Abstract: The magnetised near detector (ND280) of the T2K long-baseline neutrino oscillation experiment has been recently upgraded aiming to satisfy the requirement of reducing the systematic uncertainty from measuring the neutrinonucleus interaction cross section, which is the largest systematic uncertainty in the search for leptonic charge-parity symmetry violation. A key component of the upgrade is SuperFGD, a 3D segmented plastic scintillator detector made of approximately 2,000,000 optically-isolated 1 cm3 cubes. It will provide a 3D image of GeV neutrino interactions by combining tracking and stopping power measurements of final state particles with sub-nanosecond time resolution. The performance of SuperFGD is characterized by the precision of its response to charged particles as well as the systematic effects that might affect the physics measurements. Hence, a detailed Geant4 based optical simulation of the SuperFGD building block, i.e. a plastic scintillating cube read out by three wavelength shifting fibers, has been developed and validated with the different datasets collected in various beam tests. Abstract: This paper presents the multiplicity of neutral and charged hadrons produced in 90 GeV$/c$ proton-carbon interactions from a dataset taken by the NA61/SHINE experiment in 2017. Particle identification via dE/dx was performed for the charged hadrons $\pi^\pm$, $K^\pm$, and $p / \bar{p}$; the neutral hadrons $K^0_S$, $\Lambda$, and $\bar{\Lambda}$ were identified via an invariant mass analysis of their decays to charged hadrons. Double-differential multiplicity results as a function of laboratory momentum and polar angle are presented for each particle species; these results provide vital constraints on the predicted neutrino beam flux for current and future long-baseline neutrino oscillation experiments.

3. Random Markov property for random walks in random environments

Abstract: We consider random walks in dynamic random environments and propose a criterion which, if satisfied, allows to decompose the random walk trajectory into i.i.d. increments, and ultimately to prove limit theorems. The criterion involves the construction of a random field built from the environment, that has to satisfy a certain random Markov property along with some mixing estimates. We apply this criterion to correlated environments such as Boolean percolation and renewal chains featuring polynomial decay of correlations.

Abstract: Since its inception nearly a half century ago, CHARMM has been playing a central role in computational biochemistry and biophysics. Commensurate with the developments in experimental research and advances in computer hardware, the range of methods and applicability of CHARMM have also grown. This review summarizes major developments that occurred after 2009 when the last review of CHARMM was published. They include the following: new faster simulation engines, accessible user interfaces for convenient workflows, and a vast array of simulation and analysis methods that encompass quantum mechanical, atomistic, and coarse-grained levels, as well as extensive coverage of force fields. In addition to providing the current snapshot of the CHARMM development, this review may serve as a starting point for exploring relevant theories and computational methods for tackling contemporary and emerging problems in biomolecular systems. 5. Reference CC3 Excitation Energies for Organic Chromophores: Benchmarking TD-DFT, BSE/$GW$ and Wave Function Methods

Abstract: To expand the QUEST database of highly-accurate vertical transition energies, we consider a series of large organic chromogens ubiquitous in dye chemistry, such as anthraquinone, azobenzene, BODIPY, and naphthalimide. We compute, at the CC3 level of theory, the singlet and triplet vertical transition energies associated with the low-lying excited states. This leads to a collection of more than 120 new highly-accurate excitation energies. Subsequently, we employ these reference values to benchmark a series of lower-order wave function approaches, including the popular ADC(2) and CC2 schemes, as well as time-dependent density-functional theory (TD-DFT), both with and without applying the Tamm-Dancoff approximation (TDA). At the TD-DFT level, we evaluate a large panel of global, range-separated, local, and double hybrid functionals. Additionally, we assess the performance of the Bethe-Salpeter equation (BSE) formalism relying on both $G_0W_0$ and ev$GW$ quasiparticle energies evaluated from various starting points. It turns out that CC2 and ADC(2.5) are the most accurate models amongst those with respective $\mathcal{O}(N^5)$ and $\mathcal{O}(N^6)$ scalings with system size. In contrast, CCSD does not outperform CC2. The best performing exchange-correlation functionals include BMK, M06-2X, M06-SX, CAM-B3LYP, $\omega$B97X-D, and LH20t, with average deviations of approximately 0.20 eV or slightly below. Errors on vertical excitation energies can be further reduced by considering double hybrids. Both SOS-$\omega$B88PP86 and SOS-$\omega$PBEPP86 exhibit particularly attractive performances with overall quality on par with CC2, whereas PBE0-DH and PBE-QIDH are only slightly less efficient. BSE/ev$GW$ calculations based on Kohn-Sham starting points have been found to be particularly effective for singlet transitions, but much less for their triplet counterparts., Comment: 27 pages, 10 figures (Supporting Information available)

6. Stepping on the Edge: Curvature Aware Learning Rate Tuners

Abstract: Curvature information -- particularly, the largest eigenvalue of the loss Hessian, known as the sharpness -- often forms the basis for learning rate tuners. However, recent work has shown that the curvature information undergoes complex dynamics during training, going from a phase of increasing sharpness to eventual stabilization. We analyze the closed-loop feedback effect between learning rate tuning and curvature. We find that classical learning rate tuners may yield greater one-step loss reduction, yet they ultimately underperform in the long term when compared to constant learning rates in the full batch regime. These models break the stabilization of the sharpness, which we explain using a simplified model of the joint dynamics of the learning rate and the curvature. To further investigate these effects, we introduce a new learning rate tuning method, Curvature Dynamics Aware Tuning (CDAT), which prioritizes long term curvature stabilization over instantaneous progress on the objective. In the full batch regime, CDAT shows behavior akin to prefixed warm-up schedules on deep learning objectives, outperforming tuned constant learning rates. In the mini batch regime, we observe that stochasticity introduces confounding effects that explain the previous success of some learning rate tuners at appropriate batch sizes. Our findings highlight the critical role of understanding the joint dynamics of the learning rate and curvature, beyond greedy minimization, to diagnose failures and design effective adaptive learning rate tuners.

7. Reducibility points and characteristic $p$ local fields I- Simple supercuspidal representations of symplectic groups

Abstract: Let $F$ be a non-Archimedean local field with odd characteristic $p$. Let $N$ be a positive integer and $G=Sp_{2N}(F)$. By work of Lomel\'i on $\gamma$-factors of pairs and converse theorems, a generic supercuspidal representation $\pi$ of $G$ has a transfer to a smooth irreducible representation $\Pi_\pi$ of $GL_{2N+1}(F)$. In turn the Weil-Deligne representation $\Sigma_\pi$ associated to $\Pi_\pi$ by the Langlands correspondence determines a Langlands parameter $\phi_\pi$ for $\pi$. That process produces a Langlands correspondence for generic cuspidal representations of $G$. In this paper we take $\pi$ to be simple in the sense of Gross and Reeder, and from the explicit construction of $\pi$ we describe $\Pi_\pi$ explicitly. The method we use is the same as in our previous paper arXiv:2310.20455, where we treated the case where $F$ is a $p$-adic field, and $\pi$ a simple supercuspidal representation of $G=Sp_{2N}(F)$. It relies on a criterion due to Moeglin on the reducibility of representations parabolically induced from $GL_M(F)\times G$ for varying positive integers $M$. We extend this criterion to the case when $F$ has any positive characteristic. The main new feature consists in relating reducibility to $\gamma$-factors for pairs., Comment: 8 pages

8. Learning with Fitzpatrick Losses

Abstract: Fenchel-Young losses are a family of convex loss functions, encompassing the squared, logistic and sparsemax losses, among others. Each Fenchel-Young loss is implicitly associated with a link function, for mapping model outputs to predictions. For instance, the logistic loss is associated with the soft argmax link function. Can we build new loss functions associated with the same link function as Fenchel-Young losses? In this paper, we introduce Fitzpatrick losses, a new family of convex loss functions based on the Fitzpatrick function. A well-known theoretical tool in maximal monotone operator theory, the Fitzpatrick function naturally leads to a refined Fenchel-Young inequality, making Fitzpatrick losses tighter than Fenchel-Young losses, while maintaining the same link function for prediction. As an example, we introduce the Fitzpatrick logistic loss and the Fitzpatrick sparsemax loss, counterparts of the logistic and the sparsemax losses. This yields two new tighter losses associated with the soft argmax and the sparse argmax, two of the most ubiquitous output layers used in machine learning. We study in details the properties of Fitzpatrick losses and in particular, we show that they can be seen as Fenchel-Young losses using a modified, target-dependent generating function. We demonstrate the effectiveness of Fitzpatrick losses for label proportion estimation.

Abstract: The Super-Kamiokande and T2K collaborations present a joint measurement of neutrino oscillation parameters from their atmospheric and beam neutrino data. It uses a common interaction model for events overlapping in neutrino energy and correlated detector systematic uncertainties between the two datasets, which are found to be compatible. Abstract: Artificial intelligence has recently experienced remarkable advances, fueled by large models, vast datasets, accelerated hardware, and, last but not least, the transformative power of differentiable programming. This new programming paradigm enables end-to-end differentiation of complex computer programs (including those with control flows and data structures), making gradient-based optimization of program parameters possible. As an emerging paradigm, differentiable programming builds upon several areas of computer science and applied mathematics, including automatic differentiation, graphical models, optimization and statistics. This book presents a comprehensive review of the fundamental concepts useful for differentiable programming. We adopt two main perspectives, that of optimization and that of probability, with clear analogies between the two. Differentiable programming is not merely the differentiation of programs, but also the thoughtful design of programs intended for differentiation. Abstract: Measurements of $K_S^0$ meson production via its $\pi^{+} \pi^{-}$ decay mode in inelastic $\textit{p+p}$ interactions at incident projectile momenta of 31, 40 and 80 GeV/$c$ ($\sqrt{s_{NN}}=7.7, 8.8$ and $12.3$ GeV, respectively) are presented. The data were recorded by the NA61/SHINE spectrometer at the CERN Super Proton Synchrotron. Double-differential distributions were obtained in transverse momentum and rapidity. The mean multiplicities of $K_S^0$ mesons were determined to be $(5.95 \pm 0.19 (stat) \pm 0.22 (sys)) \times 10^{-2}$ at 31 GeV/$c$, $(7.61 \pm 0.13 (stat) \pm 0.31 (sys)) \times 10^{-2}$ at 40 GeV/$c$ and $(11.58 \pm 0.12 (stat) \pm 0.37 (sys)) \times 10^{-2}$ at 80 GeV/$c$. 14. How do Transformers perform In-Context Autoregressive Learning?

Abstract: Transformers have achieved state-of-the-art performance in language modeling tasks. However, the reasons behind their tremendous success are still unclear. In this paper, towards a better understanding, we train a Transformer model on a simple next token prediction task, where sequences are generated as a first-order autoregressive process $s_{t+1} = W s_t$. We show how a trained Transformer predicts the next token by first learning $W$ in-context, then applying a prediction mapping. We call the resulting procedure in-context autoregressive learning. More precisely, focusing on commuting orthogonal matrices $W$, we first show that a trained one-layer linear Transformer implements one step of gradient descent for the minimization of an inner objective function, when considering augmented tokens. When the tokens are not augmented, we characterize the global minima of a one-layer diagonal linear multi-head Transformer. Importantly, we exhibit orthogonality between heads and show that positional encoding captures trigonometric relations in the data. 15. Implicit Diffusion: Efficient Optimization through Stochastic Sampling

Abstract: We present a new algorithm to optimize distributions defined implicitly by parameterized stochastic diffusions. Doing so allows us to modify the outcome distribution of sampling processes by optimizing over their parameters. We introduce a general framework for first-order optimization of these processes, that performs jointly, in a single loop, optimization and sampling steps. This approach is inspired by recent advances in bilevel optimization and automatic implicit differentiation, leveraging the point of view of sampling as optimization over the space of probability distributions. We provide theoretical guarantees on the performance of our method, as well as experimental results demonstrating its effectiveness. Abstract: Direct alignment from preferences (DAP) methods, such as DPO, have recently emerged as efficient alternatives to reinforcement learning from human feedback (RLHF), that do not require a separate reward model. However, the preference datasets used in DAP methods are usually collected ahead of training and never updated, thus the feedback is purely offline. Moreover, responses in these datasets are often sampled from a language model distinct from the one being aligned, and since the model evolves over training, the alignment phase is inevitably off-policy. In this study, we posit that online feedback is key and improves DAP methods. Our method, online AI feedback (OAIF), uses an LLM as annotator: on each training iteration, we sample two responses from the current model and prompt the LLM annotator to choose which one is preferred, thus providing online feedback. Despite its simplicity, we demonstrate via human evaluation in several tasks that OAIF outperforms both offline DAP and RLHF methods. Abstract: Aligning language models with human preferences is crucial for reducing errors and biases in these models. Alignment techniques, such as reinforcement learning from human feedback (RLHF), are typically cast as optimizing a tradeoff between human preference rewards and a proximity regularization term that encourages staying close to the unaligned model. Selecting an appropriate level of regularization is critical: insufficient regularization can lead to reduced model capabilities due to reward hacking, whereas excessive regularization hinders alignment. Traditional methods for finding the optimal regularization level require retraining multiple models with varying regularization strengths. This process, however, is resource-intensive, especially for large models. To address this challenge, we propose decoding-time realignment (DeRa), a simple method to explore and evaluate different regularization strengths in aligned models without retraining. DeRa enables control over the degree of alignment, allowing users to smoothly transition between unaligned and aligned models. 18. Routers in Vision Mixture of Experts: An Empirical Study

Abstract: Mixture-of-Experts (MoE) models are a promising way to scale up model capacity without significantly increasing computational cost. A key component of MoEs is the router, which decides which subset of parameters (experts) process which feature embeddings (tokens). In this paper, we present a comprehensive study of routers in MoEs for computer vision tasks. We introduce a unified MoE formulation that subsumes different MoEs with two parametric routing tensors. This formulation covers both sparse MoE, which uses a binary or hard assignment between experts and tokens, and soft MoE, which uses a soft assignment between experts and weighted combinations of tokens. Routers for sparse MoEs can be further grouped into two variants: Token Choice, which matches experts to each token, and Expert Choice, which matches tokens to each expert. We conduct head-to-head experiments with 6 different routers, including existing routers from prior work and new ones we introduce. We show that (i) many routers originally developed for language modeling can be adapted to perform strongly in vision tasks, (ii) in sparse MoE, Expert Choice routers generally outperform Token Choice routers, and (iii) soft MoEs generally outperform sparse MoEs with a fixed compute budget. These results provide new insights regarding the crucial role of routers in vision MoE models.

Abstract: The critical point of strongly interacting matter is searched for at the CERN SPS by the NA61/SHINE experiment in central $^{40}$Ar +$^{45}$Sc collisions at 13$A$, 19$A$, 30$A$, 40$A$, and 75$A$ GeV/$c$. The dependence of the second-order scaled factorial moments of proton multiplicity distributions on the number of subdivisions in transverse momentum space is measured. The intermittency analysis uses statistically independent data sets for every subdivision in transverse and cumulative-transverse momentum variables. The results obtained do not indicate the searched intermittent pattern. Abstract: Abstract This paper presents the energy dependence of multiplicity and net-electric charge fluctuations in p+p interactions at beam momenta 20, 31, 40, 80, and 158 $$\text{ Ge }\hspace{-1.00006pt}\text{ V }\!/\!c$$ Ge V / c . Results are corrected for the experimental biases and quantified with the use of cumulants and factorial cumulants. Cumulant ratios are an essential tool in the search for the critical point of strongly interacting matter in heavy ion collisions. Measurements performed in p+p interactions provide a vital baseline estimation in these studies. The measured signals are compared with the string hadronic models Epos1.99 and FTFP-BERT.

Abstract: Abstract Cardiovascular-kidney-metabolic health reflects the interactions between metabolic risk factors, chronic kidney disease, and the cardiovascular system. A growing body of literature suggests that metabolic syndrome (MetS) in individuals of normal weight is associated with a high prevalence of cardiovascular diseases and an increased mortality. The aim of this study was to establish a non-invasive preclinical model of MetS in support of future research focusing on the effects of novel antidiabetic therapies beyond glucose reduction, independent of obesity. Eighteen healthy adult Beagle dogs were fed an isocaloric Western diet (WD) for ten weeks. Biospecimens were collected at baseline (BAS1) and after ten weeks of WD feeding (BAS2) for measurement of blood pressure (BP), serum chemistry, lipoprotein profiling, blood glucose, glucagon, insulin secretion, NT-proBNP, angiotensins, oxidative stress biomarkers, serum, urine, and fecal metabolomics. Differences between BAS1 and BAS2 were analyzed using non-parametric Wilcoxon signed-rank testing. The isocaloric WD model induced significant variations in several markers of MetS, including elevated BP, increased glucose concentrations, and reduced HDL-cholesterol. It also caused an increase in circulating NT-proBNP levels, a decrease in serum bicarbonate, and significant changes in general metabolism, lipids, and biogenic amines. Short-term, isocaloric feeding with a WD in dogs replicated key biological features of MetS while also causing low-grade metabolic acidosis and elevating natriuretic peptides. These findings support the use of the WD canine model for studying the metabolic effects of new antidiabetic therapies independent of obesity.

Abstract: Abstract Background Severe trauma is the leading cause of disability and mortality in the patients under 35 years of age. Surgical site infections (SSI) represent a significant complication in this patient population. However, they are often inadequately investigated, potentially impacting the quality of patient outcomes. The aim of this study was to investigate the epidemiology of SSI and risk factors in severe trauma patients. Methods We conducted a multicenter retrospective cohort study screening the severe trauma patients (STP) admitted to two intensive care units of an academic institution in Marseille between years2018 and 2019. Those who underwent orthopedic or spinal surgery within 5 days after admission were included and classified into two groups according to the occurrence of SSI (defined by the Centers for Disease Control (CDC) international diagnostic criteria) or not. Our secondary goal was to evaluate STP survival at 48 months, risk factors for SSI and microbiological features of SSI. Results Forty-seven (23%) out of 207 STP developed an SSI. Mortality at 48-months did not differ between SSI and non-SSI patients (12.7% vs. 10.0%; p = 0.59). The fractures of 22 (47%) severe trauma patients with SSI were classified as Cauchoix 3 grade and 18 (38%) SSI were associated with the need for external fixators. Thirty (64%) severe trauma patients with SSI had polymicrobial infection, including 34 (72%) due to Gram-positive cocci. Empirical antibiotic therapy was effective in 31 (66%) cases. Multivariate analysis revealed that risk factors such as low hemoglobin, arterial oxygenation levels, hyperlactatemia, high serum creatinine and glycemia, and Cauchoix 3 grade on the day of surgery were associated with SSI in severe trauma patients. The generated predictive model showed a good prognosis performance with an AUC of 0.80 [0.73–0.88] and a high NPV of 95.9 [88.6–98.5] %. Conclusions Our study found a high rate of SSI in severe trauma patients, although SSI was not associated with 48-month mortality. Several modifiable risk factors for SSI may be effectively managed through enhanced perioperative monitoring and the implementation of a patient blood management strategy.

Abstract: Abstract The yields of $$K^0_S$$ K S 0 mesons have been measured in inelastic p+p interactions at incident projectile momenta of 31, 40 and 80 $$\text {Ge}\hspace{-1.00006pt}\text {V}\!/\!c$$ Ge V / c ( $$\sqrt{s_{NN}}=7.7, 8.8$$ s NN = 7.7 , 8.8 and 12.3 $$\text {Ge}\hspace{-1.00006pt}\text {V}$$ Ge V , respectively). The data were recorded by the NA61 $$/$$ / SHINE spectrometer at the CERN Super Proton Synchrotron and the $$K^0_S$$ K S 0 mesons identified via their decays into $$\pi ^{+} \pi ^{-}$$ π + π - pairs. Double-differential distributions are presented as function of transverse momentum and rapidity. Abstract: Abstract Background Microsatellite instability (MSI) due to mismatch repair deficiency (dMMR) is common in colorectal cancer (CRC). These cancers are associated with somatic coding events, but the noncoding pathophysiological impact of this genomic instability is yet poorly understood. Here, we perform an analysis of coding and noncoding MSI events at the different steps of colorectal tumorigenesis using whole exome sequencing and search for associated splicing events via RNA sequencing at the bulk-tumor and single-cell levels. Results Our results demonstrate that MSI leads to hundreds of noncoding DNA mutations, notably at polypyrimidine U2AF RNA-binding sites which are endowed with cis-activity in splicing, while higher frequency of exon skipping events are observed in the mRNAs of MSI compared to non-MSI CRC. At the DNA level, these noncoding MSI mutations occur very early prior to cell transformation in the dMMR colonic crypt, accounting for only a fraction of the exon skipping in MSI CRC. At the RNA level, the aberrant exon skipping signature is likely to impair colonic cell differentiation in MSI CRC affecting the expression of alternative exons encoding protein isoforms governing cell fate, while also targeting constitutive exons, making dMMR cells immunogenic in early stage before the onset of coding mutations. This signature is characterized by its similarity to the oncogenic U2AF1-S34F splicing mutation observed in several other non-MSI cancer. Conclusions Overall, these findings provide evidence that a very early RNA splicing signature partly driven by MSI impairs cell differentiation and promotes MSI CRC initiation, far before coding mutations which accumulate later during MSI tumorigenesis.

Abstract: Abstract The critical point of strongly interacting matter is searched for at the CERN SPS by the NA61/SHINE experiment in central $$^{40}$$ 40 Ar + $$^{45}$$ 45 Sc collisions at 13 A, 19 A, 30 A, 40 A, and 75 A GeV/c. The dependence of the second-order scaled factorial moments of proton multiplicity distributions on the number of subdivisions in transverse momentum space is measured. The intermittency analysis uses statistically independent data sets for every subdivision in transverse and cumulative-transverse momentum variables. The results obtained do not indicate the searched intermittent pattern. An upper limit on the fraction of correlated protons and the intermittency index is obtained based on a comparison with the Power-law Model.

Abstract: Abstract This paper presents a lifting‐line implementation in the framework of a Lagrangian vortex particle method (LL‐VP). The novelty of the present implementation lies in the fluid particles properties definition and in the particles shedding process. In spite of mimicking a panel method, the LL‐VP needs some peculiar treatments described in the paper. The present implementation converges rapidly and efficiently during the shedding sub‐iteration process. This LL‐VP method shows good accuracy, even with moderate numbers of sections. Compared to its panel or vortex filaments counterparts, more frequently encountered in the literature, the present implementation inherently accounts for the diffusion term of the Navier‐Stokes equations, possibly with a turbulent viscosity model. Additionally, the present implementation can also account for more complex onset flows: upstream ambient turbulence and upstream turbine wakes. After validation on an analytical elliptic wing configuration, the model is tested on the Mexnext‐III wind turbine application, for three reduced velocities. Accurate results are obtained both on the analytical elliptic wing and on the New MEXICO rotor cases in comparison with other similar numerical models. A focus is made on the Mexnext‐III wake analysis. The numerical wake obtained with the present LL‐VP is close to other numerical and experimental results. Finally, a last configuration with three tidal turbines in interaction is considered based on an experimental campaign carried out at the IFREMER wave and current flume tank. Enhanced turbine‐wake interactions are highlighted, with favourable comparisons with the experiment. Hence, such turbine interactions in a farm are accessible with this LL‐VP implementation, be it wind or tidal energy field.

Abstract: Scedosporium spp. and Lomentospora prolificans are emerging non-Aspergillus filamentous fungi. The Scedosporiosis/lomentosporiosis Observational Study we previously conducted reported frequent fungal vascular involvement, including aortitis and peripheral arteritis. For this article, we reviewed 7 cases of Scedosporium spp. and L. prolificans arteritis from the Scedosporiosis/lomentosporiosis Observational Study and 13 cases from published literature. Underlying immunosuppression was reported in 70% (14/20) of case-patients, mainly those who had solid organ transplants (10/14). Osteoarticular localization of infection was observed in 50% (10/20) of cases; infections were frequently (7/10) contiguous with vascular infection sites. Scedosporium spp./Lomentospora prolificans infections were diagnosed in 9 of 20 patients ≈3 months after completing treatment for nonvascular scedosporiosis/lomentosporiosis. Aneurysms were found in 8/11 aortitis and 6/10 peripheral arteritis cases. Invasive fungal disease–related deaths were high (12/18 [67%]). The vascular tropism of Scedosporium spp. and L. prolificans indicates vascular imaging, such as computed tomography angiography, is needed to manage infections, especially for osteoarticular locations.

Abstract: IntroductionWell-organized nasal-associated lymphoid tissue (NALT) has been identified in the pharyngeal and tubal tonsils of both adults and children, and diffuse NALT has been identified in the superior, middle and inferior turbinate regions of children. However, it is not clear how to target these NALT sites with aerosolized vaccines. We explored whether head position and/or angle and distance of device insertion could be used to target fluorescein aerosol to NALT sites in three-dimensional printed models of the intranasal airways of an 18- and a 5-year-old (yo).MethodsThree head positions (upright [Up], tilted back 45° [45] and supine [Su]), two angles of insertion (30° and 45°) and two distances of insertion (6 mm and 9 mm) were tested. Fluorescein aerosol was generated by an Aptar Pharma BiVax 200 µL intranasal atomizer. Percent fluorescein deposition was quantified in the anterior nose, the upper horizontal third of the model (superior turbinate region), middle third (middle turbinate), lower third (inferior turbinate and nasopharynx combined) and exit filter.ResultsMean percent deposition in both models was 45 > Su (p = 0.03).DiscussionThese results suggest that, in individuals with similar nasal airway dimensions as our models: 1) supine and upright head positions might be used to target delivery of aerosolized vaccines generated by the BiVax intranasal atomizer to NALT sites in the middle turbinate and the inferior turbinate and nasopharynx combined, respectively; 2) delivery to the middle turbinate may be higher in children ≤5-yo; and 3) deposition in the anterior nose may be higher in adults, for all head positions. In vivo tests are needed to confirm these findings.

Abstract: The Oligocene–Miocene transition witnessed great environmental and faunal changes, spanning from late Oligocene to early Miocene (MP28–MN3). Its drivers and consequences on mammals are, however, poorly understood. Rhinocerotoids are among the most affected taxa, reflected by great taxonomical and morphological changes. However, potential associated changes in ecology have not been explored. Here, we investigated the palaeoecology of 10 rhinocerotid species coming from 15 localities across Western Europe and ranging from MP28 to MN3. We explored evolutionary trends for diet, physiology and habitat via dental wear, hypoplasia, body mass and stable isotopy. All rhinocerotids studied were C3 feeders, whether browsing or mixed-feeding, but clear dietary differences were observed at some localities and between Oligocene and Miocene rhinocerotids. The prevalence of hypoplasia was low (less than 10%) to moderate (less than 20%), but there were great differences by loci, species and localities. Body mass covaried with hypoplasia prevalence, suggesting that larger species might be more susceptible to stresses and environmental changes. We reconstructed similar warm conditions at all localities except Gaimersheim, but found greater variations in precipitation. Indeed, a clear shift in δ13C values was noticed at the end of the Oligocene, consistent with climatic and vegetation changes reported at that time.

Abstract: The infectious pseudoaneurysm of the common carotid artery is a rare observation in children. We present a case of ruptured infectious common carotid aneurysm in a 14-year-old girl managed in the Democratic Republic of Congo. We performed an arterial reconstruction with a saphenous vein patch. Post-operative course was simple. The patient was discharged from the hospital at post-operative day 15. She was doing well and had returned to school. Rupture of an infectious pseudoaneurysm of the common carotid artery constitutes a serious, life-threatening accident. As we have shown in this case report, its management with successful results is indeed possible, even in a resource-limited environment.

Abstract: Abstract Background Cervical cancer is the fourth most common cancer among women worldwide, both for incidence and mortality. Prevention relies on screening with a Pap test to detect precancerous lesions, which can then be treated. Access to this screening is currently both improvable and inequitable. Pregnancy may be an ideal moment for women to catch up on their overdue cervical cancer screening. In the general population, women's risk of not being screened is associated with their place of birth and other social factors; this may be true as well among pregnant women. Our objective was to study the association between women's place of birth and their failure to catch up with this screening during pregnancy. Methods The 2016 French National Perinatal Survey included 13,147 women who gave birth after 21 weeks of gestation. The association between their place of birth and failure to catch up on this screening (defined by the absence of a Pap test during pregnancy for women overdue for it) was adjusted for age, parity, education level, health insurance, and when they began prenatal care with logistic regression models. Results Among the women for whom screening was then recommended, 49% were not up to date at the start of pregnancy, and of these, 53% were not caught up before delivery. After adjustment for other risk factors, maternal place of birth was not associated with a higher risk of failure to catch up with this screening during pregnancy. However, factors identified as associated with this risk included a low education level and late start of prenatal care. Conclusion About half of women overdue for cervical cancer screening did not catch up with it during their pregnancy. Professionals should pay special attention to women with lower education levels and late initiation of prenatal care, who constitute a group at high risk of not catching up on this screening during pregnancy.

Abstract: Abstract The NA61/SHINE experiment at the CERN Super Proton Synchrotron studies the onset of deconfinement in strongly interacting matter through a beam energy scan of particle production in collisions of nuclei of varied sizes. This paper presents results on inclusive double-differential spectra, transverse momentum and rapidity distributions and mean multiplicities of $$\pi ^\pm $$ π ± , $$K^\pm $$ K ± , p and $$\bar{p}$$ p ¯ produced in $$^{40}\hbox {Ar+}^{45}\hbox {Sc}$$ 40 Ar+ 45 Sc collisions at beam momenta of 13A, 19A, 30A, 40A, 75A and 150A $$\text{ Ge }\hspace{-1.00006pt}\text{ V }\!/\!c$$ Ge V / c . The analysis uses the 10% most central collisions, where the observed forward energy defines centrality. The energy dependence of the $$K^\pm $$ K ± / $$\pi ^\pm $$ π ± ratios as well as of inverse slope parameters of the $$K^\pm $$ K ± transverse mass distributions are placed in between those found in inelastic $$p+p$$ p + p and central Pb + Pb collisions. The results obtained here establish a system-size dependence of hadron production properties that so far cannot be explained either within statistical or dynamical models.

41. Perioperative outcomes of the surgical management of achalasia in two tertiary Cameroonian hospitals: a cohort study

Abstract: Abstract Introduction Achalasia is a rare esophageal disease with potentially lethal complications. Knowledge of the outcomes of the different surgical treatment modalities for achalasia by Heller’s cardiomyotomy (HCM) helps to choose the safest and most effective option. However, data on the management of achalsia using a Heller myotomy is limited in Africa. Thus, our aim was to determine the perioperative morbidity, mortality and short-term functional outcomes of HCM in Cameroon. Methodology We conducted a cohort study throughout a 10-year chart review of patients who underwent HCM for achalasia and were followed up postoperatively for at least three months at two tertiary health centers in Cameroon. We analyzed demographic data, preoperative clinical and imaging data, treatment details, and outcomes at three to twelve months after HCM using the Eckardt score. Results We enrolled 29 patients with achalasia having a mean age of 24 ± 16 years and predominantly females (M/F of 1/3.8). The mean symptom duration was 51 ± 20 months. In 80% of cases, the diagnosis was made through a conventional x-ray contrast imaging or “barium swallow test” (93%) and/or an upper gastrointestinal endoscopy (86%). The gold standard diagnostic method via esophageal manometry was unavailable. Preoperatievly, all patients had symptoms suggestive of an active achalasia. HCM was performed via laparotomy in 75% as opposed to 25% laparoscopic HCM procedures. Dor’s anterior partial fundoplication was the main anti-reflux procedure performed (59%). Mucosal perforations were the only intraoperative complications in eight patients (2 during laparoscopy vs. 6 during laparotomy; p > 0.5) and were managed successfully by simple sutures. Postoperative complications were non-severe and occurred in 10% of patients all operated via laparotomy. The mean postoperative length of hospital stay was 7 ± 3 days for laparotomy vs. 5 ± 2 days for laparoscopy; p > 0.5. The perioperative mortality rate was nil. Abstract: Therapeutic monoclonal antibodies have been successful in protecting vulnerable populations against SARS-CoV-2. However, their effectiveness has been hampered by the emergence of new variants. To adapt the therapeutic landscape, health authorities have based their recommendations mostly on in vitro neutralization tests. However, these do not provide a reliable understanding of the changes in the dose-effect relationship and how they may translate into clinical efficacy. Taking the example of EvusheldTM (AZD7442), we aimed to investigate how in vivo data can provide critical quantitative results and project clinical effectiveness. We used the Golden Syrian hamster model to estimate 90 % effective concentrations (EC90) of AZD7442 in vivo against SARS-CoV-2 Omicron BA.1, BA.2 and BA.5 variants. While our in vivo results confirmed the partial loss of AZD7442 activity for BA.1 and BA.2, they showed a much greater loss of efficacy against BA.5 than that obtained in vitro. We analyzed in vivo EC90s in perspective with antibody levels measured in a cohort of immunocompromised patients who received 300 mg of AZD7442. We found that a substantial proportion of patients had serum levels of anti-SARS-CoV-2 spike protein IgG above the estimated in vivo EC90 for BA.1 and BA.2 (21 % and 92 % after 1 month, respectively), but not for BA.5. These findings suggest that AZD7442 is likely to retain clinical efficacy against BA.2 and BA.1, but not against BA.5. Overall, the present study illustrates the importance of complementing in vitro investigations by preclinical studies in animal models to help predict the efficacy of monoclonal antibodies in humans.

46. Efficacy of a mitral regurgitation severity index to predict long‐term outcome in dogs with myxomatous mitral valve disease

Abstract: Abstract Background Predicting progression of myxomatous mitral valve disease (MMVD) in dogs can be challenging. Hypothesis/Objectives The mitral regurgitation severity index (MRSI) will predict time to congestive heart failure (CHF) and all‐cause death in dogs with MMVD. Animals Eight hundred sixty‐nine client‐owned dogs. Methods Retrospective study pooling data from 4 previous samples including dogs with MMVD stage B2 or C. MRSI was calculated as: (heart rate [HR]/120) × left atrium‐to‐aorta ratio (LA:Ao) × (age in years/10) × 100. Alternative MRSI formulas substituting radiographic measures of left atrial size were also calculated. Cox proportional hazard modeling and time‐dependent receiver‐operator characteristic curves quantified prognostic performance. Results For Stage B2 pooled samples, MRSI > 156 was predictive of time to CHF (median 407 vs 1404 days; area under the curve [AUC] 0.68; hazard ratio 3.02 [95% CI 1.9‐4.9]; P 173 was predictive of all‐cause death (median survival 868 vs 1843 days; AUC 0.64; hazard ratio 4.26 [95% CI 2.4‐7.5]; P

47. Breakdown of the velocity and turbulence in the wake of a wind turbine – Part 1: Large-eddy-simulation study

Abstract: A new theoretical framework, based on an analysis in the moving and fixed frames of reference (MFOR and FFOR), is proposed to break down the velocity and turbulence fields in the wake of a wind turbine. This approach adds theoretical support to models based on the dynamic wake meandering (DWM) and opens the way for a fully analytical and physically based model of the wake that takes meandering and atmospheric stability into account, which is developed in the companion paper. The mean velocity and turbulence in the FFOR are broken down into different terms, which are functions of the velocity and turbulence in the MFOR. These terms can be regrouped as pure terms and cross terms. In the DWM, the former group is modelled, and the latter is implicitly neglected. The shape and relative importance of the different terms are estimated with the large-eddy-simulation solver Meso-NH coupled with an actuator line method. A single wind turbine wake is simulated on flat terrain, under three cases of stability: neutral, unstable and stable. In the velocity breakdown, the cross term is found to be relatively low. It is not the case for the turbulence breakdown equation where even though the cross terms are overall of lesser magnitude than the pure terms, they redistribute the turbulence and induce a non-negligible asymmetry. These findings underline the limitations of models that assume a steady velocity in the MFOR, such as the DWM or the model developed in the companion paper. It is also found that as atmospheric stability increases, the pure turbulence contribution becomes relatively larger and pure meandering relatively smaller.

48. Breakdown of the velocity and turbulence in the wake of a wind turbine – Part 2: Analytical modelling

Abstract: This work aims to develop an analytical model for the streamwise velocity and turbulence in the wake of a wind turbine where the expansion and the meandering are taken into account independently. The velocity and turbulence breakdown equations presented in the companion paper are simplified and resolved analytically, using shape functions chosen in the moving frame of reference. This methodology allows us to propose a physically based model for the added turbulence and thus to have a better interpretation of the physical phenomena at stake, in particular when it comes to wakes in a non-neutral atmosphere. Five input parameters are used: the widths (in vertical and horizontal directions) of the non-meandering wake, the standard deviation of wake meandering (in both directions) and a modified mixing length. Two calibrations for these parameters are proposed: one if the users have access to velocity time series and the other if they do not. The results are tested on a neutral and an unstable large-eddy simulation (LES) that were both computed with Meso-NH. The model shows good results for the streamwise velocity in both directions and can accurately predict modifications due to atmospheric instability. For the axial turbulence, the model misses the maximum turbulence at the top tip in the neutral case, and the proposed calibrations lead to an overestimation in the unstable case. However, the model shows encouraging behaviour as it can predict a modification of the shape function (from bimodal to unimodal) as instability and thus meandering increases.
