Objective Management of neuropathic pain following spinal cord injury (SCI) can be a frustrating experience for patients since it poses a therapeutic challenge. In this article the authors describe the clinical characteristics of a group of patients with pain after spinal cord injury. Methods In this retrospective study, 213 patients with SCI and neuropathic pain were assessed. We analyzed clinical characteristics, treatment options, and pain intensity for these patients. Results The main cause of SCI was spine trauma, which occurred in 169 patients, followed by tumors and infection. Complete lesions were verified in 144 patients. In our study, patients with traumatic SCI and partial lesions seem to be presented with more intense pain; however, this was not statistically significant. Conclusions Neuropathic pain is a common complaint in patients with SCI and presents a treatment challenge. Knowledge of the clinical characteristics of this group of patients may help determine the best approach to intervention.

Literature has shown that extent of tumor resection has an impact on quality of life and survival of patients with gliomas. Intraoperative MRI has been used to increase resection while preserving procedure's safety. METHOD: The first five patients with gliomas operated on at the University of São Paulo using intraoperative MRI are reported. All but one patient had Karnofsky Performance Status of 100% before surgery. Presentation symptoms were progressive headache, seizures, behavior disturbance, one instance of hemianopsia, and another of hemiparesis. RESULTS: Gross total removal was achieved in two patients. Surgical resection was limited by tumor invasion of critical areas like the internal capsule or the mesencephalon in the remaining patients. CONCLUSION: Intra-operative MRI is an important tool that helps surgeons to remove glial tumors, however, knowledge of physiology and functional anatomy is still fundamental to avoid morbidity.

OBJECTIVE: Colloid cysts (IIIVT CC) are benign neuroepithelial cysts located in the anterior third ventricle. The authors propose the use of Nd:YAG laser stereotactic neuroendoscopic for guided resection of the third ventricle colloid cysts. METHOD: Eleven patients presented third ventricle colloid cysts and were treated by Nd:YAG laser guided with stereotactic endoscopy (n=7) , stereotactic endoscopy (n=3) or stereotactly guided puncture (n=1). The patients were followed prospectively (average 33 months, range 19-64 months). The clinical data, neuroimaging findings, hospitalization stay, outcomes and complications of the method were evaluated. RESULTS: All patients presented headache; six had papilledema, one had gait disturbance and one had third-nerve palsy. Neuroimaging showed hydrocephalus and a IIIVT CC with 14.4-mm mean diameter. After surgery all patients presented clinical and image improvement. Only two patients presented transient morbidities that were easily treated: One had diabetes insipidus that lasted for two days and was treated with a single dose of DDAVP, and another had chemical aseptic meningitis, probably due to the contact of the cyst content with the CSF. This patient was treated with antibiotics and corticosteroids with complete resolution of the problem without sequels. The other patients were discharged from the hospital 48 h after surgery. CONCLUSION: The stereotactic neuroendoscopy-guided procedure with Nd:YAG laser allowed the complete removal of the third ventricle colloid cysts, without definitive morbidities, sequels or recurrence of the lesion.OBJETIVO: Os cistos colóides (CC IIIVT) são lesões neuroepiteliais benignas localizadas anteriormente no terceiro ventrículo. Nós propomos a ressecção neuroendoscópica com o uso do Nd:YAG laser guiada por estereotaxia. MÉTODO: Onze pacientes portadores de cisto colóide do terceiro ventrículo foram tratados por neuroendoscopia estereotática com laser Nd:YAG (n=7), neuroendoscopia estereotática (n=3) ou punção estereotática (n=1) e foram seguidos prospectivamente (média 33 meses, variação 19-64 meses). Os dados clínicos, achados de neuroimagem, tempo de hospitalização, evolução e complicações referentes à técnica foram avaliadas. RESULTADOS: Todos pacientes apresentaram cefaléia; seis tiveram papiledema, um apresentou distúrbio de marcha e um apresentou paresia do terceiro nervo. Os exames de neuroimagem evidenciaram hidrocefalia e CC IIIVT com diâmetro médio de 14,4 mm. Depois da cirurgia todos os pacientes apresentaram melhora clínica e de imagem. Apenas dois pacientes apresentaram morbidades transitórias que foram facilmente tratadas: um apresentou diabetes insipidus que durou dois dias e foi tratada com uma única dose de DDAVP, o outro apresentou meningite química asséptica, provavelmente pelo contato do conteúdo do cisto com o líquor. Este paciente recebeu antibióticos e corticóide com resolução completa do problema. Os outros pacientes receberam alta hospitalar após 48 h. CONCLUSÃO: A ressecção neuroendoscópica com o uso do Nd:YAG laser guiada por estereotaxia possibilitou a remoção completa do cisto colóide sem morbidades definitivas, seqüelas ou recorrência da lesão.

OBJECTIVE: In the majority of cases, the correct treatment of brain lesions is possible only when the histopathological diagnosis is made. Several deep-seated lesions near eloquent areas are not safely approached by the classical neurosurgical procedures. These patients can get benefit by a minimally invasive procedure. METHOD: We present a series of 176 consecutive patients submitted to stereotactic biopsies due to a great variety of brain lesions. RESULTS: Histological diagnosis found in this series: glioma in 40.1% of the patients, other neoplasms in 12.2% and infectious or inflammatory diseases in 29.1 %. The result was inconclusive in 5.2% of the procedures. One patient died (0.6%) and two (1.2%) presented operative complications. The criteria, advantages and risks of the stereotactic biopsies are discussed. CONCLUSION: The efficacy of the method is adequate and morbid-mortality rates were low.OBJETIVO: O diagnóstico anatomopatológico das lesões encefálicas é muitas vezes necessário para a instituição do tratamento adequado. Entretanto, muitas lesões localizadas profundamente no encéfalo ou em centros nervosos de grande importância funcional não podem ser acessadas sem riscos, com a aplicação dos procedimentos neurocirúrgicos habituais. MÉTODO: Apresentamos uma série de 176 doentes submetidos a biópsias estereotáxicas de lesões encefálicas. RESULTADOS: Em 40,1% dos casos, o diagnóstico foi de glioma, em 12,2% de outras neoplasias e em 29,1%, de doenças infecciosas ou inflamatórias. O resultado foi inconclusivo em 5,2% dos doentes. Um (0,6%) doente faleceu e dois (1,2%) apresentaram graves complicações operatórias. Os critérios de seleção, as vantagens e os riscos da biópsia estereotáxica são discutidos. CONCLUSÃO: A eficácia do método é boa e a morbimortalidade das biópsias estereotáxicas é baixa.

OBJECTIVE: The main objective when resecting benign brain lesions is to minimize risk of postoperative neurological deficits. We have assessed the safety and effectiveness of craniotomy under local anesthesia and monitored conscious sedation for the resection of lesions involving eloquent language cortex. METHODS: A retrospective review was performed on a consecutive series of 12 patients who underwent craniotomy under local anesthesia between 2001 and 2004. All patients had lesions close to the speech cortex. All resection was verified by post-operative imaging. Six subjects were male and 6 female, and were aged between 14 and 52 years. RESULTS: Lesions comprised 7 tumour lesions, 3 cavernomas and 1 dermoid cyst. Radiological gross total resection was achieved in 66% of patients while remaining cases had greater than 80% resection. Only one patient had a post-operative permanent deficit, whilst another had a transient post-operative deficit. All patients with uncontrollable epilepsy had good outcomes after surgery. None of our cases subsequently needed to be put under general anesthesia. CONCLUSION: Awake craniotomy with brain mapping is a safe technique and the "gold standard" for resection of lesions involving language areas.OBJETIVO: O presente estudo visa discutir as vantagens e as limitacões do uso da técnica de mapeamento cortical da área da fala com o paciente acordado. MÉTODO: esta é uma revisão retrospectiva dos casos em que foi realizado monitoramento cortical intraoperatório em cirurgias para ressecção de lesões intracranianas localizadas próximas à área da fala. Todos os pacientes foram submetidos a avaliação neuropsicológica no pré e intra-operatório. O grau das ressecções foi verificado através de exames de imagem pós-operatórios. Foram avaliados um total de 12 pacientes. Destes, 6 eram do sexo masculino e 6 do feminino. RESULTADOS: 7 lesões eram tumorais. A ressecção total foi atingida em 66% e ressecção subtotal nos remanescentes. Apenas 1 paciente apresentou déficit motor permanente no pós-operatório e todos os pacientes com quadro prévio de epilepsia refratária obtiveram bom controle das crises no pós-operatório. Em nenhum caso houve necessidade de conversão da anestesia para geral. CONCLUSÃO: O mapeamento funcional intraoperatório na craniotomia com o paciente acordado otimiza a extensão da ressecção da lesão minimizando morbidade permanente. Esta é uma técnica eficaz no manejo de lesões em íntimo contato com o córtex eloqüente, que outrora, seriam designadas inoperáveis.
