Objective: Febrile neutropenia (FN) is a serious clinical event, associated with significant morbidity and mortality. Imaging has a central role in the identification of the fever cause. The study objectives were to assess abnormalities of potential infectious origin on paranasal sinus, chest and abdomen/pelvis CT scans performed during an episode of FN, in patients with or without specific clinical signs, and to evaluate their impact on the frequency of changes in treatment. Materials and methods: This retrospective study included 306 patients with FN from a single academic center between October 2018 and December 2019. Clinical and biological data, final clinical diagnosis for the FN episode, and change in treatment following CT scans were collected from medical records. CT images were reviewed for positive findings in each anatomical region. Results: Overall, 188 patients (61.4%) had at least one positive CT finding on paranasal sinus, chest and/or abdomen/pelvis regions, and 153 patients (50.0%) received a final clinical diagnosis of infection, based on microbiological and/or radiological findings. There were more chest and abdomen/pelvis CT positive findings in patients with specific clinical signs than in patients without (65.7% vs. 25.8%, and 59.0% vs. 22.7% respectively, p <0.001), with a higher frequency of therapeutic modifications in positive CTs. There were significantly more paranasal sinus CT abnormalities in symptomatic patients (33.3% vs. 8.5%, p = 0.03), but without any impact on treatment. Conclusion: These results support recommendations to perform systematic chest CT in patients with FN and may encourage the widespread use of abdomen/pelvis CT in this indication, even without symptoms. BACKGROUND: Sugarcane (Saccharum sp.) growers in Louisiana report that the widely used soil residual herbicide, pendimethalin, is less effective at controlling itchgrass (Rottboellia chochinchinensis), possibly through accelerated dissipation from repeated applications. Some growers have started to use another herbicide, clomazone, in place of pendimethalin for itchgrass control when pendimethalin does not provide the necessary control. Little is known about the dissipation of these two herbicides, especially when applied together, in diverse sugarcane soils in Louisiana. Thus, the objective of the research was to measure the dissipation of pendimethalin and clomazone in soils having high itchgrass pressure. Soil samples from five fields were fortified (4 μg g−1) with clomazone and pendimethalin and monitored over the course of 163 days under laboratory conditions. Herbicide levels were determined by extracting the herbicides from soil with acetonitrile and analyzing the extracts with high performance liquid chromatography (HPLC). RESULTS: The results indicate that clomazone is persistent in the soils studied, with a degradation half‐life (DT50) of ≥79 days. Pendimethalin dissipates more rapidly in soil with a DT50 ranging from 7 to 59 days. All itchgrass plants showed injury to herbicide but many grew out of symptoms, and responses were not consistent between soil types. CONCLUSION: Pendimethalin dissipation varied greatly between soil types, yet clomazone was generally persistent in each soil tested. However, when these soils were seeded with itchgrass, pendimethalin was more effective at controlling itchgrass than clomazone. Although persistent in the soils tested, the leaching potential of clomazone may limit its long‐term effectiveness. Published 2024. The present study aimed to assess the mediating role of adjustment processes in known risk factors associated with prolonged grief disorder. Data were collected in March–April 2021 through an online survey of 542 Canadian adults bereaved since March 2020. The mediating role of satisfaction with funeral rituals, bereavement support, and coping strategies on grief outcomes was tested using structural equation modeling. Results showed that such adjustment processes played a significant role in the grief process and that they were better predictors than risk factors alone. Since they are more amenable determinants of grief reactions, they should be further studied using a longitudinal design. [ABSTRACT FROM AUTHOR]
