Abstract Near continuous phase transitions, universal power-law scaling, characterized by critical exponents, emerges. This behavior reflects the singular responses of physical systems to continuous control parameters like temperature or external fields. Universal scaling extends to non-equilibrium dynamics in isolated quantum systems after a quench, where time takes the role of the control parameter. Our research unveils critical scaling in time also during the relaxation dynamics of an open quantum system. Here we experimentally realize such a system by the spin of individual Cesium atoms dissipatively coupled through spin-exchange processes to a bath of ultracold Rubidium atoms. Through a finite-size scaling analysis of the entropy dynamics via numerical simulations, we identify a critical point in time in the thermodynamic limit. This critical point is accompanied by the divergence of a characteristic length, which is described by critical exponents that turn out to be unaffected by system specifics.

The following case report demonstrates a case of a chronic irreducible patellar dislocation, age‐indeterminate associated with a large medial patellar avulsion fracture that was treated with a vastus medialis obliquus advancement following an extensive lateral release. This case is the only known report of this kind in the literature. The patient is a 41‐year‐old Caucasian female who presented to the clinic with an age‐indeterminate, chronically dislocated patella. She has a past medical history of hypertension and ischemic stroke 1 year prior to presentation, leading to expressive aphasia and lower extremity weakness in addition to patellar instability dating back to age 13. An unsuccessful patellofemoral reduction was performed at an outside clinic, and she was placed in a knee immobilizer and referred to our office. Clinically, the patient had limited knee range of motion with a fixed lateral patellar dislocation that was confirmed on imaging. This case report demonstrates a surgical reduction technique that can be utilized by orthopedic surgeons for chronic patellar dislocations that are not amenable to routine patellar instability surgeries due to the contraction of surrounding soft tissue, chronic bone abnormalities, and position of the chronic dislocation. An extensive lateral release followed by vastus medialis obliquus advancement was performed to center the patella within the trochlear groove and to allow for stable articulation throughout range of motion. The patient was able to regain painless, full range of motion of her knee postoperatively with patellar stability noted on both physical exam and radiographic imaging. [ABSTRACT FROM AUTHOR]

Combined medial and lateral posterior meniscal root tears in the setting of an acute anterior cruciate ligament (ACL) rupture are extremely rare. The following case report demonstrates a high school football player who sustained a noncontact knee injury while performing a spin move at practice. The patient is a 17‐year‐old high school football defensive end who was presented to the clinic 1 week following the injury complaining of persistent knee pain with associated swelling, limited range of motion (ROM), and complaint of instability. During physical examination, the patient was found to have anterior cruciate laxity. Magnetic resonance imaging (MRI) demonstrated a complete midsubstance tear of the ACL and increased signal within the posterior horn of the medial meniscus with no obvious signs of pathology localized to the lateral meniscus. ACL reconstruction (ACLR) was performed and intraoperatively, both medial and lateral root tears were found. A standard bone patellar‐tendon bone (BTB) autograft ACLR was performed with combined medial and lateral root repair utilizing a transtibial pull‐out method for both. The clinical importance is root tears with associated ACL tears can be hard to diagnose on preoperative MRI, especially laterally, so careful assessment of both meniscal roots at the time of arthroscopy is critical. Furthermore, careful creation of the needed root repair tunnels for transtibial repair is critical to avoid coalescence with the ACL tibial tunnel. [ABSTRACT FROM AUTHOR]

Abstract As cities continue to grow globally, characterizing the built environment is essential to understanding human populations, projecting energy usage, monitoring urban heat island impacts, preventing environmental degradation, and planning for urban development. Buildings are a key component of the built environment and there is currently a lack of data on building height at the global level. Current methodologies for developing building height models that utilize remote sensing are limited in scale due to the high cost of data acquisition. Other approaches that leverage 2D features are restricted based on the volume of ancillary data necessary to infer height. Here, we find, through a series of experiments covering 74.55 million buildings from the United States, France, and Germany, it is possible, with 95% accuracy, to infer building height within 3 m of the true height using footprint morphology data. Our results show that leveraging individual building footprints can lead to accurate building height predictions while not requiring ancillary data, thus making this method applicable wherever building footprints are available. The finding that it is possible to infer building height from footprint data alone provides researchers a new method to leverage in relation to various applications.

Single element detection, spatial frequency modulation imaging (SPIFI) is deployed in a laser processing environment. SPIFI images are used to monitor a laser melting process, like that encountered during additive manufacturing, with an exposure time of 120μs, which enables real-time, in-situ monitoring of melt track formation coaxial with the processing laser. SPIFI images from a single photodiode are shown to be comparable to or better than white light camera images taken at the same numerical aperture. SPIFI deployed in this manner represents a disruptive new sensor metrology system that, when coupled with developing process models, can ultimately be used to validate statistically significant process parameter-signature-quality relationships with quantified uncertainty.

Abstract Leveraging high performance computing, remote sensing, geographic data science, machine learning, and computer vision, Oak Ridge National Laboratory has partnered with Federal Emergency Management Agency (FEMA) to build a baseline structure inventory covering the US and its territories to support disaster preparedness, response, and recovery. The dataset contains more than 125 million structures with critical attribution, and is ready to be used by federal agencies, local government and first responders to accelerate on-the-ground response to disasters, further identify vulnerable areas, and develop strategies to enhance the resilience of critical structures and communities. Data can be freely and openly accessed through Figshare data repository, ESRI’s Living Atlas or FEMA’s Geodata platform.

The article contains a description of the design solutions proposed by the authors for a hybrid wind turbine bearing, in which the sliding part takes over the load to the turbine shaft after reaching the shaft rotation speed, ensuring hydrodynamic lubrication of the plain bearing and relieving the rolling bearing. This allows for low starting resistance of the power plant and ensures quiet operation during use. Two conceptual solutions of a hybrid bearing were presented, differing in the shape of the plain bearing journal. A mechanism for automatic switching of the load between a rolling and a plain bearing was developed. A solid simulation model of this mechanism was built in the Autodesk Inventor—Dynamic Simulation software Inventor Professional 2023 environment, and its operation was simulated. The results confirmed the usefulness of using this design in shaft-bearing systems of wind turbines with a vertical axis of rotation. Based on the simulation, the speed at which the thrust roller bearing will be released was determined. Technical parameters of a plain bearing with a spherical journal shape were calculated. The height of the lubrication gap and the shaft rotational speed at which the bearing load capacity index reaches a critical value were determined.

This study aimed to understand the effect of C. jejuni challenge on the cecal microbiota and short-chain fatty acid (SCFA) concentration to form a better understanding of the host–pathogen interaction. Sixty broilers were randomly allocated into two treatments: control and challenge. Each treatment was replicated in six pens with five birds per pen. On day 21, birds in the challenge group were orally gavaged with 1 × 108C. jejuni/mL, while the control group was mock challenged with PBS. The C. jejuni challenge had no effect on body weight, feed intake, and feed conversion ratio compared to the control group. On day 28, the C. jejuni challenge decreased the observed features and Shannon index compared to the control group. On the species level, the C. jejuni challenge decreased (p = 0.02) the relative abundance of Sellimonas intestinalis on day 28 and increased (p = 0.04) the relative abundance of Faecalibacterium sp002160895 on day 35 compared to the control group. The C. jejuni challenge did not change the microbial function and the cecal concentrations of SCFA on days 28 and 35 compared to the control group. In conclusion, C. jejuni might alter the gut microbiota’s composition and diversity without significantly compromising broilers’ growth.

Hypertrophic cardiomyopathy (HCM) is a heterogeneous genetic disorder, most often caused by sarcomeric gene mutations, with a small proportion due to variants in non-sarcomeric loci. Phospholamban (PLN) is a phosphoprotein associated with the cardiac sarcoplasmic reticulum, a major determinant of cardiac contractility and relaxation. We conducted a retrospective study to determine the prevalence, phenotypical spectrum and clinical course of patients carrying the PLN p.Leu39* variant. A cohort including 11 PLN patients was identified among all patients with HCM (9/189, 4.8%) and DCM (2/62, 3.2%) who underwent genetic testing from two tertiary centers and five more were detected through cascade screening. Complete phenotyping was performed. PLN p.Leu39* variant-driven cardiomyopathy presented mostly as hypertrophic, with frequent progression to end-stage dilated HCM. We proceeded to compare these results to a similar analysis of a control cohort consisting of age-matched individuals that inherited pathogenic or likely pathogenic variants in common sarcomeric genes (MYBPC3/MYH7). Overall, the clinical characteristics and examination findings of patients carrying PLN p.Leu39* were not different from patients with cardiomyopathy related to sarcomeric mutations except for the presence of pathological Q waves and the incidence of non-sustained ventricular arrhythmias, which were higher in PLN patients than in those with MYBPC3/MYH7-related diseases.

Background: Chemotherapy with SOX (S-1 and oxaliplatin) regimen showed good efficacy and a favorable safety profile in advanced gastric cancer. Anti-programmed cell death protein 1 (PD-1) antibody camrelizumab also demonstrated antitumor activity in this setting in a phase I study. However, the efficacy and safety of camrelizumab plus SOX for advanced gastric cancer have not been investigated. Thus, this study aimed to address this objective., Methods: This phase II study evaluated the efficacy and safety of camrelizumab plus SOX in previously untreated advanced gastric cancer. Patients received camrelizumab (200 mg intravenously every 3 weeks) plus SOX (S-1, 40 mg/m 2 orally twice daily on days 1-14; oxaliplatin, 130 mg/m 2 intravenously on day 1 every 3 weeks) until disease progression, death, or intolerable toxicity. Camrelizumab was prescribed for up to a year. The primary endpoint was progression-free survival (PFS). The second endpoints were overall survival (OS), objective response rate (ORR), and disease control rate (DCR)., Results: A total of 25 patients were enrolled and received at least 1 dose of study drug. The median PFS was 7.4 months [95% confidence interval (CI): 5.6-16.4]. The median OS was 20.2 months (95% CI: 10.5-29.9); ORR was 36% (95% CI: 18.0-57.5%); DCR was 92% (95% CI: 74.0-99.0%). Purpose: To evaluate ≥2-year patient outcomes after primary all-soft tissue quadriceps tendon autograft (ASTQ) anterior cruciate ligament reconstruction (ACLR) with suture tape augmentation (STA) in skeletally mature high school and collegiate athletes., Methods: All high school and collegiate athletes who underwent primary ASTQ ACLR with STA with a minimum of 2-year follow-up were analyzed retrospectively. Patients were administered validated patient-reported outcome measures (PROMs) pre- and postoperatively. The minimal clinically important difference was calculated for each PROM based on this study population and applied to the individual patient. Return to sport, subsequent surgical intervention including contralateral ACLR, and KT-1000 arthrometer measurements for knee laxity were collected. Complications were assessed by physical examination, radiologic studies, or obtained via telephone., Results: In total, 60 patients were included in the final data analysis, with a mean age of 16.8 years (95% confidence interval 13-23) and mean final follow-up of 37.1 months (95% confidence interval 33.1-41.1). Twelve patients (20%) required subsequent surgery on the ipsilateral knee, which included 7 patients having a subsequent meniscal procedure and 3 patients who underwent arthrolysis. None sustained a graft failure, and 6 patients sustained a contralateral ACL injury necessitating surgery. All PROMs improved at the final follow-up (P < .001). In addition, KT-1000 arthrometer measurements significantly improved postoperatively at 1-year clinical follow-up (P < .001). Most patients obtained the minimal clinically important difference thresholds for each PROM at the final follow-up. There were 48 patients (80%) who participated in pivoting sports. The return-to-sport rate at same level was 54 patients (90%), with 6 patients (10%) not returning to the same level because of graduation., Conclusions: ASTQ ACLR with STA in a young athletic patient population may result in a low graft failure rate while maintaining satisfactory patient outcomes at short-term follow-up, including a return to sport at the same level of 90%., Level of Evidence: Level IV, retrospective case series., (Copyright © 2024 Arthroscopy Association of North America. Purpose: Despite educational mandates to assess resident teaching competence, limited instruments with validity evidence exist for this purpose. Existing instruments do not allow faculty to assess resident-led teaching in a large group format or whether teaching was interactive. This study gathers validity evidence on the use of the Resident-led Large Group Teaching Assessment Instrument (Relate), an instrument used by faculty to assess resident teaching competency. Relate comprises 23 behaviors divided into six elements: learning environment, goals and objectives, content of talk, promotion of understanding and retention, session management, and closure., Methods: Messick's unified validity framework was used for this study. Investigators used video recordings of resident-led teaching from three pediatric residency programs to develop Relate and a rater guidebook. Faculty were trained on instrument use through frame-of-reference training. Resident teaching at all sites was video-recorded during 2018-2019. Two trained faculty raters assessed each video. Descriptive statistics on performance were obtained. Validity evidence sources include: rater training effect (response process), reliability and variability (internal structure), and impact on Milestones assessment (relations to other variables)., Results: Forty-eight videos, from 16 residents, were analyzed. Rater training improved inter-rater reliability from 0.04 to 0.64. The Φ-coefficient reliability was 0.50. There was a significant correlation between overall Relate performance and the pediatric teaching Milestone, r = 0.34, P = .019., Conclusion: Relate provides validity evidence with sufficient reliability to measure resident-led large-group teaching competence.
