Introduction: This study evaluated hand and forearm injuries caused by angle grinders and identify the risk factors. Methods: Between 2020 and 2022, 79 patients with hand and forearm injuries due to angle grinder were retrospectively evaluated. Data were obtained from the hospital database and telephone interviews with patients. Age, gender, education level, employment status and work experience, smoking, and alcohol use were recorded. The injury location and structures were evaluated. The time of injury and use of protective gloves were recorded. The Modified Hand Injury Severity Score (MHISS) was used to determine the severity of the injury. The return to work times of actively working patients were recorded. Results: Seventy-nine patients with angle grinder injuries were included in the study. All patients were male. Dominant side injury was in 17 (21.5%) patients. The most frequently injured area was the dorsal hand, and the most frequently injured structure was the extensor tendon. The mean MHISS was 49.15 (4-280) and 39 (49.4%) patients had minor injuries. The mean return to work was 7.13 (2-32) weeks. No correlation was found between age, work experience, and MHISS (p=0.167 p=0.389). There was a correlation between the use of protective gloves and MHISS (p=0.002). Conclusion: Injuries to the hand and forearm caused by angle grinder usage can result in serious work-related disabilities. These injuries can be reduced by increasing safety precautions and training.

Difficult airway is a serious condition that can be fatal and is frequently encountered during general anesthesia applications. Tracheobronkopathy osteochondroplasty (TBO) is a rare benign disease that is one of the causes of unpredictable difficult airway. A 45-year-old male patient with no comorbidities was transferred to the operating room for elective cholecystectomy. He couldn’t be intubated after standard monitoring and induction of anesthesia. The patient, who had papillomatous lesions in the trachea detected by bronchoscopy, was awakened to be scheduled for rigid bronchoscopy. TBO was detected in pathological examination of the samples taken in rigid bronchoscopy. TBO is a rare benign disease. Its etiology is unknown. It may be asymptomatic or may present with persistent dry cough, hemoptysis, dyspnea, recurrent lower respiratory tract infection, atelectasis and difficult intubation. Some patients may be diagnosed for the first time in the operating room due to difficult intubation.In cases with advanced tracheal stenosis, invasive airway intervention may be required or it may have a fatal course. The use of a laryngeal mask in the perioperative period may be an ideal option in a patient known to have TBO before. Unexpected difficult airway management in the operating room is important for anesthesiologists. According to the difficult airway society algorithm, waking the patient from a planned operation is the safest way until the problem is detected. We did not need invasive intervention because our patient also had elective surgery and ventilation was not difficult. In addition, we think that it is an important decision to make a diagnosis using fiberobtic bronchoscopy in operating room conditions and to wake up our patient with sugammadex without repeated attemps of tracheal intubation and delay elective surgery. In coclusion, it should be kept in mind that TBO may be one of the unpredictable causes of difficult intubation in the operating room.

In this study, the 5S method was applied in the tool room of a company operating in the aviation field and engaged in machining. Preparations for the 5S study were carried out in June 2020, and after this month, production was started according to the 5S method. Outputs such as defective parts rates due to cutting tools, costs due to tool room, machine waiting due to tool supply and tool preparation time were determined as performance evaluation criteria. Within the scope of preparation, first of all, lean production and 5S trainings were given to company employees. Personnel for the area where 5S will be applied have been determined and necessary planning has been made. Sorting, organizing, cleaning, standardization and discipline steps related to 5S have been successfully implemented, respectively. For the recovery data, the outputs of the first half of 2020 and the last half were compared and evaluated. As a result of this study, 66% improvements were observed in the cost of the tool room, 76% in the downtime rate of the machines due to the tool room, 78% in the scrap rate due to the faulty cutting tool, and 85% in the unnecessary movement load rates in the tool room. As a result of these improvement works, the efficiency of production has increased. With the gray relational analysis, where all outputs are evaluated jointly, it has been revealed that the most productive month in terms of production is November, when production is made by applying the 5S approach. Doris Lessing’s novel, Mara and Dann: An Adventure reflects a multi-layered exploration of trauma’s enduring impact on both individuals and their environments. In Lessing’s novel, the shattered world left by the apocalypse continues to breed a multitude of traumas: war, violence, discrimination, and the enduring wounds of inequality. This article explores how the novel employs space as a tool to create a powerful metaphor, where both physical landscapes and the body serve as maps representing profound traumas. Drawing on theories of trauma and postcolonial ecocriticism and inspired by the idea that the novel is an “epistemological narrative form” (Tally, 11), that “projects, describes, and figuratively maps the social spaces” (96), this article examines how Lessing utilizes spatial imagination to enable the characters in her novel, particularly the protagonist Mara, to create new maps: physical, mental, and social. These maps enable them to bear witness to the past traumas of land annihilation and cultural loss, as well as the present traumas of bodily violation, ultimately providing a context for trauma resilience. Objective: The study aimed to retrospectively evaluate postoperative results of patients with duplex system ureterocele (DSU) who underwent endoscopic ureterocele incision (UI), lower urinary tract reconstruction (LUTR) or both, and investigate the necessity of upper pole heminephrectomy (UPH) in ureterocele treatment. Patients and methods: In total, 44 patients with DSU who were operated on in December 2005 and December 2018 were evaluated for patient characteristics, ureterocele location, differential renal function (DRF), vesicoureteral reflux (VUR), postoperative incontinence, proteinuria, hypertension and urinary tract infection (UTI). Results: UI was performed in 27 of 44 cases (61.3%) as the first-line treatment. Ten of them (37%) had no urinary infection or renal function loss at a mean of 3 years (1.5–7 years). LUTR was needed in 17 patients after UI due to VUR in 14 patients and bladder outlet obstruction (BOO) in 3 patients. In total, 34 patients who underwent LUTR, proteinuria or incontinence were not detected in the mean 6 years' follow-up. Thirty-three patients (97%) had identical renal scans and similar DRF before and after the operation. No patient underwent UPH. Conclusion: UI must be considered the first-line treatment in DSU in infancy. In older patients, especially with VUR, LUTR is effective and safe and UPH mostly is not necessary. Background. Pandemics, such as COVID-19, have the potential to adversely affect children's development due to a variety of negative factors at the level of children, families, and services. In this study the effect of the pandemic on the cognitive, language and motor development of premature babies who are among the most vulnerable group, were evaluated. Methods. The study included 236 premature infants who were followed at Hacettepe University Department of Developmental Pediatrics. The Bayley-Third Edition Developmental Assessment (Bayley III) was used to evaluate the neurodevelopment of 152 premature infants from the pre-pandemic group and 84 from the postpandemic group at the corrected age of 18-24 months. The perinatal and sociodemographic risks were also evaluated. Results. No difference in Bayley III scores (cognitive, language, and motor) was found between the pre- and post-pandemic groups. Furthermore, the multivariate covariance analysis displayed that regardless of the pandemic, infants with higher maternal education consistently scored higher in the cognitive, language, and motor domains; and the motor area scores of infants with moderate perinatal risk were also significantly higher than infants with high perinatal risk. Conclusions. It is crucial to monitor the development of vulnerable children who encounter developmental risks, such as premature babies. Fortunately, no significant effect was encountered during the COVID-19 pandemic. However, this does not underweigh the need for close supervision in extraordinary circumstances. Additionally, it should be noted that severe postnatal comorbidities, perinatal risks, and social factors, such as maternal education level, interact to influence the neurodevelopmental outcomes of preterm infants. This article discusses the case of a patient who experienced progression of disc herniation after receiving a transforaminal epidural steroid injection (TFESI) for low back and leg pain. The patient initially had a positive response to the injection, but two days later, she reported increased pain and difficulty walking. An MRI revealed an increase in the size of the herniation. The article suggests that further research is needed to determine if this is a possible side effect of the treatment. [Extracted from the article]

OBJECTIVES. We investigated the effect of vitamin D3 deficiency on peripheral and central odor centres by using cranial magnetic resonance imaging (MRI). MATERIAL AND METHODS. In this retrospective study, cranial MRI images of 23 adolescents with vitamin D3 deficiency and 18 adolescents with normal vitamin D3 levels (≥ 20 ng/ml) were evaluated. Blood tests for vitamin D3, B12, hemoglobin and calcium levels were also taken from the database of our hospital. Peripheral odor centers, olfactory bulb (OB) volume and olfactory sulcus (OS) depth, and also central odor centers, insular gyrus and corpus amygdala area were measured on cranial MRI. RESULTS. The insular gyrus and corpus amygdala area were not different between the study group with vitamin D3 deficiency and the control group (p>0.05). There was no difference between OB volume of the study and control groups (p>0.05). Although the right insular area was lower in girls than males (p<0.05), there was no correlation between OB volume and gender (p>0.05). Higher hemoglobin levels were related to higher OB volume on the right side (p<0.05). CONCLUSION. In the group with vitamin D3 deficiency, olfactory bulb volume of the left side was asymmetrically larger than the one on the right side. Therefore, the measurements of vitamin D3 should be performed in adolescents, and the patients with the vitamin D3 levels of < 20 ng/ml should be supplemented with medical treatment and reinforcing vitamin D synthesis by the exposure of sunlight more. Objective: We aimed to examine the relationship between chronic rhinosinusitis without nasal polyps and microplastics. Methods: A total of 80 patients participated in this prospectively planned study. The patients were divided into two groups. Group 1 had 50 patients with chronic rhinosinusitis without nasal polyps, whereas Group 2 had 30 healthy volunteers. The age and gender of the participants were noted. Nose Obstruction Symptom Evaluation questionnaire was applied to the patients. The patients performed nasal lavage with saline. Microplastics were examined in the collected nasal lavage fluids, and their numbers were noted. The groups were compared on these values. Results: The mean age was 38.06 ± 14.15 years in the chronic rhinosinusitis group without nasal polyps and 33.60 ± 11.68 years in the control group. There was no significant difference between the groups in terms of age and gender. There was a significant difference in the number of microplastics between the chronic rhinosinusitis group without nasal polyps and the control group (p < 0.001). Microplastics were detected in all participants. Conclusions: We found more microplastics in patients with chronic rhinosinusitis without nasal polyps. Cervical transforaminal epidural steroid injections (TESIs) have technical difficulties and a risk of complications due to the cervical spine anatomy. A 52-year-old female patient was admitted to our outpatient clinic with complaint of neuropathic pain radiating to her right arm. Right C7 TESI was planned for patient who did not respond to conservative treatment. Immediately after the procedure, flaccid paralysis was detected in the right side and, at the end of 24 h, the patient completely regained her former muscle strength. In conclusion, patient selection for the cervical TESI should be done carefully and kept in mind that transient spinal cord ischemia may develop. [ABSTRACT FROM AUTHOR]

Objective: This study, myotonometric properties of longissimus thoracis, multifidus lumborum, rectus abdominis in adolescent idiopathic scoliosis (AIS) patients were examined according to pelvic axial rotation (PAR) direction and convex/concave side of major curvature. As a result of study, information about biomechanical properties of muscles will be obtained and treatment will be more effective. It is aimed to be contribute to literature. Methods: Individuals between ages of 10-18 were diagnosed with AIS were included in this prospective and descriptive study. Cobb angle and PAR were measured on radiographs. Tone, hardness, elasticity of relevant muscles were measured with MyotonePRO device. Data analysis used with SPSS25 program. Results: A total of 38 patients (male: 11, female: 27) were examined. They were grouped according to PAR direction and the convex/concave side of major curvature, and myotonometric properties of relevant muscles were analyzed. On the concave/convex sides; A moderate positive correlation was found between tone of multifidus lumborum and longissimus thoracis, a strong positive correlation was found between tone of multifidus lumborum and rectus abdominis. A weak positive correlation was found between hardness of multifidus lumborum and longissimus thoracis on convex/concave sides, and moderate positive correlation was found between hardness of multifidus lumborum and rectus abdominis. A weak positive correlation was found between elasticity of multifidus lumborum and rectus abdominis on convex/concave sides. Conclusion: In the tone, stiffness and elasticity of convex/concave side muscles of AIS patients, agonist antagonist muscle influence is more prominent than influence of synergist muscles. This effect should be taken into consideration when planning treatment programs, and it will contribute to shaping of treatment and will contribute to literature since there are not enough studies on this subject. Objective: Adolescent idiopathic scoliosis (AIS) is a 3D spinal deformity consisting of lateral deviation and axial rotation of spine that develops during growth. Spinal musculus is closely related to head and neck muscles, such as sternocleidomastoideus muscle. In this study, according to axial trunk rotation in AIS patients, We aimed to contribute to literature by examining changes in biomechanical properties of sternocleidomastoideus muscle. Methods: This study was planned as prospective and descriptive. Study included 46 individuals (34 females, 12 males) who applied to Gaziantep University Department of Orthopedics and Traumatology, were between ages of 10-18, were diagnosed with AIS by relevant physician, and hadn't spinal deformity on all spine radiographs. Individuals who had undergone spine surgery, didn't have idiopathic scoliosis, had orthopedic, muscular, neurological diseases accompanying their scoliosis weren't included in study. Cobb angle was measured on AP radiographs. MyotonePro device, which is valid in literature was used to measure tone, hardness and elasticity of m. sternocleidomastoideus. Results: There is a statistically significant difference between pre-post-exercise D measurements of left m.sternocleidomastoideus in the participants. Left m. sternocleidomastoideus D measurement increased in second measurement. A statistically significant positive relationship was found between right sternocleidomastoideus S measurement and thoracic Cobb angle (p=0.045). A statistically significant negative relationship was found between left m.sternocleidomastoideus D measurement and lumbar Cobb angle (p=0.045). Conclusion: It has been determined that hardness and elasticity of m. sternocleidomastoideus may affect thoracic and lumbar Cobb angles. Objective: Adolescent idiopathic scoliosis(AIS) is three-dimensional spinal deformation consisting of lateral deviation and axial rotation of vertebrae that develops during body growth. It has been reported that scoliosis may cause asymmetries in upper and lower extremities and biomechanical changes in musculoskeletal system. Best indicator of this transmission is one of most important muscles of thigh ligament, which is tendon of quadriceps femoris muscle. It is thought to be patella. Aim of this study is to determine direction of pelvic asymmetry in AIS patients. It is examination of changes in patella features. Treatment plan can be shaped by evaluating effect of this change on patient's walking patterns. Methods: Prior to study, approval was obtained from Gaziantep University Clinical Research Ethics Committee (2023/295). 36 individuals applied to Gaziantep University Department of Orthopedics and Traumatology were diagnosed with scoliosis were included. MyotonePro device, which is valid in literature and is non-invasive and doesn't cause complications in patient, was used to measure muscle tone, hardness, elasticity in myofacial tissues. Results: It was determined that right-left ligamentum patella stiffness was statistically higher in individuals with right pelvic asymmetry than in individuals with left pelvis asymmetry. It was found that elasticity of left ligamentum patella was significantly higher in individuals with right pelvic asymmetry. Conclusion: It has been determined that direction of pelvic asymmetry may affect biomechanical properties of lig.patella. Giriş: HIV pozitif hastalarda Pneumocystis jirovecii olgularının %90'ından fazlasında CD4+T lenfosit sayısı <200 hücre/mm3 olduğu görülmüştür. HIV pozitif Pneumocystis jirovecii pnömonisi (PCP) olgularında pnömotoraks gelişme insidansı bir çalışmada %9 bulunmuş olup çoğu olguda göğüs tüpü uygulanmadan tedavi başarısı sağlanmıştır. Başka bir retrospektif çalışmada HIV pozitif PCP olgularında pnömotoraks insidansı %4,9 bulunmuştur. Bizim olgumuzda CD4 sayısı 1 olup PCP tanısı ile trimetoprim (TMP) sulfametoksazol (SMX) tedavisi almakta iken pnömotoraks gelişen bir olgu irdelenmiştir. Olgu: Elli iki yaşındaki kadın hasta nefes darlığı şikayetiyle 26.03.2024'te hastanemize başvurdu. Öyküsünde 15 gün önce son 2 aydır giderek artan nefes darlığı, sırt ağrısı, 8 kg kilo kaybı ve ateş yüksekliği şikayetleri ile dış merkeze başvurduğu öğrenildi. Yapılan tetkiklerinde akciğer tomografisinde bilateral yaygın infiltrasyonlar görülmesi ve anti-HIV (+) saptanması üzerine bakteriyel pnömoni ve PCP pnömonisi ön tanıları ile hastaya piperasilin tazobaktam 4x4,5 gr intravenöz (iv) ve TMP-SMX (15 mg-75 mg/kg/gün) iv, (4 eşit doza bölünerek) başlandığı, prednizolon tedavisi almadığı, indüklenmiş balgam örneğinde PCP PCR 1,262,556 kopya/uI sonuçlanan hastanın takiplerinde nefes darlığı şikayetinin gerilemesi üzerine tedavinin 10. gününde TMP-SMX forte 2x1 po ile taburcu edildiği öğrenildi. Özgeçmişinde kronik obstrüktif akciğer hastalığı, hipertansiyon tanıları mevcuttu. Tarafımızca acil serviste değerlendirilen hastada pnömotoraks (Şekil 1) saptanması üzerine hastaya göğüs cerrahisi tarafından göğüs tüpü takıldı. Takipneik seyreden hastanın arter kan gazında parsiyel oksijen basıncı (pO2) 58 mmHg saptandı. Tedavisi TMP-SMX (15 mg-SMX 75 mg/kg/gün) iv, (4 eşit doza bölünerek) ve prednizolon ilk 5 gün 2x40 mg iv (21 günde tedrici olarak azaltılıp stoplanmak üzere) olarak düzenlendi. Hastanın takipleri devam etmektedir. Sonuç: CD4 sayısı 200'ün altında olan hastalarda PCP gelişebileceği akılda tutulmalı ve pnömoni ile başvuran hastalarda ayırıcı tanıda mutlaka Pneumocystis jirovecii akla gelmelidir. PCP olan olgularda ilk tercih olarak TMP-SMX önerilirken arter kan gazında parsiyel oksijen basıncı <70 mmHg veya alveol-arterial oksijen gradienti ≥35 mmHg olan hastalarda tedaviye steroid eklenmesi de önerilmektedir. Uygun tedavi rejimini alsalar dahi PCP olgularında bir komplikasyon olarak pnömotoraks gelişebileceği hatırlanmalı, uygun tedavi altında nefes darlığında artış olan hastalar bu komplikasyon açısından tekrar değerlendirilmelidirler.
