Simple Summary: Lung cancer, the second most common cancer in both men and women, is a major health issue, and early detection is key to reducing deaths from the disease. Radiologists, who read CT scans, play an important role in spotting lung cancers early, even those that are not clear nodules. One particular type of lung cancer, associated with cystic airspaces, has received growing attention in research, though it remains under-studied. These cancers, mainly adenocarcinomas, often appear in the outer parts of the lungs and can be confused with other pathologies like infections or emphysema. While it is estimated that lung cancers associated with cystic airspaces account for about a quarter of lung cancer diagnoses that are missed or delayed, more research is needed to better understand them. Raising awareness about these types of cancer could help catch them earlier and potentially save lives, all while being a cost-effective approach. Limited investigations with small sample sizes have estimated their prevalence and explored their radiological and pathological features. Lung cancers associated with cystic airspaces exhibit varying complexities within their cystic components and demonstrate suspicious changes over time. Adenocarcinoma is the predominant histological type, often with a peripheral location. Differential diagnosis on CT scans includes inflammatory processes or emphysema-related changes. Unfortunately, prospective studies specifically analyzing the prevalence of cystic airspace-associated lung cancers are lacking. However, it is estimated that they constitute approximately one-fourth of delayed radiological diagnoses. Hereditary congenital facial palsy (HCFP) is a medical condition caused by dysfunction of the seventh cranial nerve. HCFP is characterized by feeding difficulties and dysmorphic features in the orofacial region. In some cases hearing loss, strabismus, limb malformations, and musculoskeletal defects may be associated. There are three types of HCFP: HCFP3 (OMIM 614744) results from autosomal recessive pathogenic variants in the HOXB1 gene, while HCFP1 and 2 (OMIM 601471, 604185) are autosomal dominant, genetically less defined conditions. We report on a case of congenital bilateral facial palsy due to two novel compound heterozygous variants in the HOXB1 gene, found by exome sequencing (ES), in a child with facial nerve axonal neuropathy without evidence of nerve hypoplasia on neuroimaging. The results of this report suggest that in individuals with congenital facial paralysis and preserved ocular motor skills, with or without facial nerve hypoplasia and with confirmed facial nerve axonal neuropathy, HOXB1 variants and therefore a diagnosis of HCFP3 should be primarily considered.
