an exercise intervention compared with an inactive control condition focusing on static and dynamic balance performance (perturbed and non-perturbed single leg stance) in healthy children, adults and seniors were screened for eligibility., Data Extraction: Eligibility and study quality [Physiotherapy Evidence Database (PEDro) scale] were independently assessed by two researchers. Standardized mean differences (SMDs) calculated as weighted Hedges' g served as main outcomes in order to compare slackline training versus inactive control on slackline standing as well as dynamic and static balance performance parameters. Statistical analyses were conducted using a random-effects, inverse-variance model., Results: Eight trials (mean PEDro score 6.5 ± 0.9) with 204 healthy participants were included. Of the included subjects, 35 % were children or adolescents, 39 % were adults and 26 % were seniors. Slackline training varied from 4 to 6 weeks with 16 ± 7 training sessions on average, ranging from 8 to 28 sessions. Mean overall slackline training covered 380 ± 128 min. Very large task-specific effects in favor of slackline training compared with the inactive control condition were found for slackline standing time {SMD 4.63 [95 % confidence interval (CI) 3.67-5.59], p < 0.001}. Small and moderate pooled transfer effects were observed for dynamic [SMD 0.52 (95 % CI 0.08-0.96), p = 0.02] and static [SMD 0.30 (95 % CI -0.03 to 0.64), p = 0.07] standing balance performance, respectively., Conclusions: Slackline training mainly revealed meaningful task-specific training effects in balance performance tasks that are closely related to the training content, such as slackline standing time and dynamic standing balance. Transfer effects to static and dynamic standing balance performance tasks are limited. As a consequence, slackline devices should be embedded into a challenging and multimodal balance training program and not used as the sole form of training.

Background: Balance training is considered an important means to decrease fall rates in seniors. Whether virtual reality training (VRT) might serve as an appropriate treatment strategy to improve neuromuscular fall risk parameters in comparison to alternative balance training programs (AT) is as yet unclear., Objective: To examine and classify the effects of VRT on fall-risk relevant balance performance and functional mobility compared to AT and an inactive control condition (CON) in healthy seniors., Data Sources: The literature search was conducted in five databases (CINAHL, EMBASE, ISI Web of Knowledge, PubMed, SPORTDiscus). The following search terms were used with Boolean conjunction: (exergam* OR exer-gam* OR videogam* OR video-gam* OR video-based OR computer-based OR Wii OR Nintendo OR X-box OR Kinect OR play-station OR playstation OR virtua* realit* OR dance dance revolution) AND (sport* OR train* OR exercis* OR intervent* OR balanc* OR strength OR coordina* OR motor control OR postur* OR power OR physical* OR activit* OR health* OR fall* risk OR prevent*) AND (old* OR elder* OR senior*)., Study Selection: Randomized and non-randomized controlled trials applying VRT as interventions focusing on improving standing balance performance (single and double leg stance with closed and open eyes, functional reach test) and functional mobility (Berg balance scale, Timed-up and go test, Tinetti test) in healthy community-dwelling seniors of at least 60 years of age were screened for eligibility., Data Extraction: Eligibility and study quality (PEDro scale) were independently assessed by two researchers. Standardized mean differences (SMDs) served as main outcomes for the comparisons of VRT versus CON and VRT versus AT on balance performance and functional mobility indices. Statistical analyses were conducted using a random effects inverse-variance model., Results: Eighteen trials (mean PEDro score: 6 ± 2) with 619 healthy community dwellers were included. The mean age of participants was 76 ± 5 years. Meaningful effects in favor of VRT compared to CON were found for balance performance [p < 0.001, SMD: 0.77 (95 % CI 0.45-1.09)] and functional mobility [p = 0.004, SMD: 0.56 (95 % CI 0.25-0.78)]. Small overall effects in favor of AT compared to VRT were found for standing balance performance [p = 0.31, SMD: -0.35 (95 % CI -1.03 to 0.32)] and functional mobility [p = 0.05, SMD: -0.44 (95 % CI: -0.87 to 0.00)]. Sensitivity analyses between "weaker" (n = 9, PEDro ≤5) and "stronger" (n = 9, PEDro ≥6) studies indicated that weaker studies showed larger effects in favor of VRT compared to CON regarding balance performance (p < 0.001)., Conclusions: Although slightly less effective than AT, VRT-based balance training is an acceptable method for improving balance performance as well as functional mobility outcomes in healthy community dwellers. VRT might serve as an attractive complementary training approach for the elderly. However, more high-quality research is needed in order to derive valid VRT recommendations compared to both AT and CON.

Annually, one in three seniors aged over 65 years fall. Balance and strength training can reduce neuromuscular fall risk factors and fall rates. Besides conventional balance and strength training, explosive or high-velocity strength training, eccentric exercises, perturbation-based balance training, trunk strength, and trunk control have also been emphasized. In contrast, aerobic exercise has to date not been included in fall-prevention studies. However, well-developed endurance capacity might attenuate fatigue-induced declines in postural control in sports-related or general activities of daily living. Physical performance indices, such as balance, strength, and endurance, are generally addressed independently in exercise guidelines. This approach seems time consuming and may impede integrative training of sensorimotor, neuromuscular, and cardiocirculatory functions required to deal with balance-threatening situations in the elderly. An agility-based conceptual training framework comprising perception and decision making (e.g., visual scanning, pattern recognition, anticipation) and changes of direction (e.g., sudden starts, stops and turns; reactive control; concentric and eccentric contractions) might enable an integrative neuromuscular, cardiocirculatory, and cognitive training. The present paper aims to provide a scientific sketch of how to build such an integrated modular training approach, allowing adaptation of intensity, complexity, and cognitive challenge of the agility tasks to the participant's capacity. Subsequent research should address the (1) link between agility and fall risk factors as well as fall rates, (2) benefit-risk ratios of the proposed approach, (3) psychosocial aspects of agility training (e.g., motivation), and (4) logistical requirements (e.g., equipment needed).

Background: The promotion of sport and physical activity (PA) for children is widely recommended to support a healthy lifestyle, but being engaged in sport bears the risk of sustaining injuries. Injuries, in turn, can lead to a reduction in current and future involvement in PA and, therefore, may negatively affect future health as well as quality of life. Thus, sports injury prevention is of particular importance in youth., Objective: The aim of this systematic review was to quantify the effectiveness of exercise-based injury prevention programs in child and adolescent sport in general, and with respect to different characteristics of the target group, injury prevention program, and outcome variables., Data Sources: An Internet-based literature search was conducted in six databases (CINAHL, Cochrane, EMBASE, ISI Web of Science, PubMed, SPORTDiscus) using the following search terms with Boolean conjunction: (sport injur* OR athletic injur* OR sport accident*) AND (prevent* OR prophylaxis OR avoidance) AND (child* OR adolescent OR youth)., Study Selection: Randomized controlled trials and controlled intervention studies in organized sport, published in English in a peer-reviewed journal, analyzing the effects of an exercise-based injury prevention program in athletes younger than 19 years of age., Data Extraction: Two reviewers evaluated eligibility and methodological quality. Main outcome extracted was the rate ratio (RR). Statistical analyses were conducted using the inverse-variance random effects model., Results: Twenty-one trials, conducted on a total of 27,561 athletes (median age 16.7 years [range 10.7-17.8]), were included. The overall RR was 0.54 (95% CI 0.45-0.67) [p < 0.001]. Girls profited more from injury prevention than boys (p = 0.05). Both prevention programs with a focus on specific injuries (RR 0.48 [95% CI 0.37-0.63]) and those aiming at all injuries (RR 0.62 [95% CI 0.48-0.81]) showed significant reduction effects. Pre-season and in-season interventions were similarly beneficial (p = 0.93). Studies on programs that include jumping/plyometric exercises showed a significant better (p = 0.002) injury preventive effect (RR 0.45 [95% CI 0.35-0.57], Z = 6.35, p < 0.001) than studies without such exercises (RR 0.74 [95% CI 0.61-0.90], Z = 3.03, p = 0.002)., Conclusions: The results provide good evidence and clearly demonstrate beneficial effects of exercise-based injury prevention programs in youth sports as they can result in statistically significant and practically relevant injury reduction. In particular, multimodal programs including jumping/plyometric exercises can be recommended. However, there is a considerable lack of data for children (under 14 years of age) and for individual sports in general. Future research should include these groups and focus on the effect of specific exercises and compliance.

Football (soccer) is the world's most popular sport with most players being younger than 18 years. Playing football can induce beneficial health effects, but there is also a high risk of injury. Therefore, it is necessary to implement measures for preventing injuries. The present review analyzes and summarizes published scientific information on the incidence and characteristics of football injuries in children and adolescent players to arrive at sound conclusions and valid considerations for the development of injury-prevention programs. A literature search was conducted up to November 2012. Fifty-three relevant scientific publications were detected. Thirty-two studies fulfilled the inclusion criteria for pooled analysis. Additional information from the remaining 21 studies was considered where appropriate to obtain a broader perspective on the injury problem in children and youth football. Training injury incidence was nearly constant for players aged 13-19 years, ranging from 1 to 5 injuries per 1,000 h training. Match injury incidence tended to increase with age through all age groups, with an average incidence of about 15 to 20 injuries per 1,000 match hours in players older than 15 years. Between 60 and 90 % of all football injuries were classified as traumatic and about 10-40 % were overuse injuries. Most injuries (60-90 %) were located at the lower extremities with the ankle, knee, and thigh being mostly affected. The frequency of upper-extremity and head/face injuries was higher in those studies that analyzed match injuries only. The most common injury types were strains, sprains, and contusions (10 up to 40 % each). There is some evidence that the risk of traumatic injuries and, in particular, of sustaining a fracture, contusion, or concussion was higher during match play than in practice sessions. Fractures were more frequent in children younger than 15 years than in older players. About half of all time-loss injuries led to an absence from sport of less than 1 week, one third resulted in an absence between 1 and 4 weeks, and 10 to 15 % of all injuries were severe. Separate data for players under the age of 11 years are almost absent. Maturation status seems to have an influence on injury characteristics, although evidence is not conclusive at this time. Three main areas seem to be of particular relevance for future prevention research in young football players: (1) the substantial number of severe contact injuries during matches, (2) the high number of fractures in younger players, and (3) the influence of maturation status and growth spurts.

Pre-cooling is used by many athletes for the purpose of reducing body temperature prior to exercise and, consequently, decreasing heat stress and improving performance. Although there are a considerable number of studies showing beneficial effects of pre-cooling, definite conclusions on the effectiveness of pre-cooling on performance cannot yet be drawn. Moreover, detailed analyses of the specific conditions under which pre-cooling may be most promising are, so far, missing. Therefore, we conducted a literature search and located 27 peer-reviewed randomized controlled trials, which addressed the effects of pre-cooling on performance. These studies were analysed with regard to performance effects and several test circumstances (environmental temperature, test protocol, cooling method, aerobic capacity of the subjects). Eighteen studies were performed in a hot (>26°C) environment and eight in a moderate. The cooling protocols were water application (n = 12), cooling packs (n = 3), cold drinks (n = 2), cooling vest (n = 6) and a cooled room (n = 4). The following different performance tests were used: short-term, high-intensity sprints (n = 2), intermittent sprints (n = 6), time trials (n = 10), open-end tests (n = 7) and graded exercise tests (n = 2). If possible, subjects were grouped into different aerobic capacity levels according to their maximal oxygen consumption (VO(2max)): medium 55-65 mL/kg/min (n = 11) and high >65 mL/kg/min (n = 6). For all studies the relative changes of performance due to pre-cooling compared with a control condition, as well as effect sizes (Hedges' g) were calculated. Mean values were weighted according to the number of subjects in each study. Pre-cooling had a larger effect on performance in hot (+6.6%, g = 0.62) than in moderate temperatures (+1.4%, g = 0.004). The largest performance enhancements were found for endurance tests like open-end tests (+8.6%, g = 0.52), graded exercise tests (+6.0%, g = 0.44) and time trials (+4.2%, g = 0.44). A similar effect was observed for intermittent sprints (+3.3%, g = 0.43), whereas performance changes were smaller during short-term, high-intensity sprints (-0.5%, g = 0.03). The most promising cooling methods were cold drinks (+15.0%, g = 1.68), cooling packs (+5.6%, g = 0.70) and a cooled room (+10.7%, g = 0.49), whereas a cooling vest (+4.8%, g = 0.31) and water application (+1.2%, g = 0.21) showed only small effects. With respect to aerobic capacity, the best results were found in the subjects with the highest VO(2max) (high +7.7%, g = 0.65; medium +3.8%, g = 0.27). There were four studies analysing endurance-trained athletes under time-trial conditions, which, in a practical sense, seem to be most relevant. Those studies found an average effect on performance of 3.7% (g = 0.48). In summary, pre-cooling can effectively enhance endurance performance, particularly in hot environments, whereas sprint exercise is barely affected. In particular, well trained athletes may benefit in a typical competition setting with practical and relevant effects. With respect to feasibility, cold drinks, cooling packs and cooling vests can be regarded as best-practice methods.

During the last nearly 50 years, the blood lactate curve and lactate thresholds (LTs) have become important in the diagnosis of endurance performance. An intense and ongoing debate emerged, which was mainly based on terminology and/or the physiological background of LT concepts. The present review aims at evaluating LTs with regard to their validity in assessing endurance capacity. Additionally, LT concepts shall be integrated within the 'aerobic-anaerobic transition' - a framework which has often been used for performance diagnosis and intensity prescriptions in endurance sports. Usually, graded incremental exercise tests, eliciting an exponential rise in blood lactate concentrations (bLa), are used to arrive at lactate curves. A shift of such lactate curves indicates changes in endurance capacity. This very global approach, however, is hindered by several factors that may influence overall lactate levels. In addition, the exclusive use of the entire curve leads to some uncertainty as to the magnitude of endurance gains, which cannot be precisely estimated. This deficiency might be eliminated by the use of LTs. The aerobic-anaerobic transition may serve as a basis for individually assessing endurance performance as well as for prescribing intensities in endurance training. Additionally, several LT approaches may be integrated in this framework. This model consists of two typical breakpoints that are passed during incremental exercise: the intensity at which bLa begin to rise above baseline levels and the highest intensity at which lactate production and elimination are in equilibrium (maximal lactate steady state [MLSS]). Within this review, LTs are considered valid performance indicators when there are strong linear correlations with (simulated) endurance performance. In addition, a close relationship between LT and MLSS indicates validity regarding the prescription of training intensities. A total of 25 different LT concepts were located. All concepts were divided into three categories. Several authors use fixed bLa during incremental exercise to assess endurance performance (category 1). Other LT concepts aim at detecting the first rise in bLa above baseline levels (category 2). The third category consists of threshold concepts that aim at detecting either the MLSS or a rapid/distinct change in the inclination of the blood lactate curve (category 3). Thirty-two studies evaluated the relationship of LTs with performance in (partly simulated) endurance events. The overwhelming majority of those studies reported strong linear correlations, particularly for running events, suggesting a high percentage of common variance between LT and endurance performance. In addition, there is evidence that some LTs can estimate the MLSS. However, from a practical and statistical point of view it would be of interest to know the variability of individual differences between the respective threshold and the MLSS, which is rarely reported. Although there has been frequent and controversial debate on the LT phenomenon during the last three decades, many scientific studies have dealt with LT concepts, their value in assessing endurance performance or in prescribing exercise intensities in endurance training. The presented framework may help to clarify some aspects of the controversy and may give a rationale for performance diagnosis and training prescription in future research as well as in sports practice.
