6. Analysis of Gradient Descent With Varying Step Sizes Using Integral Quadratic Constraints

Abstract

The framework of integral quadratic constraints is used to perform an analysis of gradient descent with varying step sizes. Two performance metrics are considered: convergence rate and noise amplification. We assume that the step size is produced from a line search and varies in a known interval. Modeling the algorithm as a linear parameter-varying (LPV) system, we construct a parameterized linear matrix inequality condition that certifies algorithm performance, which is solved using a result for polytopic LPV systems. Our results provide convergence rate guarantees when the step size lies within a restricted interval. Moreover, we recover existing rate bounds when this interval reduces to a single point, i.e., a constant step size. Finally, we note that the convergence rate depends only on the condition number of the problem. In contrast, the noise amplification performance depends on the individual values of the strong convexity and smoothness parameters, and varies inversely with them for a fixed condition number.

Abstract

We discuss the use of machine learning models for finding "good coordinates" for polynomial ideals. Our main goal is to put ideals into quasi-stable position, as this generic position shares most properties of the generic initial ideal, but can be deterministically reached and verified. Furthermore, it entails a Noether normalisation and provides us with a system of parameters. Traditional approaches use either random choices which typically destroy all sparsity or rather simple human heuristics which are only moderately successful. Abstract

Summary: Background: Lyme disease (LD) is caused by Borrelia burgdorferi and is the most common tickborne disease in the northern hemisphere. Although classical characteristics of LD are well-known, the diagnosis and treatment are often delayed. Laboratory diagnosis by serological testing is recommended for most LD manifestations. The objective of this study was to describe clinical characteristics and associated serological profiles in children with LD. Methods: This retrospective cohort study included children aged 0–18 years, diagnosed with LD according to current guidelines at University Children's Hospital Zurich between January 1, 2006 and December 31, 2020. Two-tier serological testing with the recomWell enzyme-linked immunosorbent assay and recomLine Western blot (MIKROGEN Diagnostik, MIKROGEN GmbH, Neuried, Germany) was performed at the Institute of Medical Microbiology, University of Zurich. Findings: In total, 469 children diagnosed with LD were included (median age, 7.9 years); 190 patients (40.5%) with Lyme neuroborreliosis (LNB), 171 (36.5%) patients with skin manifestations (erythema migrans, n = 121; multiple erythema migrans, n = 11; borrelial lymphocytoma, n = 37; and acrodermatitis chronica atrophicans, n = 2), and 108 (23.0%) patients with Lyme arthritis. We observed seasonal variations for patients with skin manifestations and LNB, with high prevalence in May–October, but not for patients with Lyme arthritis. Significant differences between LD manifestation groups were found for age, inflammatory parameters, and specificity and concentration of B. burgdorferi-specific serum antibody responses. We observed distinct patterns of pronounced serum antibody responses against B. burgdorferi antigens in LNB (IgM against VlsE, p41, and OspC) and Lyme arthritis (IgG against p100, VlsE, p58, p41, p39, and p18). Interpretation: Our study is one of the largest and most detailed for children with LD. We present unique findings regarding the differences in clinical characteristics and immune responses between various manifestations of LD in children. 28. Exploring diagnostic stewardship in the emergency department evaluation of pediatric abdominal pain in a statewide quality collaborative.

Abstract

Background: Diagnostic stewardship is the effort to optimize diagnostic testing to reduce errors while avoiding overtesting and overtreatment. Abdominal pain and appendicitis in children are essential use cases. Delayed diagnosis of appendicitis can be dangerous and even life-threatening, but overtesting is harmful., Methods: We conducted a retrospective cohort study of children aged 5-17 years presenting with abdominal pain to 26 EDs within the Michigan Emergency Department Improvement Collaborative (MEDIC) from May 1, 2016, to February 29, 2024. We defined two outcome measures summarized by ED. First, we describe the cross-sectional imaging:appendicitis visits ratio, defined as the count of ED visits resulting in any cross-sectional imaging (CT or MRI) divided by the count of ED visits with a diagnosis of appendicitis. Second, we describe the delayed diagnosis rate, defined by an ED visit for abdominal pain resulting in a discharge and subsequent return visit with a diagnosis of appendicitis within 7 days., Results: The sample included 120,112 pediatric visits for abdominal pain at 26 EDs; 4967 (4.1%) were diagnosed with appendicitis. The cross-sectional imaging:appendicitis visits ratio varied by site, from as low as 0.2 (95% confidence interval [CI] 0.1-0.2) at a pediatric site to as high as 7.9 (95% CI 4.8-16.4) at an urban ED. The proportion of pediatric ED visits for abdominal pain that resulted in an identified delayed diagnosis of appendicitis was 0.1% (141/120,112). All but four sites had fewer than 10 cases of delayed diagnosis across the study period., Conclusions: In this retrospective cohort study of 120,000+ ED visits for pediatric abdominal pain, we found that the ratio of visits with cross-sectional imaging to diagnosed cases of appendicitis varied widely across EDs. Delayed diagnosis of appendicitis was uncommon. Adherence to best practices and improved imaging quality may hold promise to improve diagnostic stewardship for children with abdominal pain across EDs., (© 2025 The Author(s).
