2. A Multi‐Scale Particle‐In‐Cell Simulation of Plasma Dynamics From Magnetotail Reconnection to the Inner Magnetosphere.

During magnetospheric substorms, plasma from magnetic reconnection in the magnetotail is thought to reach the inner magnetosphere and form a partial ring current. We simulate this process using a fully kinetic 3D particle‐in‐cell (PIC) numerical code along with a global magnetohydrodynamics (MHD) model. The PIC simulation extends from the solar wind outside the bow shock to beyond the reconnection region in the tail, while the MHD code extends much further and is run for nominal solar wind parameters and a southward interplanetary magnetic field. By the end of the PIC calculation, ions and electrons from the tail reconnection reach the inner magnetosphere and form a partial ring current and diamagnetic current. The primary source of particles to the inner magnetosphere is bursty bulk flows (BBFs) that originate from a complex pattern of reconnection in the near‐Earth magnetotail at xGSM=−18RE ${x}_{\text{GSM}}=-18{R}_{\mathrm{E}}$ to −30RE ${-}30{R}_{\mathrm{E}}$. Most ion acceleration occurs in this region, gaining from 10 to 50 keV as they traverse the sites of active reconnection. Electrons jet away from the reconnection region much faster than the ions, setting up an ambipolar electric field allowing the ions to catch up after approximately 10 ion inertial lengths. The initial energy flux in the BBFs is mainly kinetic energy flux from the ions, but as they move earthward, the energy flux changes to enthalpy flux at the ring current. The power delivered from the tail reconnection in the simulation to the inner magnetosphere is >2×1011 ${ >} 2\times 1{0}^{11}$ W, which is consistent with observations. Plain Language Summary: During intervals of increased solar activity, the magnetic field in Earth's stretched night‐side tail undergoes intense reconfiguration that can energize particles. This process is called magnetic reconnection. Ions and electrons from reconnection can reach the inner magnetosphere. In this paper, we simulate this process using a novel model that includes Earth's global magnetic field configuration and self‐consistently models particle physics for both electrons and ions. We find that the particles are accelerated significantly near the sites of magnetic field reconfiguration with ions gaining 10 s of keV energy. As they propagate earthward, they end up contributing energetically to the formation of a ring current system partially encircling Earth. - Abstract
BackgroundSARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice.MethodsThis is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure.ResultsNine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout.There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management.ResultsNine hundred fifty-four surgeons completed the survey. In this work, using the theory of first-order macroscopic crowd models, we introduce a compartmental advection–diffusion model, describing the spatio-temporal dynamics of a population in different human behaviors (alert, panic and control) during a catastrophic event. For this model, we prove the local existence, uniqueness and regularity of a solution, as well as the positivity and L 1 -boundedness of this solution. This article focuses on the analysis of dynamics emerging in a network of Hodgkin–Huxley reaction–diffusion equations. The network has three levels. The three neurons in level 1 receive a periodic input but do not receive inputs from other neurons. The three neurons in level 2 receive inputs from one specific neuron in level 1 and all neurons in level 3. The neurons in level 3 (all other neurons) receive inputs from all other neurons in levels 2 and 3. Furthermore, the right-hand side of pre-synaptic neurons is connected to the left-hand side of the post-synaptic neurons. The synchronization phenomenon is observed for neurons in level 3, even though the system is initiated with different functions. As far as we know, it is the first time that evidence of the synchronization phenomenon is provided for spatially extended Hodgkin–Huxley equations, which are periodically forced at three different sites and embedded in such a hierarchical network with space-dependent coupling interactions. Spectral double power laws are common in solar high-energy phenomena such as flares and interplanetary energetic-electron events. However, the physical mechanism that produces the changes in power-law index within a single spectrum is unclear. We developed a fully analytical method of forming single power-law spectra from sequential acceleration of particles orbiting inside and hopping between simulated large-scale magnetic islands formed by flare reconnection. Here, we extend the analytical method to the formation of double power-law spectra by assuming sequential acceleration in two successive regions with different acceleration and particle-transport rates. The resulting spectral distribution is continuous and smooth, with a flattening at low energies, two power-law regions at mid-energies, and a steep rollover at high energies. The model provides analytical expressions for the spectral indices, all energy breaks, and normalization constants as functions of just three physical parameters of each acceleration region: (1) the energy gain in each accelerator, (2) the percentage of particles transferred between accelerators, and (3) the number of accelerators visited. One of the most salient predictions of our work is that the spectral index at high (low) energies is determined by the parameters of the first "seed" (second) acceleration region. By constructing the spectral distribution through an iterative analytical process, the evolution toward a double power law is easily characterized and explained. Our analytical model provides tools to interpret space- and ground-based observations from RHESSI, FOXSI, NuSTAR, Solar Orbiter/STIX, EOVSA, and future high-energy missions. [ABSTRACT FROM AUTHOR]

We give a series of numerical examples of competitive evolution in the predation system, showing in some cases how the choice is made to increase the efficiency of the predation mechanism (or other significant parameters) to the detriment of populations (both of prey and predators). We then develop the mathematical theory that enables us to understand the causality involved, and we identify a trend towards the emergence of the functional predation mechanism as such (and not of populations of the species involved). The realization of this trend only takes place when the conditions for it are offered by the hazards proposed to successive competitive choices. In this paper, we investigate the mathematical analysis of a mathematical model describing the virotherapy treatment of a cancer with logistic growth and the effect of viral cycle presented by a time delay. The cancer population size is divided into uninfected and infected compartments. Depending on time delay, we prove the positivity and boundedness and the stability of equilibria. We give conditions on which the viral cycle leads to “Jeff’s phenomenon” observed in laboratory and causes oscillations in cancer size via Hopf bifurcation theory. We establish an algorithm that determines the bifurcation elements via center manifold and normal form theories. We give conditions which lead to a supercritical or subcritical bifurcation. Several rural Moroccan people depend on natural remedies such as Smen (ghee) before visiting a health center due to their low cost and accessibility. However, knowledge of Smen traditional medicine is not documented but rather transferred orally from generation to generation. This is the first qualitative ethnomedicinal study that provides and documents information about the medicinal use of Smen in Northern Morocco. The present study aimed to investigate and gather information on the traditional medicinal practices of using Smen among rural people in Northern Morocco for a document and suggest the exploration of this product and its bioactive compounds in medical applications. The investigation was carried out by conducting individual semistructured interviews with 630 elderly people from March to April 2022. Data were analyzed using descriptive statistics and expressed as a percentage of responses to each question in the survey. The study revealed that 66.5% of the people surveyed were aware of the medical benefits of Smen , which had been passed down from their ancestors. It has been used to treat hemorrhoids, common colds, detoxification, rheumatism, and wound/burn injuries. This study showed that Moroccan's ethnomedicinal knowledge is closely related to Ayurveda, ancient Indian traditional medicine. For centuries, Smen has been used traditionally for medical purposes, just as it has been used in cooking. The diversity of ghee medicinal use in Northern Morocco could contribute to the discovery and development of ghee-based drugs, which have fewer side effects.

Iatrogenic aneurysms of the splenic artery constitute a rare yet potentially severe complication arising from diverse medical or surgical interventions. The clinical complexity and challenging management strategies associated with these aneurysms pose significant difficulties for clinicians. This circumstance is exemplified in our case report, detailing an iatrogenic aneurysm of the splenic artery that emerged secondary to a pancreatic fistula following a median pancreatectomy performed for a Frantz tumor. 16. Generation of Anti-Epidermal Growth Factor Receptor-2 (HER2) Immunoliposomes Using Microbial Transglutaminase (mTG)-Mediated Site-Specific Conjugated Antibodies.

- Abstract
Nanocarriers have found their interests in many fields including drug delivery and labeling of cells with the aim to target and eradicate tumor cells. One of the approaches to specifically address nanocarriers, such as liposomes, to their target is to attach antibodies of interest to their surface. To date, the development of immunoliposomes has been widely explored but has mainly involved chemical and unspecific reactions that could impair antibody stability, integrity, and orientation, thus reducing optimized immunoliposomes generation. In this study, we report the use of the patented COVISOLINK technology and the strain-promoted alkyne-azide cycloaddition (SPAAC) to generate immunoliposomes that target HER2 positive breast cancer with Trastuzumab as the antibody to be coupled. The efficacy of our two-step functionalization strategy and the successful specific coupling of the antibodies were validated by high-performance liquid chromatography-size exclusion chromatography (HPLC-SEC), which allowed a precise quantification of antibodies conjugated to liposomes and confirmed by cryo-TEM and sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) analyses. We also demonstrate by flow cytometry and epifluorescence microscopy that the produced anti-HER2 immunoliposomes were able to interact specifically with their target cells (SK-BR-3) while remaining negative with cells that express HER2 at a low level (MDA-MB-231). Tumors originating from soft tissues are uncommon, among these tumors, liposarcomas are the most frequent. These tumors remain asymptomatic for a long time, and only revealing themselves when they reach an important size. In such cases, treatment is difficult, requiring extensive surgery procedures that can excise several adjacent structures, potentially completed by adjuvant radiotherapy. Despite successful treatment, the recurrence rate remains very high. The prevalence of intracranial aneurysms (IA) is higher in patients with stenosis of the internal carotid artery (ICA), the intracavernous internal carotid aneurysm is an intracranial aneurysm causing major functional and vital complications. We report the case of a 26-year-old man who consulted for a reduction in visual acuity, converging strabismus and ptosis of the right eye evolving for 7 months before his consultation, the various neuro-radiological examinations made it possible to identify diagnose a giant aneurysm of the right intracavernous internal carotid artery associated with severe stenosis of the contralateral internal carotid artery, hence the performance of a therapeutic arteriography consisting of an occlusion of this aneurysm. In summary, we describe successful management of a giant aneurysm of the intracavernous portion of a single functional internal carotid artery, while preserving optimal cerebral vascularization., (© 2024 The Authors. 19. Barriers to clinical learning skills development among midwifery students and newly qualified midwives in Morocco: A qualitative study.

Background: In Morocco, despite various initiatives to improve the quality of reproductive healthcare, the indicators remain unfavorable. To strengthen the skills of midwives and support the achievement of Millennium Development Goals (MDGs) 4 and 5 by 2030, Morocco has developed and implemented a competency-based training framework. However, there is a lack of information on the successful implementation of this program and its impact on the quality of student midwives' preparation for practice., Objective: This study aimed to gain a deeper understanding and explore the barriers affecting the development of clinical learning skills among midwifery students and newly qualified midwives., Methods: This study employed a descriptive, exploratory qualitative approach. Data were collected through nine focus group discussions with 30 midwifery students and 24 newly graduated midwives between April and June 2023 from six higher education institutions in northern and central Morocco. Inductive content analysis was used to analyze the transcribed discussions., Results: The barriers varied between locations. Overall, the internship design was inadequate, and the clinical environment was insufficient. Participants highlighted limited opportunities to experience the full range of midwifery skills. However, most midwifery students and new graduates had deficient competencies in managing high-risk situations and emergencies., Conclusion: This work provides an overview of the nature and extent of the challenges faced by many student midwives in Morocco, resulting in newly graduated midwives being poorly prepared for the full range of midwifery practice. 21. Antibody and antibody fragments site-specific conjugation using new Q-tag substrate of bacterial transglutaminase.

- Abstract
During the last few years Antibody-Drug Conjugates (ADCs) have become one of the most active and very promising therapeutic weapons. Lessons learned from the traditional chemical conjugations (via lysine or cysteine residues of the antibodies) and the clinical studies of the developed ADCs have recently paved the way to the improvement of the conjugation technologies. Use of site-specific conjugation is considered as the promising path for improving the design and development of homogeneous ADCs with controlled Drug-Antibody ratio (DAR). Moreover, some of these conjugations can be applied to antibody fragments such as Fab, scfv and VHH for which random and chemical conjugation showed significant limitations. In this study, we identified a novel small peptide substrate (Q-tag) with high affinity and specificity of bacterial transglutaminase which can be genetically fused to different formats of antibodies of interest for the development of enzymatic site-specific conjugation we named "CovIsolink" platform. We describe the synthesis of chemically defined drugs conjugation in which the site and stoichiometry of conjugation are controlled using a genetically encoded Q-tag peptide with specific amino acids which serves as a substrate of bacterial transglutaminase. This approach has enabled the generation of homogeneous conjugates with DAR 1,7 for full IgG and 0,8 drug ratio for Fab, scfv and VHH antibody fragments without the presence of significant amounts of unconjugated antibody and fragments. As a proof of concept, Q-tagged anti Her-2 (human IgG1 (Trastuzumab) and the corresponding fragments (Fab, scfv and VHH) were engineered and conjugated with different aminated-payloads. The corresponding Cov-ADCs were evaluated in series of in vitro and in vivo assays, demonstrating similar tumor cell killing potency as Trastuzumab emtansine (Kadcyla®) even with lower drug-to-antibody ratio (DAR)., (© 2024.
