We present initial results from the Polarization Sky Survey of the Universe's Magnetism (POSSUM), analysing 22 817 Faraday rotation measures (RMs) with median uncertainties of 1.2 rad m |$^{-2}$| across 1520 deg2 to study magnetized gas associated with 55 nearby galaxy groups (⁠|$z\lesssim 0.025$|⁠) with halo masses between |$10^{12.5}$| and |$10^{14.0}$| M |$_\odot$|⁠. We identify two distinct gas phases: the intragroup medium (IGrM) within 0–2 splashback radii and the warm-hot intergalactic medium (WHIM) extending from 2 to 7 splashback radii. These phases enhance the standard deviation of residual (i.e. Galactic foreground RM-subtracted) RMs by |$6.9\pm 1.8$| rad m |$^{-2}$| and |$4.2 \pm 1.2$| rad m |$^{-2}$|⁠ , respectively. Estimated magnetic field strengths are several μ G within the IGrM and 0.1–1 μ G in the WHIM. We estimate the plasma |$\beta$| in both phases, and show that magnetic pressure might be more dynamically important than in the ICM of more massive clusters or sparse cosmic web filaments. Our findings indicate that 'missing baryons' in the WHIM likely extend beyond the gravitational radii of group-mass haloes to Mpc scales, consistent with large-scale, outflow-driven 'magnetized bubbles' seen in cosmological simulations. We demonstrate that RM grids are an effective method for detecting magnetized thermal gas at galaxy group interfaces and within the cosmic web. This approach complements X-ray and Sunyaev-Zel'dovich effect methods, and when combined with fast radio burst dispersion measures, data from the full POSSUM survey – comprising approximately a million RMs – will allow direct magnetic field measurements to further our understanding of baryon circulation in these environments and the magnetized universe. Arteriovenous fistulas (AVFs) are frequently used for hemodialysis access, but approximately 15%-20% of AVFs fail to mature within 3 months of their creation. Vascular access interventional therapy (VAIVT) is typically performed for treating an immature AVF. However, it should be performed 4-6 weeks or later after AVF creation. Here, we present a case in which VAIVT effectively addressed AVF occlusion that occurred only 9 days after its creation. The patient was an 82-year-old woman with chronic kidney disease who underwent surgery to create a left radiocephalic AVF for hemodialysis. Nine days postoperatively, disappearance of the AVF sound prompted an angiography, revealing thrombosis near the AVF anastomosis. VAIVT successfully restored blood flow, resulting in AVF maturation, and with no thrombosis or flow dysfunction at 21 months after VAIVT. A 29-year-old man with diabetic nephropathy presented with fever and chills 4 days postdischarge following hospitalization for hyperglycemia. Abdominal computed tomography revealed a splenic abscess. Percutaneous drainage was performed, and intravenous meropenem was administered. Subsequent culture of the drained abscess identified Lancefieldella rimae . Based on the antimicrobial susceptibility results, the patient was switched to oral levofloxacin. This combined treatment led to the resolution of the abscess, with no recurrence after 6 months. This is the first case of a splenic abscess caused by L. rimae successfully managed by prompt percutaneous drainage and appropriate antibiotics., (© 2024 The Authors. Objective: This study aimed to clarify the molecular mechanism of remnant pancreatic cancer (PC) development after primary PC resection., Summary Background Data: Molecular mechanisms of the development of remnant PCs following primary PC resection are largely unknown., Methods: Forty-three patients undergoing remnant PC resection after primary PC resection between 2001 and 2017 at 26 institutes were retrospectively analyzed. Clinicopathological features and molecular alterations detected by targeted amplicon sequencing of 36 PC-associated genes were evaluated., Results: These patients showed significantly lower body mass indices and higher hemoglobin A1c values at remnant PC resection than at primary PC resection. A comparison of the molecular features between primary and remnant PCs indicated that remnant PCs were likely to develop via three different molecular pathways: successional, showing identical and accumulated alterations (n=14); phylogenic, showing identical and distinct alterations (n=26); and distinct, showing independent distinctive alterations (n=3). The similarity of gene alterations was associated with time to the remnant PC development (r=－0.384, P=0.0173). Phylogenic pathways were significantly associated with the intraductal spread of carcinoma (P=0.007). Patient survival did not differ significantly depending on these molecular pathways., Conclusion: Molecular profiling uncovered three pathways for the development of remnant PCs, namely, successional, phylogenic, and distinct pathways. The vast majority of remnant PCs are likely to be molecularly associated with primary PCs either in the successional or phylogenic way. Background: The purpose of this study is to evaluate the potential of a novel surgical procedure, the Total Sealing Technique (TST), using the latest bipolar vessel sealing system (BVSS; LigaSure™ Exact Dissector) to reduce lymphatic leakage and seroma formation after electrocautery axillary lymph node dissection (ALND) in breast cancer surgery. Prolonged drainage is a common occurrence after ALND, primarily due to lymphatic leakage. In addition, the presence of seroma often leads to delays in the administration of postoperative adjuvant chemotherapy even after drain removal., Methods: We conducted a comparative analysis of 36 patients who underwent total mastectomy with ALND using conventional electrocautery technique (CONV) during the first 3 years, and 35 patients who underwent the same procedure using TST during the subsequent 3 years. The following factors were compared to assess the impact of TST: operation time, blood loss, total drainage volume, mean time to drain removal, postoperative hospital stay, mean time to initiation of postoperative chemotherapy, and postoperative complications in each group., Results: TST significantly reduced drainage volume (360.5 vs. 820.6 mL, p  < 0.001), days to drain removal (4.8 vs. 6.8 days, p  < 0.001), postoperative hospital stay (5.9 vs. 9.6 days, p < 0.001), the incidence of seroma (28.6 % vs. 65.9 %, p  = 0.001), and time to chemotherapy initiation (33.1 vs. 61.4 days, p  < 0.001) compared to CONV., Conclusions: TST in total mastectomy with ALND effectively decreases the incidence of lymphorrhea and seroma formation; thus, it can be recommended for total mastectomy with ALND., Competing Interests: All the authors declare no potential conflict of interest., (© 2024 The Authors.)
