Lichen sclerosus (LS) is a chronic inflammatory disease of the skin, and the gold standard for treatment is the use of the very potent topical steroids, but they can have side effects. Previously, we demonstrated that OZOILE (stable ozonides) were effective in children affected by LS, reducing the inflammatory process and stimulating tissue regeneration of the foreskin, showing a similar efficacy to steroid treatment. In this study, the modulation of inflammatory and oxidative stress pathways was evaluated by qRT-PCR and Western blotting in foreskins affected by LS removed from patients untreated or treated with OZOILE or corticosteroid cream formulations for 7 days before circumcision. OZOILE induced a significant increase in NRF2 and SOD2 levels, while it did not produce change in MIF, NF-kB subunits, and MMPs in comparison to untreated foreskins. Conversely, steroid topical treatment produced a significant reduction in the expression of p65, MIF, and MMP9, but it did not cause variation in NRF2 and SOD2 levels. These results demonstrate that the use of OZOILE as cream formulation exhibits effects on NRF2 signaling, and it does not induce NF-κB activation, unlike corticosteroids. On the basis of our biochemical data, further studies evaluating the role of NRF2 signaling cascade are necessary.

Abstract While alterations in nucleus size, shape, and color are ubiquitous in cancer, comprehensive quantification of nuclear morphology across a whole-slide histologic image remains a challenge. Here, we describe the development of a pan-tissue, deep learning-based digital pathology pipeline for exhaustive nucleus detection, segmentation, and classification and the utility of this pipeline for nuclear morphologic biomarker discovery. Manually-collected nucleus annotations were used to train an object detection and segmentation model for identifying nuclei, which was deployed to segment nuclei in H&E-stained slides from the BRCA, LUAD, and PRAD TCGA cohorts. Interpretable features describing the shape, size, color, and texture of each nucleus were extracted from segmented nuclei and compared to measurements of genomic instability, gene expression, and prognosis. The nuclear segmentation and classification model trained herein performed comparably to previously reported models. Features extracted from the model revealed differences sufficient to distinguish between BRCA, LUAD, and PRAD. Furthermore, cancer cell nuclear area was associated with increased aneuploidy score and homologous recombination deficiency. In BRCA, increased fibroblast nuclear area was indicative of poor progression-free and overall survival and was associated with gene expression signatures related to extracellular matrix remodeling and anti-tumor immunity. Thus, we developed a powerful pan-tissue approach for nucleus segmentation and featurization, enabling the construction of predictive models and the identification of features linking nuclear morphology with clinically-relevant prognostic biomarkers across multiple cancer types.

Background: Nordic walking (NW) has several potential benefits for individuals with cardiovascular (CV) disease, type 2 diabetes, and obesity and/or overweight. NW improves cardiovascular health, including exercise capacity and blood pressure control. NW enhances glycemic control and insulin sensitivity in diabetes, and aids in weight management and body composition improvement. NW offers additional advantages, such as improvement in muscular strength, joint mobility, physical activity levels, and psychological well-being. Methods: This open-label study with 3 arms will aim to evaluate the efficacy, safety, and adherence to exercise prescription in obese and/or overweight diabetic patients with CV complications. The primary objective will be to assess the CV performance of participants after a 6-month and a 12-month follow-up period, following a 3-month NW intervention, compared with standard rehabilitation, and with cardiological counseling (control group) training lasting 3 months. Results: The results of the study will provide valuable insights into the comparative effectiveness of a NW intervention vs standard rehabilitation and control group training in improving CV performance in obese and/or overweight diabetic patients with CV complications. Additionally, safety and adherence data will help inform the feasibility and sustainability of the exercise prescription over an extended period. Conclusions: These findings may have implications for the development of tailored exercise programs for this specific patient population, with the aim of optimizing CV health outcomes. Clinical Trials Registration: NCT05987410 Résumé: Contexte: La marche nordique offre plusieurs bienfaits potentiels aux personnes atteintes d’une maladie cardiovasculaire (CV), de diabète de type 2, de surpoids ou d’obésité. Elle améliore la santé cardiovasculaire, notamment l’endurance à l’effort et la régulation de la pression artérielle, en plus de favoriser l’équilibre glycémique et d'accroître la sensibilité à l’insuline chez les personnes diabétiques. Elle facilite également la gestion du poids et l’amélioration de la composition corporelle. Par ailleurs, la marche nordique présente d’autres avantages, comme l’augmentation de la force musculaire, de la mobilité articulaire, du niveau d’activité physique et du bien-être psychologique. Méthodologie: Cette étude ouverte à 3 groupes vise à évaluer l’efficacité, la sécurité et l’observance des exercices prescrits chez des sujets diabétiques obèses ou en surpoids présentant des complications CV. Le principal objectif consistera à évaluer la performance CV des participants au cours d'une période de suivi de 6 et 12 mois après un programme de marche nordique de 3 mois, comparativement à un programme de réadaptation standard et à un programme d’encadrement en soins CV (groupe témoin) de 3 mois. Résultats: Les résultats de l’étude fourniront de précieux renseignements sur l’efficacité d’un programme de marche rapide comparativement à un programme de réadaptation standard et à un programme d’encadrement (groupe témoin) pour améliorer la performance CV chez des sujets diabétiques obèses ou en surpoids présentant des complications CV. Les données relatives à la sécurité et à l’observance permettront également d’évaluer la faisabilité et la viabilité de la prescription d’exercices sur une longue période. Conclusions: Ces résultats pourraient s’avérer utiles dans l’élaboration de programmes d’exercices spécifiquement conçus pour cette population de patients, afin d’optimiser les résultats en santé CV. Abstract The gold standard for enrollment and endpoint assessment in metabolic dysfunction‐associated steatosis clinical trials is histologic assessment of a liver biopsy performed on glass slides. However, obtaining the evaluations from several expert pathologists on glass is challenging, as shipping the slides around the country or around the world is time‐consuming and comes with the hazards of slide breakage. This study demonstrated that pathologic assessment of disease activity in steatohepatitis, performed using digital images on the AISight whole slide image management system, yields results that are comparable to those obtained using glass slides. The accuracy of scoring for steatohepatitis (nonalcoholic fatty liver disease activity score ≥4 with ≥1 for each feature and absence of atypical features suggestive of other liver disease) performed on the system was evaluated against scoring conducted on glass slides. Both methods were assessed for overall percent agreement with a consensus “ground truth” score (defined as the median score of a panel of three pathologists’ glass slides). Each case was also read by three different pathologists, once on glass and once digitally with a minimum 2‐week washout period between the modalities. From a physiological standpoint, aging is a progressive reduction in each organ system’s capacity to maintain homeostasis in the face of illness or stressors. With advancing age, gastrointestinal (GI) symptoms and signs may increase, not only due to the aging processes but also to the superimposed effects of comorbidities, which can badly affect digestive functions (i.e., diabetes, malignancy, etc.) and environmental exposure. In general, gastrointestinal symptoms in older people more often underlie organic pathologies, while GI functional disorders are less frequently diagnosed in this age group. Moreover, gastrointestinal disease can also present in a nuanced and atypical manner, making the diagnostic hypothesis and, consequently, the correct diagnosis and therapy more challenging. In addition, with reference to this age group, the clinical implications of gastrointestinal pathologies can be more severe due to a decreased physiologic reserve, with a higher risk for malnutrition resulting in falls, depression, social isolation, and a deterioration of functional status. In this review, we focused on the most frequent GI tract disorders, highlighting the main age-related changes, their epidemiological, pathophysiological and clinical implications, and any differences with younger patients.

Background/Aim: Prevalence of gastroduodenal endoscopic and histological lesions may modify over time due to different factors. We assessed both macroscopic and histological lesions currently detected at upper endoscopy performed in routine practice. Patients and Methods: Clinical, endoscopic, and histological data of consecutive adult patients referred for upper endoscopy in the 28 participating centres were analysed. Only patients who underwent the first endoscopic examination were considered. Prevalence of erosive/ulcerative lesions, cancers and extensive precancerous lesions in the stomach, and Helicobacter pylori infection was computed. Results: A total of 1,431 patients underwent endoscopy for gastro-oesophageal reflux symptoms (31.5%), dyspepsia (29.4%), or alarm symptoms (18.5%). Erosive oesophagitis or Barrett’s oesophagus was detected in 210 (14.7%) cases, peptic ulcer in 49 (3.4%), and a neoplastic lesion in 17 (1.2%). H. pylori was present in 201 (22.6%) cases, and extensive precancerous lesions on gastric mucosa in 46 (5.6%) patients. Gastric lesions were more prevalent in patients aged ≥50 years (26% vs. 18%; p = 0.001), and peptic ulcers were more frequently detected in patients with H. pylori (9.4% vs. 2.3%; p = 0.001) and in males (5.8% vs. 1.6%; p = 0.001), while neoplastic lesions in patients with alarm symptoms (3.8% vs. 0.6%; p = 0.001). Conclusions: The overall endoscopic lesions were more prevalent in patients aged ≥50 years, peptic ulcer and erosions were more frequent in H. pylori-infected patients, and extensive gastric precancerous lesions were present in less than 6% of cases.

Importanti contributi alla definizione della nozione di estetica e creatività colgono il contemporaneo mutamento di paradigma, il quale in modo trasversale alle scienze umane e a quelle biologiche mette in evidenza l’ineludibile dimensione incarnata dell’intelligenza e della creatività umana, traendone ispirazione e anche importanti sollecitazioni per ridefinire nozioni, concetti e teorie. Al fine di attivare una sensibilità etica ed estetica, in cui gioca un ruolo fondamentale la coscienza corporea, ci è sembrato necessario partire dagli aspetti epistemologici, per poi soffermarci sul “come” avviene e “cosa” emerge dall’integrazione dell’arte con la scienza. Tale incontro, reso possibile dall’interazione con un prodotto artistico, ad un livello fenomenologico ha messo in comunicazione due mondi: quello dell’artista e quello del fruitore artistico. Dall’osservazione della qualità di questo incontro, al “confine di contatto” tra i due mondi, emerge una connotazione poetica e poietica di nuovi legami associativi e, dunque, nuove sensazioni che si aggiungono alla costruzione mentale del mondo. Ciò permette di aprirci a nuove possibilità di espressione della creatività della mente umana e al superamento di qualunque forma di stereotipo, giudizio e pregiudizio, a cui siamo sempre più esposti. Partendo dagli Archetipi e dalle ultime ricerche nel campo della neuroestetica, si vuole utilizzare nella disciplina della psicosomatica e come approccio pedagogico all’arte un metodo di “Esperienza Estetica Incarnata Creativa Consapevole”® di stampo bio-psico-sociale. In un’ottica Gestaltica, questa ipotesi metodologica mira a comprendere i meccanismi e i processi che sono alla base delle transizioni efficaci tra l’individuo e l’ambiente, agevolando un sano sviluppo del Sé creativo. Tale proposta metodologica fonde tecniche corporee con quelle immaginative attraverso la visualizzazione e l'interpretazione gestuale di un'opera d'arte, per consentire di avviare un percorso di empowerment psicocorporeo e permettere una gestione creativa del proprio mondo emozionale. Il metodo, con la sua sperimentazione ormai decennale, è stato già utilizzato come intervento di Medical Humanities nell’Azienda Sanitaria Locale di Napoli, nei Centri di riabilitazione e nei Corsi Universitari. Nello specifico di questo studio pilota, l'efficacia del metodo è stata monitorata da osservazioni partecipate di psicologi tutor circa i vissuti personali post-laboratoriali dei partecipanti, inoltre è stato utilizzato un questionario auto valutativo degli indicatori rappresentativi della qualità della comunicazione non verbale: la Diade Rev.3. Al fine di sostenere il processo di co-creazione dell’esperienza relazionale vissuta durante i laboratori sono state raccolte testimonianze narrative riferite alle percezioni personali di ciascun partecipante, poi archiviate in un e-book quale report dell’intero percorso esperienziale. L’obiettivo è stato sviluppare un percorso di riappropriazione corporea e di decodificazione imitativa dell’opera d’arte, al fine di attivare l'esperienza empatica attraverso l'esplorazione dell’aspetto motorio, delle life skills, della creatività, delle capacità di coping ed il miglioramento delle capacità relazionali. Attraverso la rappresentazione artistica, l’utilizzo di tecniche psicocorporee e la catarsi è stato possibile prendere consapevolezza di aspetti profondi della propria realtà psicologica ed esistenziale e avviare un processo di risoluzione di conflitti interni e con l'ambiente esterno, condividendone il senso per farne cultura.

Introduction The treatment of patients with cardiogenic shock (CS) encompasses several health technologies including Impella pumps and venoarterial extracorporeal membrane oxygenation (VA-ECMO). However, while they are widely used in clinical practice, information on resource use and quality of life (QoL) associated with these devices is scarce. The aim of this study is, therefore, to collect and comparatively assess clinical and socioeconomic data of Impella versus VA-ECMO for the treatment of patients with severe CS, to ultimately conduct both a cost-effectiveness (CEA) and budget impact (BIA) analyses.Methods and analysis This is a prospective plus retrospective, multicentre study conducted under the scientific coordination of the Center for Research on Health and Social Care Management of SDA Bocconi School of Management and clinical coordination of Istituto di Ricovero e Cura a Carattere Scientifico (IRCCS) San Raffaele Scientific Institute in Milan. The Impella Network stemmed for the purposes of this study and comprises 17 Italian clinical centres from Northern to Southern Regions in Italy. The Italian network qualifies as a subgroup of the international Impella Cardiac Surgery Registry. Patients with CS treated with Impella pumps (CP, 5.0 or 5.5) will be prospectively recruited, and information on clinical outcomes, resource use and QoL collected. Economic data will be retrospectively matched with data from comparable patients treated with VA-ECMO. Both CEA and BIA will be conducted adopting the societal perspective in Italy. This study will contribute to generate new socioeconomic evidence to inform future coverage decisions.Ethics and dissemination As of May 2024, most of the clinical centres submitted the documentation to their ethical committee (N=13; 76%), six centres received ethical approval and two centres started to enrol patients. Study results will be published in peer-reviewed publications and disseminated through conference presentations.

Mild cognitive impairment (MCI) is a heterogeneous condition definable as the intermediate clinical state between normal aging and dementia. As a pre-dementia condition, there is a recent growing interest in the identification of non-invasive markers able to predict the progression from MCI to a more advanced stage of the disease. Previous evidence showed the close link between gut microbiota and neurodegenerative diseases, such as Alzheimer's (AD) and Parkinson's disease (PD). Conversely, the actual relationship between gut microbiota and MCI is yet to be clarified. In this work, we provide an overview about the current knowledge regarding the role of gut microbiota in the context of MCI, also assessing the potential for microbiota-targeted therapies. Through the review of the most recent studies focusing on this topic, we found evidence of an increase of Bacteroidetes at phylum level and Bacteroides at genus level in MCI subjects with respect to healthy controls and patients with AD. Despite such initial evidence, the definitive identification of a typical microbiota profile associated with MCI is still far from being achieved. These preliminary results, however, are growingly encouraging research on the role of gut microbiota modulation in improving the cognitive status of pre-dementia subjects. To date, few studies evaluated the role of probiotics in MCI subjects, and they showed favorable results, although still biased by small sample size, heterogeneity of study design and short follow-up.

Acute diarrhea represents a major public health issue, and the management of adult patients admitted to the emergency department (ED) for this problem is still challenging. In a retrospective analysis on more than 20,000 patients visiting a tertiary ED for acute diarrhea and then being discharged home, we found that age > 65 years, onset of symptoms > 24 h since ED admission, refusal of hospitalization, and a history of chronic renal and liver diseases were independently associated with ED readmission for abdominal symptoms within 7 days. In the younger group, the presence of comorbidities significantly impacted on ED readmission, while fever and alteration of serum creatinine were the main determinants in the older group. Antibiotics were prescribed in about 25% of patients, although diarrhea etiology (viral or bacterial) was usually not available. According to international guidelines, fluoroquinolones were the most prescribed class, showing an inverse correlation to ED readmission. However, β-lactams and probiotics were also commonly prescribed; the latter were independently correlated to ED readmission in the elderly group. A comprehensive, guideline-based approach, including a detailed clinical history and laboratory and comorbidity assessment, should be encouraged to support physicians in the management of different age subgroups of adults admitted to the ED for acute diarrhea.

OBJECTIVES:. In patients with COVID-19 respiratory failure, controlled mechanical ventilation (CMV) is often necessary during the acute phases of the disease. Weaning from CMV to pressure support ventilation (PSV) is a key objective when the patient’s respiratory functions improve. Limited evidence exists regarding the factors predicting a successful transition to PSV and its impact on patient outcomes. DESIGN:. Retrospective observational cohort study. SETTING:. Twenty-four Italian ICUs from February 2020 to May 2020. PATIENTS:. Mechanically ventilated ICU patients with COVID-19-induced respiratory failure. INTERVENTION:. The transition period from CMV to PSV was evaluated. We defined it as “failure of assisted breathing” if the patient returned to CMV within the first 72 hours. MEASUREMENTS AND MAIN RESULTS:. Of 1260 ICU patients screened, 514 were included. Three hundred fifty-seven patients successfully made the transition to PSV, while 157 failed. Pao2/Fio2 ratio before the transition emerged as an independent predictor of a successful shift (odds ratio 1.00; 95% CI, 0.99–1.00; p = 0.003). Patients in the success group displayed a better trend in Pao2/Fio2, Paco2, plateau and peak pressure, and pH level. Subjects in the failure group exhibited higher ICU mortality (hazard ratio 2.08; 95% CI, 1.42–3.06; p < 0.001), an extended ICU length of stay (successful vs. failure 21 ± 14 vs. 27 ± 17 d; p < 0.001) and a longer duration of mechanical ventilation (19 ± 18 vs. 24 ± 17 d, p = 0.04). CONCLUSIONS:. Our study emphasizes that the Pao2/Fio2 ratio was the sole independent factor associated with a failed transition from CMV to PSV. The unsuccessful transition was associated with worse outcomes.

Crocus sativus L., commonly known as saffron, is a precious spice coming from Asia, in particular from Iran, the country leader in its production. The spice is derived exclusively from dried stigmas and it is the most expensive one in the world. The areas of application of saffron are multiple, in fact ranging across the food, drinks, pharmaceuticals and cosmetics sectors. As is the case with other phytochemicals, not only the final product but also saffron by-products are considered a valuable source of bioactive natural compounds. In fact, its healthy effects, especially as antioxidants and anti-inflammatories (via reducing pro-inflammatory cytokines), are well-recognized in internal medicine. In particular, its healthy effects are related to counteracting degenerative maculopathy, depression and anxiety, neurodegenerative diseases, metabolic syndrome, cancer and chronic kidney disease, by promoting glucose metabolism. In this review, we summarize the most important papers in which saffron has turned out to be a valuable ally in the prevention and treatment of these pathologies. Moreover, we would like to promote the use of saffron by-products as part of a bio-circular economy system, aimed at reducing wastes, at maximizing the use of resources and at promoting environmental and economic sustainability.

Platelets have a fundamental role in mediating hemostasis and thrombosis. However, more recently, a new idea is making headway, highlighting the importance of platelets as significant actors in modulating immune and inflammatory responses. In particular, platelets have an important role in the development of vascular amyloid-b-peptide(ab) deposits, known to play a relevant role in Alzheimer’s disease (AD) through accumulation and deposition within the frontal cortex and hippocampus in the brain. The involvement of platelets in the pathogenesis of AD opens up the highly attractive possibility of applying antiplatelet therapy for the treatment and/or prevention of AD, but conclusive results are scarce. Even less is known about the potential role of platelets in mild cognitive impairment (MCI). The aim to this brief review is to summarize current knowledge on this topic and to introduce the new perspectives on the possible role of platelet activation as therapeutic target both in AD and MCI.

Three-dimensional printing technologies are becoming increasingly attractive for their versatility; the geometrical customizability and manageability of the final product properties are the key points. This work aims to assess the feasibility of producing radiopaque filaments for fused deposition modeling (FDM), a 3D printing technology, starting with zinc oxide (ZnO) and polylactic acid (PLA) as the raw materials. Indeed, ZnO and PLA are promising materials due to their non-toxic and biocompatible nature. Pellets of PLA and ZnO in the form of nanoparticles were mixed together using ethanol; this homogenous mixture was processed by a commercial extruder, optimizing the process parameters for obtaining mechanically stable samples. Scanning electron microscopy analyses were used to assess, in the extruded samples, the homogenous distribution of the ZnO in the PLA matrix. Moreover, X-ray microtomography revealed a certain homogenous radiopacity; this imaging technique also confirmed the correct distribution of the ZnO in the PLA matrix. Thus, our tests showed that mechanically stable radiopaque filaments, ready for FDM systems, were obtained by homogenously loading the PLA with a maximum ZnO content of 6.5% wt. (nominal). This study produced multiple outcomes. We demonstrated the feasibility of producing radiopaque filaments for additive manufacturing using safe materials. Moreover, each phase of the process is cost-effective and green-oriented; in fact, the homogenous mixture of PLA and ZnO requires only a small amount of ethanol, which evaporates in minutes without any temperature adjustment. Finally, both the extruding and the FDM technologies are the most accessible systems for the additive manufacturing commercial apparatuses.

Positive interactions among foundation species play a crucial role in achieving and maintaining a good state of the marine environment, enhancing the habitat stability, productivity, and the whole ecosystem functioning. In this regard, macroalgal canopies may affect the biotic and abiotic environmental features, improving the habitat quality for the establishment of other sedentary organisms. In this study, we investigated the effect of intertidal canopy-forming algae on the colonization success of the central-Mediterranean vermetid reef-builder Dendropoma cristatum (Biondi 1859) on artificial substrates produced for reef restoration purposes. An in situ experiment was carried out along the northwestern coast of Sicily during the breeding season of the vermetid snails, by using geopolymer concrete settlement discs with a topographic design to facilitate the vermetid settlement. The discs were placed on the seaward reef rim, underneath the macroalgal canopy, and on adjacent control bare reef areas. The canopy effect on the understory algal colonization and the environmental temperature at the vermetid settlement substrate were also surveyed. After 35 days of field exposure, the vermetid settlement increased by 1.7-fold on the discs underneath the macroalgal canopy compared with that on the bare settlement discs. Moreover, the understory algae showed a higher homogeneity and a lower percent cover on the settlement discs underneath the macroalgal canopy. The peaks of temperature were higher on the bare reef, and the macroalgal canopy also reduced temperature variability under maximum sun irradiance during the diurnal low tides. The vermetid settlement was positively correlated to the canopy cover, which, directly or indirectly, improved the success of colonization of the reef-builder snails on artificial substrates. The deployment of settlement discs where canopy-forming algae are naturally present may facilitate the gardening of vermetid clusters that may be translocated to restock the reef-builder density at degraded areas.

The occurrence of an abdominal tumor invading the spinal canal and causing symptoms of epidural compression is rare in an infant, and exceptional at birth. Peripheral neuroblastic tumors are by far the most common cause. Emergency chemotherapy is commonly curative, though permanent sequelae are possible. Although other malignancies may be involved, no case of rhabdoid tumors at birth has been reported. We describe the case of a neonate who presented symptoms of spinal epidural compression at birth secondary to a rhabdoid tumor. As expected with this highly malignant tumor, the patient experienced a rapidly progressive clinical course and died within three months of diagnosis.

Long coronavirus disease (COVID) syndrome leads to chronic inflammatory state onset that can have a multisystem impact and compromise organ function. Moreover, long COVID syndrome is often characterized by the presence of chronic fatigue, which affects subjects’ daily activities and worsens their quality of life. The aim of our double-blind, placebo-controlled randomized trial (protocol code RS 150.21, approved on 4 November 2021) was to evaluate the beneficial effects of the consumption of 2 cps/day, for two months, of an oral food supplement (OFS), based on Echinacea angustifolia, rosehip, propolis, royal jelly and zinc, in long COVID patients, compared to a two-month placebo period. The OFS’s vitamin C content was equal to 22.17 mg/g (8.87 mg/capsule). The OFS’s total polyphenol content was 43.98 mg/g gallic acid equivalents. At the end of the in vivo study, we highlighted a significant decrease in the inflammatory parameters in the OFS period, compared to the placebo period (neutrophil-to-lymphocyte ratio, p = 0.0455; monocyte to-lymphocyte ratio, p = 0.0005; C-reactive protein, p = 0.0145). Our study also highlighted a significant increase in vitamin D serum values (p = 0.0005) and, at the same time, an improvement in patients’ life quality and a reduction in fatigue, monitored by the fatigue severity scale. This study showed the OFS’s beneficial effects on the inflammatory state, fatigue and quality of life in long COVID patients.
