Cases of extramedullary multiple myeloma occurring in the oral cavity are rare. Herein, we present a unique case of extramedullary disease occurring twice in the oral cavity. A 72-year-old woman was diagnosed with multiple myeloma and was undergoing melphalan and prednisone therapy, along with thalidomide. The patient visited our hospital with a complaint of swelling of the left mandibular gingiva and an outward mass located on the buccal side of the left mandibular second molar. Radiographic and computed tomography examinations showed no bone resorption disease. Biopsy was performed for a definitive diagnosis, which confirmed extramedullary multiple myeloma. Radiation therapy was administered, and the lesion disappeared after 2 months. However, 2 months thereafter, a tumor appeared in the upper right maxillary gingiva, which disappeared following another round of radiation therapy. Subsequently, an extramedullary tumor developed systemically. The patient died due to poor management of the underlying disease. The development of extramedullary tumors is associated with a low survival rate, which should be given crucial consideration in treatment decisions. [ABSTRACT FROM AUTHOR]

Reflection electron energy loss spectroscopy (REELS) has played a pivotal role in allowing researchers to explore the characteristics of various bulk materials. This study presents results for the low-loss region of REELS with a new cylindrical lens spectrometer integrated into a low-voltage scanning electron microscope. The operational principles and implementation of the spectrometer are explained through comparisons between electron optical simulations and experimental results. Notably, the analysis shows the ability to distinguish samples in film and bulk forms. Graphene and graphite, despite their identical elemental composition and crystalline structure, are found to have distinct energy spectra as indicated by plasmon peaks. Furthermore, the study explores the bandgap measurement of SiO2 at low-energy conditions of 2.5 keV, highlighting the proposed instrument's advantages in the measurement without the harmful effect of Cherenkov loss. Additionally, this method reaffirms the capability to measure multiple plasmon peaks from the energy spectra of bulk gold samples, thus introducing a pioneering avenue in energy spectrum measurement. Leveraging the compact size and simple experimental setup of the spectrometer for REELS, the method enables the measurement of energy spectra of both bulk- and film-formed samples under low electron energy conditions, marking a significant advancement in the field., Competing Interests: Conflict of Interest. Background: In Japan, sentinel surveillance is used to monitor the trend of infectious gastroenteritis. Another method of pathogen surveillance, wastewater-based epidemiology, has been used recently because it can help to monitor infectious disease without relying on patient data. Here, we aimed to determine the viral trends reflected in the number of reported patients and number of gastroenteritis virus-positive samples. We focused on gastroenteritis viruses present in wastewater and investigated the usefulness of wastewater surveillance for the surveillance of infectious gastroenteritis., Methods: Real-time polymerase chain reaction was used for viral gene detection in wastewater. The number of reported patients per pediatric sentinel site and number of viral genome copies were compared for correlation potential. The number of gastroenteritis virus-positive samples reported by National Epidemiological Surveillance of Infectious Disease (NESID) and the status of gastroenteritis viruses detected in wastewater were also evaluated., Results: Genes of norovirus genotype I, norovirus genotype II, sapovirus, astrovirus, rotavirus group A, and rotavirus group C were detected in wastewater samples. Viruses were detected in wastewater during periods when no gastroenteritis virus-positive samples were reported to NESID., Conclusion: Norovirus genotype II and other gastroenteritis viruses were detected in wastewater even during periods when no gastroenteritis virus-positive samples were found. Therefore, surveillance using wastewater can complement sentinel surveillance and is an effective tool for the surveillance of infectious gastroenteritis.
