Abstract

: Introduction: Transgender women are at increased risk of acquiring HIV. Earlier studies reported lower retention in HIV care, antiretroviral therapy uptake, adherence and viral suppression. We assessed the stages of the HIV care continuum of transgender women in the Netherlands over an 11‐year period. In addition, we assessed new HIV diagnoses and late presentation, as well as disengagement from care, between 2011 and 2021. Methods: Using data from the Dutch national ATHENA cohort, we separately assessed viral suppression, as well as time to achieving viral suppression, among transgender women for each year between 2011 and 2021. We also assessed trends in new HIV diagnoses and late presentation (CD4 count of Results: Between 2011 and 2021, a total of 260 transgender women attended at least one HIV clinical visit. Across all years, Conclusions: Between 2011 and 2021, less than 90% of transgender women linked to HIV care were virally suppressed. Late presentation at the time of diagnosis and disengagement from care were common. Efforts are needed to identify barriers to early HIV diagnosis and to optimize the different steps across the care continuum for transgender women., INTRODUCTION Transgender women are at increased risk of human immunodeficiency virus (HIV) acquisition [1−5]. Abstract

Background: Mortality among people with HIV declined with the introduction of combination antiretroviral therapy. We investigated trends over time in all-cause and cause-specific mortality in people with HIV from 1999-2020. Methods: Data were collected from the D:A:D cohort from 1999 through January 2015 and RESPOND from October 2017 through 2020. Age-standardized all-cause and cause-specific mortality rates, classified using Coding Causes of Death in HIV (CoDe), were calculated. Poisson regression models were used to assess mortality trends over time. Results: Among 55716 participants followed for a median of 6 years (IQR 3-11), 5263 participants died (crude mortality rate [MR] 13.7/1000 PYFU; 95%CI 13.4-14.1). Changing patterns of mortality were observed with AIDS as the most common cause of death between 1999- 2009 (n = 952, MR 4.2/1000 PYFU; 95%CI 4.0-4.5) and non-AIDS defining malignancy (NADM) from 2010 -2020 (n = 444, MR 2.8/1000 PYFU; 95%CI 2.5-3.1). In multivariable analysis, all-cause mortality declined over time (adjusted mortality rate ratio [aMRR] 0.97 per year; 95%CI 0.96, 0.98), mostly from 1999 through 2010 (aMRR 0.96 per year; 95%CI 0.95-0.97), and with no decline shown from 2011 through 2020 (aMRR 1·00 per year; 95%CI 0·96-1·05). Mortality due all known causes except NADM also declined over the entire follow-up period. Conclusion: Mortality among people with HIV in the D:A:D and/or RESPOND cohorts decreased between 1999 and 2009 and was stable over the period from 2010 through 2020. The decline in mortality rates was not fully explained by improvements in immunologic-virologic status or other risk factors.

Abstract

RIVM rapport:In 2023 heeft 4 procent meer mensen zich bij een Centrum voor Seksuele Gezondheid (CSG) laten testen op seksueel overdraagbare aandoeningen (soa) dan in 2022. Het percentage dat een soa had (21 procent) was hetzelfde als in 2022. Mensen die via een partner een melding ontvingen voor een soa of zelf klachten hadden, hadden het vaakst een soa. Het RIVM beschrijft in dit overzicht de ontwikkelingen van soa in Nederland, waaronder het aantal testen en diagnoses per soa bij de CSG's. In 2023 waren er in totaal 172.113 consulten. Bij CSG's kunnen mensen met een grotere kans op soa, zich gratis laten testen. Sinds augustus 2019 bieden deze centra ook zorg aan mannen die seks hebben met mannen (MSM) die een geneesmiddel krijgen dat hiv voorkomt (Pre-Expositie Profylaxe, PrEP). Dit is een pilot van vijf jaar, waarin PrEP-gebruikers elke drie maanden worden getest op soa (MSM-PrEP). Chlamydia In 2023 waren er 24.048 chlamydia-diagnoses, iets minder dan in 2022 (24.684). Het percentage vrouwen met chlamydia daalde van 17,9 procent in 2022 naar 16,8 procent in 2023. Het percentage heteroseksuele mannen met chlamydia daalde van 21,2 procent in 2022 naar 19,6 procent in 2023. De percentages MSM en MSM-PrEP met chlamydia daalden naar respectievelijk 10,2 en 9,1 procent in 2023 (respectievelijk 10,9 en 9,4 procent in 2022). Gonorroe Het aantal diagnoses gonorroe (13.853) was veel hoger dan in 2022 (10.600), een stijging van 31 procent. De percentages vrouwen en heteroseksuele mannen met gonorroe zijn in 2023 toegenomen naar respectievelijk 4,1 en 3,5 procent (dat waren respectievelijk 2,3 en 2,4 procent in 2022). Dit is het hoogste percentage onder vrouwen en heteroseksuele mannen sinds 2014. De toename begon in de tweede helft van 2022 en zette heel 2023 door. In this overview, RIVM describes the developments of STIs in the Netherlands, including the number of tests and diagnoses per STI at the SHCs. In 2023, there were a total of 172,113 consultations. SHCs offer free STI testing to people at high risk of acquiring an STI. Since August 2019, SHCs have also provided care to men who have sex with men (MSM) who receive a drug to prevent HIV (Pre-Exposure Prophylaxis, PrEP). This is a five-year long pilot, in which PrEP users are tested for STIs every three months (MSM-PrEP). Chlamydia There were 24,048 chlamydia diagnoses in 2023, slightly less than in 2022 (24,684). The percentage of women with chlamydia decreased from 17.9 per cent in 2022 to 16.8 per cent in 2023. The percentage of heterosexual men with chlamydia decreased from 21.2 per cent in 2022 to 19.6 per cent in 2023. The percentages of MSM and MSM-PrEP with chlamydia decreased to 10.2 and 9.1 per cent respectively (10.9 and 9.4 per cent respectively in 2022). Gonorrhoea The number of gonorrhoea diagnoses (13,853) was much higher than in 2022 (10,600), an increase of 31 per cent. The percentages of women and heterosexual men with gonorrhoea both increased in 2023, to 4.1 and 3.5 per cent respectively (2.3 and 2.4 per cent respectively in 2022). This is the highest percentage among women since 2014. The increase started in the second half of 2022 and continued throughout 2023. Abstract

Background: Men and women with a migration background comprise an increasing proportion of incident human immunodeficiency virus (HIV) cases across Western Europe., Methods: To characterize sources of transmission in local transmission chains, we used partial HIV consensus sequences with linked demographic and clinical data from the opt-out AIDS Therapy Evaluation in the Netherlands (ATHENA) cohort of people with HIV in the Netherlands and identified phylogenetically and epidemiologically possible HIV transmission pairs in Amsterdam. We interpreted these in the context of estimated infection dates, and quantified population-level sources of transmission to foreign-born and Dutch-born Amsterdam men who have sex with men (MSM) within Amsterdam transmission chains., Results: We estimate that Dutch-born MSM were the predominant sources of infections among all Amsterdam MSM who acquired their infection locally in 2010-2021, and among almost all foreign-born Amsterdam MSM subpopulations. Stratifying by 2-year intervals indicated time trends in transmission dynamics, with a majority of infections originating from foreign-born MSM since 2016, although uncertainty ranges remained wide., Conclusions: Native-born MSM have predominantly driven HIV transmissions in Amsterdam in 2010-2021. Abstract

BackgroundIn the Netherlands, HIV pre-exposure prophylaxis (PrEP) has been available since 2019. However, the extent of PrEP use prior to HIV diagnosis and development of PrEP-resistance-associated mutations (RAMs) is not known.AimWe assessed prior PrEP use and potential transmission of PrEP RAMs among men who have sex with men (MSM) and transgender persons (TGP) with a new HIV diagnosis in the Netherlands.MethodsData on prior PrEP use between 1 January 2018 and 31 December 2022 were available from the Dutch national ATHENA cohort. We assessed proportion of prior PrEP use, detected PrEP associated RAMs and assessed potential onward transmission of RAMs between 2010 and 2022 using a maximum likelihood tree.ResultsData on prior PrEP use were available for 583/1,552 (36.3%) individuals, with 16% (94/583) reporting prior PrEP use. In 489 individuals reporting no prior PrEP use, 51.5% did not use PrEP due to: low HIV-risk perception (29%), no access (19.1%), personal preference (13.1%), and being unaware of PrEP (19.1%). For PrEP users, 13/94 (13.8%) harboured a M184V/I mutation, of whom two also harboured a K65R mutation. In people with a recent HIV infection, detection of PrEP RAMs increased from 0.23% (2/862) before 2019 to 4.11% (9/219) from 2019. We found no evidence of onward transmission of PrEP RAMs.ConclusionThe prevalence of PrEP-associated RAMs has increased since PrEP became available in the Netherlands. More widespread access to PrEP and retaining people in PrEP programmes when still at substantial risk is crucial to preventing new HIV infections.

14. Discrimination of the Veterans Aging Cohort Study Index 2.0 for Predicting Cause-specific Mortality Among Persons With HIV in Europe and North America.

Background: Predicting cause-specific mortality among people with HIV (PWH) could facilitate targeted care to improve survival. We assessed discrimination of the Veterans Aging Cohort Study (VACS) Index 2.0 in predicting cause-specific mortality among PWH on antiretroviral therapy (ART)., Methods: Using Antiretroviral Therapy Cohort Collaboration data for PWH who initiated ART between 2000 and 2018, VACS Index 2.0 scores (higher scores indicate worse prognosis) were calculated around a randomly selected visit date at least 1 year after ART initiation. Missingness in VACS Index 2.0 variables was addressed through multiple imputation. Cox models estimated associations between VACS Index 2.0 and causes of death, with discrimination evaluated using Harrell's C-statistic. Absolute mortality risk was modelled using flexible parametric survival models., Results: Of 59 741 PWH (mean age: 43 years; 80% male), the mean VACS Index 2.0 at baseline was 41 (range: 0-129). For 2425 deaths over 168 162 person-years follow-up (median: 2.6 years/person), AIDS (n = 455) and non-AIDS-defining cancers (n = 452) were the most common causes. Predicted 5-year mortality for PWH with a mean VACS Index 2.0 score of 38 at baseline was 1% and approximately doubled for every 10-unit increase. The 5-year all-cause mortality C-statistic was .83. Discrimination with the VACS Index 2.0 was highest for deaths resulting from AIDS (0.91), liver-related (0.91), respiratory-related (0.89), non-AIDS infections (0.87), and non-AIDS-defining cancers (0.83), and lowest for suicides/accidental deaths (0.65)., Conclusions: For deaths among PWH, discrimination with the VACS Index 2.0 was highest for deaths with measurable physiological causes and was lowest for suicide/accidental deaths., Competing Interests: Potential conflicts of interest. Abstract

Introduction: Mortality rates for people living with HIV (PLHIV) on antiretroviral therapy (ART) in high-income countries continue to decline. We compared mortality rates among PLHIV on ART in Europe for 2016-2020 with Spectrum's estimates., Methods: The AIDS Impact Module in Spectrum is a compartmental HIV epidemic model coupled with a demographic population projection model. We used national Spectrum projections developed for the 2022 HIV estimates round to calculate mortality rates among PLHIV on ART, adjusting to the age/country distribution of PLHIV starting ART from 1996 to 2020 in the Antiretroviral Therapy Cohort Collaboration (ART-CC)'s European cohorts., Results: In the ART-CC, 11,504 of 162,835 PLHIV died. Between 1996-1999 and 2016-2020, AIDS-related mortality in the ART-CC decreased from 8.8 (95% CI: 7.6 to 10.1) to 1.0 (0.9-1.2) and from 5.9 (4.4-8.1) to 1.1 (0.9-1.4) deaths per 1000 person-years among men and women, respectively. Non-AIDS-related mortality decreased from 9.1 (7.9-10.5) to 6.1 (5.8-6.5) and from 7.0 (5.2-9.3) to 4.8 (4.3-5.2) deaths per 1000 person-years among men and women, respectively. Adjusted all-cause mortality rates in Spectrum among men were near ART-CC estimates for 2016-2020 (Spectrum: 7.02-7.47 deaths per 1000 person-years) but approximately 20% lower in women (Spectrum: 4.66-4.70). Adjusted excess mortality rates in Spectrum were 2.5-fold higher in women and 3.1-3.4-fold higher in men in comparison to the ART-CC's AIDS-specific mortality rates., Discussion: Spectrum's all-cause mortality estimates among PLHIV are consistent with age/country-controlled mortality observed in ART-CC, with some underestimation of mortality among women.
