Er was eens een bos waar de bewoners genoeg hadden van al die groene bladeren. Ieder jaar kwamen er meer van die bladeren aan de bomen te hangen, en nu was het genoeg! Tijd om daar paal en perk aan te stellen. Er volgde een verkiezing, en het resultaat was overweldigend en ingrijpend. Gevolg: geen nieuwe bladeren meer in het bos! En met het verdwijnen van al dat groen restte het de bomen slechts langzaam, maar massaal te vergrijzen… Gelukkig is dit een sprookje en is er niks van waar. Hoewel de eerste indruk bij de foto toch is dat hier iets niet pluis is, wat het dus wel is!

Wij, in ons mooie Europa, zijn doorgaans voorstander van dunningspaden in onze bossen om de belasting van de bosbodem door voertuigen voor houtoogst tot een minimum te beperken. Dat dit leidt tot een rechte padenstructuur in onze overigens steeds minder rechte bossen is daarbij lastig. Dus wij, als ware Hollanders, maken deze paden dan toch maar weer krom. Daardoor wel meer belaste bosbodem, maar het oog wil ook wat, toch?

Er zijn sombere berichten over de vlinders. Jaar in jaar uit zijn er minder. Ook dit jaar lijkt het niet goed te gaan en laten veel vlinders een negatieve trend zien. Maar is dat wel zo? Als je de NDFF gebruikt, het meest complete databestand over de natuur in Nederland, dan lijkt het best mee te vallen en gaan veel soorten juist vooruit. Maar klopt die conclusie wel?

(Protein-energy) malnutrition in individuals living with obesity presents complex diagnostic challenges due to the distinctive physiological characteristics of obesity. This narrative review critically examines the identification of malnutrition within the population with obesity, distinguishing malnutrition from related conditions such as sarcopenic obesity. While noting some shared features, the review highlights key differences between these conditions. The review also highlights the limitations of current malnutrition screening tools, which are not designed for individuals living with obesity. These tools primarily rely on anthropometric measurements, neglecting (among others) nutrient intake assessment, which hinders accurate malnutrition detection. Additionally, this review discusses limitations in existing diagnostic criteria, including the Global Leadership Initiative on Malnutrition (GLIM) criteria, when applied to individuals living with obesity. Challenges include the identification of appropriate cut-off values for phenotypic criteria (unintentional weight loss, low body mass index and muscle mass) and etiological criteria such as reduced food intake and inflammation for the population with obesity. Overall, this review emphasises the need for modified screening tools and diagnostic criteria to recognise and assess malnutrition in obesity, leading to improved clinical outcomes and overall well-being.

In de schemergebieden die de overgang vormen van onze dagelijkse wereld naar de onderwereld, heersen de trollen. Deze schepsels – als je ze zo mag noemen – blinken uit in hun penetrante lijfgeur, lelijkheid en klein denkraam. Logisch dus dat zelfs een wip voor hen een onoverkomelijke intellectuele uitdaging vormt. Doch in dit geval bevinden we ons gelukkig niet in dit duistere overgangsgebied, maar gewoon ergens in een van voorjaarsgeuren vervuld hedendaags bos. Geen mislukte wip dus. Wat u ziet oogt eenvoudig, doch de functie is u wellicht een raadsel. Daarom dus de vraag: wat ziet u hier?

The aim of this study was to elucidate the nature of T cell abnormalities in bipolar disorder (BD). With the use of multicolor flow cytometry, we first quantified the composition of the different memory and pro-inflammatory immune subpopulations in samples of 58 patients with BD and compared them to 113 healthy controls. Second, to assess if cytomegalovirus infection was related to the resulted immune subpopulation compositions in the two groups, we measured cytomegalovirus-specific antibodies in serum. Thirdly, we assessed differences between the two groups in the serum levels of the immune cell differentiation factor interleukin-7. Compared to healthy controls, patients showed significantly higher T helper-17, T regulatory and T central memory cells (CD4+ and CD8+). Besides, patients showed significantly lower CD4+ T effector memory and CD4+ T effector memory re-expressing RA cells. Cytomegalovirus infection was not related to the observed abnormalities, with the exception of T helper-17 cells. This immune subpopulation was significantly higher only in patients seropositive to cytomegalovirus infection. Finally, interleukin-7 levels were significantly lower in BD compared to healthy controls. In conclusion, the aberrant levels of T memory cell populations in BD may suggest a T cell differentiation abnormality. The role of interleukin-7 in this putative abnormality should be further investigated.
