Agriculture faces significant challenges due to climate change and unpredictable environmental factors, which impact crop yields and threaten food security. This study proposes a novel approach to crop yield prediction and risk assessment using neutrosophic logic and least squares regression. By integrating these methods, we aim to improve accuracy in predicting crop losses under uncertain conditions. The model classifies crops based on profitability and environmental risks, utilizing the independence test to evaluate the relationships between crop attributes. Our approach leverages deep learning techniques, such as restricted Boltzmann machines (RBM), to enhance the analysis of crop data and provide farmers with actionable insights for decisionmaking. The results demonstrate that the proposed method effectively estimates crop yield damage with the ability to assess risk under varying conditions, such as extreme weather. This approach can help farmers optimize crop management strategies, minimize losses, and improve overall productivity. This study introduces an innovative enhancement to the U-Net architecture, termed Modified DRU-Net, aiming to improve the segmentation of cell images in Transmission Electron Microscopy (TEM). Traditional U-Net models, while effective, often struggle to capture fine-grained details and preserve contextual information critical for accurate biomedical image segmentation. To overcome these challenges, Modified DRU-Net integrates dense residual connections and attention mechanisms into the U-Net framework. Dense connections enhance gradient flow and feature reuse, while residual connections mitigate the vanishing gradient problem, facilitating better model training. Attention blocks in the up-sampling path selectively focus on relevant features, boosting segmentation accuracy. Additionally, a combined loss function, merging focal loss and dice loss, addresses class imbalance and improves segmentation performance. Experimental results demonstrate that Modified DRU-Net significantly enhances performance metrics, underscoring its effectiveness in achieving detailed and accurate cell image segmentation in TEM images. Introduction: Stress response to surgery is characterized by increase in secretion of endocrinal system and activation of immunological and sympathetic nervous system. Alpha 2-adrenoceptor agonists have several beneficial actions during the perioperative period. They decrease sympathetic tone, with attenuation of the neuroendocrine and hemodynamic responses to anaesthesia and surgery; reduce anaesthetic and opioid requirements; and cause sedation and analgesia. Aims and objectives; The objective of this study is to evaluate the intra-operative, and postoperative haemodynamic stress responses in patients, when Dexmedetomidine is used as an anaesthetic adjunct. Materials and Method: After the approval from Ethical Committee of Guntur medical college, Guntur, A.P, 100 patients scheduled for elective surgery planned to be done under general anaesthesia were included in the study.Patients were randomly divided into two equal groups, according to the study drug administered, each group include 50 patients in number (n=50), group I (the control or placebo group) and group II (Dexmedetomidine group). Patients received either Dexmedetomidine hydrochloride (available as 100 μg/ml ) in group II or saline solution in group I in equal amount. Data were collected and entered into the software STATA (version 11.0). P<0.05 was considered significant and P>0.05 was considered insignificant. Results: There was no significant difference in the base line parameters among the groups. Dexmedetomidine decreased heart rate significantly compared to control group. Dexmedetomidine reduced the intraoperative use of Propofol and Fentanyl significantly compared to control group. Conclusion: The perioperative infusion of Dexmedetomidine appears to be effective to attenuate stress induced haemodynamic fluctuation, reduced Propofol requirements, decreased the intra and post-operative narcotic analgesic administration, produced perioperative sedation in patients who underwent elective surgeries when compared with placebo. The fish's immune system consists of two major components, innate and adaptive immunities. Innate immunity is non-specific and acts as the primary line of protection against pathogen invasion while adaptive immunity is more specific to a certain pathogen/following adaptation. Innate immunity consists of the non-specific cellular and the nonspecific humoral components. On the other hand, the adaptive immune system consists of highly specialized systemic cells and processes that are separated into two main components: the humoral and cellular components. Three types of antibodies, the IgM, IgD and IgT, are the major constituents of the humoral immunity, which act on invaded extracellular pathogens. The cytotoxic T-lymphocyte cells are the major component of the cellular immunity that frequently kills virus-infected and intracellular bacterial or parasitic-infected cells. The software reliability model estimates the probability of data failure in a specific environment, significantly impacting reliability and trustworthiness. The paper study focuses on cluster crime data, i.e., indeterministic in Neutrosophic Logic, using a software reliability model. The study utilizes reinforcement learning, Neutrosophic logic, and non-homogeneous Poisson process crime data to estimate indeterministic cluster data in crime. The "Non-homogeneous Poisson Process with Neutrosophic Logic" technique performs well in evaluating and deterring crime based on crime data analysis. The crime cluster involving offenders correctly classified as failure to accomplish does better than uncertain cluster reliability estimation with least squares and logistic regression analysis. Prostate cancer is a leading cause of cancer-related mortality in men, with metastatic castration-resistant prostate cancer presenting a substantial treatment challenge. The authors focuse on prostate-specific membrane antigen (PSMA) radiotheranostics, particularly lutetium 177 ( 177 Lu)-PSMA radioligand therapy, as an emerging treatment modality for metastatic castration-resistant prostate cancer. The U.S. Food and Drug Administration approval of 177 Lu-PSMA-617 marked a substantial advancement in the treatment paradigm of metastatic castration-resistant prostate cancer, based on the VISION trial that showed improved overall survival and quality of life compared with those for standard care. PSMA expression, assessed via PSMA PET, is crucial for patient selection and assessment of treatment eligibility. The authors discuss current practices, including therapy administration, dosing, and side effects, with a particular focus on eligibility criteria and the added value of posttherapy imaging. Response assessment criteria using PSMA PET are discussed, although these require further validation. The discussion of future directions highlights ongoing trials investigating PSMA targeting agents, the extension of radioligand therapy to earlier stages of prostate cancer, and combination therapies. Abstract: Chronic pancreatitis is a benign disease which causes recurrent abdominal pain and loss of pancreatic function. Lateral pancreaticojejunostomy (LPJ) is a commonly performed drainage procedure for this condition. While usually performed through an open approach, there have been few cases of laparoscopic approaches for this condition. A literature review was conducted to understand the current status of laparoscopic longitudinal pancreatojejunostomy (LLPJ). We conducted a comprehensive literature search using PubMed, Embase and Cochrane Library to find the articles published until 1 st October 2023. We excluded studies involving paediatric patients or robotic assisted surgeries. Our evaluation focussed on pain relief scores, morbidity, hospital stay length, mortality rates and the development of endocrine and exocrine deficiencies in the patients. The patients in the analysis had a mean age of 36.5 and a male to female ratio of 1.4:1. The mean main pancreatic duct diameter was 11.5 mm. Tropical pancreatitis was identified as the primary cause. The surgical procedure was performed using 4 ports with minimal bleeding and a 13% morbidity rate. The conversion rate was 15%. The average operative time was 260 min and the mean hospital stay was 5.7 days. The results for pain control were excellent, as 90% of patients did not report pain in most series at the end of 3 years. The laparoscopic surgical management of chronic calcific pancreatitis with LPJ offers a safe and effective solution for pain relief in carefully chosen patients. Modern and upcoming high-speed optoelectronics as well as secure data storage or solar energy harvesting technologies integrating stimuli-responsive materials fully rely on the fundamental concept of rapid transitions between discrete states possessing different properties. Relatively slow transition kinetics between those states for commonly used classes of photochromic compounds in solution or bulk solids severely restrict the applicability of stimuli-responsive materials for device development. Herein, we report a multivariate strategy based on a photochromic spirooxazine derivative, coordinatively integrated in the solvent-free confined space of a solid-state matrix, such as a metal-organic framework (MOF), for the first time, resulting in the fastest photoresponse reported for any solid-state material to date. The photoisomerization rate for the developed photochromic material was estimated to be 126 s -1 , surpassing any literature reports to the best of our knowledge. We also shed light on the fundamentals of the correlation between framework topology, the nature of organic linkers, and the presence/absence of organic solvent within the scaffold voids on the material photoresponse using a series of isoreticular frameworks. Overall, the presented conceptual approach allows for tailoring the isomerization kinetics of photochromic molecules in the solid state over a range of 4 orders of magnitude-an unprecedented span that provides a pathway for addressing challenges associated with the response rate and photoisomerization, which are key criteria in stimuli-responsive material development.

This study investigated the clinical and physiological response to use of the BEACON Caresystem, a bedside open-loop decision support system providing advice to guide clinicians when weaning patients from invasive mechanical ventilation. Multicenter prospective study conducted in five adult intensive care units in the UK. Following screening and assent, intubated patients mechanically ventilated for > 24 h were randomized to intervention or usual care. Intervention consisted of application of the BEACON Caresystem's advice on tidal volume/inspiratory pressure, inspired oxygen, respiratory rate and PEEP. Usual care was defined as local clinical practice. The primary outcome was duration of mechanical ventilation. Secondary outcomes quantified prolonged intubation and survival; adverse events; ventilator settings and physiological state; time spent in ventilator modes; links to other therapy; the frequency of advice utilization and time spent outside normal physiological limits. The study was terminated early with a total of 112 patients included. Fifty-four were randomised to the intervention arm and fifty-eight to usual care. The study was underpowered and no significant differences were seen in duration of mechanical ventilation (p = 0.773), prolonged intubation or survival. Intervention arm patients had lower rates of adverse events (p = 0.016), including fewer hypoxaemic events (p = 0.008) and lower values of PEEP (p = 0.030) and tidal volume (p = 0.042). Values of peak inspiratory pressure and pressure support were reduced but at the boarder of statistical significance (p = 0.104, p = 0.093, respectively). No differences were seen for time in ventilator mode or other therapy. Advice presented by the decision support system was applied at the beside an average of 88% of occasions, with a significantly increased number of changes only in inspired oxygen fraction. No significant differences were seen in time spent outside physiological limits. This study investigated the use of the BEACON Caresystem, an open loop clinical decision support system providing advice on ventilator settings. It was terminated early, with no significant difference shown in duration of mechanical ventilation, the primary outcome. Angelman syndrome (AS) is a rare genetic neurodevelopmental disorder with profoundly debilitating symptoms with no FDA-approved cure or therapeutic. Brain-derived neurotrophic factor (BDNF), and its receptor tropomyosin receptor kinase B (TrkB), have a well-established role as regulators of synaptic plasticity, dendritic outgrowth and spine formation. Previously, we reported that the association of postsynaptic density protein 95 (PSD-95) with TrkB is critical for intact BDNF signaling in the AS mouse model, as illustrated by attenuated PLCγ and PI3K signaling and intact MAPK pathway signaling. These data suggest that drugs tailored to enhance the TrkB-PSD-95 interaction may provide a novel approach for the treatment of AS and a variety of neurodevelopmental disorders (NDDs). To evaluate this critical interaction, we synthesized a class of high-affinity PSD-95 ligands that bind specifically to the PDZ3 domain of PSD-95, denoted as Syn3 peptidomimetic ligands. We evaluated Syn3 and its analog D-Syn3 (engineered using dextrorotary (D)-amino acids) in vivo using the Ube3a exon 2 deletion mouse model of AS. Following systemic administration of Syn3 and D-Syn3, we demonstrate improvement in the seizure domain of AS. Learning and memory using the novel object recognition assay also illustrated improved cognition following Syn3 and D-Syn3, along with restored long-term potentiation. A pharmacokinetic analysis of D-Syn3 demonstrates that it crosses the blood-brain barrier (BBB), and the brain influx rate is in the range of CNS therapeutics. Finally, D-Syn3 treated mice showed a partial rescue in motor learning. Neither Syn3 nor D-Syn3 improved gross exploratory locomotion deficits, nor gait impairments that have been well documented in the AS rodent models. Purpose: To elucidate etiology and management of retropupillary sulcus migration of intravitreal gas after uneventful retinal detachment repair surgery., Methods: 70 year old Caucasian man presented with a temporal macula-off rhegmatogenous retinal detachment. 25-gauge (25G) pars plana vitrectomy was performed with cryopexy to retinal tear and 12% C3F8 gas tamponade under sub-Tenon's anaesthesia. At one week review, there was an elevated IOP of 28mmHg with migration of gas to the retropupillary space. Superiorly, iris was displaced anteriorly causing iridocorneal touch. There was no phacodonesis nor subluxation and retina was attached with a cryopexy scar under a 80% vitreous cavity gas fill., Result: On treatment with topical IOP-lowering agents until two-week review, IOP had normalised to 18mmHg with persistent 50% gas fill in the retropupillary sulcus and superior iridocorneal touch.Retropupillary gas resorbed at week four with normalisation of IOP, a localised superior anterior subcapsular cataract with associated posterior synechiae, and no iridocorneal touch. Best-corrected visual acuity was 6/12 Snellen following resorption of vitreous cavity gas., Conclusion: Medical management may be adequate if there is no complete pupil block and adequate posterior gas fill. An ultra-sensitive, high-performance liquid chromatography-based laser-stimulated fluorescence detection system was developed and validated for the simultaneous detection of 20 derivatized amino acids. Dansyl chloride was used as a derivatizing agent, and key derivatization parameters, such as reaction time and temperature, were optimized to enhance sensitivity and reproducibility. The majority of amino acids showed a relative standard deviation of less than 5%, indicating the reliability of the approach. The method demonstrated excellent sensitivity for all 20 amino acids, with detection limits ranging from 4.32 to 85.34 femtomoles. It also exhibited good linearity, with regression ( R 2 ) values greater than 0.98 for the amino acids. The system's performance was tested on human serum, and the eluted amino acids were identified. This method has great potential for analyzing amino acids in various body fluids and can be used in various clinical applications. Purpose: The purpose of this study was to review the pharmacokinetic profile of gentamicin among neonates with hypoxic ischemic encephalopathy (HIE) undergoing therapeutic hypothermia (TH) treatment., Methods: This was a retrospective study of neonates with HIE undergoing TH in the neonatal intensive care unit who received gentamicin between 2009 and 2014. Demographic information, diagnoses, laboratory test results, and medication administration and monitoring information were collected, and data were analyzed using SciPy., Results: A total of 57 neonates were analyzed. The median birth weight was 3.25 kg (interquartile range [IQR] 2.8-3.68), and median gestational age was 39 weeks (IQR 38-40). An elevated gentamicin trough level (defined as > 2 mg/L) was observed in 61% (35/57) of neonates. Half of the neonates (49%) had multiple gentamicin trough levels obtained, and 4% of the neonates were switched to an alternate agent. There was a significant difference in the number of dosing interval changes in neonates with elevated gentamicin trough levels compared with those with therapeutic gentamicin trough levels (P < 0.001). Of the neonates with elevated gentamicin trough levels, 14% (5/35) failed their hearing screen (P = 0.389)., Conclusions: Neonates with HIE undergoing TH may have an altered pharmacokinetic profile, requiring multiple blood draws so medication levels can be monitored and doses adjusted. Traditional gentamicin dosing regimens may not be ideal for this patient population, and further guidance is required for alternative treatment regimens., (© 2024. Photochromic materials with properties that can be dynamically tailored as a function of external stimuli are a rapidly expanding field driven by applications in areas ranging from molecular computing, nanotechnology, or photopharmacology to programable heterogeneous catalysis. Challenges arise, however, when translating the rapid, solution-like response of stimuli-responsive moieties to solid-state materials due to the intermolecular interactions imposed through close molecular packing in bulk solids. As a result, the integration of photochromic compounds into synthetically programable porous matrices, such as metal-organic frameworks (MOFs), has come to the forefront as an emerging strategy for photochromic material development. This review highlights how the core principles of reticular chemistry (on the example of MOFs) play a critical role in the photochromic material performance, surpassing the limitations previously observed in solution or solid state. The symbiotic relationship between photoresponsive compounds and porous frameworks with a focus on how reticular synthesis creates avenues toward tailorable photoisomerization kinetics, directional energy and charge transfer, switchable gas sorption, and synergistic chromophore communication is discussed. Background: While many patients seem to recover from SARS-CoV-2 infections, many patients report experiencing SARS-CoV-2 symptoms for weeks or months after their acute COVID-19 ends, even developing new symptoms weeks after infection. These long-term effects are called post-acute sequelae of SARS-CoV-2 (PASC) or, more commonly, Long COVID. The overall prevalence of Long COVID is currently unknown, and tools are needed to help identify patients at risk for developing long COVID., Methods: A working group of the Rapid Acceleration of Diagnostics-radical (RADx-rad) program, comprised of individuals from various NIH institutes and centers, in collaboration with REsearching COVID to Enhance Recovery (RECOVER) developed and organized the Long COVID Computational Challenge (L3C), a community challenge aimed at incentivizing the broader scientific community to develop interpretable and accurate methods for identifying patients at risk of developing Long COVID. From August 2022 to December 2022, participants developed Long COVID risk prediction algorithms using the National COVID Cohort Collaborative (N3C) data enclave, a harmonized data repository from over 75 healthcare institutions from across the United States (U.S.)., Findings: Over the course of the challenge, 74 teams designed and built 35 Long COVID prediction models using the N3C data enclave. The top 10 teams all scored above a 0.80 Area Under the Receiver Operator Curve (AUROC) with the highest scoring model achieving a mean AUROC of 0.895. Included in the top submission was a visualization dashboard that built timelines for each patient, updating the risk of a patient developing Long COVID in response to clinical events., Interpretation: As a result of L3C, federal reviewers identified multiple machine learning models that can be used to identify patients at risk for developing Long COVID. Background: As an investigative program around the development of novel oomycetes fungicides, a systematic exploration of piperidinyl thiazole fungicides employing a bioisosteric replacement strategy was planned., Results: A series of novel piperidinyl thiazole analogues were designed, synthesised, and evaluated as fungicides in vitro and in vivo against three oomycete fungal pathogens: Phytophthora infestans (P. infestans), Plasmopara viticola (P. viticola), and Pseudoperonospora cubensis (P. cubensis). Variation at four different regions of piperidinyl thiazole fungicides like Oxathiapiprolin and Fluoxapiprolin was considered. All variations delivered excellent in vitro and in vivo activity against P. infestans at extremely low concentrations. O-linked heterocyles, bicyclic piperidines, and sulfoximine-substituted aryls demonstrated the most robust transfer of in vitro activity into the greenhouse, while pyridones, acylated sulfoximines, and sultams showed less consistent results concerning the transfer. Specific compounds were selected for deeper evaluation of oomycetes activity. Several of them with representatives of all the series showed an excellent in vivo activity against P. viticola and P. cubensis. Among the most active compounds, P14 and P25 were further profiled in advanced studies and were found to have a robust curative potential. P14, P15, and P25 provided excellent control of P. infestans (in potato), P. viticola (in grapes), and P. cubensis (in cucumber) in field trials at dose rates of 20 and 30 g per hectare., Conclusions: A bioisosteric replacement strategy was successfully employed to identify novel piperidine thiazole derivatives with robust and commercially interesting control of oomycetes diseases in the greenhouse and under field conditions. Background: Psychiatric social workers typically play an important role in the treatment and management of psychosocial issues at different levels, such as individual, family, and community levels. However, despite the field having a long history, there are significantly fewer standardized guidelines for interventions in mental health settings., Methodology: This study aims to develop guidelines for psychiatric social work interventions in mental health settings. Psychiatric social workers with a degree of M.Phil. in Psychiatric Social Work, 2 years of working experience, and working in mental health settings were selected for this study. Using purposive sampling, 15 psychiatric social workers were selected for this study and were interviewed using a semi-structured interview schedule. The data were analyzed using the qualitative method of content analysis., Results: There is a high level of satisfaction rate among psychiatric social workers about their work and profession, however, they face challenges because of non-clarity of roles and responsibilities, lack of theories, and difficulties in implementing theories into practice. More training, discussion, supervision, and exposure to different cases help improve the interventions' quality., Conclusion: Although psychiatric social work interventions are effective in addressing the treatment gap by providing curative, preventive, promotive, and rehabilitative services, there is no uniformity in the interventions, which in turn, explains the need for standardized guidelines for intervention. Background: Most extremely preterm (EP) infants are vitamin D deficient (serum 25-hydroxyvitamin D levels below 20 ng/mL), and optimal supplementation practices for EP infants remain unknown. Our objective is to assess current vitamin D supplementation practices in U.S. neonatal intensive care units (NICU) for EP infants to provide baseline information for the design of future clinical trials., Methods: We conducted an online survey to study vitamin D intake and supplementation practices in U.S. NICUs caring for EP infants. Descriptive statistics compared responses by affiliation and level of care., Results: We analyzed responses from 253 NICUs, representing the majority of academic and level IV centers. Nearly all centers (97%) provided enteral vitamin D supplementation during the NICU stay, with 400 IU/day as the most common dosage (77%). Over half (56%) used feeding volume to initiate supplementation, with 71% of centers starting after achieving at least 120 ml/kg/day. Additionally, 94% of NICUs reported prescribing a vitamin D supplementation at discharge., Conclusions: Most NICUs in the U.S. supplement EP infants with 400 IU/day of enteral vitamin D. Clinical trials of vitamin D supplementation comparing the most common regimen to earlier and higher doses are needed to identify adequate regimens for EP infants., Impact: Despite the prevalence of vitamin D deficiency in extremely preterm (EP) infants at birth, optimal levels and supplementation strategies remain debated. Recent studies have suggested benefits of early high-dose vitamin D supplementation (800 IU/day) for reducing complications like bronchopulmonary dysplasia, infections, and disability. There is US center variation in timing and dose of vitamin D supplementation, being the most common regimen 400 IU/d started after established feedings (≥120 ml/kg/day). These findings inform and highlight the need for clinical trials of usual vs. early, higher-dose vitamin D supplementation to advance clinical outcomes and define desirable blood levels of EP infants., (© 2024. Purpose: Thyroid eye disease (TED) phenotype varies by ethnicity/race and genetic/environmental factors. This study compared demographic and clinical characteristics of TED patients from the US and India., Design: Observational pilot study ., Methods: Sixty-four patients with TED ages ≥18 years old with active disease (onset of symptoms ≤18 months or presenting clinical activity score (CAS) ≥4) were recruited between March and October 2021 from clinical practices in the United States (Philadelphia, PA) (n = 30) and India (Hyderabad, India) (n = 34). Data collection at baseline and 3 months included clinical measurements, thyroid disease history, and photographs. Ocular symptoms and quality of life data were obtained by phone interview. CAS was calculated using the standard 7-point scale., Results: There was no statistically significant difference in age, TED duration, or smoking status between patient groups. Both groups had good vision, a low rate of optic neuropathy, and comparable exophthalmometry. US patients were predominantly female (86.7% [26/30]), with a history of hyperthyroidism (96% 29/30). In comparison, Indian patients were 52.9% (18/34, P = .004) female; underlying thyroid disease was distributed between hyperthyroidism (52%), hypothyroidism (17.6%) and euthyroid (9.7%). Mean (SD) CAS in the US cohort was double the score in the India cohort (4.2 (1.7) vs 1.65 (1.7), respectively) (P < .0001). However, patients at both sites experienced subjective symptoms of TED at a similar frequency., Conclusions: Observed differences in TED between US and Indian patients warrant further investigation to elucidate differences in pathogenesis, disease expression, or other factors that may influence TED in diverse populations., (Copyright © 2024 Elsevier Inc. Prostate specific membrane antigen (PSMA) directed PET imaging has rapidly transformed prostate cancer workup over the past decade and paved the way for a theranostic approach using 177Lu-labelled PSMA radioligand therapy (RLT). This review gives an overview of the underlying principles behind PSMA as a target; the current use of PSMA PET in prostate cancer imaging and benefits compared to conventional imaging; and therapeutic applications including optimisation of patient selection. It also explores the evidence base of PSMA PET for other indications not in routine clinical use and the future of PSMA-directed RLT., (© The Author(s) 2024.
