Purpose: The purpose of this study is to discuss the feasibility of developing, and the challenges of implementing, a structured dysphagia program in a rural patient population with preliminary data. Method: The Dysphagia Pathway Program (DPP) is a standardized, prophylactic swallow therapy program for head and neck cancer patients initiated in 2019 as a multidisciplinary approach at Penn State Hershey Medical Center. An interrupted time series study was conducted comparing the swallow and nutritional status of patients pre- and postimplementation of the DPP. Data were analyzed using the chi-square test and linear mixed-effects models to determine statistical significance for categorical and continuous variables, respectively. Results: The DPP group included 52 patients, and the pre-implementation group included 109 patients. A higher percentage of patients in the DPP group tolerated a by mouth (PO)-only diet at the last day of treatment time point (p < .001). The DPP group showed similar changes in body mass index and albumin over time but had lower rates of gastrostomy tube placements (p = .002) compared to the pre-implementation group. The average adherence to speech and swallow therapy in the DPP group decreased over time. Conclusions: Although we encountered many obstacles in the development of the DPP, we found prophylactic swallow therapy to be feasible to implement and beneficial in our rural patient population. In this pilot study, our results demonstrated that prophylactic swallow therapy shows a favorable trend in swallowing and nutritional status. Background: Differences in treatment outcomes between community or academic centers are incompletely understood. Methods: Retrospective review of head and neck cancer patients between 2010 and 2020 in a rural health region. Kaplan–Meier curves and log‐rank tests were used to evaluate survival outcomes, along with bivariate and multivariable Cox proportional hazards models. Linear regression was used for functional outcomes of tracheotomy and gastrostomy tube dependence. Results: Two hundred and forty‐eight patients treated at an academic center were compared with 94 patients treated in community centers. In multivariable analysis, the risk of death (HR = 0.60, p = 0.019), and risk of recurrence were lower (HR = 0.29, p < 0.001) for patients treated in academic centers. Patients treated in community centers had longer gastrostomy tube dependence (p = 0.002). Conclusion: Our findings suggest that treatment at an academic center was associated with a lower risk of recurrence and shorter gastrostomy tube dependence compared to treatment in the community. Objectives: The aim of this study was to examine students' perceptions of pass/fail scoring for Step 1, particularly when applying to competitive specialties. This study also investigated whether this transition increased anxiety among medical students and elicited student perspectives on the most critical components of residency applications., Introduction: The United States Medical Licensing Exam (USMLE) Step 1 Board Exam transitioned from a traditional numeric score to pass/fail for exams taken on or after January 26, 2022. One justification for this change, according to a statement from USMLE, was to ease the transition between undergraduate and graduate medical education. Although many factors are considered when selecting candidates to interview for residency, Step 1 scores were historically used as an important metric. Few studies specifically evaluated medical student's attitudes toward the change in Step 1 grading., Methods: An online survey was distributed to Penn State College of Medicine medical students in the 2024 to 2027 classes. Statistical analysis was conducted on responses to research plans, attitudes regarding Step 1 pass/fail, match rates, and importance of application components., Results: There was a 21.3% response rate (127/596) to the survey with 30.7% pursuing a competitive specialty. Students applying into competitive specialties felt their chances of matching changed significantly compared to students applying to other specialties (either increased (25.6% vs 10.2%, P  < .001), or decreased (28.2% vs10.3%, P  < .001). Majority of students (73.2%) agreed with the decision to make Step 1 pass/fail, however, students reported increased anxiety surrounding Step 2 scores. There was a significant association between which aspects students believed were most important for matching (letters of recommendation and performance on Acting Internships) and those they believed program directors would weigh heaviest ( P  < .001)., Conclusion: Medical students report increased anxiety with Step 2 after Step 1 became pass/fail, although students overwhelmingly agreed with this change. To address this, medical schools should offer further guidance and resources to students, focusing on mentorship and methods to optimize residency applications for their chosen specialties., Competing Interests: Dr. Karen Y. Purpose: Papillary thyroid carcinoma detection has increased dramatically in the United States. However, the indolent nature of papillary thyroid microcarcinoma (mPTC) has led the American Thyroid Association (ATA) to advocate for more conservative management. The 2015 ATA recommendations advocated for observation or lobectomy for mPTC. However, the majority of mPTCs continue to be treated with more aggressive surgical management. In this study, we aim to understand the management of mPTC based on facility variables., Materials and Methods: A retrospective observational study of patients diagnosed with mPTC between 2004 and 2018 was performed using the National Cancer Database incidence data. We collected data on patient sex, age, tumor size, race, ethnicity, geographic location, thyroid surgical volume at the facility, and treatment modality for mPTC were collected. Conservative and non-conservative treatment modalities based on patient and facility characteristics were compared both longitudinally and cross-sectionally between pre- and post-2015 ATA recommendations., Results: Total thyroidectomy with or without radioactive iodine ablation (RAI) remains the treatment of choice regardless of patient and facility characteristics. Patients treated at low-volume facilities were actually more likely to be treated conservatively., Conclusions: Despite 2015 ATA recommendations advocating for observation or lobectomy for mPTC, patients with mPTC are still more likely to be treated with total thyroidectomy with or without RAI, especially at high-volume facilities., Competing Interests: Declaration of competing interest The authors have no conflicts of interests., (Copyright © 2024.
